Objective To assess the risk factors for pneumonia in patients with acute stroke. Methods Three hundred and forty patients with acute stroke were studied, and risk factors for pneumonia such as demographics, history of hypertension, diabetes and smoking, initial blood pressure, body temperature, fasting plasma glucose level, C-reactive protein, National Institutes of Health Stroke Scale ( NIHSS) scores, degree of dysphagia and mechanical ventilation were evaluated. Besides, the correlation between incidence of pneumonia and pneumonia scores in patients with acute stroke was studied. Results Pneumonia was developed in 63 patients (18.5%). Logistic regression analysis revealed that age, gender, initial NIHSS scores, dysphagia and mechanical ventilation were independent predictors for pneumonia (P <0.05). The incidence of pneumonia gradually increased with the pneumonia scores. Conclusion The occurrence of pneumonia after acute stroke is associated with older age, male gender, high NIHSS scores, dysphagia and mechanical ventilation. The pneumonia scores can be adopted to identify the risks of pneumonia after acute stroke.

Objective To analyze the clinical features and causes of benign paroxysmal positional vertigo(BPPV).Methods The clinical data of 521 patients with BPPV were collected. The percentage of patients with BPPV in all the outpatients with dizziness and vertigo, the clinical characteristics of BPPV,and the prevalence of comorbidity with migraine were analyzed. Results BPPV accounted for 35.7% of patients with vertigo and 12.1% of patients with dizziness.Among the 521 patients with BPPV,158(30.3%)were male and 363(69.7%)were female, with an average age of(57.5?12.8)years old(20 to 93 years old).Right semicircular canals were involved in 323 cases(62.0%),and left semicircular canals in 187 cases(35.9%).The latency period of nystagmus of vertical canals was(3.22?2.37)s,and time of persistence was(8.31?7.98)s.The latency period of nystagmus of horizontal canals was(2.33?1.50)s,and the time of persistence was(14.77?11.40)s.There were significant differences in the latency period of nystagmus and time of persistence between the vertical and horizontal canals(P=0.001 and P=0.000).Fifteen patients(2.9%)had a history of head trauma and 39(7.5%)were complicated with migraine. Conclusion BPPV is prevalent in patients with dizziness and vertigo, and misdiagnosis and missed diagnosis should be avoided.

Objective To investigate the risk factors for deep vein thrombosis (DVT) of lower limbs after laparoscopic surgery. Methods A total of 230 patients underwent laparoscopic surgery were selected in Huanggang Central Hospital from January 2014 to August 2016.The patients were divided into DVT group and non-DVT group according to DVT of the lower limbs, and the related risk factors for DVT of lower limbs after laparoscopic surgery were analyzed. Results In the 230 patients, there were 10 patients with DVT of lower limbs and 220 patients without DVT of lower limbs, the incidence of DVT of lower limbs was 4.35% (10/230).Logistic regression analysis showed that age ≥40 years old, operation time ≥120 min, postoperative ambulation time ≥ 30 h,the intraoperative position of high-head and low-foot,plasma D-Dimer level ≥0.5 mg · L-1 were the independent risk factors for DVT of lower limbs after laparoscopic surgery (P < 0.05).Conclusion There is a certain incidence of DVT of the lower limbs after laparoscopic surgery. The effective prevention measures should be taken to reduce the incidence of DVT after laparoscopic surgery according to the related factors of DVT.

Journal of International Pharmaceutical Research 2015;(4):483-487
Under physiological conditions, many vital functions of the body are controlled by transient release of bioactive substances at a specific time and site. Based on the circadian rhythm character of disease and chronotherapeutic conceptions, pulsatile delivery system has been designed to achieve optimal therapeutic effect and reduce the toxic and side-effect. In recent years, more and more studies are focused on the pulsatile multiparticulate drug delivery system. Pulsatile multiparticulate system possesses many benefits, such as no risk of dose dumping, predictable gastric emptying, flexible release patterns and increased bioavailability. Based on these premises, the aim of this review is to summarize the major design methods of pulsatile multiparticulate and the research progress.

Objective To evaluate if the enteral nutrition have effect on the immune function and in- flammatory reaction after operating about gastric carcinoma. Methods 58 postoperative patients suffering from stomach cancer and colon cancer were randomly divided into the EN group and the TPN group. On the first postoperative day, nutrition fibre were given via nasal intestinal tube, increasing the capacity and drop- ping speed day by day until patients can eat liquid diet. While patients in the TPN group didn't eat anything until enterokinesia completely recovered.Observing on preoperative day 1 and on postoperative day 3 and day 8 respectively to check IgA,IgG,IgM,C3,C4,CRP,LYM,LYM%,TP,ALB,PA.Results The results showed that on the postoperative day 8,the target ascension extent was higher than that in the PN group. The statistical significance was very obvious(P

Objective Through the application of recombinant human Ⅱ tumor necrosis factor-Fc function protein (rhTNFR:Fc) in the treatment of juvenile idiopathic arthritis (JIA) with randomized control study, clinical characteristic and clinical effect were summarized. Methods According to the randomized controlled principle,124 patients with JIA were divided into control group and treatment group. The basic treatment in two groups were one antirheumatic slow-acting drug, nonsteroidal drug, adrenal cortical hormone. There were no significant differences between clinical type and basic treatment in two groups (P > 0.05).Sixty-two patients of JIA treated with rhTNFR:Fc by subcutaneous injection. The doses was 0.8mg /kg per week. There were 17 cases of oligoarthritis,15 cases of polyarthritis,30 cases of systemic arthritis in the treatment group and control group respectively. The basic antirheumatic drugs, nonsteroidal anti-inflamatory drugs ( NSAIDs),adrenal cortex hormone were allowed to continued.Clinical evaluation index included ACR Pedi 30,ACR Pedi 50 and ACR Pedi 70.The adverse drug reactions were recorded. Results The remission rate of ACR Pedi 30,50,70 in 2 weeks, one month, three monthes and six monthes were different in types of JIA patients in the treatment group ( P < 0.05 ).The remission rate of systemic arthritis was lower than the other two groups of arthritis ( P < 0.05 ).Only 44% ACR Pedi 50 remission was achieved after three monthes medication in systemic arthritis and 41.7% ACR Pedi 50,29.2% ACR Pedi 70 were achieved after six monthes. The remission rate in the types of oligoarthritis and polyarthritis at different time points (2 weeks, one month, three monthes, six monthes) of ACR Pedi 30,50,70 were similar. After six monthes, more than 80% reached ACR Pedi 50 remission, more than half of patients reached ACR Pedi 70 remission.Three cases of macrophage activation syndrome in systemic arthritis group was effective treated with rhTNFR:Fc.In the treatment group,2 cases of systemic arthritis appeared ache and discomfort after one week treatment,3 cases appeared repeated mild upper respiratory tract infection and diarrhea during treatment, including one varicella infection. The incidence of adverse reactions in the treatment group of systemic arthritis were 16.7%,Other 2 types of patients did not show adverse reaction during rhTNFR:Fc treatment. Conclusion rhTNFR:Fc has good effect on oligoarthritis and polyarthritis of JIA.The adverse reactions of six monthes were rare. The cases of systemic arthritis could reach some clinical remission, but need to guard against infection and the occurrence of adverse reactions. To whom did not respond to conventional therapy in systemic arthritis or systemic arthritis combined with macrophage activation syndrome,it could be considered with rhTNFR:Fc.
