Wound repair is a complex biological process and great progresses have been achieved during the last ten years because of the integration of molecular biology and traumatology. However, some fundamental principles in these fields, which are closely related to developmental biology and comparative biology have not been fully understood. Therefore, more attention should be paid to the incorporation basic knoweldge with technology of developmental biology and comparative biology in the research of wound repair before better understanding and new discoveries are achieved in this field.
3.Arthroscopic PCL reconstruction with an implant fixation of hamstring tendon knot and bottle-neck technique

Objective To evaluate arthroscopic PCL (posterior cruciate ligament) reconstruction with an implant fixaliun of hamstring tendon knot and bottle-neck technique.Methods Thirteen cases with PCL injury were treated with arthroscopic PCL reconstruction with an implant fixation of hamstring tendon knot and bottle-neck technique between April,2002 to June,2005.The patients were followed up fur at least seven months to evaluate the clinical effects by means of ROM of the joint,post drawer test,Lachman test,Lysholm and IKDC (International Knee Documents Committee) scores.Results The patients were followed up for an average of 13.4 months.No limitation of knee extension was found six months after operation.Eleven patients had normal knee flexion(>140?), one had knee flexion of 120?and one 100?.In posterior drawer test,11 patients were gradeⅠand two gradeⅡ.In Lachman test,11 patients were rated as within gradeⅠand two gradeⅡ.By Lysholm knee function scale,the average knee scores were 90.47?4.13 and 78.82?2.46 respectively before and after operation,and the dif- ference was of statistical significance (t=2.416,P=0.029).By IKDC scale,six were grade A,five grade B and one grade C.Conclusion Arthroseopic PCL reconstruction with an implant fixation of hamstring tendon knot and bottle-neck technique is a reliable method to restore knee function and will have a prospect of wide application be- cause of its good tissue biocompatibility,less invasion,and less operative expenditure.
4.Arthroscopic medial patellofemoral ligament reconstruction combined with lateral retinacular release for recurrent patellar dislocation.

OBJECTIVETo evaluate the clinical results of the medial patellofemoral ligament (MPFL) reconstruction combined with the lateral retinacular release for the treatment of recurrent patellar dislocation.
METHODSFrom March 2011 to June 2013, 15 patients with recurrent patellar dislocation underwent arthroscopic MPFL reconstruction combined with the lateral retinacular release. The graft was autogenous semitendinosus and semimembranosus tendon. There were 5 males and 10 females with an average age of 19.4 years old (ranged,14 to 32 years old). The patients suffered recurrent patellar dislocation at least twice preoperatively. Preoperative conventional X-ray, CT, and MR examination were used to analyze the causes of the patellofemoral joint and MPFL injury. Preoperative Lysholm score was 69.85 ± 11.52. During operation, the arthroscopic examination was performed to evaluate the patellofemoral alignment and patellar tracking.
RESULTSAll the patients were followed up for an average of 27.6 months (ranged,12 to 36 months) with no recurrent dislocation and sub-dislocation. All the patients showed negative apprehension test at straight and 30 ° flexions of knee. The range of motion of knee returned to normal level at 12 months after operation. There were no patients with subjective discomfort of knee. Postoperative Lysholm score was improved to 92.60 ± 5.75.
CONCLUSIONThe technique of arthroscopic MPFL reconstruction combined with the lateral retinacular release is an effective surgical procedure for the treatment of recurrent patellar dislocation, which can relieve the symptom of knee and improve the patella stability and knee function.

5.Effect of miR-24-2 on Proliferation of U-2OS Cell Line

Objective To explore the effect of miR-24-2 on the in vitro proliferation of U-2OS cells. Methods U-2OS cells were randomly allocat-ed into 4 groups:miR-24-2 mimic group,miR-24-2 inhibitor group,negative control group,and normal control group. MicroRNAs were transfected into U-2OS cells using Lipofectamine?2000. miR-24-2 expression level and the proliferation of U-2OS cells after transfection were detected by real-time quantitative RT-PCR(RT-qPCR)and MTT proliferation assays,respectively. Results RT-qPCR results showed that miR-24-2 level was sig-nificantly higher in the miR-24-2 mimic group and lower in the miR-24-2 inhibitor group than those in the controls,indicating the successive trans-fection. MTT proliferation assay results proved that the cell viability was significantly lower in the U-2OS cells transfected with miR-24-2 mimic and higher in inhibitor groups compared to the control. Conclusion MiR-24-2 inhibits growth of the U-2OS cells,which could be a potential biomarker in the treatment of osteosarcoma.

7.Clinical Analysis of Complications of 115 Children with Serious Pneumonia with Ventilatory Support

Objective To explore the clinical characteristics of children with serious pneumonia with ventilatory support,and combined with heart failure,encephalopathy,hyponatramia,acute respiratory distress syndrome(ARDS) and multiple organ dysfunction syndrome(MODS).Methods One hundred and fifteen children(81 males,34 females) with serious pneumonia in ICU from Jul.2005 to Jul.2007 were reviewed,The duration of ventilation support was over 48 h.And blood electrolyte,blood gas,cerebrospinal fluid and CT were detected.Results Ninety-three cases had a good outcome,7 cases were given up,15 cases were dead.The complications included heart failure in 60 cases(52.17%);encephalopathy in 40 cases(34.78%),in which tics type in 38 cases(95%),apnea type in 11 cases(27.5%);hyponatremia in 42 cases(36.52%),of which tics type in 31 cases(73.81%),syndrome of inappropriate secretion of antidiuetic hormone in 27 cases(72.97%)resulted in hyponatremia;ARDS in 8 cases(6.96%),pa(O2)/FiO2
8.Experience in treatment of poorly differentiated thyroid carcinoma

Objective:To discuss strategies in treatment of poorly differentiated thyroid carcinoma (PDTC) .Methods:Clinical data of 31 cases with PDTC were reviewed retrospectively, who were treated in Department of Head & Neck Surgery, Sichuan Cancer Hospital & Institute by primary surgical resection with or without adjuvant therapy. 27 cases had total thyroidectomy compounded neck dissection or extened total thyroidectomy when trachea or esophagus involved. 4 cases underwent partial resection of tumor. 11 cases were treated with external beam radiotherapy (EBRT) after surgery, 10 cases were treated with postoperative radioiodine, and 8 cases had chemotherapy.Results:The median follow-up time was 18 months (ranged from 3-96 months) . 19 patients died of local recurrence or distant metastasis. Kaplan-Meier analysis and Log-rank analysis was used to compare the differences between groups. Five-year survival was 35.9%. Compared to the cases with partial resection, the cases with surgical disease clearance had longer survival ( P=0.00) . The same statistical difference was found between patients with or without radioiodine ( P=0.017) . The patients treated with radioiodine had longer survival. No statistical differences were found among patients with or without chemotherapy or EBRT. COX regression analysis showed stage of tumor ( P=0.005) , total resection ( P=0.006) and postoperative radioiodine ( P=0.013) were same to predict longer survival. Conclusions:Thorough resection of tumor is the most important therapy for PDTC. Postoperative radioiodine is recommended for patients with high recurrence risk. EBRT is recommended to control local unresectable PDTC.
9.The biomechanical study and clinical application of internal fixation with Kirschner wire placed in the nerval fossa of ulna for GartlandⅢsupracondylar fractures of humerus in children

Objective To perform a biomechanical study on and compare the stability of internal fixation with closed Kirschner wire in different spots from the epicondylus medialis to sulcus nervi ulnaris and epicondylus lateralis of humerus in the treatment of GartlandⅢsupracondylar fractures of humerus in children in order to provide basis for avoiding cubitus varus in clinic.Methods Twenty models of su- pracondylar fractures were made from ten humerus samples,then divided into two groups(group A and group B).They were fixed with closed Kirschner'wires.Ulnar Kirschner wire varied.In group A five spots were selected averagely from the epicondylus medialis to the sulcus nervi ulnaris.Kirschner wires entered from one of the five spots into another spot of humerus.In group B Kirschner wires entered defferent spots of humerus from the central spot of sulcus nervi ulnaris.Biomechanical tests were carried out,such as axial load,axial displacement,stress intensity,axial rigidity and anti-twisty ability.Forty-six children with Gart- landⅢsupracondylar fracture of humerus were treated and followed up for more than one year.Results In beth groups,with the angle of Kirschner wires and fracture side increasing,the strain and the vertical displacement decreased,the stress intensity and the axial rigidity increased,and the anti-twists ability was enhanced.There was statistic difference(P<0.05).The follow-up of the 46 patients showed the elbow joint function recovered well with no cubitus varus malformation.Conclusion Internal fixation with closed Kirschner wires in sulcus nervi ulnaris from the epicondylus medialis and the epicondylns lateralis of humerus for GartlandⅢsupracondylar fracture of humerus in children has the advantages of enhancing bio- mechanical stability,effective prevention of secondary replacement of outlying black of the fracture and a- voidance of cubitus varus.It can be widely used clinically.
10.Transpedicular bone graft for the treatment of thoracolumbar vertebral fractures through Wiltse approach.

OBJECTIVETo explore methods and therapeutic effects of transpedicular bone graft in treating thoracolumbar fractures through Wiltse approach.
METHODSFrom March 2009 to February 2012,56 patients with thoracolumbar fractures were treated by transpedicular bone graft through Wiltse approach. Among them, there were 36 males and 20 females, ranging in age from 14 to 55 years old (mean, 41 years old). The time from injury to operation from 2 to 15 d (mean,3 d). Twenty-five cases were caused by falling down, 7 cases were caused by slipping, 20 cases were caused by car accident and 4 cases were caused by crush trauma. MRI was performed before operation to exclude pathological fracture. The distance between multifidus muscle and longissimus to midcourt line was measured. Self-made trocar was applied in operation. According to AO classification,there were 33 cases with type A1 compression fracture,5 cases with type A2 cleavage fracture and 18 cases with type A3 burst fracture. Sixteen cases of the 56 cases combined with spinal cord injury. Based on Frankel neurologic grading system, preoperative neurological function was grade B in 5 cases, grade C in 2 cases, gade D in 9 cases. Preoperative Denis gading were P5. Frankel and lumbago Denis clssification were used to evaluate neurological function and lumbago. The imaging data before, after operation and the latest follow-up were used to evaluate correction vision.
RESULTSAll patients were followed up over 24 months. At the time of the latest follow-up, Frankel B were 3 cases, Frankel C were 2 cases, Frankel D were 4 cases and Frankel E were 7 cases. According to lumbago Denis clssification, P1 (painlessness) were 32 cases, P2 ( slight pain without treatment) were 18 cases, P3 ( moderate pain and taking medicine occasionally) were 6 cases. The anterior vertebral height improved from preoperative (13.38 +/- 4.72)mm to postoperative (22.18 +/- 1.44)mm. The Cobb's angle decreased from preoperative (28.39 +/- 2.64) degrees to (10.07 +/- 3.05) degrees. There were no nails broken, rod broken, internal fixation lossen and vertebral body recompression.
CONCLUSIONTranspedicular bone graft for thoracolumbar fractures through Wiltse approach can reduce intraoperative blood loss and postoperative complications, and aviod "eggshell" vertebral body. Mastering revealed way, drafting detailed preoperative plan and eariler exercise is the key to the success of treatment.
