Objective To probe into the relationship of the cumulative effect of childhood trauma types and symptoms of depression and anxiety among pregnancy women. Methods A total of 276 cases of pregnancy women were investigated by using Childhood Trauma Questionnaire (CTQ- SF), the Edinburgh Postnatal Depression Scale (EPDS) and the State-Trait Anxiety Inventory (STAI). Results The cumulative number of childhood trauma types were positively correlated with the scores of EPDS, state anxiety and trait anxiety (r=0.245, 0.262 and 0.292, P<0.01);the scores of CTQ-SF, EPDS, state anxiety and trait anxiety of multi-CTQ group were higher than that of non-CTQ group, as well as the positive rate of depression symptom and anxiety symptom (P<0.05 or 0.01);Logistic regression analysis showed that the cumulative number and accumulation group of CTQ may be predictors or risk factors of depression and anxiety of women during pregnancy, and existed cumulative effect. When the cumulative number of childhood trauma types more than two types, it increased 2.37 and 3.12 times likelihood of depression and state anxiety comparing to non-CTQ group. Conclusions It suggested that childhood trauma experience may be a risk factors of depression and anxiety during pregnancy, and exist cumulative effect.

Objective To investigate the occurrence of posttraumatic stress disorder (PTSD) and the relationship between PTSD and emotional regulation strategies of mothers with neonates hospitalized in the neonatal intensive care unit (NICU). Methods One hundred and twenty-seven postpartum women with neonate hospitalized in the NICU were selected from a general hospital. All the cases were investigated by PTSD check list-Civilian version (PCL-C) and emotion regulation questionnaire (ERQ). Results The incidence of PTSD of postpartum women with neonate in NICU was 13.4%(17/127). The expressive suppression scores in positive PTSD group were (17.76±2.46) points, which were significantly higher than (14.16 ± 5.14) points in negative PTSD group (t=-4.659, P<0.01). Expressive suppression contributed significantly positive correlation to the total and the dimension scores of PCL-C (r=0.187-0.243, P<0.05 or 0.01). Expressive suppression was found to contribute significantly to the explained variance in mothers′ PTSD (P<0.05), whereas no association was found between PTSD and cognitive reappraisal (P>0.05). Conclusions The admission of neonates to the NICU is a stressful event for mothers, which could cause PTSD symptoms. PTSD has a closer relationship with expressive suppression, rather than cognitive reappraisal. Therefore, health workers should instruct mothers with neonates in NICU to regulate emotions by appropriate coping strategies, decreasing the possibility of psychological problems, promoting the mental health of women in the perinatal period.

Objective:To evaluate the efficiency of the tibial coronal alignment after total knee arthro-plasty with the extramedullary cutting system guided by the tibial tubercle and anterior tibial tendon, and to discuss the potential risk factors for the postoperative malalignment. Methods:A retrospective study of 212 primary unilateral TKA surgeries (in 188 patients)in our hospital between June to December in 2014 had been analysed. All the cases were performed by one surgical group. An extramedullary tibial cutting guide system had been used, with a landmark of one third inner-medial tibial tubercle as a proxi-mal anatomy reference, and anterior tibial tendon as a distal marker. The mechanical axis of lower ex-tremity in full-length X-ray was measured before surgery, and the tibial prosthetic coronal alignment was checked two weeks postoperation, evaluating the accuracy of this extramedullary cutting system guided by our method. Results:Since good alignment was defined as an angle within 3 degrees of the perpendicular to the mechanical axis, the tibial component positions were acceptable in 191 knees (90.1%),five knees were in valgus postoperative, and sixteen knees were in varus. There were seventeen(8.7%)in 179 knees with a preoperative varus presented malalignment after surgery, and four in 12 preoperative valgus kneess howed malalignment also,no statistical difference was found by Chi-square test (χ2 =2.778,P=0.096),which cannot define the relationship between the varus or valgus deformity preopera-tion and the malalignmentposition in tibial prosthesis after surgery.Twenty-two knees suffered a severe preoperative deformity as a varus or valgus angle larger than 20 degrees with absolute value of mechanical axis before surgery and tibial prosthetic coronal alignment were 21.95°(20.00°,26.90°)and 1.85° (0.10°,7.10°),showed a significant difference (Z =2.11,P =0.035 )compared with the data [10.65°(0.50°,19.80°)in preoperative mechanical axis and 1.10°(0.00°,4.60°)in the tibial pros-thetic coronal alignment]of 190 knees who presented a mild deformity before surgery(less than 20 de-grees),the result indicated the severe preoperative deformity might be a potential mal-alignment risk fac-tor within this cutting system in TKA surgery. Conclusion:The tibial coronal alignment after total knee arthroplasty could achieve good results with extramedullary cutting guide,by using one third inner-medial tibial tubercle and anterior tibial tendon as a proximal and distal anatomy marker.Postoperative varus might occur in this system, and tibial prosthetic malalignment appeared more often in the patients who suffered a severe deformity before surgery.

Objective:To investigate the effects of hydromorphone on early postoperative recovery, postoperative analgesia and stress response in patients undergoing laparoscopic radical resection of colorectal cancer.Methods:120 patients with colorectal cancer who underwent laparoscopic radical resection of colorectal cancer in Taizhou Hospital of Zhejiang Province from June 2019 to June 2020 were included in this study. They were randomly assigned to undergo either postoperative patient-controlled intravenous analgesia with hydromorphone (observation group, n = 60) or postoperative patient-controlled intravenous analgesia with sugentanil (control group, n = 60). The changes of perioperative indexes, postoperative recovery, Visual Analogue Scale score at 3, 12 and 24 hours post-surgery, stress response [norepinephrine, interleukin-6 and cortisol] before and 3 days after surgery were compared between the two groups. Results:There were no significant differences in operative time and intraoperative blood loss between the two groups (both P > 0.05). The time to intestinal peristalsis, the time to getting out of bed, and the time to drainage tube removal in the observation group were (57.83 ± 8.98) hours, (43.12 ± 2.34) hours, and (121.38 ± 10.29) hours, which were significantly shorter than those in the control group [(65.21 ± 7.45) hours, (45.46 ± 2.19) hours and (150.28 ± 13.42) hours, t = 4.899, 5.656 and 13.238, all P < 0.05]. Visual Analogue Scale score in the observation group at 12 and 24 hours after surgery was (1.89 ± 0.27) points and (1.45 ± 0.23) points, respectively, which was significantly lower than that in the control group [(2.19 ± 0.24) points, (1.84 ± 0.20) points, t = 6.433, 9.911, both P < 0.05]. At 3 days after surgery, serum levels of norepinephrine, interleukin-6 and cortisol in the observation group were (185.49 ± 18.29) ng/L, (59.91 ± 6.89) ng/L, and (109.21 ± 15.46) μg/L, respectively, which were significantly lower than those in the control group [(235.41 ± 16.57) ng/L, (73.24 ± 7.68) ng/L, (128.39 ± 10.32) μg/L, t = 15.668, 10.008, 7.993, all P < 0.05]. Conclusion:Hydromorphone exhibits a good effect on laparoscopic radical resection of colorectal cancer because it can promote early postoperative recovery, has an obvious postoperative analgesic effect and little influence on stress response.

PURPOSE: The role of systemic sensitization in the pathophysiology of chronic rhinosinusitis with nasal polyps (CRSwNP) remains elusive. This study sought to characterize the pattern of cytokines in polyp tissues from atopic and nonatopic patients with CRSwNP. METHODS: Atopic and nonatopic polyp and normal tissues were collected from 70 CRSwNP patients and 26 control subjects, respectively. The distribution of inflammatory cells (eosinophils, neutrophils, mast cells, etc.) were examined using immunohistochemistry, the mRNA levels of the transcription factors GATA-3, T-bet, RORc, and FOXP3 were determined using quantitative real-time polymerase chain reaction. The levels of inflammatory mediators (IFN-gamma, IL-5, IL-17A, etc.) in tissue homogenates were measured using enzyme-linked immunosorbent assay (ELISA). Moreover, the levels of inflammatory mediators in the supernatant of anti-IgE stimulated polyp tissues were measured using ELISA. RESULTS: Atopic CRSwNP patients were characterized by increased eosinophil accumulation, enhanced eosinophilic inflammation (elevated IL-5, ECP, and total IgE), and significantly increased GATA-3 mRNA levels (P<0.05), whereas both atopic and non-atopic CRSwNP patients showed decreased FOXP3 mRNA expression (P<0.05). After addition of anti-IgE stimulation, atopic CRSwNP patients produced more IL-5, IL-2, IL-10, IL-17A, and PGD2 in the supernatant of stimulated polyp tissues than nonatopic CRSwNP patients did. CONCLUSIONS: Atopic and nonatopic CRSwNP patients may possess the patterns of inflammatory response in polyp tissues.
