1.Meticillin Resistance and Glycopeptide Resistance in Clinical Coagulase-negative Staphylococci:Detection and Analysis

OBJECTIVE To provide laboratory evidence for the prevention and control of coagulase-negative staphylococci(CNS) and study the prevalence of meticillin resistance and vancomycin resistance in clinical CNS in our hospital.METHODS Meticillin-resistant coagulase-negative staphylococci(MRCNS) were detected with cefoxitin disk diffusion and mecA-PCR.CNS with reduced susceptibilities to vancomycin was detected with vancomycin agar screen test.Their MIC was determined with E-test and gene van was detected with multiplex PCR.RESULTS CNS was highly resistant to cephem,?-lactam,aminoglycoside,macrolide,and lincosamide.The confirm rate of cefoxitin disk diffusion to mecA-PCR was 96.2% and 92.3% of mecA in 52 CNS strains were detected.Four strains of staphylococci with heteroresistance to vancomycin(VRS) were screened in 256 CNS ones which were all MRCNS but without gene van detected.CONCLUSIONS It is important for clinical laboratory to detect VRS and MRCNS with suitable methods to prevent VRS infection and prevalence.
2.Effect of nicotine intake on the periodontal tissue during orthodontic tooth movement

BACKGROUND:Cigarette smoking can seriously damage the periodontal tissues and root, and the nicotine in tobacco accelerates the progression of periodontal diseases. OBJECTIVE: To investigate the effect of different doses of nicotine on the expression of cyclooxygenase-2 mRNA in the periodontal tissue during orthodontic tooth movement. METHODS: Totaly 110 Sprague-Dawley rats were randomly divided into blank control group (n=10), normal saline group (n=25), 0.5 mg/kg nicotine group (n=25), 0.75 mg/kg nicotine group (n=25), 1 mg/kg nicotine group (n=25). Rats in the normal saline groups were injected intraperitonealy with 0.1 mL normal saline, and those in the three nicotine groups were respectively injected with 0.5, 0.75, 1 mg/kg nicotine tartrate solution. Except the blank control group, the unilateral maxilary first molars of rats in the other four groups were exposed to 50 g force. At 1, 3, 5, 7, 14 days under force, the rats were sacrificed to take the maxilary tissues. Hematoxylin-eosin staining was used to observe the changes of periodontal tissues, immunohistochemical staining was employed to count positive cels, andin situ hybridization staining was adopted to detect the mean absorbance value of cyclooxygenase-2 in the periodontal tissues. RESULTTS AND CONCLUSION:The number of odontoclasts and the expression of cyclooxygenase-2 in the nicotine groups were higher than those in the non-nicotine groups. With the increasing dose of nicotine, the number of odontoclasts gradualy increased, and the difference was statisticaly significant (P < 0.05). At 7 days under force, the number of odontoclasts reached the peak. With the increasing dose of nicotine, the positive expression intensity of cyclooxygenase-2 was also increased, and the difference was statisticaly significant (P < 0.05). The expression of cyclooxygenase-2 reached peak at 5 days under force. These findings indicate that with the increasing dose of nicotine, the number of odontoclasts and the expression intensity of cyclooxygenase-2 are both increased at the same time point under force. During the orthodontic tooth movement, the intake of nicotine can damage the periodontal tissue, and the dose of nicotine can directly influence the severity of damage to the periodontal tissue.
3. Study on the design and application of nasal endoscope screw self-open cleaning loading technology

Objective:
To improve the cleaning and loading technology of nasal endoscope in order to ensure the cleaning effect, to improve the working efficiency, and to reduce the cleaning cost.
Methods:
A total of 480 reusable contaminated nasal endoscopes were divided into two groups according to random number table: Clean and load the nasal endoscopes (group A) with the usage of metal parts to open up the leaf; Clean and load the nasal endoscope (group B) with the usage of screw driver to open up the leaf. Each group has 240 pieces. After mechanical cleaning, observe and analyze the cleaning effect, loading effectiveness, loading capacity, loading time and CSSD operators′ degree of satisfaction.
Results:
The number of qualified cleaning joint parts of the two groups were 218 and 231 respectively, and the number of qualified cleaning leaf of the two groups were 214 and 227 respectively, and there were significant differences between the two groups (
4.Predictive value of pre-surgical 18F-FDG PET/CT metabolic parameters for mediastinal lymph node metastasis in patients with lung adenocarcinoma

Objective:To investigate predictive value of model based on pre-surgical 18F-FDG PET/CT metabolic parameters for mediastinal lymph node metastasis (LNM) in lung adenocarcinoma. Methods:A total of 288 patients with lung adenocarcinoma (135 males, 153 females, age (61.6±8.5) years) who diagnosed and treated in the Fourth Hospital of Hebei Medical University from January 2016 to February 2021 were enrolled retrospectively. All patients underwent 18F-FDG PET/CT examination within 1 month before operation, and underwent complete resection of primary lung tumor and standard lymph node dissection. PET/CT parameters were extracted (PET metabolic parameters: minimum SUV(SUV min), SUV max, SUV mean, SUV standard deviation (SUV std), metabolic tumor volume (MTV) and total lesion glycolysis (TLG); CT parameters: minimum CT value (HU min), maximum CT value (HU max), mean CT value (HU mean), CT value standard deviation (HU std)). Multivariate logistic regression analysis was used for screening parameters and establishing model to predict LNM. ROC curves analyses were used to evaluate the predictive performance of models. Results:Among 288 patients, 90 had LNM, and 361 metastatic lymph nodes (N1: 186, N2: 175) were reported by pathology. SUV min (odds ratio ( OR)=1.859, 95% CI: 1.074-3.220, P=0.027), SUV max ( OR=2.255, 95% CI: 1.306-3.893, P=0.004), SUV mean ( OR=0.277, 95% CI: 0.115-0.665, P=0.004) were predictors of LNM. The AUC of PET/CT model was 0.849 (95% CI: 0.804-0.893), and the sensitivity, specificity, accuracy, and positive and negative predictive values were 87.8%(79/90), 72.2%(143/198), 77.1%(222/288), 59.0%(79/134) and 92.9%(143/154), respectively. Conclusion:The model based on 18F-FDG PET/CT metabolic parameters can improve the accuracy of pre-surgical N-staging in patients with lung adenocarcinoma.
5. The efficacy and safety of denosumab and bisphosphonates on glucocorticoid induced osteoporosis patients:a meta-analysis

Objective:
In order to evaluate the therapeutic effects and safety of denosumab and bisphosphonates in glucocorticoid induced osteoporosis patients.
Methods:
Standard studies were obtained by searching CNKI, CBM, VIP, Wanfang, Pubmed, Embase and Cochrane databases.
Results:
Three RCTs with 869 patients were included in this study. It showed that the mean changes of lumbar spine, total hip and femoral neck bone mineral density (BMD) for patients in denosumab group were increased by 2.47%, 1.43% and 1.07% respectively compared to those of bisphosphonates group.There was no statistically significant difference between patients receiving denosumab and those receiving bisphosphonate in terms of adverse events and serious adverse events.
Conclusions
Denosumab has an effective increase for lumbar spine, total hip and femoral neck bone mineral density, and the safety of both is similar.
