1.Clinical analysis of 45 patients with community acquired pneumonia complicated with type two diabetes

Objective To investigate clinical characteristics about patients of community acquired pneumonia complicated with type two diabetes and the methods of diagnosis and therapy, in order to improve recovery rate.Methods To analyze clinical data methods of diagnosis and therapy and therapeutic effect of forty five patients who were diagnozed community acquired pneumonia complicated with type two diabetes. Results The clinical characteristics of patients of community acquired pneumonia complicated with type two diabetes are delitescence of symptom,metabolic disturbance,severe pathogenetic condition,difficult control of infection and high fatality rate cetera,after afford homologue treatments including regimenting blood glucose, selecting sensitive antibiotics to control infection, ameliorating pulmonary function and nutritional support and so on, thirty eight patients were cured, two patients were given up because of severe pathogenetic conditions,five patients died because of multiple organ failure,recovery rate was 84. 44%. Conclusion Compared to patients of purecommunity acquired pneumonia, the patients of community acquired pneumonia complicated with type two diabetes had unique clinical characteristics, and need to give special treatment
3.Effect of Serum containing Ziqi Ruangan Pill on Apoptosis Related Genes in LO2

Objective To study the influence of serum containing Ziqi Ruangan Pill on inhibitory effect of alcohol LO2 and to investigate the regulation on apoptosis related genes and the protective mechanism for LO2. Methods Rabbit sera saline containing Ziqi Ruangan Pill was prepared. The apoptotic LO2 model was established by being cultured with alcohol. The effect of serum on the model was detected by MTT method. Identification of apoptosis was carried out using flow cytometry and Hoechst3325 fluorescent staining. The expression of apoptotic inducing gene,casepase3 and NF-?B was detected by Real-time RT PCR and Western-Blot. Results The serum containing Ziqi Ruangan Pill could regulate the expression of the casepase3,NF-?B and reduce the apoptosis. Conclusion Ziqi Ruangan Pill can protect the LO2 by regulating the expression of apoptosis stimulative gene casepase3 and NF-?B.
4.Anterior approach operation of thoracolumbar fractures through limited incision

[Objective]To investigate the surgical method and clinical effects of the anterior approach operation in the treatment of thoraco-lumbar vertebral fractures through limited incision.[Method]Thirty-four cases of bursting fracture of thoracolumbar vertebral body were treated with the operation of anterior decompression,bone grafting and internal fixation by the anterior retropleuro-peritoneum approach.The MRI combined with clinical examination,X-ray,CT results were taken as selection criteria of treatment.[Result]Thirty-four cases were treated with this operative technich.Hemorrhage capacity is about 300-800ml with average 600ml.The operation time was 2-2.5 hours.All patients were followed-up with an average period of 18 months(ranged,6-24months).No aggravation of nerve injury appeared in 34 cases.X-ray manifested good fusion,no broken nails and no recurrence of kyphosis was found in all cases.[Conclusion]Anterior approach operation through limited incision is an effective way for severe thoracolumbar fractures. It has significantly low complication and allow early recovery of function.
5.Drug Resistance of 96 Fungal Strains from Urinary System

OBJECTIVE To study the distribution and the drug resistance of fungus species in hospital in one year,and to provide the evidence for clinical treatment.METHODS The fungus isolations were detected by colored separated-cuture,API 20C AUX,VITEK-60 identification system,and YBC identification card.The drug sensitivity was detected by ATB Fungus 3 INT strip.RESULTS Of the 96 islatens,37.5% were Candida glabrata,30.2% C.albicans and 27.1% were C.tropicalis.All the tested isolates were sensitive to 5-flucytosine(5Fc) and amphotericinB(AMB).CONCLUSIONS The hospital fungal infection of urinary system is caused by C.glabrata,C.albicans and C.tropicalis mostly.The clinical treatment should base on the drug sensitivity results.
6.Clinical evaluation of ginkgo leaf capsule combined with comprehensive rehabilitation training in treating chronic schizophrenia

AIM:To evaluate the therapeutic effects of treating chronic schizophrenia with ginkgo leaf capsule combined with comprehensive rehabilitation training. METHODS:110 chronic schizophrenia patients who had been treated with systemic antipsychotic for more than 12 weeks with unsatisfactory therapeutic effects and a long history of hospitalization were enrolled in this study. Ginkgo leaf capsule combined with comprehensive rehabilitation training was applied for 12 weeks and the therapeutic effects were evaluated. The positive and negative syndrome scale (PANSS) and treatment emergent symptom scale(TESS) for the side effects that needed to be treated during treating were used to evaluate the therapeutic effects and side effects. RESULTS:The treatment of ginkgo leaf capsule combined with comprehensive rehabilitation training was effective for the chronic schizophrenia patients with a long history of hospitalization,the total clinical effective rate was 96.36% and the cure rate together with the apparent rate was 67.27%,which were significantly better. CONCLUSION:Ginkgo leaf capsule improves the therapeutic effects of antipsychotic in the treatment of chronic schizophrenia and reduces the side effects,so it is a safe and effective drug for treating the chronic schizophrenia patients with a long history of hospitalization.
7.Successful transplantation of double unit cord blood from unrelated donors in high risk leukemia

Objective To enhance engraftment by infusing two partially matched cord blood (CB) units from unrelated donors simultaneously. Methods Two patients with high-risk leukemia(1 case of ALL and 1 case of CML-BC) received total body irradiation with antithymocyte globulin (ATG) and then were transplanted with two HLA-mismatched CB units from unrelated donors. A combination of cyclosporine A, methylprednisonelone and mycophenolate mofetil (MMF) was administered for graft-versus-host disease (GVHD) prophylaxis. Results Two patients were all engrafted and the time to neutrophil≥ 0.5 ?10 9/L and platelet≥20?10 9/L were 21, 22 days and 51, 28 days post-transplant respectively, and DNA analysis showed only one donor DNA was detectable after engraftment. There was no serious GVHD. And these 2 patients are still alive and disease free for more than 34 and 30 months repectively. Conclusion Double units cord blood from unrelated donors can restore hematopoiesis and is suitable in adults without bone marrow donors.
8.16 cases of gastrointestinal stromal tumor of small intestine: the comparative study of imaging findings and clinic pathological analysis

Objective To evaluate the barium meal radiography and CT scans in diagnosing malignant gastrointestinal stromal tumors (GIST) of small intestine. Methods The clinical and imaging data of 16 patients with GISTs of small intestine which were diagnosed surgically and pathologically were analyzed and summarized. In this group CT scan and gastrointestinal barium exam(GI) was performed. Results The tumors included 6 low-malignant GISTs of small intestine and 10 high-malignants. The diameter varied from 3.2 cm to 7.2 cm, the average size was 5.6 cm. The main signs of barium meal of malignant GIST of small intestine included flattened or destroyed mucosa runae, partial lumen stenosis, and barium fleck and fistula in the tumor. On CT scans, the main manifestation was an extraluminal mass with multiple necrosis areas of low density. Conclusion Barium examination and helical CT scan are the major imaging techniques in the determination of the location of GIST of small intestine. The appearance of imperfect lumen and the irregular tumor with multiulcerations or low density regions and inhomoneneous enhancement may lead to the correct diagnosis of malignant GIST.
9.Optic nerve injury model in rats set up by fluid percussion brain injury device

Objective To observe whether the animal model of optic nerve injury in rats can be set up by fluid percussion brain injury device (FPI) or not.Methods Seventy-one healthy female Wister rats were randomly divided into 2 groups,inlcuding model group with 66 rats and control group with 5 rats.The rats in model group were randomly divided into 3 groups.Eight rats in group 1 were examined by flash-visual evoked potential (F-VEP) and magnetic resonance imaging (MRI) examines before and 1,3 days,1,2,4,6,and 8 weeks after injury;56 rats in group 2 were randomly divided into 7 subgroups with 8 rats in each subgroup,and were detected by histopathologieal and terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling (TUNEL) apoptosis examines 1,3 days,1,2,4,6,8 weeks after injury;2 rats in group 3 were examined by electron microscopy 4 and 8 weeks aher injury.According to the degree of injury,the injured eyes were divided into 2 groups including severe injury group with the beat pressure of (699.14±60.79) kPa and mild injury group with the beat pressure of (243.18±20.26) kPa.The right and left eyes in rats in each group were in severe and mild injury group,respectively.Results One day after injury,the latency duration of F-VEP prolonged in severe injury group,wich differed much form which in the normal control group (P<0.05);the amplitude was gradually reduced during the first 2 weeks after injury and kept steady after that (P>0.05).The latency duration prolonged in mild injury group,and its difference with the normal control group was statistically significant (P<0.05);the amplitude was gradually reduced during the first 4 weeks after injury and kept steady after that (P>0.05).The abnormal high signal could be seen on optic nerve 1 day after injury,and was still obvious 8 weeks later.The results of histopathological examination showed ruptured capillary in ganglion cell layer 1 day after injury;retinal ganglion ceils without nucleus could be seen 4 weeks after injury.The apoptosis of positive cells was found in each layer of the retina 3 days after injury.TUNEL results indicated that the number of apoptotic positive cells increased significantly 1-2 weeks after injury.Conclusion An animal model of optic nerve injury can be successfully set up using FPI in rats.
10.Risk Factors and Outcome of Stroke-associated Pneumonia in Emergency Ward

Objective To explore the risk factors and outcome of stroke-associated pneumonia (SAP) in emergency ward. Methods The baseline characteristics and laboratory data of 306 patients with stroke in emergency ward of our hospital from April, 2014 to December, 2016 were reviewed. Patients were allocated into SAP group (n=120) and non-SAP group (n=186). The multivariable Logistic regression was applied to investigate risk factors for the progression and death of SAP. Results Totally, 120 (39.2%) patients developed SAP, in which 34 (28.3%) cases died. There were significant differences in age, the scores of Glasgow Coma Score (GCS), Sequential Organ Failure As-sessment (SOFA) and National Institute of Health Stroke Scale (NIHSS), and mechanical ventilation, dysphagia, antiacid and tube feeding (P<0.05). The multivariable Logistic regression analysis revealed that age over 65 years, hypertension, GCS<9, NIHSS, dysphagia and anti-acid were the risk factors for the development of SAP;the SOFA, NIHSS>15 and mechanical ventilation were the risk factors for 30-day mortality in SAP group. Conclusion SAP is a severe complication of stroke, it is associated with a variety of risk factors that affect its out-come in emergency ward.
