ObjectiveTo observe the effect of nerve block combined with sequential analgesia in peri-operative knee arthroscopy.Methods Two hundred patients undergoing knee arthroscopy,were performed lumbar plexus and sciatic nerve block anesthesia by nerve stimulator with ultrasound guidance.Celecoxib was combined for sequential analgesia.The vital signs,the effect of anesthesia in the surgery,the duration of postoperative analgesia,and patient's satisfaction to analgesia were observed to evaluate the clinical effects.ResultsThe vital signs of the patients in the surgery were stable,the effect of anesthesia was good,180 cases of excellent,16 cases of good,4 cases of poor.The duration of postoperative analgesia was 6-10 (8.2 ± 0.5) h.Combined with sequential analgesia program,there was high patient's satisfaction [ excellent 75％ (150/200),good 10％ (20/200),general 10％ (20/200),poor 5％ ( 10/200 ) ].Conclusion The nerve block combined with sequential analgesia is safe and effective in knee arthroscopy,it achieves high patient's satisfaction,this maybe a routine in knee arthroscopy.

Purpose: The prevalence of frailty among patients with heart failure is about 45%. Frailty may result in patients' functional decline, falls, disability, and decreased quality of life. Qualitative studies can explore older patients' perceptions of frailty and help patients cope with it. However, a qualitative approach that explores the experience of frailty in older patients living with heart failure is lacking. This study aimed to explore the lived experience of frailty in older patients with heart failure. Methods: This qualitative study applies Giorgi's phenomenological method. Data were collected from October 2019 to August 2020. Thirteen older patients with heart failure aged at least 60 years were recruited using purposive sampling from a medical center in Taiwan. The participants participated in an in-depth interview using a semistructured interview guide. Results: Seven themes were identified: “being reborn at the end of the road but having difficulty recovering”, “living with a disease with an ineffable feeling”, “feeling like being drained: physical weakness and a dysfunctional body”, “struggling with impaired physical mobility and facing unexpected events”, “suffering from mental exhaustion”, “receiving care from loved ones”, and “turning over a new leaf”. Conclusions Frailty in older patients with heart failure was obscure and difficult to describe. Frailty could be improved by medical intervention, self-management, and social support but was difficult to reverse. Patients with heart failure should be evaluated for frailty using multidimensional assessment tools at first diagnosis and provided frailty-related information so that patients have proper insight into their disease as early as possible.

Objective To study the affect of psychological intervention to anxiety-depression emotion and T Lvmlcih cells in breast cancer patients during the peri-operation. Methods We divided 60 cases into two groups at random. the experiment group(30) and the control group(30).The psychological situation Was evaluated according to the scores of Self-Rating Anxiety Status Inventory(SAS)and Depression Status Inventory (SDS).2 ml of venous blood Was taken to measure the level of T lymph cells. Then the experiment group Was given the specific and generalized psychological interventions. while the control group was given only the common nursing care. Results The scores of anxiety(8.65±3.69)and depression(31.70±5.79)after the intervention in the experiment group were lower than those before the intervention。and had significant difference (P<0.05).After the operation，the average value of CD3，CD4，CD4／CD8 in the experiment group was higber than that of the control group，and the values of CD3 and CD4 had statistical meaning(P<0.05).After the intervention，the vatues of CD3,CD4，CD4／CD8 of the experiment group were higher than those before the intervention. and the values of CD3 and CD4 had obvious difference(P<0.05).Conclusions In this study, a suitable and effective psychological intervention can improve the status of anxiety and depression of patients with breast cancer during the peri-operative period, and at the same time can modulate the immune status.

INTRODUCTION: Cigarette smoking is a leading cause of morbidity and mortality, and has a deleterious effect on dermatological conditions, such as skin cancers, hidradenitis suppurativa and psoriasis. The study aimed to evaluate the efficacy of a pharmacist-led smoking cessation clinic in reducing cigarette smoking at a tertiary referral dermatology centre. We described the impact of this clinic to provide guidance on how such a model could be further improved and implemented more widely. METHODS: In this single-centre, retrospective study, 74 currently smoking patients who received counselling at a structured smoking cessation clinic between January 2010 and March 2013 were identified. Information on baseline demographic characteristics and detailed past medical history, including smoking history, was collected. Follow-up was conducted at two weeks and three months. RESULTS: At the first follow-up at two weeks, which was attended by 57 patients, 9 (15.8%) had stopped smoking and 26 (45.6%) showed reduction in the number of cigarette sticks smoked per day, with an average reduction of 4.1 cigarette sticks per day. However, a few patients also reported no change or increased number of cigarette sticks smoked per day following counselling. CONCLUSION A structured pharmacist-led smoking cessation clinic is effective and can be made a part of the holistic management of dermatological conditions.

INTRODUCTION: Postgraduate medical education in Singapore underwent a major transition recently, from a British-style system and accreditation to a competency-based residency programme modelled after the American system. We aimed to identify the relative importance of factors influencing the choice of sponsoring institution (SI) for residency among medical students during this transition period. METHODS: A questionnaire-based cross-sectional study of Singapore undergraduate medical students across all years of study was performed in 2011. Participants rated the degree of importance of 45 factors (including research, academia and education, marketing, reputation of faculty, working conditions, posting experience and influence by peers/seniors) to their choice of SIs on a five-point Likert scale. Differences in gender and seniority were compared. RESULTS: 705 out of 1,274 students completed the survey (response rate 55.3%). The top five influencing factors were guidance by mentor (4.48 ± 0.74), reputation for good teaching (4.46 ± 0.76), personal overall experience in SI (4.41 ± 0.88), quality of mentorship and supervision (4.41 ± 0.75), and quality and quantity of teaching (4.37 ± 0.78). The five lowest-rated factors were social networking (2.91 ± 1.00), SI security (3.01 ± 1.07), open house impact (3.15 ± 0.96), advertising paraphernalia (3.17 ± 0.95) and research publications (3.21 ± 1.00). Female students attributed more importance to security and a positive working environment. Preclinical students rated research and marketing aspects more highly, while clinical students valued a positive working environment more. CONCLUSION Quality of education, mentorship, experiences during clerkship and a positive working environment were the most important factors influencing the choice of SI.
