1.Analysis and countermeasure research on configuration of orthopedic nursing manpower resource in grade-Ⅲ general hospital in Shanghai

Objective To understand and analyze the situation of orthopedic nursing manpower resources in grade- Ⅲ general hospital in Shanghai and provide empirical evidence for appropriate allocation and exploitation of nursing manpower resources. Methods Data were collected by questionnaire and then underwent analysis. Results In 2009,the number of authorized beds was 1866,the actual number of open beds was 2007.The number of nurses was 595 (2 male).The ratio of ward bed and nurse were 1:0.319 and 1:0.296, calculated by the authorized beds and authorized beds, respectively.The average age of head nurses was (37.53±6.11 ) years old, average working life span was (17.87 ± 6.16) years; college degree or above accounted for 95.56%; middle professional titles accounted for 62.22%. College degree or above accounted for 75.13% in nurses, junior professional titles accounted for 92.1%; age group is mostly concentrated in the 20 to 29 years of age, accounted for 67.06%; working life span concentrated within 10 years, accounted foi 73.11%. The education level mainly is primary school in the care workers (carer), accounted for 45.83%; concentrated in the 40 to 49 years of age, accounted for 60.42%; working life span concentrated in more than 6 years, accounted for 55.21%. Conclusions There are shortage of orthopedic nursing manpower resources, unreasonable allocated structure, lack of male nurses in grade- Ⅲ general hospital in Shanghai. The management of the care workers should be standardized.
2.An extensive DeBakey type IIIb aortic dissection with massive right pleural effusion presenting as abdominal pain and acute anemia:particular case report

We describe the case of a 79-year-old male presented with sudden onset of abdominal pain and mild breathlessness, and complicated acute progressive anemia with haemoglobin which declined from 120 g/L to 70 g/L within five days. An urgent computed tomography an-giography showed acute thoracic aortic dissection, DeBakey type IIIb, a dissecting aneurysm in the proximal descending thoracic aorta start-ing immediately after the origin of the left subclavian artery and extending distally below the renal arteries with evidence of rupture into the right pleural cavity for massive pleural effusion. Plasma D-dimer, brain natriuretic peptide and C reactive protein level were elevated. Our case showed that D-dimer can be used as a‘rule-out’ test in patients with suspected aortic dissection. A raised BNP may exert a protective role through anti-inflammatory endothelial actions in the systemic circulation.
3.Synthesis and properties of poly(hydroxyethyl methacrylate) hydrogel for IOL materials.

Poly (hydroxyethyl methacrylate) (PHEMA) hydrogel for intraocular lens (IOL) materials was synthesized by solution polymerization using 2-hydroxyethyl methacrylate (HEMA) as raw material, ammonium persulfate and sodium pyrosulfite (KPS/SMBS) as catalyst, and trietyleneglycole dimethacrylate (TEGDMA) as cross-linking additive. Effects of reaction time, temperature, dosage of catalyst and cross-linking additive on mechanical strength and the equilibrium water content (EWC) of the PHEMA hydrogel were systematically investigated and their structure and optical property were also characterized. The experimental results showed that the optimum conditions for preparing PHEMA hydrogel are: catalyst 0.5 wt%, cross-linking additive 1.0 wt%, reaction temperature 40 degrees C, reaction time 36 h. Under the optimum conditions, the tensile strength of PHEMA hydrogel prepared is as high as 0.57 MPa, hardness of Shore A is 23.0, EWC is over 40%, and light transmittance is over 97%.

Objective To investigate the endoscopic prevalence of erosive esophagitis (EE) among 13 hospitals in Guangdong province of China. Methods Retrospectively reviewed all the cases (63459 cases) that received oesophagogastrodeuodenoscopy in 13 main hospitals in Guangdong province of China in 2003. Los Angeles criteria for classification of erosive esophagitis were employed as the basis of analysis. 5.Effects of disease symptoms and the degree of care of patients′ spouses on postoperative depression in female with breast cancer

Objective To investigate the prevalence situation and the main influencing factors of postoperative depression in female patients with breast cancer, and provide basis for response measures. Methods Totally 307 cases of patients with breast cancer from June 2014 to May 2015 in a tertiary hospital in Tangshan were selected as the research object, and Hamilton Depression Scale ( HAMD) was used to measure the depressive and symptoms of the disease.Results The total HAMD score of 307 patients with breast cancer after surgery was (7.46±6.44);130 patients had depression mood, which accounted 42.3%. Univariate analysis showed that spouses care about image, metastasis, ill-limb activity, nausea, vomiting, shortness of breath, loss of appetite, insomnia, joint pain, insomnia and hair loss were related to total depression score of female patients with breast cancer after operation ( t/F=3. 707, 3. 522, 3. 005, 5. 690, 4. 978, 10. 805, 18. 836, 28. 478, 7. 586, 28. 478,8. 540;P<0. 05). Multiple regression analysis showed that metastasis, spouses care about image, shortness of breath, insomnia, loss of appetite, hair loss were associated with postoperative depression in female with breast cancer (P<0.05).Conclusions As the prevalence rate of depression after breast cancer surgery was high, we should pay more attention on disease symptoms and improving the degree of spouses support to reduce the occurrence of postoperative depression in patients with breast cancer.
6.Influencing factors of immune-related thyroid dysfunction and overall survival in cancer patients treated with PD-1/PD-L1 inhibitors

[Objective] To investigate the influencing factors of immune-related thyroid dysfunction (irTD) treated with programmed death-1 (PD-1)/programmed death ligand-1 (PD-L1) inhibitors and their impact on overall survival (OS) of cancer. [Methods] We enrolled 211 cancer patients treated with PD-1/PD-L1 inhibitors. Clinical differences between irTD groups were compared, and subgroup analysis was performed. Multifactor Logistic regression analysis was used to identify influencing factors of irTD, while survival analysis was used to explore the relationship between the occurrence of irTD and OS, and the log-rank test was used for comparison between groups. The multi-model COX regression analysis was used to evaluate the impact of irTD on OS. [Results] The incidence rate of irTD was 26.1%, with 13.3%, 10.0% and 2.8%, respectively for grade 1, grade 2, and grades 3-4, and the median time of occurrence was at week 9 (IQR: 5-25 weeks). Significant differences were observed between the irTD and non-irTD groups in terms of gender, smoking history, targeted therapy history, and baseline thyroid antibody status (P<0.05). In irTD patients, thyroglobulin antibody (TGAb) levels began to increase from week 3, remained above the baseline from week 6 to week 30, and then gradually declined to the baseline level after week 30. The change in thyroid microsomal antibody (TMAb) levels was less pronounced than that of TGAb. Subgroup analysis showed that patients with hyperthyroidism were younger at the time of initial immunotherapy than those with hypothyroidism (P<0.05) and had lower baseline TSH levels (P<0.05). Multifactor Logistic regression analysis revealed that patients with positive baseline thyroid antibodies had a 4.595-fold higher risk of developing irTD compared to those with negative antibodies (95% CI: 2.286-9.239, P<0.001). Survival analysis revealed that patients with irTD had a longer OS and the multi-model COX regression analysis revealed that after adjusting for factors such as age, gender, chemotherapy, tumor type and tumor metastasis status, patients with irTD had a significantly longer OS (HR=0.228, 95% CI: 0.079-0.656, P=0.006). [Conclusion] The severity of irTD was predominantly grades 1-2, with grades 3-4 being rare. Positive baseline thyroid antibodies were an independent risk factor for the development of irTD. Patients who develop irTD have a longer OS, which may be due to their stronger immune response.
