1.Clinical effect of two kinds of infusion jejunum nutrition colostomy of patients after Whipple operation

Objective To compare the effect of enteral nutrition by jejunum colostomy nutrition infusion pump of patients after Whipple surgery as well as reduce adverse reactions in patients.Methods Sixty-five cases with the implementation of Whipple and jejunum of colostomy were selected as our subjects,who were hospitalized in the Affiliated hospital of Hebei United University from Feb.2009 to Nov.2013.All patients were divided into observation group (33 cases) and control group (32 cases) according to the methods of nutrient input.Patients in observation group were given nutrition infusion pump pumping (15 to 50 ml/h) ;and patients in control group were adopted disposable infusion connection infusion with the speed of 30 drops/min with the thermostat heating temperature and the water pipe.The blood glucose,serum albumin,blood electrolyte concentration of postoperative,and the adverse reactions during input nutrient solution including vomiting,abdominal distention,diarrhea and other adverse circumstance were recorded.Results At 1st,3rd,5th day,there was no statistically significant difference in terms of the levels of glucose,blood albumin,blood C1,Na +,K + between two groups(blood glucose:F inner grouP =3.01,P > 0.05 ; F between group =2.90,P > 0.05 ; F cross group =2.87,P > 0.05 ; serum albumin:F inner group =2.94,P > 0.05 ; F between group =2.89,P > 0.05 ; F cross group =2.76,P > 0.05 ; blood Cl:F inner group =1.78,P > 0.05 ; F between group =1.96,P > 0.05 ; F cross group =1.88,P > 0.05 ; blood Na +:F inner group =1.06,P > 0.05 ; F between group =1.35,P > 0.05 ; F cross group =1.27,P > 0.05 ; blood K +:F inner group =3.12,P > 0.05 ; F between group =3.04,P > 0.05 ; F cross group =2.93,P > 0.05).There were significant differences regarding of the rate of vomiting,abdominal distention,diarrhea and other adverse conditions compared with the infusion enteral nutrition has good clinical effect,postoperative blood (x2 =4.029,4.381,4.905 respectively; P < 0.05).Conclusion The methods of colostomy enteral nutrition with infusion pump after Whipple surgery is proved to be with the better clinical effect in reducing postoperative vomiting,abdominal distention,diarrhea and other adverse conditions compared with the infusion enteral nutrition,and there are no significant difference in the terms of the levels of glucose,blood albumin,blood Cl,Na +,K +.
2.Dengzhan Xixin injection as an adjuvant treatment for angina pectoris: a systematic review and Meta-analysis of randomized controlled trials.

The paper is to systematically evaluate the efficacy and safety of Deng Zhan Xi Xin injection ( DZXXI) as an adjuvant treatment for patients with angina pectoris. The Cochrane Library, Medline, EMbase, CBM, CNKI, VIP, and Wan fang Data base were searched. Randomized controlled trials (RCTs) of DZXXI combined with western medicine routine treatment versus western medicine routine treatment alone for angina pectoris patients were all included. All trials were assessed according to the Cochrane Reviewer' s Handbook 5.1 for Systematic Reviews of Intervention and Meta analyses were performed by RevMan 5. 2 Software. A total of 30RCTs (3 086 patients including 1 572 patients of treatment group and 1 514 patients of control group) were included. Meta-analysis of treatment group compared with control group showed superior effect over reducing cardiovascular events ( OR = 0.33; 95% CI: [0.16, 0.67], P = 0.002, improving effective rate of DZXXI as adjuvant treatment for angina pectoris patients (OR = 3.97; 95% CI: [3.15, 5.02]; P < 0.000 010 and electrocardiogram curative effect (OR = 2.21; 95% CI; [1.83, 2.68]; P < 0.000 010. Funnel figure seemed that there was publication bias. The current limited evidence showed that when compared with the control group, treatment group was superior in improving patients with angina pectoris. But based on the limitations of the study, rigorous design with long follow up clinical trials are necessary for further evidence.

3.Determination of three glycosides from herbs of Swertia punicea by RP-HPLC.

OBJECTIVETo develop a RP-HPLC method for determination of three glycosides in Swertia punicea.
METHODChromatographic column: Alltimal C18 (4.6 mm x 250 mm, 5 microm). Mobile phase: methanol-water (including 0.05% H3PO4), and gradient elution. Flow rate: 1 mL x min(-1). Wavelength: 254 nm. Column temperture: 30 degrees C.
RESULTThe calibration curves of gentiopicroside, mangiferin and swertrianolin were in good linearity over the range of 31.3-281.7, 0.31-2.78, 0.55-4.91 microg, (r = 0.9996, 0.9993, 0.9995). The average recoveries were 103.36%, 101.42% and 97.39%, with RSD less then 3% (n = 5).
CONCLUSIONIt is a simple and sensitive meathod in controlling the quality of S. punicea.

4.Survey on health-promoting lifestyle of kidney transplant recipients

Objective By discussing the state of lifestyle as well as affecting factors of kidney transplant recipients,to provide more health education content and basis for their physical and psychological health.Method With internationally accepted Health-Promoting Lifestyle Profile (HPLP),we surveyed the lifestyle of the selected 104 long-term follow-up cases of kidney transplant recipients.At the same time,163 sex-and age-matched healthy volunteers,who had no obvious abnormalities in the medical test,were chosen.Result For HPLP scores among the 104 cases of recipients,20 cases were excellent,67 cases good,17 cases common,and none was inferior.As a whole,its excellent rate was 83.65%.As for 129 healthy volunteers,their overall excellent rate was 70.55%.The HPLP scores for the nutrition behavior ranked top in kidney transplant recipients,followed by healthy responsibility behavior,and lowest for exercise behavior.For the healthy volunteers,the HPLP scores for interpersonal support behavior ranked top,followed by nutrition behavior,and lowest for healthy responsibility.Correlation analysis revealed that the HPLP scores in kidney transplant recipients were significantly and positively correlated with age (r =0.307,P =0.002) and educational level (r=0.370,P =0.000),and not with gender,ethnicity,occupation and kidney sources (P>0.05).The HPLP scores in idney transplant recipients were higher than those in healthy volunteers,among which self-actualization and healthy responsibility showed statistically significant differences (P < 0.05),there was no significant difference in exercise,nutrition,interpersonal support and stress management between recipients and healthy volunteers (P>0.05).Conclusion The HPLP scores in kidney transplants was higher than in healthy volunteers,thereinto,stress management behavior and exercise behavior were relatively weak,which were the focused improvement projects of lifestyle of kidney transplant recipients.
5.MRI demonstrates abnormalities of motor nerves and extraocular muscles in congenital fibrosis syndrome due to mutations in KIF21 A

AIM:To observe the structural basis of ocular motility abnormalities in patients with congenital fibrosis of the extraocular muscles type Ⅰ ( CFEOM Ⅰ) due to missense mutations in the developmental kinesin KIF21A using high - resolution magnetic resonance imaging ( MRI) .
METHODS: Totally 11 affected individuals reported KIF21A mutations were correlated with MRI studies demonstrating extraocular muscles ( EOMs ) size, location, contractility, and innervation. EOMs and the motor nerve in the orbits were imaged with T1 weighted in a triplanar scan using a dual-phased coils with 2. 0mm thick. Motor nerves were imaged at the brainstem using head coils and 3D-FIESTA with 0. 6-mm thick.
RESULTS: Patients with CFEOM Ⅰ exhibited different degrees of hypoplasia of oculomotor nerve, the abducens nerve and the trochlear nerve were also affected, of which 8 cases of orbital section could see the signal of abnormal nerve dominated by oculomotor nerve to lateral rectus. The both sides of six EOMS in all patients exhibited variable atrophy and abnormal bright internal signal on T1 imaging, particularly severe for the superior rectus and levator muscles.
CONCLUSION: High - resolution MRI can directly demonstrate pathology of motor nerves,affected EOMs, and ‘Pulley' hypoplasia caused by CFEOM Ⅰ due to mutations in KIF21A,and these findings suggest that the neuronal hypoplasia is the etiological factor of CFEOM.
6.Clinical significance of setting nasojejunal nutrition tube in Whipple operation

Objective To investigate the clinical significance of setting nasojejunal nutrition tube in Whipple operation,which can promote patient recovery and prevent complications.Methods Fourty-one patients were undergone Whipple operation and they were randomly divided into eternal nutrition (EN) and parenteral nutrition group (PN).Patients in EN group were set nasojejunal nutrition tube during Whipple operation in order to supply eternal nutrition at earlier period,while in PN group were supplied parenteral nutrition in earlier period.The operative procedure time,the complication in post-operation,the cost of hospitalization and the periods of hospitalization of two groups were recorded.The scores of nutritional status were measured.Results The operative time in EN group and PN group were (200.71 ±51.33)min and (160.48 ± 47.62) min,and no significant difference was found between two groups (t =-1.524,P > 0.05).The hospitalization expenses in EN group was (38835.65 ± 537.69)yuan,lower than that in PN group ((47833.18 ±659.24) yuan,t =2.073,P < 0.05).The hospital periods of EN group and PN group were 10 (3) d and 18(3) d,and the difference was significant (Z =-5.374,P <0.001).There were 2 cases who occurred complications after operation in EN group,including 1 cases with incision infection,and 1 cases of pancreatic fistula.Nine cases occurred complications in PN group,including 3 cases with incision infection,2 cases with pancreatic fistula,2 cases with pulmonary infection,2 cases with gastric paralysis,and all were cured by conservative treatment.There was statistically significant difference in the incidence of postoperative complications (P =0.027).All cases with complication were recovery after corresponding treatment.At 7th day after operation,hemoglobin in EN group and PN group were (113.09 ± 12.35) g/L and (107.04 ± 11.81) g/L,and the difference was significant (t =2.035,P < 0.05).The levels of retinol binding protein,serumalbumin,serum prealbumin,serum transferrin in EN group were (42.62 ± 5.64) mg/L,(40.24 ± 6.79) g/L,(321.43±31.28) mg/L,(32.86±4.67) mg/L,(32.86 ±4.67) mg/L,significant different from those in PN group ((15.50 ± 4.26) mg/L,(31.52 ± 5.92) g/L; (197.86 ± 37.71) mg/L,(23.59 ± 4.32) mg/L; t=2.398,2.606,2.119,2.569; P <0.01 or P < 0.05).Conclusion Nutritional status and prognosis are improved obviously by setting nasojejunal nutrition tube in Whipple operation.And the cost of hospitalization,the periods of hospitalization are decrease.

8.Optimization for ISSR-PCR system of traditional Chinese medicine Lysimachia christinae by orthogonal design.

In order to establish the stable andreliable ISSR-PCR System of Lysimachia christinae, L16 (4(5)) orthogonal design, which based on 7 levels of single factor experiment, were used in this study. The variance analysis was carried out by SPSS 19.0, and 5 main factors affecting the reaction system were optimized in 4 levels. The best annealing temperature was selected by the optimized reaction system. And the stability and reliability of this system was tested by 23 samples from different origins. The results showed that the five factors (DNA template, primer, dNTP, Mg2+ and Taq enzyme) were the most impacts on the amplified results of ISSR-PCR of L. christinae. The order of the influence was: primer > Taq enzyme > DNA template > Mg2+ > dNTP. The optimal system, which was determined by multiple comparison on different levels of each factor, was total volume of 25 microL, including DNA template 60 ng, primer 0.3 micromol x L(-1), dNTP 0.2 mmol x L(-1), Mg2+ 1.8 mmol x L(-1), Taq enzyme 1.25 U. The optimal system was stable and reliable tested by 23 samples from different origins. This study lays the foundation for genetic diversity analysis, fine varieties selection and molecular identification of L. christinae, and provides reference for optimization on ISSR-PCR system of other speciesin future.

9. Analgesic alkaloids from Urticae Fissae Herba

Objective: To investigate the analgesic substances in the aerial part of Urtica fissa (Urticae Fissae Herba), commonly used for rheumatoid and rheumatism arthritis. Methods: The analgesic constituents were isolated with the active guidance of hot plate and acetic acid writhing models, and identified by comprehensive spectroscopic analysis. Results: Thirteen alkaloids (1–13), two lignans (14, 15), and three amides (16–18) were isolated from the active fractions. Among them, compound 1 was a new alkaloid, and compound 6 was a new natural product. The activity evaluation in vivo indicated that various pyrrole alkaloids (1, 3, 6, and 12) possessed significant analgesic activities, they could significantly inhibit the mice pain response induced by acetic acid and hot plate at the dosage of 2 mg/kg BW. Conclusion: The study revealed that the pyrrole alkaloids played important roles in the analgesic activities of Urticae Fissae Herba.
10.Effect of total alkaloids of Rubus alceaefolius poiron on gene expressions of CYP2E1 and CYP3A1 in rats with acute liver injury.

OBJECTIVETo explore the effects of total alkaloids of Rubus alceaefolius Poiron (RAP) on gene expressions of drug-metabolic enzymes, CYP2E1 and CYP3A1 in liver.
METHODSSixty SD rats were randomly divided into six groups (10 rats in each), the blank control group, the model control group, the bifendate group and the three RAP treated groups treated respectively with low-, middle- and high-dose of RAP. The model of acute hepatic injury was established with intra-peritoneal injection of carbon tetrachloride. Serum levels of alanine aminotransferase (ALT) and aspartate aminotransferase (AST), and severity of hepatic tissue injury were measured, and the mRNA expressions of CYP2E1 and CYP3A1 in liver tissue were detected by RT-PCR.
RESULTSAs compared with the model group, serum levels of ALT and AST were significantly lower in the high- and middle-dose ARP group (P <0.01), but in the low-dose group, only ALT was significantly lower (P<0.01); the severity of liver injury was milder in the RAP groups (P<0.01); and both CYP2E1 and CYP3A1 mRNA expressions in liver were significantly lower in the bifendate and all RAP treated groups (P<0.01 or P<0.05).
CONCLUSIONRAP could significantly reduce the ALT and AST levels, protect liver cells from injury, and inhibit the mRNA expressions of CYP2E1 and CYP3A1 in liver tissue.
