1.Application of thromboelastography in perioperative evaluation of benign prostatic hyperplasia patients treated with anticoagulants

Objective To explore the application value of thromboelastography(TEG)in perioperative evaluation of benign pros-tatic hyperplasia(BPH) patients treated with anticoagulants .Methods To retrospective analyze the intraoperative bleeding ,postop-erative bleeding and transfusion treatment of 120 patients with BPH surgery ,who were treated with anticoagulants .The results of TEG detection and coagulation function test were compared .Results There was no patients with significant intraoperative bleed-ing ,including 40 cases of bleeding after surgery ,80 cases of patients with no bleeding .In patients of bleeding ,there were 32 benign prostatic hyperplasia patients(80 .00% )with low coagulation function detected by TEG detection ,and 18(45 .00% ) with low coagu-lation function detected by routine coagulation function test .The difference of detection rates was statistical significant between the two methods(χ2 =12 .709 ,P=0 .002) .In patients of non-bleeding ,there were 76 benign prostatic hyperplasia patients(95 .00% ) with non-low coagulation function detected by TEG detection ,and 67(83 .75% )with non-low coagulation function detected by rou-tine coagulation function test .The difference in detection rate was significant between the two methods(χ2 =6 .679 ,P=0 .022) . Conclusion TEG could effectively evaluated postoperative bleeding in BPH patients ,guide postoperative transfusion therapy ,which is better than routine coagulation function test .
2.The false-negative reasons of positioning test in benign paroxysmal positional vertigo.

OBJECTIVE:
To analyze the false-negative proportion and reasons of positioning test in benign paroxysmal positional vertigo (BPPV).
METHOD:
Sixty-eight cases were reviewed and analyzed during February to April 2009. All patients had typical clinical history of BPPV and diagnosed by positioning test,and all were treated with PRM successfully. If positioning test at the first time were negative, examinations repeated after either head shaking or return visits.
RESULT:
Thirteen patients (19.12%) appeared false-negative in positoning test, and 11 cases showed typical nystagmus after head shaking,while 2 cases were diagnosed by return visitis the next day. There were no statistically difference between false-negative group and positive group in age (P > 0.05), gender (P > 0.05), course of disease (P > 0.05) and types of semi-circular canal (P > 0.05).
CONCLUSION
Re-examinations after head shaking or return visits are useful for the diagnosis in the suspected BPPV patients who have negative positioning test.

3.Efficacy comparison between endolymphatic sac surgery and semicircular canal occlusion in the treatment of stage 4 Ménière disease.

OBJECTIVE:
To compare the vertigo controlling situation between the endolymphatic sac decompression(ELSD) and semicircular canal occlusion (SCO) in stage 4 Ménière disease.
METHOD:
Fourteen patients who underwent endolymphatic sac decompression and 9 patients who underwent semicircular canal occlusion from 2009 to 2013 were followed. All patients has complete preoperative examination and postoperative follow-up.
RESULT:
The vetigo control of the patients underwent endolymphatic sac decompression: completely control 35.7%; basic control 28.6%; partly control 14.3%; the vetigo control of the patients underwent semicircular canal occlusion: completely control 88.9%; basic control 11.1%.
CONCLUSION
ELSD and SCO are alternative methods for the patients of stage 4. SCO has a much highter complete vertigo control rate. For the patients without practical listening, SCO is a better choice.

4.Optimization of routine trizol extraction method for promoting the production of serum microRNA

Objective To investigate which methods can promote the serum microRNA extract production.Methods The sera of healthy persons were treated with 10% SDS (sodium dodecyl sulfate) and/or sonication at first,then extracted miRNA by routine Trizol method,or the serum miRNA was just extracted by routine Trizol method.The contents of serum miRNA-218 and miRNA-346 were detected by quantitative reverse transcription polymerase chain reaction (qRT-PCR) technique,and qRT-PCR relative quantities among different groups were compared to evaluate influence of different methods on microRNA extract efficacy.Results Compared to routine Trizol extraction method,the qRT-PCR relative quantities of miRNA-218 and miRNA-346 were improved when serum was treated with 10% SDS and/or sonication at first (P <0.05),which was most obviously improved when treated with 10% SDS combined with sonieation.Conclusions The method that serum was treated with SDS incubation and sonication at first can promote sensitivity of serum microRNA detection.
5.Postoperative delirium in elderly patients after gastrointestinal surgery

Objective To study postoperative delirium in elderly patients.Methods We investigate the morbidity of postoperative delirium in 142 elderly patients (≥ 60 years)after gastrointestinal surgery by using Confusion Assessment Method (CAM) and Delirium Rating Scale Revised-98 (DRS-R98) scores.Data were analyzed using Student's t test and Chi-squaretest respectively with SPSS 19.0.Results Of 142 patients,delirium was diagnosed in 36 patients(25.4%),delirium developed in 4,7,17,7,1 patients in posto perative 1,2,3,4-7,7 + days respectively.There were significant difference in hospital stay:17.7 ± 2.6 days (postoperative delirium) and 13.4 ± 2.3 days (no postoperative delirium),t =4.608,P =0.000 1.The postoperative complications (52.8% / 23.6%,x2 =10.710,P =0.001) and ICU admission (22.2%/6.6%,x2 =6.939,P =0.008) significantly increased.Conclusions Postoperative delirium is recognized as one of the most common surgical complications in elderly patients with gastrointestinal surgery leading to other major postoperative complications,and prolonged hospitalization.
6.Therapeutic effect of acupoint catgut embedding therapy on transient cer-ebral perfusion in patients with transient ischemic attacks

Objective To discuss the effect of acupoint catgut embedding therapy on transient cerebral perfusion in patients with transient ischemic attacks (TIA).Methods 67 patients with TIA were ran-domly divided into treatment group and control group.The patients of the control group took orally aspirin enteric-coated tablets 100 mg per night,in addition,those of treatment group underwent acupoint catgut embedding therapy with the embedding points of Fēngchí(GB 20),Dàzhuī(GV 14),Fēnglóng (ST 40).In order to understand the cerebral blood flow perfusion and curative effect,CT perfusion imaging was performed before and one month after treatment.Results After one month of treatment,the efficacy of the treatment group with effective rate of 91.18 % (remarkable effect 18 cases,effect 13 cases and in-efficacy 3 cases)was significantly better than that of the control group with effective rate of 63.64 % (re-markable effect 10 cases,effect 11 cases,and inefficacy 12 cases).And the improvement of cerebral is-chemia in CTPI imaging was better than that in the control group (P <0.01).Conclusion The positive therapeutic effect of acupoint catgut embedding therapy has been proved on TIA,which can significantly improve the patients with cerebral perfusion.

Nonalcoholic fatty liver disease (NAFLD) is a pathological manifestation of obesity and metabolic syndrome involving the liver.It can cause disability and mortality related to liver disease,and it is closely associated with the high incidence rates of type 2 diabetes and cardiovascular diseases.At present,NAFLD has become the most common chronic liver disease worldwide,and the development and implementation of the clinical guideline for the diagnosis and treatment of NAFLD help to improve patients' prognosis.This article provides an interpretation of the latest Europeans guidelines through a comparative analysis of the previous and latest Europeans guidelines.
