Background: Femoral shaft fracture, in company with the development of morden society and the increase in means of transport, is increasing in the number of case and the complicated degree of injury. Objectives: 1. To note indication for interlocking intramedullary SIGN nailing in femoral shaft fracture in adult patients. 2. To assess result of interlocking intramedullary SIGN nailing in treatment of femoral shaft fracture in adult patients. Subjects and method: A prospective, descriptive study was conducted in the 40 patients between the ages of 17 and 80 who were dignosed femoral shaft fracture caused by injury and operated for bone union by interlocking intramedullary SIGN nailing at Viet Duc hospital, from April/2004 to July/2005. Results: In this study: 5 patients with I degree fracture (12.5%), 20 with II degree (25%) and most of patients with III and IV degree fracture (62.5%), of which 14 patients with IV degree (35%). Because the SIGN nailing\u2019s structure was solid, so it had a good capacity for patient\u2019s weight. 4 weeks after operation, 85% patients were able to move around without crutches. There was no complications. Conclusion: SIGN nailing enables surgeons to drive interlocking intramedullary nailing for long bone without image intensifier. It also enables both surgeons and patients to appoarch modern, high effective methods without needing expensive machines.

OBJECTIVE: To evaluate the analgesic effect and application advantage of acupuncture combined with local anesthesia of lidocaine in percutaneous vertebroplasty (PVP) for the patients with osteoporotic vertebral compression fracture (OVCF). METHODS: A total of 60 patients with OVCF and receiving PVP at single vertebra under local anesthesia were selected and randomized into an acupuncture plus medication group and a simple medication group, 30 cases in each one. In the simple medication group, the local laying infiltration anesthesia with 1% lidocaine 30 mL was used. In the acupuncture plus medication group, firstly, filiform needles were used to stimulate Hegu (LI 4), Neiguan (PC 6), Jinmen (BL 63) and Yintang (GV 29) with reducing technique, and then the epidermal infiltration anesthesia was followed with 1% lidocaine 4 mL. The needles were retained till the end of operation. Successively, before operation (T RESULTS: In the acupuncture plus medication group, MAP and HR were lower than those in the simple medication group at T CONCLUSION Acupuncture combined with medication reduces the dose and adverse reactions of anesthetics, alleviates pain degree of patients, shortens the duration of operation and improves patients' subjective satisfaction in PVP for OVCF.

Fracture nonunion is the cessation of a normal reparative process of fracture healing. With an incidence of 2.5%-4.4%, it is one of difficult problem for orthopedics surgeons. Definition, classification, treatment and causes of fracture nonunion are reviewed in this article. The therapeutic principles and methods are introduced systematically combined with causes of fracture nonunion.

When compared to adults, pediatric fractures unite readily and nonunion is quite rare. Nonunion is often associated with open fractures, operative interventions, infection, pediatric osteogenesis imperfecta and neurofibromatosis. There are only a few studies and reports mentioning nonunion following conservative ma- nagement of closed pediatric fractures. We report here a case of an eighteen-month-old child who developed nonunion following treatment of fracture of both forearm bones with cast and was successfully treated with plating. To the best of our knowledge, this is the youngest reported case of nonunion following conservative management of closed diaphyseal pediatric fracture.

The forearm distal bone fracture is the most com-mon fracture in children. Scaphoid frac-ture is rare because the scaphoid is largely cartilaginous in children. Some cases associating both distal radius and scaphoid fractures were reported in the literature. Scaphoid fracture often occurs without displacement and can be easily overlooked. We reported a case of children wrist injury combining scaphoid fracture with ipsilateral distal fracture.
