1. Three-dimensional finite element biomechanical analysis of stage II adult acquired flatfoot deformity after medial column stabilization

BACKGROUND: Flatfoot is a commonly seen disease in foot and ankle surgery, and stage II adult acquired flatfoot is mostly seen in clinic, so this stage is a key to treatment. However, medial column instability occurs in stage II adult acquired flatfoot, which is an important cause for arch collapse. Medial column stabilization can correct the deformity to great extent, but there is a lack of biomechanical study to assess the effect of medial column stabilization on the whole foot. OBJECTIVE: To investigate the biomechanical effects of medial column stabilization on stage II adult acquired flatfoot. METHODS: A three-dimensional finite element model of stage IIa and IIb adult acquired flatfoot was established. Geomagic software, Solidwork software and Abaqus software were used to simulate medial column stabilization operation (naviculocuniform joint fusion, tarsometatarsal joint fusion, and both fusion). The maximum pressure of plantar soft tissue, medial column bone and medial ligaments was compared before and after simulated single-foot weight loading. Meanwhile, the related parameters were measured to carry out a comprehensive comparison. RESULTS AND CONCLUSION: (1) The maximum plantar stress was located under the first metatarsal head after the simulated medial column stabilization operation. The maximum plantar stress increased significantly after the medial column stabilization in stage IIa flatfoot model, but did not change significantly after the medial column stabilization in stage IIb model. (2) After medial column fusion, the stress of the corresponding joint was reduced, but increased for the other joints of the first metatarsal column. (3) The stress of medial ligament and plantar fascia was not alleviated after medial column fusion. (4) These results indicate that simple medial column stabilization surgery cannot reduce the pressure of medial column of flatfoot in stage II acquired flatfoot adults. It can only be used as a combined surgery to stabilize joints with excessive motion and correct the deformity of supination of forefoot.
2.Overexpression of sarcoplasmic reticulum calcium ATPase induced hemodynamic and proteomic changes in a dog model of heart failure.

OBJECTIVEOverexpression of SERCA2a could improve cardiac function in human and experimental heart failure (HF) models. We observed the proteomics changes post SERCA2a overexpression in a pacing induced HF model in dogs.
METHODSBeagles were divided into four groups: control group, HF group (230 beats/min for 4 weeks), HF + EGFP group (myocardial injection of 1 x 10(12) v.g recombinant adeno-associated virus carrying enhanced green fluorescent protein gene, rAAV2/1-EGFP) and HF + SERCA2a group (myocardial injection of 1 x 10(12) v.g recombinant adeno-associated virus carrying SERCA2a gene, rAAV2/1-SERCA2a). Thirty days after gene transduction, left ventricular systolic and diastolic functions were measured by echocardiography and invasive hemodynamics in all animals. By use of 2-dimensional gel electrophoresis (2-DE), -500 distinct protein spots were detected in myocardium of all animals. Protein spots observed to be altered between failing and SERCA2a overexpressed hearts were subjected to tryptic peptide mass fingerprinting for identification by MALDI-TOF mass spectrometry in combination with LC/MS/MS analysis.
RESULTSAt 30 day after gene transfer, HF signs were significantly reduced, cardiac function [LVSP: (214.72 +/- 31.74) mm Hg (1 mm Hg = 0.133 kPa) vs. (139.32 +/- 36.79) mm Hg, +dp/dt(max): (6779.43 +/- 217.58) mm Hg/s vs. (2746.85 +/- 931.23) mm Hg/s and -dp/dt(max): (-4341.42 +/- 322.02) mm Hg/s vs. (-2531.14 +/- 616.15) mm Hg/s, LVEDP: (21.86 +/- 6.95) mm Hg vs. (59.78 +/- 6.92) mm Hg] significantly improved in HF + SERCA2a dogs than those in HF + EGFP group(all P < 0.05) and parameters were comparable between HF + SERCA2a and control groups. We identified alterations in the expression level of more than 10 proteins in myocardium. These protein changes were observed mainly in two subcellular compartments: the cardiac contractile apparatus and metabolism/energetics.
CONCLUSIONThese results showed that overexpression of SERCA2a could improve cardiac function accompanied with numerous alterations in protein expressions involved in calcium handling, myofibrils, and energy production in this dog model of chronic heart failure.

4.Study on the molecular epidemiology of people infected with human immunodeficiency virus-1 in Shandong province.

OBJECTIVETo study the distribution of human immunodeficiency virus-1 (HIV-1) strains subtypes in Shandong province and to study their source in order to predict the epidemic trend.
METHODSEpidemiological investigation was made and 93 DNA fragments of HIV-1 env, gag, tat gene were amplified by nested polymerase chain reaction from people infected with HIV-1, in 2002 - 2003. Their C2-V3, P17/P24, 1st exon of tat and adjacent region were sequenced.
RESULTSSequence analysis showed that there were 7 HIV-1 strains or circulating recombinant forms (CRFs), B' (n = 71), CRF01-AE (n = 9), CRF07-BC (n = 3), CRF08-BC (n = 3), B (n = 2), C (n = 2), CRF02-AG (n = 2). B' strains was the predominant which, covered 10 cities and 4 kinds of population including blood donors, blood receivers, spouses of the infected people and clients of the sex workers. CRF07-BC, CRF08-BC strains were identified in 5 cities, mainly from injecting drug users. CRF01-AE and other strains were found distributed in developed cities, among sex workers.
CONCLUSIONThere were many kinds of subtypes and CRFs of HIV and their genomes which generated obvious variation in Shandong province, suggesting that they might facilitate the spread of the disease in Shandong province.

5.Epidemiological feature of metabolic syndrome in Shanghai residents aged 15 - 74 years.

OBJECTIVETo assess the prevalence of the metabolic syndrome in Shanghai residents.
METHODSA cross-sectional study was conducted in Shanghai with a representative sample of 14,327 Chinese adults from 15 to 74 years of age. The sample was randomly selected using stratified cluster sampling. The criteria of International Diabetes Federation (IDF), the US National Cholesterol Education Program Adult Treatment Panel III (ATP III) guidelines and the China Diabetes Society (CDS), respectively, were applied for diagnosis.
RESULTSDiagnosed by using the IDF criterion, the crude prevalence of metabolic syndrome was 17.51%, and the age and gender-adjusted prevalence was 12.81% (95% CI: 12.26 - 13.36). The prevalence of the metabolic syndrome was higher in females than in males (14.79% and 10.93%, respectively), and higher in urban residents than in rural ones (13.71% and 10.72%, respectively). The age-specific prevalence of the metabolic syndrome increased in both male and female residents and the prevalence of metabolic syndrome in females 45 years of age or elder was significantly increased. The prevalence of having at least 3 parameters of metabolic syndrome was 22.28%. According to the ATP III guideline, the crude and age-adjusted prevalence of metabolic syndrome was 12.03% and 8.62%, respectively. Based on the criterion of CDS, they were 13.98% and 10.41%.
CONCLUSIONThere is a high prevalence of metabolic syndrome in Shanghai residents. Metabolic syndrome has become a noteworthy public health problem. It suggests that community-integrated control of metabolic syndrome should have its priority.

7.Optimization of culture measure for bovine-bovine and goat-bovine cloned embryos in vitro.

UNLABELLEDThis study is conducted to explore an effective culture method for supporting the embryo development. The cattle fetal ear fibroblasts and the goat fetal ear fibroblasts are transplanted into the enucleated cattle oocytes separately by oocyte intraplasmic nuclear injection method to construct bovine cloned embryos and goat-bovine cloned embryos. The embryos are first cultivated in modified charles rosenkrans 2 amino acid medium (mCR2aa) and modified synthetic oviduct fluid medium (mSOF) separately. Then BSA (8 mg/mL) or FBS (10%) can be added to mSOF according to the different culture period. The supplements and orders, added during the first three days and after three days are as follow: BSA and BSA, BSA and FBS, FBS and BSA, FBS and FBS. On the basis of the cleavage rate, 8/16-cell rate, blastocysts rate and total cell number of blastocysts, the best culture way can be screened out.
RESULTFirst, cleavage rate, 8/16-cell rate, blastocysts rate and total cell number of blastocysts, cultivated in mSOF solution are all higher than those cultivated in mCR2aa( P < 0.05). Second, the cleavage rate and 8/16-cell rate, adding BSA and FBS into mSOF, are in turn 79.8% +/- 7.1%, 49.7% +/- 3.5%, 21.5% +/- 1.8%, and 115.2 +/- 4.3 in bovine cloned embryo, and 40.1% +/- 6.3%, 29.2% +/- 2.0%, 13.4% +/- 2.1% and 100.1 +/- 3.0 in goat-bovine cloned embryo, which are significant higher than other culture groups (P < 0.05).
CONCLUSIONThe goat-bovine cloned embryo can be cultivated by the optimized culture measure of bovine cloned embryo. The best culture ways of bovine cloned embryo and goat-bovine cloned embryo are all to use mSOF supplemented BSA in the first three days and then use mSOF supplemented FBS in the next five days.

8.Design and realization of remote psychological service platform

Objective To design a remote psychological service platform to solve the problems of elementary troops in lack of psychologists,psychological intervention means and effects.Methods A remote psychological service platform was developed with B/S architecture,ASP.NET and ADO.NET technologies,Visual Studio.NET 2008 development platform and C# language,which was composed of four functional modules of psychological classroom,video teaching and psychological counseling,psy-chological assessment and file management,and system management.Results The platform developed provided elementary troops with remote psychological service for psychological assessment,psychological counseling,psychological guidance,mental health education,professional training for health personnel and emergency intervention.Conclusion The platform developed gains advantages in deployment,professional service,timely response and confidentiality,and facilitates the same psychological service effect as face-to-face communication.[Chinese Medical Equipment Journal,2023,44(9):49-52]
9.Modified CT imaging scale in assessment of prognosis of comatose patients with severe traumatic brain injury

Objective To explore the application of modified CT imaging scale in the assessment of prognosis of comatose patients with severe traumatic brain injury. Methods Retrospective analysis was performed on the clinical data of 90 comatose patients with severe traumatic brain injury; according to the prognosis,they were divided into conscious group (n=47) and coma group (n=43).The modified CT imaging scale was used to analyze the relationship between these CT imaging data and prognosis,and the relationship between these CT imaging data and severity of illness. Results The scores of CT imaging scale in comatose patients were significantly higher than those in conscious patients (P<0.05). Patients with 3-5 scores in GCS enjoyed higher scores of CT imaging scale as compared with those with 6-8 scores (P<0.05).The scores of modified CT imaging scale and the GCS scores in these 90 patients showed negative correlation (Spearman's correlation coefficient:r=-0.79,P=0.000). Conclusion The modified CT imaging scale is helpful in assessing the prognosis and guiding the clinical treatment of comatose patients with severe traumatic brain injury.
10.The epidemiological study on human immunodeficiency virus infection among paid blood donors living in Shandong provincial China Comprehensive Response Project Areas.

OBJECTIVETo understand the current epidemic situation and high risk factors of human immunodeficiency virus (HIV) infection among paid blood donors living in Shandong provincial China Comprehensive Response (CARES) Project Areas.
METHODSAll residents between 20 to 60 years old were selected from one or several counties in August 2003.
RESULTSThere were 19 HIV(+) infections among 661 subjects interviewed. HIV prevalence rate among paid blood donors was higher (3.98%) than that of others (0.48%) while HIV prevalence rate was higher in plasma donors (7.24%) than that in both plasma and full blood donors (2.90%). There was no infections identified in full blood donors. Donors who donated blood during 1994 - 1995 had a higher prevalence rate (7.07%) than those who started donation after 1995 and those began donation before 1993 (0.99%). The rate of condom use was lower among paid blood donors (13.72%) than that of others. There was a lower rate of voluntary HIV testing among paid blood donors than that of others.
CONCLUSIONThe main high risk population of HIV infection were paid donors in CARES Project Areas, whose risk factor was plasma donation and were infected mainly before 1995. Both the rate of condom use and HIV testing were all very low, indicating that they knew little on HIV/AIDS and paid less attention to self-proctection. It is necessary to enhance HIV/AIDS health education and behavior intervention on paid blood donors.
