1.Outcome of Housebound Seniors: 5 Years Follow-up

Objective To investigate the outcome of community seniors after 5 years of housebound, and the factors related with the outcome. Methods The housebound seniors confirmed 5 years ago in 5 communities in Tangshan were investigated with self-designed questionnaire and elderly depression rating scale again. Results 42.1% of the 147 housebound seniors remained housebound. There was significance between the seniors housebound and non-housebound in the factors of population sociology, such as age, spoused, economic income, housing floor, relationship with their children and social communication, and physical and psychological factors, such as health self-evaluation, loneliness and depression. Conclusion Seniors housebound in community may be reversible with some approaches of intervention.
2.Effects of self-efficacy intervention on self-management level of patients with chronic obstructive pulmonary disease

Objective To use self-efficacy theory to intervene patients with chronic obstructive pulmonary disease (COPD),and assess its efficacy on the self management level of COPD patients.Methods 106 COPD patients hospitalized in the respiratory department of a certain hospital from October 2011 to April 2012 were randomly divided into the control group and the intervention group.Each group consisted of 53 patients.Two groups of patients received routine respiratory treatment and nursing.But patients in the intervention group were also systematically intervened under the guidance of Bandura self-efficacy theory.The self-management level of both groups of patients were assessed before the treatment and six weeks after the treatment respectively.Results The self-management level of the intervention group were significantly better than that of the control group after intervention.Conclusions Self-efficacy theory can improve the self-management level of patients with COPD.
3.Effect of vaIproic acid on radiosensitivity to breast cancer ceIIs

OBJECTIVE To study the effect of valproic acid ( VPA) on radiosensitivity to MCF7 breast cancer cells. METHODS MCF7 cells were pretreated with VPA 0.5 and 1 mmol.L-1 for 0, 24, 48 and 72 h respectively, irradiated with 8 Gy lR, and at 6 h post-lR, the γ-H2 AX foci formation in MCF7 cells was tested by immunofluorescence assay. MCF7 cells were pretreated with VPA 0.5 and 1 mmol.L-1 for 72 h, irradiated with 4 Gy lR, and at 48 h post-lR, the cell survival rate was detected by MTT assay. MCF7 cells were pretreated with VPA 0.5 mmol.L-1 for 24 h, and then irradiated according to the amount of cells: 2 Gy (500 and 1000 cells per plate), 4 Gy (2000 and 4000 cells per plate), 6 Gy (8000 and 16000 cells per plate), and the cloning efficiency was calculated. MCF7 cells were pretreated with VPA 0.5 and 1 mmol.L-1 for 0, 24, 48 and 72 h respectively and the cell cycle profile was analyzed via flow cytometry. RESULTS After treatment with VPA alone for 24 h, MCF7 cells showed a significant increase in the amount of γ-H2 AX foci formation ( P < 0. 01). lt was also found that VPA increased lR-induced γ-H2 AX foci formation, which obviously prolonged the pretreatment time of VPA(P<0.01) in a time-dependent manner(r=0.98, P<0.05). VPA 0.5 and 1 mmol.L-1 had the same effect on γ-H2 AX foci formation. Furthermore, VPA was able to cause a significant decrease in lR-induced clonogenic survival but an increase in lR-induced cytotoxicity by MTT assay. Also, VPA alone decreased the plating efficiency of MCF7 cells. However, the cycle profile of MCF7 cells treated with both VPA 0.5 and 1 mmol.L-1 was not changed. CONCLUSION Without affecting the cell cycle profile, both the safe and critical dose of VPA used in clinical epilepsy treatment can significantly increase the accumulation of DNA double strand breaks in the cells and sensitize the cells to lR treatment, suggesting that VPA can induce radio-sensitization of breast cancer cells.
4.Effects of KIM-1 on high glucose induced the expression of MCP-1 and FN in rat tubular epithelial cells

Objective To evaluate the effects of KIM-1 on high glucose induced the expression of MCP-1 and FN in rat tubular epithelial cells and to explore the possible mechanisms of KIM-1 involved in renal interstitial fibrosis of DN.Methods The rat renal tubular epithelial cells (NRK52E) were cultured in vitro and divided into five groups:Normal control group (D-glucose 5.6 mmol/L),Hypertonic group (D-glucose 5.6 mmol/L + D-mannitol 24.4 mmol/L),High glucose group (Dglucose 30 rmmol/L),Control siRNA group,KIM-1 siRNA group.ELISA assay was used to assess the levels of MCP-1 and FN in the cells supernatant; Western blotting was used to detect the protein expression of KIM-1; RT-PCR was used to detect mRNA expression of KIM-1,MCP-1 and FN.Results Compared with the control group,the protein and mRNA expression of KIM-1 in the high glucose group were increased at 12 h (P < 0.05),and reached the peak at 48 h (P < 0.05); the protein and mRNA expression of MCP-1 and FN in high glucose group were increased at 24 h significantly (P < 0.05),and peaked at 48 h (P < 0.05).Compared with the high glucose group,the protein and mRNA expressions of MCP-1 and FN in KIM-1 siRNA group were decreased (P<0.05).Conclusions Down-regulating the expression of KIM-1 can significantly inhibit the expression of MCP-1 and FN,which suggests that KIM-1 may be involved in renal interstitial fibrosis of DN by regulating expression of MCP-1 and FN.
5.Investigation on Degree of Familiarity of GCP in Guang’anmen Hospital of China Academy of Chinese Medical Sciences

Objective To investigate the mastery degree of GCP (Good Clinical Practice) in Guang’anmen Hospital of China Academy of Chinese Medical Sciences (hereinafter referred to as“Guang’anmen Hospital”). Methods Totally 338 clinical researchers, covering 18 clinical professional sections and medical clinical trial institutions, in Guang’anmen Hospital with both qualification of clinical trials and GCP certificate were investigated in 23th-27th June 2014. The investigation was in the written answer sheet form, including four noun explanations and 36 multiple choice questions, which were related to GCP. Results 338 researchers could accurately explain the concepts of 4 nouns, and 57 (16.86%) researchers could do all multiple choice questions correctly. 141 (41.72%) researchers chose 1 wrong answer and 89 (26.33%) researchers chose 2 wrong answers. 39 (11.54%) researchers failed to answer 3-4 questions. Only 12 (3.55%) researchers failed to answer 5 or more questions. Conclusion The mastery degree of clinical researchers with qualification of clinical trials and GCP certificate in Guang’anmen Hospital is satisfied. In the future, pertinence and diverse training should be strengthened, in order to improve the overall level of the mastery degree of GCP.
6.Correlation of pelvic endometriosis cysts with pain and infertility

Objective To study the correlations of pelvic endometriosis cysts with pain and infertility according to the revised American Fertility Society (r-AFS) staging of endometriosis.Methods We retrospectively studied the clinical information of 258 cases patients with the ovarian endometriosis cysts who undergoing laparoscopic ovarian endometriosis cysts excision from January 2007 to December 2013.In accordance with the r-AFS,the correlation between pelvic adhesions and pain symptoms or infertility was evaluated.Results Among 258 cases with ovarian endometriosis cysts,there were 72.09% (186/258) of patients presented pelvic pain and 16.28% of patients presented infertility.The stages of r-AFS were that 16 cases in stage I,80 cases in stage Ⅱ,108 cases of in stage Ⅲ and 54 cases of stage Ⅳ.Also,186 cases with pain symptom,77.42% (144/186) of patients were in stage Ⅲ and Ⅳ,which was remarkably higher than that of no symptom group (25.00% (18/72),x2 =64.017,P < 0.05).Among 42 cases with infertility,there were 85.71% (36/42) of patients were in stage Ⅲ and Ⅳ,remarkably higher than of no infertility group (58.33% (126/216),x2 =11.283,P < 0.05).Conclusion The r-AFS stage of pelvic distribution in ovarian endometriosis cyst is closely correlated with pain symptoms and infertility.Patients in stage Ⅲ and Ⅳ with pain and infertility are more significant.
7.Investigation and analysis of consciousness of cornea donation in Chengde city

OBJECTIVE: Chengde is a city with many corneal blindness patients In north China. Lacking of corneas donation is the main reason which prevents corneal transplantation. Survey was made by questionnaire in Chengde to evaluate the current situation and the influential factors of corneal donation.METHODS: Survey was made in 3 200 Chengde residents aged 18 years or older, includes outpatients and inpatients of ophthalmology, some undergraduate students and people met accidentally in park, supermarket, station and centre for elders.48.6% are male and 51.4% are female. Self-made questionnaire includes general state, questions about cornea donation and factors influencing cornea donation.RESULTS: Among 3 200 questionnaires, 2 971 were valid. The effective rate was 92.84%. Over 50% people support donating cornea. More than 40% people intend to donate their corneas and support their relatives to donate. Among the factors for not intending to donate cornea, lacking knowledge of cornea donation was the main reason accounting for 42.81%, and worrying about the misusage of donating cornea without corresponding law became the second factor, which accounting for 21.07%. It has no influence on the consciousness of cornea donation by the difference of sex and location between city and countryside.Whereasfession and level of education indeed influence the consciousness of cornea donation, which of the people from 18 to 40 years old was greater than those of over 40, medical workers was greater than those from other fields, the people graduating from secondary specialized school or higher was greater than those graduating under secondary specialized school.CONCLUSION: People in Chengde have a positive attitude towards cornea donation. It is very necessary to enhance the education of cornea donation, establish an easy and smooth way for donation may promote cornea donating. Consummate legislation is also needed for cornea donation.
8.Effects of Self-management Program for Old Patients with Knee Osteoarthritis in Community

Objective To study the effects of self-management of knee osteoarthritis (KOA) on the function and quality of life in old patients. Methods From October 2011 to April 2012, 100 patients with KOA from 2 communities in Tangshan, Hebei were recruited, and were as control group and intervention group based on the community. The intervention group learned a KOA self-management course, which consisted of 6 classes of 1.5 to 2 hours, once a week, taught by health professionals. The control group read the KOA self-management handbook only. They were measured with the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) and Arthritis Impact Measurement Scale 2nd version- Short Form (AIMS2- SF) before and 6 months after intervention. Results The score of WOMAC and AIMS2-SF improved more in the intervention group than in the control group (P<0.05). Conclusion The KOA self-management program is effective on function and quality of life in old patients with KOA in community.
9.The level of serum hypoxia-inducible factor-1α and the regulation of pantoprazole in patients with gouty arthritis

Objective To explore the level of serum hypoxia-inducible factor (HIF)-1α and the effect of pantoprazole on HIF-1α in patients with gouty arthritis.Methods Subjects were divided into acute gouty arthritis (group A),intermittent gouty arthritis (group B) and healthy controls (group C).Patients in group A were divided into mild (A1) and severe (A2) subgroups according to the severity of inflammation.Levels of serum HIF-1α in the three groups,and the levels of interleukin (IL)-1β and tumor necrosis factor (TNF)-α in group A1 and group A2 were detected by enzyme-linked immunosorbent assay (ELISA).Patients in group A2 were randomly divided into three groups,saline 100 ml/twice a day,20% pantoprazole 100 ml,twice a day and 40% panto-prazole 100 ml,twice a day were administered respectively.After 7 days,the levels of serum HIF-1α were measured by ELISA.One-way analysis of variance (ANOVA) and two independent sample t test were conducted in this study.Results ① Levels of serum HIF-1α in group A [(48±13) ng/L] were significantly higher than those in group B [(40±12) ng/L,P<0.01] and group C [(28±12) ng/L,F=29.838,P<0.01].② Levels of serum HIF-1α [(56±10) ng/L],IL-1β [(51 ±13) ng/L] and TNF-α [(161 ±45) ng/L] in the experimental group A2 were higher than those in the experimental group A1 [HIF-1α:(42±11) ng/L,t=4.600,P<0.01;IL-1β:(42±12) ng/L,t=2.552,P<0.05;TNF-α:(122±34) ng/L,t=3.432,P<0.01].③ Levels of HIF-1α [(26±6) ng/L],IL-1β [(23±4) ng/L],TNF-α [(92±6) ng/L] in the 40% pantoprazole group were lower than those in 20% pantoprazole 100 ml,twice a day group [HIF-1α:(33±4) ng/L];IL-1β:(30±5) ng/L;TNF-α:(102±7) ng/ L] and saline group [HIF-1α:(37±5) ng/L];IL-1β:(38±5) ng/L];TNF-α:(108±9) ng/L](all P<0.05);Levels of HIF-1α,IL-1β and TNF-αin the 20% pantoprazole group were lower than those in saline group (all P<0.05).Conclusion ① HIF-1α may be one of the indicators of acute inflammation,which may reflect the degree of inflammation in patients with gout.② Pantoprazole can reduce the level of serum HIF-1α,IL-1β and TNF-α in patients with gouty arthritis,with a concentration dependent characterisit.
10.Comparative study on functional characters of MCF7 and HCC1937 cell lines in response to DNA damage

Objective The functional characters of MCF 7 and HCC1937 cell lines were compared through the activity of BRCA 1 and p53 following DNA damage in order to provide more research evidence for the related studies in both breast cancer cell lines.Methods The protein level of BRCA1 and p53 in two breast cancer cell lines and the protein level of BRCA 1 in MCF7,HCC1937 and HCC1937 wtBRCA1 breast cancer cell lines treated with 10Gy after 1 h,4 h or 8 h were detected by western blotting analysis.The distribution and foci formation of BRCA 1 in the cells were observed through immunostaining assay and the percentage of BRCA1 or Rad51 foci formation after ionizing radiation was calculated.Cell cycle profiling was analyzed using flow cytometry.Results Most of BRCA1 and p53 localized in nucleus , and both proteins responded to DNA damage in MCF 7 cells.In MCF7 cells, BRCA1 and Rad51 foci formation respectively increased to (59.40 ±3.66)%from(11.80 ±3.51)%(t=16.26,P<0.05)and (73.90 ±8.66)% from (16.70 ±3.76)%(t=10.49,P<0.05) after 10 Gy 8 h;p53 and p21 protein level was further separately induced and enhanced to (82.54 ±1.04) from (23.75 ±0.51) (t =87.90,P<0.05) and (90.95 ± 1.13) from (50.19 ±0.89)( t=49.11,P<0.05) after 10 Gy 8 h;and the cells were accumulated in G1 phase.In contrast to MCF7, in HCC1937 cell line, both of BRCA1 and p53 were defective in nucleus since both proteins were mutated;in response to DNA damage , BRCA1 foci formation was not found , p53 and p21 was not induced; there was no cell accumulation in both of G 1-S and G2-M phases.However , after complementation of wild-type BRCA1 in HCC1937 cells, DNA damage-induced Rad51 foci formation increased to (61.70 ±4.03)%from (6.22 ±2.27)%( t=20.78,P<0.05) and accumulation of cells in G2-M phase was also restored after 10 Gy 8h , which was similar to that of in MCF7 cells.Conclusions We have identified that BRCA1 and p53 have dramatically different functions in MCF 7 and HCC1937 cell lines in response to DNA damage.
