1.Alveolar soft part sarcoma: a clinicopathological analysis of 21 cases

Purpose To investigate the clinical and pathological features and differential diagnosis of alveolar soft part sarcoma (ASPS).Methods The clinical data were retrospectively analyzed in 21 cases of ASPS. Histopathological and immunohistochemical changes of the tumors were also observed in the paraffin-embedded tissue samples.Results In 21 cases, 11 were male and 10 females. Ages of the patients ranged from 4 to 56 years old,with mean 25.9 years old.The lesions were mainly located in the deep soft tissues of legs.Microscopically,tumor cells with granular cytoplasm arranged in alveolar or solid structures,and were separated by sinusoidal vessels. There existed crystals in the cytoplasm of tumor cells after PAS staining.Immunohistochemically,10 cases were positive for MyoD1, 4 positive for desmin,9 positive for S-100,11 positive for NSE, and 11 positive for Vim. All of ASPS were negative for AE1/AE3,CK, EMA, SMA, MSA and Syn.Conclusions ASPS is a rare malignant neoplasm in adolescents and young adults. It is prone to early hematogenous metastases and a high recurrence rate after conservative surgical excision. Therefore, the prognosis of ASPS is poor. Careful analysis of the clinicopathological features and immunohistochemisitry will be helpful to reach an accurate diagnosis.
2.The Speech Rate in Monologue and Reading in Normal Adults

Objective To study the characteristics and difference of speech rate in monologue and reading in normal adults .Methods Through the acquisition of spontaneous speech and reading from 30 normal adults ,the syl‐lables per minute (SPM ) and the words per minute (WPM )were separately calculated .The monologue materials in‐clude ten topics :self introduction hobby ,hometown and so on;the reading material was the stars in the sky written by Ba Jin .We also analyzed whether there was a significant difference in genders and types of expression for speech rates .Results With the expression of monologue :SPM (male)=226 .13 ± 25 .97 ,SPM (female)=198 .73 ± 40 .With the expression of reading :SPM (male)= 208 .93 .13 ± 29 .35 ,SPM (female)= 220 .53 .± 26 .52 ,WPM (male)=104 .67 ± 13 .43 ,WPM (female)=111 .80 ± 12 .73 .Pearson correlation coefficient of SPM and WPM had reached 0 .926(r=0 .926) in the spontaneous speech and 0 .969 (r=0 .969) in reading .There were no significant differences in both genders (P=0 .42) and types of expression(P=0 .72) .The interaction effect of genders and types of ex‐pression was significant (P=0 .006) .For females ,the rate of reading was significantly higher than monologue (P=0 .025);for monologue ,there was a significant difference between males and females(P=0 .035) .The male was higher than the female in the speed of monologue .Conclusion The statistics revealed a high correlation between SPM and WPM .Genders and types of expression did not play an important role on the speech rate .The interactioneffect of gender and type of expression was significantly high (P=0 .006) .
3.Uncertainty Evaluation of the Determination of Lamotrigine in Human Plasma by LC-MS/MS

Objective:To evaluate the uncertainty in the determination of lamotrigine(LTG)in human plasma by LC-MS/ MS. Methods:The uncertainty sources in the determination of LTG were analyzed and the uncertainty was evaluated and combined. Re-sults:The expanded uncertainty of LTG at low(0. 050 4 μg· ml - 1 )and high(1. 27 μg· ml - 1 )concentrations was 0. 005 18 μg· ml - 1 and 0. 066 4 μg· ml - 1 ,respectively(P = 95% ,k = 2). Conclusion:The uncertainty in the determination of LTG in human plasma by LC-MS/ MS is mainly caused by the protein precipitation recovery,matrix effect and sample preparation at low concentration, and by the matrix effect,sample preparation and repeatability at high concentration.
4.Application of LC-MS/MS Method for the Determination of Carbamazepine in Human Plasma and Exter-nal Quality Assessment

OBJECTIVE:To establish and validate the method for the determination of carbamazepine (CBZ) in human plas-ma,and to apply the method for external quality assessment. METHODS:After precipitated with acetonitrile,the plasma sample was determined by LC-MS/MS. Using loratadine as internal standard,the determination was performed on Kromasil C18 column with mobile phase consisted of water (containing 0.1% formic acid)-acetonitrile (gradient elution) at flow rate of 0.6 ml/min and column temperature of 40 ℃. The ion transitions under MRM mode by ESI+ ionization were performed at m/z 237.1→194.0 and m/z 383.1→267.0 for CBZ and internal standard,respectively. RESULTS:The linear range of CBZ were 5-1 000 ng/ml (r>0.998). The limit of quantitation was 5 ng/ml. RSDs of inter-day and intra-day were 1.00%-6.42%;relative deviation were -6.93%-0.32%. The external quality assessment of 5 samples were 679.0,475.0,104.0,29.2 and 26.2 ng/ml,respectively. The pass rate of assess-ment result was 100%. CONCLUSIONS:The method is sensitive,accurate and specific. The method is applicable for the plasma concentration determination and external quality assessment of CBZ.
5.Uncertainty Evaluation in Carbamazepine Determination in Human Plasma by HPLC-MS/MS

Objective:To evaluate the uncertainty in carbamazepine ( CBZ) determination in human plasma by HPLC-MS/MS. Methods:The whole process of CBZ determination was analyzed and the uncertainty sources were established, and then the uncertainty was evaluated and combined, and the expanded uncertainty was also calculated. Results: The expanded uncertainty of CBZ with low (7.46 ng·ml-1) and high (745 ng·ml-1) levels was 0.410 ng·ml-1 and 33.400 ng·ml-1, respectively (P=95%, k=2). Conclusion:The uncertainty in CBZ determination in human plasma by HPLC-MS/MS is mainly caused by recovery, sample prepara-tion and matrix effect for low concentration, and by sample preparation and repeatability for high concentration.
6.Proximal epithelioid sarcoma: a clinicopathological analysis of 6 cases

Purpose To study the clinicopathological features, immunophenotype and differential diagnosis of proximal epithelioid sarcoma (PES).Methods The clinical, radiological and pathological features were reviewed in 6 cases of PES.Results The patients consisted of five males and one female with ages ranging from 32 to 74 years (mean 56 years). Clinically, the tumor mass were progressively-growing solitary nodules with the fuctional disorder and pain. The sites of the tumor involvement were buttock (1 case), thigh (2 cases), iliac spine (1 case), lumbar spine (1 case) and anterior sacrum (1 case). Histologically, the tumors were mainly consisted of epithelioid cells with round or oval nuclei, prominent nucleoli and eosinophilic cytoplasm, some tumor cells appeared rhabdoid features. Immunohistochemically, the tumor cells were positive for vimentin (6/6), CK (5/6), EMA (6/6),β-catenin (6/6), CD99 (5/6), and CD34 (4/6), but negative for S-100 protein, HMB 45, CD 117 and CD 31.Conclusion PES is a distinctive moderate grade malignant tumor with prominent epithelioid and rhabdoid cells appearance.The differential diagnoses include the tumors that have similar cellular appearance with PES, and immunohistochemical staining is useful for diagnosis and differential diagnosis.

9.Mechanism of 5-hydroxytryptamine induced calcium signaling in cultured rat stomach fundus smooth muscle cells

AIM To get an insight into intracellular signaling steps, a very early step in the signaling cascade, the biphasic Ca 2+ elicited by 5 HT in rat stomach fundus smooth muscle cells was investigated. METHODS Cells were cultured and loaded with Fluo 3 AM. [Ca 2+ ] i was measured by fluorescent intensity (FI) in each cell with confocal microscopy. RESULTS The resting FI level of SFSMC was 264?15. Stimulation of SFSMCs by 5 HT produced an elevation of [Ca 2+ ] i; Depletion of external Ca 2+ by addition of EGTA led to a significant attenuation of [Ca 2+ ] i change induced by 5 HT; Pre treatment of SFSMCs with ryanodine (10 ?mol?L -1 , 5 min) in D Hanks, the effect of 5 HT was completely inhibited; The stimulation of SFSMCs by 5 HT was partly attenuated by miaserin(10 ?mol?L -1 ), however, L type Ca 2+ channel antagonist lacidipine and G protein inhibitor NEM completely abolished the increase of [Ca 2+ ] i mediated by 5 HT; 5 HT mediated Ca 2+ release was reduced by phospholipase C specific inhibitor compound 48/80(1 2 ?g?ml -1 ); When protein kinase C was activated by phorbol 12 myristate 13 acetate (PMA 0 1 ?mol?L -1 , 5 min) the effect of 5 HT was inhibited, and the inhibitory effect of PMA was reversed by D sphingosine, a PKC inhibitor. CONCLUSION Our data suggest that G protein coupled 5 HT 2B receptor in the rat stomach fundus modulates 5 HT stimulated Ca 2+ increase, and it is coupled to calcium influx through L type calcium channels, and also intracellular calcium release by the opening of ryanodine receptor. The 5 HT 2B receptor mediated signal of 5 HT is transduced by PLC and PKC.
10.Early therapeutic effect of coronary artery bypass grafting with and without cardiopulmonary bypass

Objective:To evaluate whether off pump coronary artery bypass grafting (OPCABG) has advantages over conventional coronary artery bypass grafting(CCABG). Methods:A total of 362 patients undergoing coronary artery bypass grafting were divided into 2 groups: OPCABG group:152 cases undergoing OPCABG and beating heart via sternotomy; CCABG group,210 cases undergoing CCABG with extracorporeal circulation. Preoperative and postoperative variables of 2 groups were compared. Results:There were no significant differences in sex, age, weight, acute or remote myocardium infarction, hypertension, diabetes and type of bypass grafts between 2 groups.The numbers of bridge vessels were not significantly different between the 2 groups; the incidences of low cardiac output syndrome, atrial fibrillation and atelectasis in OPCABG group were significantly less than those of CCABG group( P
