2.Arthroscopic medial patellofemoral ligament reconstruction combined with lateral retinacular release for recurrent patellar dislocation.

OBJECTIVETo evaluate the clinical results of the medial patellofemoral ligament (MPFL) reconstruction combined with the lateral retinacular release for the treatment of recurrent patellar dislocation.
METHODSFrom March 2011 to June 2013, 15 patients with recurrent patellar dislocation underwent arthroscopic MPFL reconstruction combined with the lateral retinacular release. The graft was autogenous semitendinosus and semimembranosus tendon. There were 5 males and 10 females with an average age of 19.4 years old (ranged,14 to 32 years old). The patients suffered recurrent patellar dislocation at least twice preoperatively. Preoperative conventional X-ray, CT, and MR examination were used to analyze the causes of the patellofemoral joint and MPFL injury. Preoperative Lysholm score was 69.85 ± 11.52. During operation, the arthroscopic examination was performed to evaluate the patellofemoral alignment and patellar tracking.
RESULTSAll the patients were followed up for an average of 27.6 months (ranged,12 to 36 months) with no recurrent dislocation and sub-dislocation. All the patients showed negative apprehension test at straight and 30 ° flexions of knee. The range of motion of knee returned to normal level at 12 months after operation. There were no patients with subjective discomfort of knee. Postoperative Lysholm score was improved to 92.60 ± 5.75.
CONCLUSIONThe technique of arthroscopic MPFL reconstruction combined with the lateral retinacular release is an effective surgical procedure for the treatment of recurrent patellar dislocation, which can relieve the symptom of knee and improve the patella stability and knee function.

3. Influence of impaired glucose tolerance on non-fatal stroke in elderly male persons

Objective To investigate the influence of impaired glucose tolerance (IGT) on the risk of non-fatal stroke in elderly male persons. Methods The elderly male persons with IGT and normal glucose tolerance (NGT), undergoing oral glucose tolerance test in our hospital from May 2005 to May 2007, were recruited and then followed up to March 2017 at least once a year to observe the incidence of non-fatal stroke, and then the risk of non-fatal stroke was compared between the two groups. Results A total of 201 IGT and 259 NGT subjects were enrolled in present study. The average period of follow-up was 11.2 years. Univariate Cox proportional risk regression analysis indicated that the risk of type 2 diabetes and non-fatal stroke was higher in IGT group [(HR=2.18, 95%CI 1.54-3.08, P<0.001) and (HR=2.54, 95%CI 1.38-4.68, P=0.003)] than in NGT group. After adjusting the baseline age, body mass index (BMI), smoking, insulin resistance index (HOMA2-IR), estimated glomerular filtration rate (eGFR), history of hypertension and of dyslipidemia, multivariate Cox proportional risk regression analysis indicated that the risk of type 2 diabetes and non-fatal stroke was still higher in IGT group [(HR=2.25, 95%CI 1.58-3.20, P<0.001) and (HR=2.42, 95%CI 1.30-4.52, P=0.005)] than in NGT group. Conclusion IGT is closely related to non-fatal stroke in elderly male persons, and is an independent risk factor of non-fatal stroke.
4.Relationship between depressive degree and myocardial ischemia in elderly patients with coronary artery disease

Objective To research the relationship between the myocardiac ischemic activity and degree of depression in elderly patients.MethodsThe Center For Epidemiological Studies-Depressive Scale(CES-D) was administered to evaluate depressive degree for 121 patients who were defined as coronary artery diseased by coronary angiography (CAG); group A (n=64) for no depressive symptoms,group B (n=34) for minor depression, group C (n=23) for major depression were studied to observing the myocardial ischemia using Holter's recording and exercise testing.ResultsCAG showed that single vessel disease was more frequently in group A;single,double and three-vessel disease showed approximative proportion in group B,three-vessel disease was more frequently in group C.The times of ST-depress were (4.2±1.3),lasting time of ST-depress was (35.8±9.2) min,and index of ST-depress was (3.28±0.9) min/h,which were higher than those of group A [(1.8±0.3),(16.6±4.2) min,(1.76±0.4) min/h] and group C [(2.1±0.7),(17.8±5.8) min,(1.69±0.5)min/ h](P<0.05);There was significantly frequent myocardial ischemia in group B than in group A during exercise testing (χ2=3.274,P<0.05). patients in group C tended to have approximative incidence of ischemia compared with group A during daily living,but more incidence of ischemia than group B during exercise testing(χ2=5.473,P<0.05).ConclusionThe depressive degree was associated with severity of coronary artery disease,mild to moderate symptoms of depression are associated with an increased likelihood of myocardial ischemia during daily living and exercise,but patients with severity depression absence symptoms of myocardial ischemia during daily living,and exhibit markedly myocardial ischemia during exercise.

Peroxisome proliferation is a cellular response to many chemical compounds affects including natural and modified fatty acids, phthalate and adipate ester plasticizers, leukotriene antagonists, acetylsalicylic acid and certain pathophysiological conditions including dramatic change of cellular morphology and enzymatic activity. Peroxisome proliferation phenomenon is seen primarily in liver and kidney. Hormones and nutritional factor can regulate peroxisome proliferation response. Sustained peroxisome proliferation can lead to hepatocarcinogenesis. The three types of peroxisome proliferator activated receptor, termed PPAR alpha, PPAR beta, and PPAR gamma, expressed in specific tissue, are consisted of a specific a nuclear receptor superfamily. After more than 10 years world wide research, the function of PPAR is clarified, as PPAR gamma, the master of thrifty genes, controls the expression of genes relative to adipogenesis, diabetes mellitus and obesity. The receptor is involved in transcriptional control of numerous cellular processes including cell cycle control, inflammation, immunoregulation and carcinogenesis.

The chemical investigation on Ganoderma philippii led to the isolation of sixteen compounds by silica gel and Sephadex LH-20 column chromatography. 9.Coactivators in energy metabolism: peroxisome proliferator-activated receptor-gamma coactivator 1 family.

Peroxisome proliferator-activated receptor gamma coactivator 1 (PGC1) family is highly expressed in tissues with high energy metabolism. They coactivate transcription factors in regulating genes engaged in processes such as gluconeogenesis, adipose beta-oxydation, lipoprotein synthesis and secretion, mitochondrial biogenesis, and oxidative metabolism. Protein conformation studies demonstrated that they lack DNA binding domains and act as coactivators through physical interaction with transcription factors. PGC1 activity is regulated at transcription level or by multiple covalent chemical modifications such as phosphorylation, methylation and acetylation/deacetylation. Abnormal expression of PGC1 coactivators usually is closely correlated with diseases such as diabetes, obesity, hyperglycemia, hyperlipemia, and arterial and brain neuron necrosis diseases.

10.Carriage of drug resistance genes and homology of carbapenem-resistant Acinetobacter baumannii isolated from respiratory intensive care unit

Objective To investigate drug resistance genes and epidemic characteristics of β-lactamase carried by carbapenem-resistant Acinetobacter baumannii (CRAB) in the respiratory intensive care unit(RICU) in a hospital.Methods Clinically isolated CRAB from RICU patients in October-December 2015 were collected.Five drug resistance genes (KPC-2,IMP,VIM,NDM-1,OXA-23) were specifically amplified by polymerase chain reaction (PCR),amplified products were performed agarose gel electrophoresis and sequencing analysis,the homology was analyzed with pulsed-field gel electrophoresis (PFGE).Results A total of 22 CRAB strains were isolated in October-December 2015,19 (86.36%) of which were isolated from sputum.The resistance rate of 22 CRAB strains to compound sulfamethoxazole was 59.09 %,resistance rate to minocycline was 9.09 %,all were sensitive to polymyxin B,resistance rates to other antimicrobial agents were more than 80%.Three kinds of resistance genes KPC-2,IMP and NDM-1 were not found by PCR amplification,positive rates of VIM and OXA-23 were both 100%.PFGE homology analysis revealed that 22 strains were divided into 13 different types,each type contained 1-5 strains,9 types(69.23%) contained only 1 strain respectively,the other 4 types (30.77%) contained 2-5 strains.A5,A7,and A8;A9,A11,A14,A19 and A22;A4,A10 and A12;A16 and A18 were of the same type respectively.Conclusion The main types of β-lactamase-resistant genes of CRAB in RICU are VIM and OXA-23.Homology analysis shows a small parts are of the same clone strains,which reveals epidemic of a small scale.
