1.Determination of Polysaccharide from Leaves of Sorbus tianschanica Rupr.and Its Bacteriostatic Action

Objective To establish a method for determination of polysaccharide from the leaves of Sorbus tianschanica Rupr.and its bacteriostatic action. Methods Phenol-sulfuric acid method was used for extraction and determination of polysaccharides in Sorbus tianschanica Rupr. Its antibacterial action was studied by control experiment. Results Average content of polysaccharides in Sorbus tianschanica Rupr was 2.56%.The polysaccharides exerted bacteriostatic effect, especially on Bacillus subtilis. Conclusion The method is simple and accurate, and can provide theoretical basis and reference for relative research of Sorbus tianschanica Rupr.
2.Association between early functional outcome and serum thyroid hormone level in elderly acute ischemic stroke patients

Objective To study the association between early functional outcome and serum thyroid hormone level in elderly acute ischemic stroke (AIS) patients.Methods Two hundred and twenty-four AIS patients admitted to our hospital from May to December 2016 were divided into good functional outcome group (n=166) and poor functional outcome group (n=58) according to their mRS score.The serum levels of FT3,FT4,TSH were measured.The patients were further divided into FT3≤4.05 pmol/L,FT3 4.06-4.46 pmol/L,and FT3>4.46 pmol/L.The association between early functional outcome and serum levels of FT3,FT4,TSH in elderly AIS patients were analyzed by univariate and multivariate logistic analysis.Results The AIS was severer,the BMI and incidence of AF were higher,the number of WBC was greater,the serum FT3 and CRP levels were higher in poor functional outcome group than in good functional outcome group (P<0.05,P<0.01).The rate of poor functional outcome was significantly higher in FT3≤4.05 pmol/L than in FT3 4.06-4.46 pmol/L and FT3 >4.46 pmol/L (39.5% vs 22.2%,39.5% vs 15.8%,P=0.003).Univariate logistic regression analysis and multivariate logistic regression analysis showed that FT3 was an independent risk factor for early poor functional outcome in AIS patients.Concision The lower the serum FT3 level on admission is,the poorer the early functional outcome is in AIS patients.
3.Distribution and drug resistance of Escherichia coli producing extended-spectrumβlactamases in cancer patient

Objective To study the distribution ,specimen types and characteristics of antibiotic resistance of Escherichia coli producing extended‐spectrum βlactamases(ESBLs) of the hospital in 2013 and to guide clinical drug use .Methods Analyzed the distribution and antibiotic resistance for the 375 strains of ESBLs‐producing E .coli ,and ESBLs was detected by disk diffusion phe‐notypic confirmatory test .Results The major distribution department was gynecology department which accounted for 42 .67% , followed by uropoiesis surgical department which accounted for 14 .67% ;the major specimen type was urine(55 .2% ) ,followed by puncture fluid(15 .47% )and excretion(14 .67% ) .For the 375 isolates of ESBLs‐producing E .coli ,the resistance rates to cefazolin , cefuroxime ,cefoperazone and cefotaxime were 100 .00% ,to SMZco was 78 .10% ,while the resistance rate to imipenem was 0 .00% , and to amikacin and fosfomycin were 4 .30% and 10 .10% respectvely ,the resistance rates to piperacillin/tazobactam and aztreonam were 17 .10% and 66 .70% respectvely .Conclusion ESBLs producing Escherichia coli have severe multidrug resistance .Antibiotics should be chosen and used rationally in accordance with results of drug susceptibility testing ,meanwhile the monitoring of ESBLs′infection rate and drug resistance should be strengthened .
4.Correlation of quantitative examination of urinary sediment bacteria and urine culture

Objective To evaluate quantitative examination of urinary sediment bacteria as a basic feasibility of screening indicators for urinary tract infection .Methods 191 outpatients and inpatient specimens were gathered firstly to implement a urine culture ,and then the rest of the urine were used for sediment bacteria quantitative testing .Meanwhile ,bacterial culture was conducted as the standard .According to the results of bacterial culture ,receiver operating characteristic(ROC) was drawn ,the threshold values of leukocyte and bacteria counts for diagnosis of urinary tract infection were found out and its sensitivity ,specificity ,positive / negative predictive value ,false positive/false negative rate and accuracy were calculated .Results The positive rate of urine culture was 39 .7% ,and the most common pathogen was Escherichia coli .The threshold value of bacteria and leukocyte counts for diagnosis of urinary tract infection was 1 024 .5/μL and 135 .8/μL respectively .When combined leukocyte and bacteria counts for urinary tract infection ,the optimum sensitivity was 62 .5% ,specificity was 98 .1% ,positive predictive value was 95 .7% ,negative predictive value was 79 .6% ,false positive rate was 1 .9% ,false negative rate was 37 .5% ,and accuracy was 83 .8% .Conclusion With UF‐1000i urinary sediment analyzer ,the combined determination of leukocyte and bacteria counts can remove the great mass of negative results ,Especially the results of bacterial culture positive predictability is higher ,but still can not replace of quantitative bacterial culture .
5.Effectiveness of lower-dose sexual hormone therapy to prevent bone mineral loss in Chinese postmenopausal women

Objective To determine the effects of sexual hormone therapy at varied doses on prevention of bone mineral loss in Chinese postmenopausal women. Methods From March 2002 to March 2003, 90 Chinese postmenopausal women were randomly divided into three groups, each given one of the following regiments for 12 months, estradiol valerate (EV) 1 mg plus medroxyprogesterone (MPA) 2 mg for group A (31 subjects), eonjngated estradiol (ethinylestradiol-3-cyclopentylether, CEE) 0.45 mg plus MPA 2 mg for group B (29 subjects) and livial 1.25 mg for group C (30 subjects), respectively. In addition, 400 mg of elemental calcium were given daily to all those women. Bone mineral density (BMD) of the 2nd to 4th lumbar vertebra (L2~4) and biochemical markers of bone turnover, urine N-telopeptide of type Ⅰ collagen/creatinine(NTX/Cr) and serum total alkaline phnsphatase (ALP), were measured before and after drug administration. Results After treatment for 12 months, BMD of the L2~4 increased significantly by 0.039 g/cm2(P<0.01) in group B, but not significantly in group A or group C (P<0.05). Increases in BMD of the L2~4 was more in group B than that in group A and group C, respectively (P < 0.05), but no significant difference in BMD increase between group A and group C was found (P >0.05). After treatment for 6 months, urine NTX/Cr reduced from the baseline for all the three groups (P < 0.05), but no significant difference among group A, group B and group C was found (P >0.05). After treatment for 12 months, serum ALP significantly reduced from the baseline for all the three groups (P <0.01), but no significant difference among group A, group B and group C wag found (P > 0.05). Conclusions Sexual hormone therapy at varied doses lower than regular one for 12 months was effective in preventing bone mineral less in postmenopausal women.
6.Effect of Liangruntongluo Recipe and its Modified Formula on Gastrointestinal MOT、 CCK、SS of Rats with Diabetes

Objective To study the mechanism of Liangruntongluo Recipe (Chinese medicines with functions of cooling, nourishing and dredging collaterals) and its modified formula in improving gastrointestinal function of diabete smellitus (DM) rats. Methods Male Wistar rats were used. Streptozotocin (STZ) was injected to rats to produce diabetic rat models. No Hypoglycemic drugs were administered to these rats to reduce blood glucose. After 18 weeks, warm water, Liangruntongino Recipe, Chinese medicines with function of nourishing, Chinese medicines with functions of cooling, and Cisapride were administered to the model rats. Detect the contents of plasma motilin (MOT), cholecystokinin (CCK), and somatostatin (SS) after 6 weeks. Results By affecting the secretion of gut hormone and having wide range of target, Liangrantongluo Recipe could regulate the disorder of gut hormone. The function of Liangruntongluo Recipe was better than its modified formula and cisapride.Conclusion Liangruntongluo Recipe can improve gastrointestinal dysfunction of DM rats.
7.Primary research on direct multi-slice spiral CT venography in inferior vena cava

Objective To investigate the superiority of direct multi-slice spiral CT venography in inferior vena cava. Methods Twenty-eight patients performed MSCT venography in inferior vena cava, including 2 cases with both indirect and direct venography, 10 cases with indirect venography, 20 cases with direct venography through unilateral or bilateral lower extremity venous injection. The image quality and enhancement degree of the inferior vena cava were compared in double-blind method. Results Of 10 cases with indirect venography of inferior vena cava, 1 case was failed due to mild enhancement in inferior vena cava. Image quality was good in 2 cases, poor in 7 cases, no excellent case. Of 20 cases with direct venography of inferior vena cava, the enhancement degree was scored 1, 2 degree in 16, 4 cases respectively and no case was scored 3 degree, the image quality was excellent, good in 16, 4 cases and no case was bad. The success rate was 100%. Conclusion The image quality of direct MSCT venography in inferior vena cava is better than that of indirect method.

ObjectiveTo investigate the difference in the outcomes in patients with acute anterior circulation stroke having achieved the Thrombolysis in Cerebral Infarction (TICI) 2b and TICI 3 recanalization after mechanical thrombectomy.MethodsThe patients with acute anterior circulation stroke having achieved successful recanalization after mechanical thrombectomy were enrolled retrospectively.They were divided into TICI 2b (almost complete recanalization) group and TICI 3 (complete recanalization) group.The good outcome was defined as the modified Rankin Scale score 0-2 at 3 months after onset.ResultsA total of 83 patients were enrolled in the study, including 38 patients (45.8%) with TICI 2b and 45 (54.2%) with TICI 3;49 (59.0%) had good outcome, and 34 (40.9%) had poor outcome.The good outcome rate in the TICI 3 group was significantly higher than that in the TICI 2b group (68.9% vs.47.4%;χ2=3.946, P=0.047).After adjusting for age, hypertension, diabetes, baseline systolic blood pressure, triglyceride, intravenous thrombolysis, and ASITN/SIR collateral grades, TICI 3 was an independent predictor for good outcome at 3 months after onset (odds ratio [OR] 3.759, 95% confidence interval [CI] 1.098-12.871;P=0.035), while the higher baseline National Institutes of Health Stroke Scale score (OR 0.820, 95% CI 0.715-0.941;P=0.005) and higher fasting glucose (OR 0.610, 95% CI 0.410-0.906;P=0.014) were the independent predictors for poor outcome at 3 months after onset.ConclusionsThere are difference in the outcomes in patients with successful recanalization in anterior circulation stroke treated with mechanical thrombectomy.The outcome is substantially better in TICI 3 than TICI 2b patients.

To investigate the effect of angiotensin receptor blocker ( ARB ) telmisartan on endoplasmic reticulum stress -related kidney cells apoptosis in diabetic rats.31 male SD rats were divided into 3 groups:normal control group,diabetic group,and telmisartan treated diabetic group.After 12 weeks' experiment,weight,24 h urinary albumin excretion,blood glucose,serum insulin,and serum creatinine were determined.Renal cell apoptosis was examined by using TUNEL; Renal cell endoplasmic reticulum stress signaling pathway molecules glucose regulated protien 78 ( GRP78 ),caspase12,and CHOP were detected with immunohistochemistry and realtime PCR.24 h urine protein and serum creatinine in diabetic group were significantly higher than those in the control group; while body weight and plasma insulin were significantly lower than those in the control group( P<0.05 ) ;Compared with diabetic group,24 h urinary protein and serum creatinine were decreased significantly in telmisartan treated diabetic group( P<0.05 ).The apoptosis index of telmisartan treated group was significantly lower than that of diabetic group( P<0.05 ).Endoplasmic reticulum stress signaling pathway molecules GRP78,caspase12,CHOP protein and mRNA expression in diabetic group were significantly higher than those in control group( P<0.05 ),as well as in telmisartan treated group( P<0.05 ).Endoplasmic reticulum stress took part in kidney cell apoptosis of the diabetic rat.Telmisartan possessed a protective effect on endoplasmic reticulum stress-related renal cells apoptosis.

BACKGROUND: Permanent or transient implantation of biomaterials can result in biomaterials-centered infections (BCI) in lung cancer patients.OBJECTIVE: To investigate the relationship between BCI and peripheral blood transforming growth factor β1 (TGF-β1) in patients with lung cancer.METHODS: A total of 248 lung cancer patients undergoing in vivo intravascular catheter indwelling > 7 days were included.Quantitative method was used for intubation, bacteriological culture and paired blood culture, and API Staph strips were adopted for positive patients. While enzyme-linked immunosorbent assay was used to detect TGF-β1 levels in the peripheral blood of patients with lung cancer and 75 healthy volunteers as normal controls.RESULTS AND CONCLUSION: Among the 248 patients, there were 82 BCI-positive cases, and 166 BCI-negative cases.Thirteen patients were confirmed to have catheter-related bloodstream infection. There were 48 Gram-positive bacteria, 24Gram-negative bacilli, and 10 fungal. The levels of TGF-β1 were higher in BCI-positive patients than BCI-negative patients (P < 0.05); the levels of TGF-β1 in the BCI-negative group were higher than those in the normal control group (P < 0.05). For lung cancer patients with nosocomial infection induced BCI, there are various species of pathogenic bacteria, and Gram-positive bacteria are more common. To detect TGF-β1 levels in patients with lung cancer is of significance for early prevention of BCI.
