1.Clinical evaluation of ginkgo leaf capsule combined with comprehensive rehabilitation training in treating chronic schizophrenia

AIM:To evaluate the therapeutic effects of treating chronic schizophrenia with ginkgo leaf capsule combined with comprehensive rehabilitation training. METHODS:110 chronic schizophrenia patients who had been treated with systemic antipsychotic for more than 12 weeks with unsatisfactory therapeutic effects and a long history of hospitalization were enrolled in this study. Ginkgo leaf capsule combined with comprehensive rehabilitation training was applied for 12 weeks and the therapeutic effects were evaluated. The positive and negative syndrome scale (PANSS) and treatment emergent symptom scale(TESS) for the side effects that needed to be treated during treating were used to evaluate the therapeutic effects and side effects. RESULTS:The treatment of ginkgo leaf capsule combined with comprehensive rehabilitation training was effective for the chronic schizophrenia patients with a long history of hospitalization,the total clinical effective rate was 96.36% and the cure rate together with the apparent rate was 67.27%,which were significantly better. CONCLUSION:Ginkgo leaf capsule improves the therapeutic effects of antipsychotic in the treatment of chronic schizophrenia and reduces the side effects,so it is a safe and effective drug for treating the chronic schizophrenia patients with a long history of hospitalization.
2.Comparison of Transient Evoked Otoacoustic Emissions and Distortion Products Otoacoustic Emissions as the Hearing Screening Methods in the Same Population of Normal Newborns

Objective To compare the results of TEOAE and DPOAE in the same population of normal newborns, to provide information on choosing appropriate screening tools.Methods A two-steps protocol was taken with the first screening during the first 48 to 72 hours of birth and rescreened from one to two months old if the newborns failed the first screening.For each step of screening, TEOAE and DPOAE were performed simultaneously using AccuScreen hearing screening instrument (Madsen-GN Otometrics, Taastrup, Denmark).A total of 1 062 normal newborns (F/M=508/554) delivered in Peking Union Medical College Hospital were enrolled in this research for the first screening.Infants who failed either TEOAE or DPOAE screening in the first screening were referred to a second screening.Among them, 135 performed both DPOAE and TEOAE in the second step.The newborns who failed the second screening would receive ABR when they were 3 months old.Results In the first screening,the failure rate for TEOAE was 11.0% (117/1 062) and 13.7% (145/1 062) for DPOAE.In the second screening step, the failure rates were 17.8% (24/135) and 20.7% (28/135) for TEOAE and DPOAE, respectively.Chi-square and Fisher's test showed that the failure rates of DPOAE were significant higher than TEOAE for both steps (P<0.001).The agreements between TEOAE and DPOAE were 96.0% and 95.6% for the first and second steps respectively, and the kappa values were 0.817 and 0.857.As to the average time taken to accomplish the screening for one ear, TEOAE was 24±25 s and DPOAE was 40±34 s during the first screening;in the rescreening, TEOAE was 52±41 s and DPOAE was 73±62 s.Paired-t tests showed that the differences between DPOAE and TEOAE testing time were statistically significant (P=0.000) in both screening steps.Finally, 7 newborns (10 ears) were diagnosed conductive hearing loss(except 1 ear was sensorineural hearing loss).Conclusion As a screening tool, TEOAE got lower refer rates and took less time than DPOAE implicating TEOAE a better screening tool for normal neonates.
3.A rapid quantitative ELISA for measuring D-dimer to exclude pulmonary embolism

4.Effects of Sijunzi decoction and Danggui Buxue decoction on immunologic function in tumor-bearing mice treated by chemotherapy

Objective To explore the protective effect of Sijunzi decoction and Danggui Buxue decoction on the immunological function in tumor-bearing mice treated by cyclophosphamide.Methods Transplantable carcinomas animal model were constructed by inoculating tumor cell to mice for 24 hours,and randomly divided into five groups.Tumor-bearing control group were reveived normal saline 0.2 mL per mice one day by intraperitoneal injection,and normal saline 0.4 mL per mice each day by intragastric administration.Cyclophosphamide group were reveived CTX 20 mg per kg weight each day,and normal saline 0.4 mL per mice each day.Sijunzi decoction and Danggui Buxue Decoction low dose group were reveived CTX 20 mg per kg weight each day and traditional Chinese medicine 0.1 g per mice each day.Sijunzi decoction and Danggui Buxue Decoction mediate dose group were reveived CTX 20 mg per kg weight each day and traditional Chinese medicine 0.2 g per mice each day.Sijunzi decoction and Danggui Buxue Decoction high dose group were reveived CTX 20 mg per kg weight each day and traditional Chinese medicine 0.4 g per mice each day.The weight indices of thymus gland and spleen,NK-cell activity and the proliferation of T-lymphocyte in five groups were measured after ten 10 days treatment.Results The weight indices of thymus gland and spleen,NK-cell activity and the proliferation of T- lymphocyte were increased in all three Sijunzi decoction and Danggui Buxue decoction groups,especially in Sijunzi decoction and Danggui Buxue decoction mediate and high dose group,the differences between them and CTX group were statistically significan(P<0.05,P<0.01).Conclusion Sijunzi decoction and Danggui Buxue decoction can antagonize the decrease of immunological function in tumor-bearing mice treated by chemotherapy.
5.The study of Yinaotongluo capsule preventing the microcirculation dysfunction in rats

ObjectiveTo explore the effect of Yinaotongluo capsule on preventing the microcirculation dysfunction in rats.Methods40 SD rats were randomly divided into the thrombosis model group, three Yinaotongluo capsule groups (low, moderate and high dosages) and control group with 8 rats in each group. Photosensitizer was injected through vessel. Endothelium damage, cell adhesion and thrombosis were induced by photochemistry. The change of microcirculation was recorded and analyzed by microphotography technique.ResultsYinaotongluo capsule groups could prolong the time to thrombosis induced by photochemistry compared with control group (P<0.05). However, the area of thrombus to vessel significantly decreased in all Yinaotongluo capsule groups compared with control group (P<0.01). The effect of moderate dosage group of Yinaotongluo capsule was superior to Buchangnaoxintong group.ConclusionYinaotongluo capsule can improve the microcirculation of mesentery of rats, in part it can improve the systematic microcirculation of rats.
6.Outcomes following HSCT using fludarabine replacing cyclophosphamide as a new preconditioning regimen for treatment of hematologic malignancies

Objective To evaluate the hypothesis of fludarabine replacing cyclophosphamide as a new myeloablative preconditioning for the treatment of malignant hematologic diseases in aged and/or intolerable patients receiving allogeniec stem cell transplantation (allo-HSCT). Methods Between January 2008 and November 2008,12 patients, who were intolerant to standard conditioning regimen, received allo-HSCT with HLA identical sibling (n = 9) or mismatched family donors (n = 3),including 1 case of acute lymphoblastic leukemia with Ph chromosome (Ph+ ALL) ,6 cases of acute myelogenous leukemia (AMD,3 cases of myelodysplastic syndrome-refractory anemia with excess blasts (MDS-RAEB) and 2 cases of chronic myelogenous leukemia (CML). Stem cell sources were G-CSF mobilized peripheral blood alone (n = 1) ,or with G-CSF mobilized bone marrow (n - 11), with a median of 6. 68 (4. 35 - 7. 86) × 10~8/kg MNC and 1. 50 (0. 31 - 3. 91) × 10~6/kg CD34~+ cells. Eleven patients received revised busulfan and fludarabine regimen with/without antithymocyteglobulin(ATG),and the rest one received TBI and fludarabine regimen. GVHD prophylaxis included cyclosporin A, mycophenolate mofetil and methotrexate. Results Results All patients were well tolerated to the regimen without serious regimen related toxicity. The median time of ANC≥0. 5 × 10~9/L was day 17. 5 (11 - 23), and that of BPC≥20. 0 ×10~9/L was day 14. All patients except one got donor engraftment successfully and attained CR. With a median follow-up of 418 (62-554) days, 10 of 12 patients were alive and disease-free. Conclusion Fludarabine replacing cyclophosphamide as a new preconditioning regimen is well tolerated and safe for allo-HSCT, especially in older patients or/and those with severe concurrent medical conditions.
7.Study of Acoustically Short Latency Negative Response in Tone Burst Auditory Brainstem Responses

Objective To study the characteristic of acoustically short latency negative response (ASNR) in auditory brainstem response (ABR) evoked by tone burst in children with hearing loss. Methods ABRs to click and tone burst were recorded from 0~6 years old children with hearing loss using SmartEP auditory evoked potential system. The threshold and latency was analyzed if ASNR was recorded. Results Among all the 80 ears tested, ASNR were recorded in 7 ears (8.75%) when using click, and in 40 ears (50%) when using tone burst. ASNR was most frequently evoked by 1 kHz tone burst (in 37 ears), and 2 kHz (in 25 ears) was the second. Among the ears with ASNR, the lowest threshold of ABR wave V was 65 dB nHL. The lowest threshold of ASNR was 80dB nHL. The latencies of ASNR for 0.5,1,2 and 4 kHz tone burst was 6~8, 5~7,3~5 and 3~4 ms, respectively. The latency decreased along with the increase of intensity. Conclusion ASNR can be recorded while recording tone-burst ABR, but it has no effect in predicting hearing level using the wave V threshold of tone-burst ABR.

Objective To explore the incidence, diagnosis and treatment of nephrotic syndrome ( NS ) following allogeneic hematopoietic stem cell transplantation ( allo-HSCT) in one single center.Methods Analysis were carried out based on the records of 1464 patients following allo-HSCT hospitalized in Peking University People's Hospital from January 1996 to April 2009, so as to evaluate the incidence and outcomes of NS. A total of 1464 patients whose survival time was longer than 100 d were enrolled. Eight patients (0.6%) had NS following allo-HSCT. Using the nested case-controlled study, 56 cases who received transplantation within the same period were selected as control group for risk factors analysis. Results The median time of NS was 488(54-1185)d after allo-HSCT. Lower extremities' edema and heavy proteinuria were present in all the patients. Two patients were accompanied with impaired renal function. Six patients suffered from chronic graft-versus-host disease (GVHD). Three patients with NS were accompanied with other symptoms of GVHD. Renal biopsy examination showed typical features of minimal change diseases in 3 patients and membranous nephropathy in 1 patient. All cases achieved complete remission (CR) after taking combined immunosuppressant agents. One patient relapsed soon after all the immunosuppressant agents were stopped. Six patients were still surviving with disease free. One patient died of the relapse of leukemia and another patient died of early discharge from hospital after allo-HSCT. The possible factors correlated with the occurrence of NS were analyzed, and the occurrence of chronic GVHD was identified as an independent factor of NS. Conclusions NS should be carefully taken into consideration when edema and proteinuria are present in patients following allo-HSCT. The occurrence of NS is probably related with chronic GVHD. NS responds well to immunosuppressant agents.
10.Blood stream infections during agranulocytosis period after hematopoietic stem cell transplantation in one single center

Objectlve To analyze the epidemiologic features of blood stream infection(BSI)during the period of agranulocytosis after hematopoietic stem cell transplantation(HSCT)in our hospital,and provide the basis for the empirical therapy.Methods The consecutive hematopoietic stem cell transplantation recipients conducted between January 2008 and October 2010 were studied retrospectively,to identify the current incidence,etiology for BSI and associated mortality during the period of agranulocytosis.Results Totally 75 patients were diagnosed as BSI.The incidence of BSI was 9.6%(75/784)in HSCT during the period of agranulocytosis,17.0%(75/441)in febrile patients.The pathogen testing showed that 64.4%were G-bacteria,30.1%were G+ bacterial and 5.5%were fungi.All G-bacteria except one were sensitive to carbapenems;all G+ bacteria except one were sensitive to vancomycin.Among the 75 patients,9(12.0%)experienced septic shock and 5(6.7%)died.Conclusions The pathogens of the BSI in the cohort are G-bacteria,followed by G+ bacteria and fungi.Carbapenems and vancomycin are the first drugs for the experiential therapy.
