1.Efficacy of celecoxib for postoperative analgesia after endoscopic nasal surgery

Objective To investigated the efficacy of celecoxib for postoperative analgesia after endoscopic nasal surgery. Methods One hundred and twenty ASA Ⅰ or Ⅱ patients of both sexes aged 18-64 yr were randomly assigned to one of three groups ( n = 40 each): control group ( group C); celecoxib 200 group ( group CEL1 ) and celecoxib 400 group ( group CEL2 ). The patients took celecoxib 200 and 400 mg by mouth at 1 h before induction of anesthesia in group CEL1 and CEL2 respectively. Anesthesia was induced with midazolam,propofol, remifentanil and rocuronium and maintained with iv propofol and remifentanil infusion. VAS was used to assess postoperative pain (0 = no pain, 10 = worst pain). In PACU when VAS score ≥3, the patients were given fentany125 μg iv every 5-10 min until the VAS score < 3. After being discharged from PACU, the patients received celecoxib 200 mg every 12 h for 5 days in the 2 celecoxib groups. Oxycodone 5 mg was used as rescue analgesic when VAS score ≥ 4 until the VAS score < 4. The number of patients who needed fentanyl for analgesia in PACU, the number of patients who needed oxycodone within 6 h, 6-24 h and day 2-5 after operation and side effects after operation were recorded. Analgesic efficacy was assessed at day 5 after operation and the satisfactory rate of patients calculated. Blood samples were obtained at 0, 6 and 48 h after operation for determination of PGE2,6-k-PGF1α and TXB2 concentrations. TXB2/6-k-PGF1α was calculated. Results The number of patients who needed fentanyl in PACU and the number of patients who needed oxycodone within 6 h and 6-24 h after operation were less, the satisfactory rate was significantly higher, and PGE2 concentrations in blood were significantly lower at 48 h after operation in the 2 celecoxib groups than in group C ( P < 0.05 or 0.01 ). There was no significant difference in each index between the 2 celecoxib groups ( P > 0.05 ). There was no significant difference in TXB2/6-k-PGF1α at each time point among the three groups. Nausea and vomiting occurred in one patient after operation in group C, but side effects were not observed in the 2 celecoxib groups. Conclusion Celecoxib given before and after operation can effectively relieve postoperative pain after endoscopic nasal operation by reducing PGE2 concentrations in blood.

AIM: To study the mechanism for biological effect of ? 1-adrenoceptors by inves tigating the molecular mechanism involving in the apoptotic processi on in myocardium. METHODS: The expressions of Bcl-2, Bax, cytochrome C, caspase-3 were determined by means of immunohistochemistry and molecular technique. RESULTS: Bcl-2 was increased but apoptosis, Bax, cytochrome C, caspase-3 were decreased when phenylephrine was used before ischemia. CONCLUSION: ? 1 -adrenoceptors may influence the expression of Bcl-2 ,change the permeabil ity of mitochondrion and adjust the expression of apoptotic protein to control apoptosis.
3.Study on Quanlity Standard of Youqing Granules

Objective To establish the quality control method for Youqing Granules. Methods Qualitation discrimination of Rhizoma Cyperi and Raidix Paeoniae in this formula was identified by TLC. Quantitive analysis of Astragaloside A which is the effective element of the formula was adopted by RP-HPLC-ELSD. Result The specks in TLC were clear and easy to discriminate. The precision and reproducibility of the RP-HPLC-ELSD method were fine. In 1.032~5.16 ?g, the content of Astragaloside A had good linear relationship. The average recovery rate of sample was 99.0516 %, and RSD=1.05%. Conclusion The quality standard established on this study can control the quality of the products, the method is viable.
4.Relationship between Helicobacter Pylori and Pathological Lesiens in 458 Bioptic Specimens from gastric Mucosa in children

Objectives To search for the relationship between HP and lesions of gastric mucosa of children.Methods Four hundred and fifty-eight bioptic specimens-from eastrie mueosa in children were strained with hematoxylin-eosin(HE)and Warthin-Starry for light microscopic study.Results HP was detected in 152 specimens. HP deteetable rate-in specimens with normal mucosa was 1. 1%, in compari-on with 42.3% in those with chronic superficial gastritis and 14. 3% in those with chronic atrophic gastrins .Specimens with chronic active gastritls, mostry manitesting local active lesions,had HP detectable rate of as high as 80.2%、69. 7% of the specimene with HP were found to have obvious lymph follicles reaction within lamina propria in their gastric mucosa.Conclusions There is a close relationship between HP infection and Pediatric chronic gastritis(especially chronic active gastritis). Histopathologically, HP-associated gastritis is characterized by chronic and local active inflamations, and tbe formation of lymph follicles within lamina propria.
5.Changing trend and influencing factors of health literacy among residents in Ningbo from 2015 to 2019

Objective:
To learn the health literacy status, trend and associated factors of Ningbo residents from 2015 to 2019, so as to provide basis for developing health policies and interventions.
Methods:
The monitoring data of health literacy of Ningbo residents from 2015 to 2019 was collected. The health literacy level was calculated and standardized by the population data of the sixth national census of Ningbo in 2010; five-year absolute growth and growth rate were used to reflect the changing trend. Multivariate logistic regression model was employed to analyze the influencing factors for the health literacy level.
Results:
The health literacy levels from 2015 to 2019 were 15.44%, 21.73%, 22.41%, 27.60% and 30.03%, with an annual increase trend ( P<0.05 ). The five-year absolute growth and growth rate were 14.59% and 94.49%. The Results of multivariate logistic regression analysis indicated that the year ( OR=1.158, 95%CI: 1.132-1.184 ), age ( 25-<35岁 years, OR=1.235, 95%CI: 1.039-1.468; 35-<45岁 years, OR=1.416, 95%CI: 1.193-1.681; 45-<55岁 years, OR=1.221, 95%CI: 1.024-1.455 ) , education level ( primary school, OR=1.790, 95%CI: 1.461-2.195; junior high school, OR=2.574, 95%CI: 2.102-3.154; high school/vocational high school/technical secondary school, OR=4.863, 95%CI: 3.943-5.998; college or above, OR=8.829, 95%CI: 7.109-10.965 ) , urban areas ( OR=0.934, 95%CI: 0.874-0.998 ) and occupation ( farmers, OR=0.692, 95%CI: 0.608-0.787; workers, OR=0.746, 95%CI: 0.664-0.837; enterprise staff, OR=0.822, 95%CI: 0.745-0.906; others, OR=1.106, 95%CI: 1.009-1.211 ) were the influencing factors for health literacy level.
Conclusion
The health literacy level of Ningbo residents shows an upward trend from 2015 to 2019, which are associated with age, education level, area and occupation.
6.Ethical Problems and Countermeasures of Medical Support at Chinese Second-Grade Peacekeeping Hospital in Democratic Republic of the Congo

The particularity and complexity of medical support in the United Nations peacekeeping operations bring various ethical problems to medical staff of the second-grade peacekeeping hospitals during their work.This paper analyzed the ethical dilemma in terms of the relevant provisions of the United Nations,the medical humanitarianism principle,lack of medical resources and the principle of fairness,emergency treatment and informed consent,cultural differences and the principle of optimal health,occupational protection requirements and respect to the human rights.In addition,it discussed the countermeasures from personnel selection,the laws and regulations,the training of medical norms and foreign customs,the concept of “patient-centered”,and the humanitarian treatment,providing a theoretical basis for solving the problems of medical support in peacekeeping operations.
7.Effect of Early Rehabilitation on Motor and Cognitive Function of Hemiplegia after Stroke

ObjectiveTo study the effect of early rehabilitation on disability of motion and cognition.Methods107 patients were randomly divided into two groups,rehabilitation group (51 cases) and control group (56 cases).Patients in rehabilitation group were given regular early rehabilitation,while all patients in both groups were given usual clinical treatment. The Barthel Index (BI), Fugl-Meyer Assessment (FMA), Mini-mental Status Examination (MMSE) were used to evaluate these two groups before and 3 months after treatment. ResultsAfter treatment, the scores of BI, FMA and MMSE were improved significantly(P<0.05) in rehabilitation group compared with that before treatment, so did that of BI, FMA in control group(P<0.05).The scores of BI、FMMS、MMSE in rehabilitation group improved significantly compared with that in control group(P<0.05).ConclusionEarly rehabilitation may obviously improve the motive and cognitive function and increase the quality of living of hemiplegia after stroke.
8.Application of Transnasal Gastroscope in Children With Upper Gastrointestinal Tract Disorders

Objective To explore the property and diagnostic significancec of transnasal gastroscope(EG-470N5)in children with upper gastrointestinal tract disorders.Methods Sixty children with upper gastrointestinal tract disorders were examined by using EG-470N5.The scope was inserted either through nose(n=28)or via mouth(n=32).Beside the routine preparations,nasal spray of lidocaine or nasal drib of ephedrine was added to those who received nasal intubation.Results Examination of upper gastrointestinal tract subjects in all succed with no obvious side response in 28 cases(46.7%)who received the examination via nose,27 cases(45%)had mild nausea and 5 cases(8.3%)had serious nausea and dysphoria.Children with esophagostenosis and pyloristenosis and 2-day-old neonate were examined without adverse effect.Conclusions Transnasal gastroscope is preponderant to children due to its slim diameter and supple body,especially to those having swallow difficulty and confined tract.It is more favorable practical prospect than routine gastroscope.
9.A Review:the Molecular Mechanisms of InlA- and InlB- mediated Invasion of Listeria monocytogenes into Host Cell

Gram-positive food-borne pathogen Listeria monocytogenes can invade non-phagocytic cells of the hosts by means of the special surface proteins and cause severe systemic infections. Internalins play a key role for Listeria monocytogenes in invading the non-phagocytic cells. In this study we will review and expand upon the recent advances in understanding the molecular mechanisms of InlA- and InlB- mediating the invasion of Listeria monocytogenes into host cells. This paper will also provide the theoretical base for pathogenetic mechanisms, precaution and therapy of food-borne pathogens.
