1.Relationship between hepatitis G virus infection and hepatic failure.

OBJECTIVETo study the pathogenic effect of hepatitis G virus (HGV) infection on hepatic failure.
METHODSUsing the RT-PCR and EIA techniques to detect HGV RNA and anti-HGV in sera of hepatic failure patients and compare them with their liver function and mortality rates.
RESULTSThere was no significant difference about the positive rates of HGV among acute hepatic failure, subacute hepatic failure and chronic hepatic failure groups (X(2)=2.54, P>0.05). The level of ALT in HGV-positive group was slightly lower than that in HGV-negative group. The concentration of bilirubin and globulin was higher in HGV-positive group than HGV-negative group, and the concentration of albumin in HGV-positive group was significantly lower than that in HGV-negative group (t=2.59, P<0.05). The mortality rate in HGV-positive group was significantly lower than that in HGV-negative group (X(2)=4.68, 0.01
CONCLUSIONSThe virulence of HGV is mild, and the HGV infection does not aggravate hepatic failure.

2.Immunization for International Travelers.

In 2006, the annual number of Koreans traveling internationally increased to 11 million. With this increased number of Koreans traveling to developing countries, various infectious diseases have been reported to be imported into Korea. Immunization is an important tool for the prevention of such diseases. Vaccination offered to international travelers is divided into 3 categories, i.e., obligatory vaccination, vaccines against diseases with an increased risk in developing countries, and routine vaccination. Vaccination that is obligatory for travelers includes yellow fever vaccine for travelers entering yellow fever-endemic areas and meningococcal vaccination for pilgrims traveling to Saudi Arabia. Recent reports of the adverse event following yellow fever vaccination, i.e., vaccine-associated viscerotropic disease, pose a caution in the administration of yellow fever vaccine to the elderly or individuals with thymic diseases. Protein-conjugated meningococcal vaccines are generally preferred; however, polysaccharide vaccines are also effective in short-term travelers. The vaccinations recommended for the health of travelers include administration of hepatitis A (HAV), meningococcal, typhoid, rabies, tick-borne encephalitis, and plague vaccines. As many young Korean adults lack immunity against hepatitis A, the administration of the HAV vaccine is recommended for all Korean individuals in their 20s and all Korean seronegative individuals in their 30s. The risk of acquiring typhoid fever is high among travelers traveling to the rural areas of developing countries for 2 weeks or more. The rabies vaccine may be recommended for travelers traveling for longer than 3 months and animal handlers. Both tick-borne encephalitis and plague are rarely encountered among Korean travelers; furthermore, vaccines for these diseases are not available in Korea. As Japanese encephalitis is endemic in Korea, vaccination against encephalitis is not essential for Korean adults. Pre-travel counseling is an important opportunity for catch-up vaccination of healthy travelers who missed routine vaccinations.

3.Relationship between amount of HBV DNA in serum/liver tissue and hepatitis G virus (HGV) infection in patients with chronic hepatitis B.

OBJECTIVETo observe the relationship between the amount of HBV DNA in serum/liver tissue and HGV infection in patients with chronic hepatitis B (CH-B) for exploring the effect of HGV infection on hepatitis B virus (HBV) replication of CH-B.
METHODSHGV RNA in serum, HGV nonstructural region 5 (NS5) antigen (HGV Ag) in liver tissue and the amount of HBV DNA in serum, liver tissue were detected for 56 patients with CH-B by reverse transcription-polymerase chain reaction (RT-PCR) assay, peroxidase antiperoxidase (PAP) immunohistochemical method and fluorescence quantitative PCR assay, respectively. Then the relationship between HGV Ag expression in liver tissue and HGV RNA expression in serum was analysed and the amount of HBV DNA in serum and liver tissues from the serum HGV RNA or liver tissue HGV Ag positive patients were compared with those of the serum HGV-RNA or liver tissue HGV Ag negative patients, respectively.
RESULTSTen (17.9%) and eight (14.3%) patients were positive for serum and liver tissues,respectively.HGV RNA expression in serum was closely related to HGV Ag expression in liver tissues, but there was HGV RNA in serum from some of the liver tissues HGV Ag negative patients ?cases of HGV RNA and HGV Ag positive or negative,HGV RNA positive but HGV Ag negative, HGV RNA negative but HGV Ag positive, respectively: 5,43,5,3,(P<0.01). There was no significant difference in the amount of HBV DNA in serum and liver tissues between HGV RNA or HGV Ag positive and negative patients (P>0.05).
CONCLUSIONSHGV infection may not affect HBV replication. Liver is the site of HGV replication, but HGV probably also replicates in extrahepatic tissues. HGV hepatic pathogenicity is probably mild and further studies are still needed.

4.Zika Virus Infection.

Zika virus was first isolated in from nonhuman primate in 1947. It is in the genus Flavivirus, closely related to other flavivirus like Dengue, West Nile, Yellow fever and Japanese encephalitis virus. Since 2007 epidemic in Yap island, zika virus infections had spread to the countries in Micronesia and South Pacific. In 2015, Zika virus outbreak occurred in Brazil and now more than 40 countries in American continents reported autochthonous infection. The virus is transmitted mainly by Ae. aegypti mosquito with many other Aedes mosquito species known as vector. Recently, Zika virus infection is known to cause severe neurological complications and congenital malformation. In this paper, we will review current knowledge on Zika virus history, biology, clinical characteristics and preventive method.

5.Seroprevalence of Dengue Virus Antibody in Korea

PURPOSE: The number of dengue fever cases is rising due to increasing overseas travel. Vaccination makes severe dengue fever in seronegative individuals after vaccination when they exposure to wild-type dengue virus. We investigated the seroepidemiology of the dengue virus for monitoring of Korean dengue virus immunity and establishing the prevention of dengue infection. METHODS: The study was based on 446 residual sera collected from 98 infants (2 months to 1 year old), 152 adolescents (13 to 19 years old), 90 adults (20 to 50 years old), and 106 elderly participants (more than 65 years old) for other studies. Antibody levels for dengue virus immunoglobulin G (IgG) in each age group were measured using an enzyme-linked immunosorbent assay (ELISA). For each dengue virus IgG positive or equivocal result, an IgG ELISA was performed for Japanese encephalitis virus. RESULTS: Of the 446 serum samples, only 1 (0.2%) adolescent had a positive result from the dengue IgG antibody test. In the dengue virus IgG antibody test, 14 (3.1%) samples showed equivocal results (10 adolescents and 4 elderly). In the 1 positive case of dengue virus IgG, the Japanese encephalitis IgG test was also positive. In the 14 equivocal cases of dengue virus IgG, there were 6 positive, 3 equivocal, and 5 negative of Japanese encephalitis IgG. CONCLUSIONS: The seroprevalence rate of dengue virus was very low in Koreans. This study provides important data for establishing the policy for preventive measures of dengue fever. It will be necessary to continuously monitor for dengue virus immunity.

6.The situation of dengue haemorrhagic fever and Japanese encephalitis in the first half of 1999 in Kien Giang province.

There were 714 cases of dengue haemorrhagic fever and 1 death. The age of patients from 2-10 years old, none of them over 15 years old. The morbidity of III, IV degrees concentrated in Hon Dat, Tan Hiep, An Ninh, Rach Gia and also the districts that had the greatest morbidity, 56 cases of shock. The Steering Board issued a number of guideline documents and established preventive plan right at the beginning of the year with medical speciality. There were 28 cases of Japanese encephalitis, 25 cases with the age from 13 months - 5 years old, 3 cases with the age from 6-11 years old, 64% male and 36% female. The Pasteurs Institute of Ho Chi Minh City coordinated with the local authority to control the disease transmission in order to isolate virus and propagate measures of mosquito killing and hygienic methods around living area.

7.Seroepidemiologic survey of hepatitis G virus in selected population of Shanxi, Qinghai and Xinjiang Provinces (region) of China.

OBJECTIVETo observe the serological features of hepatitis G virus (HGV or GBV) in selected population of Shaanxi, Qinghai and Xinjiang Provinces (region).
METHODSEnzyme-linked immunosorbent assay (ELISA) was used to determine IgG antibody against GBV (anti-GBV IgG) in serum specimens of 1469 individuals from the 3 provinces (region).
RESULTSThe positivity rate of serum GBV-IgG antibody in ethnic minorities (4.11% in Tibetan, 5.36% in Mongolian, 4.55% in Uigur, 4.00% in Hui population) was slightly higher than that in Han population (1.36%-1.73%), but the differences were not significant (P>0.05). The positivity rate of serum GBV-IgG antibody in drug abusers (11.30%, 34/301) was remarkably higher than that of the normal population (2.44%,18/736) (P<0.01). The positivity rate of serum GBV-IgG antibody in blood donors was 1.02%-7.68%.
CONCLUSIONSThe positivity rates of serum GBV-IgG antibody among ethnic groups in the 3 provinces (region) had no significant differences; blood-borne transmission seemed to be an important transmission route of GBV, therefore supervision of blood donors and drug abusers should be intensified.

Several research groups have recently reported that persistent GB virus C (GBV-C) co-infected with human immunodeficiency virus (HIV) leads to slower AIDSs disease progression than HIV-1 infection alone. However, these findings were not confirmed by several other studies. To investigate the association between GBV-C replication and plasma HIV loads and CD4+ T cell counts, 203 HIV-1 positive former blood/plasma donors(FBDs) were enrolled from Fuyang city of Anhui Province in China. Plasma specimens were collected from them and were tested for GBV-C using RT-PCR and ELISA. Out of 203 specimens, 52 (25.6%) cases were positive for GBV-C, including 35 male (67.3%) and 17 female (32.7%) cases. No significant association was identified between GBV-C infection and CD4+ T-cell counts or between GBV-C infection and HIV viral loads. Since all the subjects studied were naive to ART, the influence of therapy on AIDS disease progression was ruled out in this study. Overall, our data indicated that HIV-1 positive male FBDs were prone to be infected, GBV-C coinfection with HIV-1 does not significantly influence HIV/AIDS disease progression during the late stage of chronic HIV-1 infection.

9.Advanced understandings for Zika virus.

Zika virus (ZIKV) is an arthropod-borne member of the genus Flavivirus, closely related to the dengue, West Nile, Japanese encephalitis, and yellow fever viruses and is transmitted by Aedes spp. mosquitoes. It has emerged explosively since 2007 to cause a series of epidemics in Micronesia, the South Pacific, and most recently the Americas. Following the first detection of ZIKV on the American continent, autochthonous ZIKV transmission has been confirmed throughout Central and South America. The unprecedented numbers of people infected during recent outbreaks in the South Pacific and the Americas may have resulted in enough ZIKV infections to notice patterns of the associated incidence of congenital microcephaly, Gillain-Barre symdrome, and acute diffuse encephalomyelitis. Here we review the history, emergence, biology, transmission, and control strategies for the ongoing outbreak through vector-centric approaches on ZIKV to date.

10.Analysis of the Three Dimensional Structure of Envelope Protein of the Japnes Encephalitis virus Isolated in Korea.

Three dimensional structures of envelope protein from Korean isolates and Nakayama-NIH strain of Japanese encephalitis virus (JEV) were deduced by a computer program (HyperChem 4.0 Chemplus 1.0) based on the data of the three dimentional structure of Tick-borne encephalitis virus. In the three dimensional structure of envelope protein, neutralizing epitope and T-helper cell recognition site of C-terminal region of Korean isolates were structually similar to those of Nakayama-NIH but the N-terminal region was not. Korean JE isolates were compared with Nakayama-NIH strain by using cross-neutralization antibody test. Neutralizing activities of Korean isolates derived from guinea pigs were higher than those of Nakayama-NIH strain against Korean isolates, although the polyclonal antibody titers of Nakayama-NIH showed 1:160 to 1:640 against Korean isolates. According to the results from three dimentional structures and cross-neutralization analyses, the antigenic difference between Korean JE isolates and Nakayama-NIH strain may be dependent on structural difference of envelope protein.
