Epidural steroid injections (ESI) are commonly used for managing radicular pain. They are generally considered to be a safe treatment with a low incidence of complications, which may be very mild and transient symptoms. We describe here one case of a delayed onset of hiccups, facial flushing and hypertension associated with a lumbar epidural steroid injection.

It is the fact that parenteral antobiotics have no place in the treatment of the discitis after postoperative state because the antobiotics can not reach the focus of infection on account of the avascular zone of the intervetebral space. The author treats more extensive discitis to place an indwelling rubber catheter into the cavity to a contunuous irrigation with normal saline solution containing antobiotics. The catheter is left are as following ; 1. It is to effect the continuous direct administration of the antobiotics. 2. It is a mechnical effect by flushing out necrotic particles. 3. It is less recurrent infection. 4. The method is very simple, inexpensive. 5. The treated period is very short in contrast with immobilization and systemic antibacterial administration.

The purpose of this study was to evaluate various methods of decontamination of ultrasonic scalers, high-speed handpieces and air-water syringes in dental equipments. Eimination of possible sources of microbial infection in dental operatories should be of primary importance. Microbial contamination levels of high speed handpieces, air-water syringes and ultrasonic scalers of 11 dental units in Seoul National University Hospital were evaluated after flushing the lines, alcohol sponge rubbing, or soaking in 0.1% chlorhexidine for 1,3 and 5 minutes. The result suggests that flushing the lines or soaking the tips in 0.1% chlorhexidine before use of the water systems may reduce the microbial levels. Soaking in 0.1% chlorhexidine for 5 minutes was most effective in reducing bacterial contamination.

Retinoscopy was performed on 90 esotropic children (180 eyes) younger than 6 years with cyclopentolate-homatropine and atropine. This procedure involves the instillation of a combination of cyclopentolate 1% and homatropine 5% on the initial visit, (ollowed by the instillation of atropine 1% two to four weeks later. In group 1, the parents administered atropine to both eyes of the patient three times a day for 3 days prior to the refraction, and for group 2, atropine was administered twice a day alternately to both eyes for 5 days. Atropine revealed +0.74 diopters more hyperopia than cyclopentolate-homatropine in group 1, and +0.68 diopters more in group 2, No statistically significant difference in cycloplegic effect of atropine was found between the two groups. After use of the atropine, side effect of facial flushing occurred in 15 patients (30%) in group 1, and in 5 patients (12%) in group 2. This difference is statistically significant (p<0.05). The results imply that in clinical practice 5 days of alternate twice-daily instillation can be substituted for conventional "full" atropinization for 3 days.

Thirty patient with essential hypertension were administered Manidipine, a new calcium antagonist, 10~20mg once daily to evaluate the hypotensive efficacy and safety for 8 weeks. And the followings were the result. 1) Patients were consists of 14 male, 16 female, aged 53 in average and classified as mild in 21 and moderate in 9 patients. 2) Optimum intial dose was 10mg and 10 to 20mg were the doses recommended. 3) Blood pressure dropped after 8 weeks 24/13mmHg in average, rewarding 80% effectiveness and normalized in 87%. 4) Most frequent side reaction was facial flushing in 5 patiens followed by palpitation and dizziness, all of which did not disturb the continuation of medication. 5) Most of routine laboratory parameter were normal and unchanged between before and after the trial. 6) Overall rating of usefulness was 77%. In conclusion, Manidipine 10 to 20mg once daily regimen is well tolerated and effective in the treatment of mild to moderate essential hypertension.

PURPOSE: Tadalafil is a phosphodiesterase type 5 inhibitor that is used for the treatment of erectile dysfunction (ED). Previous clinical trials have assessed its efficacy and safety in Western populations, but this drug has not been investigated in a large clinical trial involving Korean men with ED. Thus, the aim of this study was to assess the efficacy and safety of 20 mg tadalafil in comparison to placebo when it is taken on demand by Korean men suffering with ED over a study period of 12 weeks. MATERIALS AND METHODS: Men more than 18 years of age with mild to severe ED of various etiologies were randomized to receive placebo or tadalafil 20 mg that was taken as needed (maximum once daily). Efficacy assessments included the International Index of Erectile Function (IIEF), the Sexual Encounter Profile (SEP) diary and Global Assessment Questions (GAQ). RESULTS: Tadalafil significantly improved erectile function, as measured by the erectile function domain of the IIEF, compared to placebo (p<0.001). At the endpoint, the patients receiving tadalafil 20mg reported a greater mean per-patient percentage of successful intercourse attempts (SEP3: 71% compared to 31% for placebo) and a greater proportion of improved erections (GAQ: 80% compared to 44%). The most common treatment emergent adverse events were headache (16.3%), flushing (5%) and eye pain (5%), and most of the adverse events were mild or moderate in severity. CONCLUSIONS: Tadalafil was an effective, well-tolerated therapy for Korean men suffering with ED of broad-spectrum severity and etiology.
