1.A case-control study on effects of irbesartan and amlodipino on insulin sensitivity in obese hypertensive pa-tients

Objective To observe the effects of irbesartan and amlodipine on insulin sensitivity in obese hypertensive patients. Methods 157 moderate hypertensive patients were divided into two greups,which received irbesartan or amlodipine for 16 weeks. The changes of blood pressure,heart rate and weight were observed. Insulin re-sistance was computed using the homeostasis model assessment (HOMA-IR). Results There was no difference in antihypertensive efficacy between the two groups(P>0.05). After 16 weeks of treatment ,the insulin resistance index decreased by 8.7±2.4 vs 6.1±2.6 (P<0.01) and body mass index decreased(28.1±3.8) kg/m2 vs (27.2± 3.2 ) kg/m2 (P<0.05 ) and mean heart rate raised by (73.1 ±10.0 ) kg/m2 vs ( 79.1±11.4) kg/m2 ( P < 0.01 ) in irbesartan group while no such changes were found in amlodipine treated group. Conclusions In obese hypertensive patients,irbsartan and amlodipine exert similar antihypertensive effect, but irbesartan may help to improve some obe-sity-related metabolize disorder,so the obese hypertensive patients may benefit more from irbsartan than from amlo-dipine.
2.Research and investigation in core profession value of nursing in modern China

Objective To discuss the core professional values of nursing(CPVN) in modern China and further promote the education of CPVN. Methods 622 clinical nurses and nursing students were re-cruited to participate the questionnaire survey to explore the understanding of CPVN. Results The top five CPVN were responsibility, accountability, aptitude, professionalism and ethics. Conclusions CPVN should be established based on the development of the times and national conditions in order to improve the overall level of nursing profession in China.
4.Psychosomatic Symptoms and Coping Styles and Their Influences on Quality of Life in Chronic Hepatitis B

Objective:To investigate the Psychosomatic symptoms and coping styles and their influences on quality of life in chronic Hepatitis B.Methods:The Chronic Hepatitis Disease Questionnair(CLDQ),Symptoms Checklist(SCL-90) and Medical Coping Modes Questionnair(MCMQ) were administered to 152 Chronic Hepatitis B.Results:The scores of SCL-90 of the patients were found to be different from the norm and the normal controls and negatively correlated with scores of CLDQ for all domains.The patients had a lower Confronce score and higher Avoidance and Acceptance-Resignation scores compared with the norm,and there was negative correlation between the scores of negative coping styles and the quality of life.Conclusion:The psychosomatic symptoms and the negative coping styles of chronic Hepatitis B can influence their quality of life.
6.Effect of Astragaloside on the Growth of Human Keloid Fibroblast

Objective:To study the inhibition of astragaloside on the proliferation of human keloid fibroblasts. Methods: Com-pared with that of normal skin, the expression of transforming growth factor-β( TGF-β) and its transduction factors Smad in the human keloid fibroblasts was detected. The optimal concentration was screened by MTT after HFF-1 human skin fibroblast was infected with astragaloside at different concentrations. The mRNA expression of Smad2, Smad3, Smad4 and Smad7 in the fibroblasts was studied by using real-time. The protein expression of TGF-βRⅡ, Smad2, Smad3, Smad4 and Smad7 in the fibroblasts was detected by using Western blot. Results: Compared with that of normal skin tissue, the expression of Smad protein was significantly increased ( P <0. 05) in the human keloid fibroblasts, and there was no significant difference in the TGF-βRⅡ expression (P>0. 05). The optimal concentration of astragaloside was 0. 5μg·ml-1 . The expression level of Smad2 protein in the two groups was significantly increased, and the level of Smad3 expression was significantly decreased (P<0. 05). Conclusion:Astragaloside can inhibit the formation of fi-broblast possibly through Smad2 over-expression and Smad3 inhibition in the TGF-β/Smad signal transduction pathway.
8.Study on Distribution and Drug Resistance of Pathogenic Bacteria in Children with Urinary Tract Infection

Objective To investigate the distribution and drug resistance of pathogenic bacteria in children with urinary tract infection.Methods All data were analyzed retrospectively.Middle segment urine samples from 573 outpatients and inpatients in children were collec-ted,cultured and identified for pathogenic bacteria by the way of ATB apparatus during Jan.2005-Dec.2007,and the drug resistance of positively cultured bacteria was tested with disc agar diffusion method.Extended spectrum ?-lactamases(ESBLs) were determined by phenotypic comfirmatory test according to the criteria of National Committee for Clinical Laboratory(NCCLS),and the identifications of methicillin-resistant staphylococcus aureus(MRS) and high level aminoglycoside resistance(HLAR) were carried out by the methods recommended from NCCLS too.Results Four hundred and eighty-two pathogenic strains were isolated from urine culture of children during 3 years,in which 385 strains(79.9%) were gram-negative(G-) and 97 strains(20.1%) were gram-positve(G+).The primary G-bacterium was Escherichia coli which accounted for 41.1%(198 strains),and the primary G+ bacterium was Enterococcus which accounted for 14.7%(71 strains).All the strains of Escherichia coli and klebsiella pneumoniae were sensitive to imipenem.Their resistance rates to sulperazone,amikacin and nitrofurantoin were less than 9.7%,while the rates to piperacillin,ceftazidime,ciprofloxacin and trimethoprim-sulfamethoxazole were more than 50%.ESBLs-producing strains accounted for 59.2% in Escherichia coli and 52.7% in klebsiella pneumoniae.Pseudomonas aeruginosa showed high multi-drug resistance.All the Enterococcus strains were sensitive to vancomycin.The resistance rate to nitrofurantoin was less than 8.1%.The detection rate of HLAR was 26.1%.All the Staphylococcus strains were sensitive to vancomycin.The resistance rate to nitrofurantoin and rifampicin were less than 22.4%.The detection rate of MRS was 52.7%.Conclusions The primary pathogenic bacterium in children with urinary tract infection is G-bacteria.The Escherichia coli is the first,the Enterococcus is the second and the klebsiella pneu-moniae is the third.All the pathogenic strains show high drug resistance to antibiotics in common use,therefore,clinicians should attach importance to the results from bacteria culture and susceptibility test,in order to obtian reference for accurate clinical diagnosis and rational use of antibiotics.
9.Experiment of turning scleral flap of self-body over drainage in trabeculectomy of glaucoma

Objective To investigate the effect of turning scleral flap of self-body over drainage in trabeculectomy.Methods The surgery was performed on one eye of a rabbit while the other eye of the same rabbit was conducted with the routine trabeculectomy as the control group. The filtering bleb, intraocular pressure and pathology were observed. Results The intraocular pressure in the experimental eye was much lower than that in the control group 3~12 weeks after the operations (P<0.05).As it was within 2 weeks after the surgeries, but there was no significant filtering difference (P>0.05).The rate of eyes with functional bleb in the experimental group was significantly higher than that in the control group at the postoperative 2~7 weeks .Conclusion This study suggests the turning of self-body over drainage surgery can help to maintain filtering function for a long time and to prevent complication after filtering surgery.
10.Inhibitive Effects of RNA Interference on Caspase-3 Gene Expression

Objective To investigate the effects of RNA interference (RNAi) on Caspase-3 expression.Methods According to the gene sequence and the secondary structure characteristics of Caspase-3, three siRNA (Si-Caspase-3-1, Si-Caspase-3-2 and Si-Caspase-3-3) were designed with 21 basic group, and siRNAs against Caspase-3 gene were synthesized. With the liposome conduct, the synthesized siRNAs were transferred into neuroglia cells. After the cells crept the slices, cells were dyed with fluor Hoechst33258; the apoptotic cells were stained with TUNEL, the normal neuroglia cells were as control, the Caspase-3 protease activity were detected with hemi-quantitative RT-PCR and Western blot.Results The synthesized siRNAs could inhibit the expression of Caspase-3 gene. For Si-Caspase-3-3, the expression inhibition rate of Caspase-3 mRNA was 86.32% in neuroglia cell.Conclusion The synthesized siRNAs can inhibit the expression of Caspase-3 gene at cell level.
