1.Clinical Study of Acupoint Catgut-embedding Therapy for Prevention and Treatment of Bone Marrow Depression in Breast Cancer Patients Induced by FEC Chemotherapy

Objective To compare the clinical efficacy of acupoint catgut-embedding therapy plus fundamental treatment versus fundamental treatment alone for the prevention and treatment of bone marrow depression in breast cancer patients induced by FEC(fluorouracil, epirubicin and cyclophosphamide) chemotherapy. Methods A total of 46 post-operation breast cancer patients accepting the first cycle of FEC chemotherapy were randomly divided into treatment group and control group, 23 cases in each group. The control group was given fundamental treatment including oral use of Chinese medicine of modified Xiangsha Liujunzi Decoction for regulating stomach and arresting vomiting, modified Guipi Decoction and Gui Lu Erxian Decoction for nourishing blood and generating blood, and western medicine of Batilol Tablets and Vitamin B4 Tablets. The treatment group was given acupoint catgut-embedding therapy on bilateral Zusanli(ST36) and Shenshu(BL23) plus fundamental treatment. The effect on bone marrow depression in the two groups was observed after treatment. Results(1) On post-chemotherapy day 7 and 8, the count of white blood cells (WBC) and neutrophils (NE) in the treatment group was higher than that in the control group, the difference being significant(P < 0.05 or P < 0.01).(2) The utilization of granulocyte colony-stimulating factor(G-CSF) in the treatment group was less than that in the control group, the difference being significant (P < 0.05). (3) On post-chemotherapy day 10, the effect on improving bone marrow depression in the treatment group was superior to that in the control group, the difference being significant (P<0.01) . Conclusion The acupoint catgut-embedding therapy plus fundamental treatment is more effective and safe for the prevention and treatment of bone marrow depression in breast cancer patients induced by FEC chemotherapy than fundamental treatment alone.
2.Awareness of chronic obstructive pulmonary disease-related knowledge in primary-care physicians at urban and suburban Shanghai and effectiveness of training

Objective To investigate awareness of definition, diagnostic criteria, severity classification and standard diagnosis and treatment for chronic obstructive pulmonary disease (COPD) among primary-care physicians at the grassroots in urban and suburban Shanghai.Methods In total, 298 physicians were surveyed by unified questionnaire for COPD-related knowledge in two secondary-care general hospitals (one in urban and suburban areas each ) and 12 community primary-care centers (four in urban and eight in suburban areas) in Shanghai.One hundred and fifteen physicians in two urban hospitals and one suburban hospital were trained professionally and then evaluated for its effectiveness.Results Awareness of diagnostic criteria of COPD was only 22.4% (35/156), 25.6% ( 10/39), 4.3 % (3/69)and 26.5% (9/34) among physicians in urban primary-care hospitals, urban secondary-care hospitals,suburban primary-care hospitals and suburban secondary-care hospitals, respectively.Their awareness of knowledge about use of anti-cholinergic agents was 13.5 % (21/156), 35.9 % ( 14/39), naught (0/69),and 14.7 % ( 5/34 ), respectively.Only 0.67 % of them ( 2/298 ) knew about severity classification of COPD.Awareness of definition and diagnostic criteria for COPD in the physicians increased to 72.2 % ( 83/115) after training from 18.2% (21/115) before it, and their awareness of severity classification for COPD increased to 45.2% (52/115) after training from 0.8 % (1/115) before it.Conclusions Awareness of COPD-related knowledge, especially of standard diagnosis and treatment for COPD, was poor in primary-care physicians at the grassroots in Shanghai, so it is necessary to strengthen training for them in COPD-related knowledge, including pulmonary function tests.
3.Distribution and drug resistance of pathogens from patients with anastomotic fistula infection after esophageal cancer surgery

Objective To understand the distribution and drug resistance of pathogens from patients with anastomotic fistula infection after esophageal cancer surgery, and provide basis for clinical diagnosis and treatment.Methods Patients were admitted to a hospital due to anastomotic fistula after esophageal cancer surgery between January 2012 and December 2015, microbial culture and antimicrobial susceptibility testing results of patients were retrospectively analyzed.Results 1 986 patients underwent radical resection of esophageal cancer within 4 years, 148 of whom developed anastomotic fistula, 104 (70.27%) were with positive microbial culture.A total of 197 pathogenic strains were isolated, 52(26.40%)and 145 (73.60%)strains were isolated from intrathoracic anastomotic fistula and cervical anastomotic fistula respectively;127 (64.47%)strains were gram-negative bacteria, the major were Pseudomonas aeruginosa, Klebsiella pneumoniae, and Acinetobacter baumannii, 62(31.47%) strains were gram-positive bacteria, the major were Staphylococcus aureus, Enterococcus spp., and Streptococcus viridans, 8 strains (4.06%) were fungi.49(47.12%) cases were with mixed pathogenic infection.The resistance rates of gram-negative bacteria to imipenem were 17.86%-47.62%, to polymyxin B was 0, resistance rates of Pseudomonas aeruginosa to other antimicrobial agents were all<50%, Klebsiella pneumoniae to piperacillin and aztreonam were both>70%, Acinetobacter baumannii to most antimicrobial agents were all>50.00%;resistance rates of gram-positive bacteria to clindamycin and tetracycline were both>50.00%, to linezolid, vancomycin, and teicoplanin were all 0, resistance rates of Staphylococcus aureus to penicillin, oxacillin, and ciprofloxacin were all>60%,resistance rate of Enterococcus spp.to quinupristin/dalfopristin was 100.00%.Conclusion Postoperative anastomotic fistula combined with infection can affect the prognosis of patients after esophageal cancer surgery, regular monitoring on distribution and drug resistance of pathogens can provide the basis for initial empirical treatment, and is conducive to the early treatment and rational use of antimicrobial agents.
4.Liquiritin improves depressive behavior in rats with post-stroke depression by reducing apoptosis of amygdala cells

Objective:To investigate the effect of liquiritin on the apoptosis of amygdala cell and the expression of apoptosis-related factors Bax and Bcl-2 protein in rats with post-stroke depression (PSD).Methods:Sixty rats were randomly divided into normal control group, stroke group, PSD group, citalopram group, liquiritin group, and normal saline control group ( n=10 in each group). The middle cerebral artery was occluded with a suture method to induce focal cerebral ischemia, and the PSD model was established by chronic and unpredictable mild stress stimulation and orphanism. At the same time every week after the model was made, the weight of rats in each group was measured and the depression behavior was evaluated, including sucrose water test and open field test. At 6 weeks after the model was made, TUNEL staining was used to detect the apoptosis of amygdala cell, immunofluorescence staining was used to detect the expression of Bax and Bcl-2 in the amygdala, and Western blot analysis was used to detect the protein expression of Bax and Bcl-2 in the amygdala. Results:Compared with the liquiritin group, citalopram group and normal control group, the body weight and sucrose solution preference of rats in the stroke group, PSD group and normal saline control group were decreased, and the horizontal and vertical movements in open field test were decreased; the differences were statistically significant (all P<0.01). TUNEL staining results showed that compared with the liquiritin group, citalopram group and normal control group, the number of apoptotic cells was significantly increased in the stroke group, PSD group, and normal saline control group; the difference was statistically significant (all P<0.01). The results of immunofluorescence staining showed that compared with the liquiritin group, citalopram group and normal control group, the number of bcl-2 immunoreactive cells in amygdala of the stroke group, PSD group and normal saline control group was significantly decreased, while the number of Bax immunoreactive cells was significantly increased; the difference was statistically significant (all P<0.01). Western blot analysis showed that compared with the liquiritin group and citalopram group, the expression of bcl 2 protein in amygdala of the stroke group, PSD group and normal saline control group was significantly decreased, while the expression of Bax protein was significantly increased; the difference was statistically significant (all P<0.01). Conclusion:Liquiritin can alleviate the symptoms of PSD, and its mechanism may be related to inhibiting the apoptosis of amygdala cells and regulating the expression of apoptosis-related factors.
5.Relationship among the dream threat level, life events and childhood trauma in obsessive-compulsive patients

ObjectiveTo explore the relationship among dream threat level,life events and childhood trauma in obsessive-compulsive patients.Methods 59 obsessive-compulsive patients and 57 healthy controls were tested with Dream Threat Questionnaire (DTQ),Childhood Trauma Questionnaire ( CTQ ) and Life Events Scale (LES).The dream threat level were compared between the two groups,and correlation and regression analysis were used to research the relationship of the variables for patients’ data.ResultsCompared with the healthy controls,patients had significantly higher DTQ score ( 1.40 ± 2.23 vs 3.31 ± 3.04,P < 0.01 ),CTQ total score (34.21 ± 8.47 vs 43.02 ± 10.78,P < 0.01 ),LES total score ( 16.88 ± 16.42 vs 45.39 ± 46.01,P < 0.01 ) and its negtive life events subscore ( 14.14 ± 14.61 vs 39.56 ± 42.94,P < 0.01 ).Correlation analysis showed that dream threat level,negative life events and childhood trauma were significantly positively correlated each other( r =0.326 ~0.402,P < 0.05 ~0.01 ).Regression analysis indicated that childhood trauma directly predicted dream threat level (β =0.104,P < 0.05 ) and completely mediated the relationship between negative life events and dream threat level.ConclusionObsessive-compulsive patients have higher dream threat level,more childhood trauma experience and life events.Childhood trauma experience is a significant factor influencing dream threat level,and as a complete mediation by which life events influence dream threat level.
6.Dynamic change of population structure of Oncomelania hupensis

Objective To understand the successive dynamic change of population structure of Oncomelania hupensis during a one-year period,so as to provide the evidence for snail control. Methods A river beach and a ditch infested with O. hupensis snails were selected and longitudinally investigated in the midmonth during one year. The snail survey indices included the sur-vival status,gender,number of whorls,length and width of shell,and gonad development status(measured by the color depth of gonad and the length ratio of gonad to liver),and the monthly snail eggs in the soil were collected and counted simultaneous-ly. In addition,the temperature and humidity of the soil and the daily data of air temperature and precipitation were measured or collected during the study period(every month). Results Both survival rate of snails and live snail density at the two environ-ments were positively correlated with the temperatures of air and soil. With a slight bimodal distribution ,the snail survival rate peaked from May to June,and in September. The living snail densities got the highest level in July and September in the river beach,and from April to May in the ditch. The regression equations of snail length(L)and width(W)were Lbeach = 2.355 +1.678W(F=2989.43,P<0.01)and Lditch=0.478+2.091W(F=2.989.43,P<0.01),respectively. The snails were the ones with 4.07-11.81 mm in the length(8.98 ± 0.92)mm in the river beach,and the snails were the ones with 3.63-9.92 mm in the length(7.03 ± 0.82)mm in the ditch. The main snails were the ones with five to eight whorls of shell in the river beach and four to seven whorls of shell in the ditch. The proportions of snails with less than or equal to five whorls(in the river beach)and four (in the ditch)were the highest in May and September,about 20%. The ratios of male and female snails were 1.66 in the river beach and 1.22 in the ditch,respectively. The gonad development status of male and female snails was basically synchronous and had a bimodal abundance period-from April to May and September to October. The numbers of snail eggs in the soil among months were significantly different,reaching the highest in June in the river beach(100.8/0.1 m2),and May in the ditch(82.5/0.1 m2). Conclusion The principal periods of breeding and alternation of generations of snails are April-May and September-October every year,which should also be the optimal time for mollusciciding in schistosomiasis susceptible zones.
7.Risk factors analysis of cervical esophagogastric anastomotic fistula after esophagectomy of esophageal cancer

Objective To investigate the risk factors of cervical esophagogastric anastomotic fistula after esophagectomy of esophageal cancer.Methods The retrospective case-control study was conducted.The clinicopathological data of 956 patients who underwent esophagectomy and cervical esophagogastrostomy from January 2012 to December 2016 in the First Affiliated Hospital of Zhengzhou University were collected.Patients underwent Sweet or Mckeown surgery.Observation indicators:(1) intra-and post-operative situations;(2) the risk factors analysis of cervical esophagogastric anastomotic fistula after esophagectomy;(3) follow-up situations.Follow-up using outpatient examination and telephone interview was performed to detect the esophagogastric anastomotic stenosis of patients up to February 2017.Measurement data with normal distribution were represented as the (x)±-s.Univariate analysis and comparison of count data were done using the chi-square test or Fisher exact probability method.Multivariate analysis was done using the Logistic regression model.Results (1) Intra-and post-operative situations:all the 956 patients underwent successful operations,including 107 with Sweet operation and 849 with Mckeown operation.Of 956 patients,336 received thoracotomy and 620 received thoracoscopic surgery.Tumors located in upper,middle and lower esophagus were respectively detected in 143,627 and 186 patients.Operation time,volume of intraoperative blood loss and number of lymph node dissected in 956 patients were (274 ± 67) minutes,(210 ± 167) mL and 18 ± 11,respectively.Of 956 patients,117 had cervical esophagogastric anastomotic fistula,with an incidence of anastomotic fistula of 12.24% (117/956).Of 117 patients with cervical esophagogastric anastomotic fistula,2 had early stage fistula,110 had middle stage fistula and 5 had later stage fistula;12 were cured by two-tube method (stomach tube and nutrition tube),24 were cured by three-tube method (stomach tube,nutrition tube and chest tube or mediastinal tube),43 were cured by open neck incision dressing,15 were cured by fistula cavity drainage and 17 were cured by esophageal stent implantation.Sixteen patients died in hospital postoperatively,including 6 with cervical esophagogastric anastomotic fistula and 10 without cervical esophagogastric anastomotic fistula.Duration of hospital stay of 956 patients was (16± 11)days,and durations of hospital stay of patients with and without cervical esophagogastric anastomotic fistula were (39± 19) days and (13±6) days.Postoperative pathological examinations:873,9 and 74 patients were respectively diagnosed with squamous cell carcinoma,adenocarcinoma and other types of cancer.TNM staging:stage 0,Ⅰ,Ⅱ,Ⅲ,Ⅳ and unidentified stage were respectively detected in 135,110,325,376,1 and 10 patients.(2) The risk factors analysis of cervical esophagogastric anastomotic fistula after esophagectomy:univariate analysis showed that gender,age,history of diabetes,surgical method,tubular stomach production,operation time,postoperative pulmonary infection and postoperative aspirating sputum through fiberbronchoscope were risk factors affecting cervical esophagogastric anastomotic fistula after esophagectomy,with statistically significant differences (x2 =4.179,6.174,4.427,4.377,6.266,7.057,55.036,51.806,P< 0.05).Multivariate analysis showed that tubular stomach production,postoperative pulmonary infection and aspirating sputum through fiberbronchoscope were independent risk factors affecting cervical esophagogastric anastomotic fistula after esophagectomy,with statistically significant differences (OR =1.922,2.907,2.323,95% confidence interval:l.203-3.070,1.682-5.023,1.235-4.370,P<0.05).(3) Follow-up situations:908 of 956 patients were followed up for 2-62 months,with a median follow-up time of 28 months.During the follow up,21 of 111 patients with cervical esophagogastric anastomotic fistula were complicated with cervical esophagogastric anastomotic stenosis,59 of 797 patients without cervical esophagogastric anastomotic fistula were complicated with cervical esophagogastric anastomotic stenosis,showing a statistically significant difference in cervical esophagogastric anastomotic stenosis (x2-16.803,P<0.05).Conclusion Tubular stomach production,postoperative pulmonary infection,postoperative aspirating sputum through fiberbronchoscope are independent risk factors affecting cervical esophagogastric anastomotic fistula after esophagectomy.
8.Role of miR-23a/PTEN axis in the occurrence and development of colorectal cancer

Objective:To analyze the expression of microRNA-23a (miR-23a) and phosphatase and tensin homolog detected on chromosome ten (PTEN) regulatory axis in LS513 cells, and to explore the effects of miR-23a and PTEN on the occurrence and development of CRC.Methods:LS513 cells were divided into blank control group (NG group) : cells were not specially treated, negative control group (NC group) : 5 μl Lipofectamine3000 and 50 pmol/μl NC were added, Inhibition of miR-23a expression group (miR-23a-inhibitor group) : miR-23a-inhibitor sequence was transfected. The mRNA levels of miR-23a and PTEN in LS513 cells were detected by real-time fluorescence quantitative PCR (qRT-PCR) ; the proliferation of LS513 cells was detected by MTT method; Transwell assay was used to detect cell migration and invasion; Western blotting was used to detect proliferation, migration, invasion and phosphatidylinositol 3 kinase/protein kinase B (PI3K/AKT) signaling pathway related proteins.Results:Compared with those in NG group and NC group, the level of miR-23a in miR-23a inhibitor group was significantly lower ( P<0.05) , and the expression levels of PTEN mRNA and protein were significantly higher ( P<0.05) . The results of MTT showed that, compared with those in NG group and NC group, the proliferation inhibition rate in miR-23a-inhibitor groupwas significantly higher ( P<0.05) , and the expression of PCNA protein was significantly lower ( P<0.05) . Compared with those in NG group and NC group, the migration number and invasion number of LS513 cells in miR-23a-inhibitor group were significantly lower ( P<0.05) , and the expression of MMP-9 and E-cadherin protein was significantly lower ( P<0.05) . Compared with those in NG group and NC group, the expression of p-pi3k/PI3K and p-atk/ATK protein in miR-23a-inhibitor group was significantly lower ( P<0.05) . Conclusion:Inhibition of miR-23a expression may up-regulate PTEN gene expression, inhibit PI3K/Akt pathway activation, and then inhibit the proliferation, migration and invasion of LS513 cells.
9.Relationship between eye expression recognition and social functioning in patients with schizophrenia

Objective To compare eye expression recognition in stable outpatients with schizophrenia with that in normal controls and to explore the relationships between eye expression recognition and social functioning.Methods 107 schizophrenic outpatients and 66 normal controls matched in age,sex and years of education were assessed with Eye Basic Emotion Discrimination Task(EBEDT) and Eye Complex Emotion Discrimination Task(ECEDT).The patients were also assessed with Social Disability Screening Schedule (SDSS).Results The correct numbers were significantly lower for patients to identify basic emotions of eye expressions(13.2±3.8 vs16.0±2.6,P<0.01) and complex emotions of eye expressions(17.9±4.3 vs 20.6±3.5,P<0.01)than those for controls respectively;the correct numbers to identify anger(3.1±1.0 vs.2.1±1.2,P<0.01),fear(1.8±1.0 vs 1.3±1.0,P<0.01) and disgust(1.8±1.1 vs 1.4±1.2,P<0.05)for controls were higher than those for patients significantly.The correct numbers to identify total basic emotions(r=-0.335,P<0.05)and total complex emotions (r=-0.374,P<0.05)in eye expressions showed negatively correlated with the total scores of SDSS in the patients after controlling age and total score of PANSS.Conclusions The ability to recognize basic and complicated emotions in eye expressions in the outpatients with schizophrenia is lower than that in the controls. It shows positively correlated with social functioning moderately in the patients.
10.SCoT-PCR Primer Selection and Reaction System Optimization of DNA of Marchantia convoluta in Guangxi

OBJECTIVE:To establish PCR reaction system of DNA of Marchantia convoluta in Guangxi marked with SCoT polymorphism marker technique by screening primer and optimizing reaction condition. METHODS:Modified CTAB method was used to extract DNA of M. convolute from Guangxi;gel electrophoresis and UV spectrophotometry were used to investigate purity and concentration of DNA. Using sample DNA as template,PCR amplification of 36 SCoT primers was conducted,and suitable primers were screened after electrophoresis,staining and imaging of products. The orthogonal experiment of 5 factors and 4 levels was conducted by 5 main factors as DNA concentration,Mg2 + concentration,dNTP concentration,primer concentration,Taq DNA polymerase concentration. The condition of SCoT-PCR reaction system was optimized. RESULTS:Extracted sample DNA bands were neat without RNA contamination,degradation or dispersion of fluorescence;sample well was clear. UV absorbance ratio ranged 1.7-2.0 at 260 nm and 280 nm;purity and concentration of DNA were both suitable for follow-up test. PCR results of 36 primers showed that product band of No. 4 primer was neat without diffuse fluorescence but with best luminance,so No. 4 primer was used for PCR reaction. The optimal SCoT-PCR reaction system contained 30.00 μg/mL DNA,2.00 mmol/L Mg2+,0.20 mmol/L dNTP, 0.40 μmol/L primer,0.50 U/mL Taq DNA polymerase(total reaction volume of 20 μL). CONCLUSIONS:Suitable SCoT-PCR primer of DNA is screened,and reaction system is optimized. It provides technologic basis for variety identification and genetic relationship analysis of M. convoluta in Guangxi.
