2.Soluble tumor necrosis factor related apoptosis inducing ligand (sTRAIL) and clinical outcome of HBV infection in human.

OBJECTIVETo explore the role of sTRAIL in the pathogenesis of HBV infection in human being.
METHODSsTRAIL in 153 patients' sera was examined with ELISA. Correlations between sTRAIL and liver functional parameters were analysed.
RESULTSThe levels of sTRAIL in patients with various clinical types of hepatitis B as well as primary hepatocellular carcinoma were all higher than that in normal persons and became almost normal in recovering stage cases. In acute and chronic HBV infection, sTRAIL level was negatively correlated with ALT, AST and total bilirubin levels, and positively correlated with serum albumin.
CONCLUSIONThe results indicated that higher level of sTRAIL expression is correlated with liver damage, and apoptosis induced by sTRAIL is one of the mechanisms of liver damage in HBV infection.

3.Effect of Foxo3a gene over-expression on the development of rat ovarian granulose cells and in prevention of cisplatin-induced ovarian damage in rats.

OBJECTIVETo evaluate the effect of Foxo3a gene over-expression on the development of rat ovarian granulosa cells and in prevention of cisplatin-induced ovarian damage in rats.
METHODSRat ovarian granulose cells released mechanically from the ovaries were cultured in vitro and identified with HE staining and immunohistochemical staining for FSHR. A recombinant adenovirus carrying Foxo3a gene was constructed for infecting the granulose cells, and the cell growth and expressions of cyclin D1, p27, Bax, and Bim were detected; the cell apoptosis and cell cycle changes were detected using Hoechst/PI 33342 staining and flow cytometry, respectively. The transfected cells were challenged with cisplatin and the cell apoptosis was detected with flow cytometry.
RESULTSOver 90% of the cultured cells survived and contained more than 95% ovarian granulose cells. Infection of the cells with the recombinant adenovirus resulted in over-expressions of Foxo3a at the mRNA and protein levels at 36 h and 48 h after the infection, respectively. The infected cells showed suppressed proliferation, increased apoptotic rate and cell cycle arrest in G1 phase with increased expressions of Bim, p27, and cyclin D1 but without significant changes in Bax expression. Cisplatin exposure caused a significantly higher apoptosis rate in the infected cells than in the control cells.
CONCLUSIONOver-expression of Foxo3a gene can promote granulose cell apoptosis by increasing Bim expression and cause cell cycle arrest in G1 phase by increasing cyclin D1 and p27 expressions, but can not prevent the toxic effects of cisplatin on ovarian granulosa cells.

4.Nosocomial septicemia in the early stage after stem cell transplantation in children with major β-thalassemia.

OBJECTIVETo explore the clinical characteristics of nosocomial septicemia in the early stage after hematopoietic stem cell transplantation (HSCT) in children with major β-thalassemia.
METHODSThe clinical data were retrospectively analyzed of 55 consecutive children with major β-thalassemia who developed septicemia early after HSCT between January, 2011 and June, 2016.
RESULTSAmong the total of 416 consecutive children with major β-thalassemia undergoing allogeneic HSCT, the incidence of nosocomial infection early after transplantation was 77.16% (321/416), and 55 (17.13%) children showed positive findings in blood culture test. The infections occurred most commonly in the oral cavity (65.5%), followed by the respiratory tract, intestinal tract and skin. Gram-negative bacteria, including Escherichia coli (27.3%), Klebsiella pneumoniae (21.8%) and Pseudomonas aeruginosa (9.1%), were the most common causes of infections. Fungal (Candida tropicalis) infection caused septicemia in 1 case. Of all the pathogens, extended-spectrum β-lactamase (ESBL)-producing bacteria were found in 6 cases, methicillin-resistant Staphylococcus aureus (MRSA) in 2 cases, and multidrug-resistant (MDR) bacteria in 2 cases.
CONCLUSIONGram-negative bacteria are the major pathogens causing septicemia in children early after HSCT for major β-thalassemia, and the bacteria show a high level of drug resistance. Adequate preventive use of antibiotics and care of the oral cavity, the respiratory tract, and the perianal region following the transplantation are important measures to control nosoconial infection in these children.
5.Resistin levels of serum and follicular fluid in non-obese patients with polycystic ovary syndrome during IVF cycles.

OBJECTIVESTo measure serum and follicular resistin, steroids hormone levels in women with PCOS (polycystic ovary syndrome) (BMI (body mass index)<25 kg/m(2)), to assess possible correlations of resistin to hormonal and metabolic parameters and to analyze the clinical outcomes of in vitro fertilization-embryo transfer (IVF-ET) in women with PCOS and tubal infertility.
STUDY DESIGNWe analyzed the clinical outcomes of IVF-ET in women with PCOS (BMI<25 kg/m(2)) and tubal infertility during the years 2002 to 2004 and compared the serum and follicular fluid resistin levels, estradiol (E(2)), progesterone (P), testosterone (T) levels in 20 PCOS and 20 healthy, age-matched women without PCOS during IVF-stimulated cycles. The correlations between the resistin levels and the outcomes of IVF-ET were evaluated.
RESULTSNo significant differences in resistin levels of either serum or follicular fluid between PCOS and control group were found. However, resistin levels in serum were higher than that in follicular fluid in both groups. Multiple regression analysis showed that resistin levels in serum did not correlate with BMI, estradiol, LH (luteinizing hormone) and insulin level in fasting blood. No significant correlations were found between follicular fluid reisistin levels and fertilization rate, implantation rate, clinical pregnancy rate or early miscarriage rate in both PCOS and control groups.
CONCLUSIONOur results show that resistin does not have correlation with the hormonal and metabolic parameters as well as the outcomes of IVF. These data suggest that resistin is unlikely to be a local determinant factor in steroidogenesis and growth and maturation of oocytes during IVF-ET in lean women with PCOS.

6.The establishment and implementation of the role of specialist stroke nurse

To better coordinate the construction of stroke center in one tertiary hospital,a specialist stroke nurse was hired,whose working scope included participation in rapid treatment of acute stroke patients,health education,stroke programme coordination,stroke quality surveillance,healthcare staff training and so on.Through the establishment of stroke center involving participation of the specialist stroke nurse,rules and regulations of the stroke center were improved while median Door-to-Needle time of intravenous thrombolysis was shortened from 98min in 2009~2010 to 58min in 2015~2016 and the percentage of zero quality defect medical records was improved from 10.8% to 81.1%(P<0.001).The involvement of specialist stroke nurse in coordination of stroke center is a new model for health quality improvement and provides some experience for specialized nurse role development and implementation.There is also a need for standardized training,professional certification and long-term management of specialist stroke nurse.
7.Feasibility assessment of Halcyon 3.0 dual-isocenter IMRT plan in postoperative radiotherapy of radical mastectomy for left side of breast cancer

Objective:To assess the feasibility of the designed dual-isocenter IMRT plans based on Halcyon 3.0 linear accelerator in postoperative radiotherapy of radical mastectomy for left side of breast cancer.Methods:A total of sixteen patients received the postoperative radiotherapy of radical mastectomy for left side of breast cancer at Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from December 2022 to June 2023 were retrospectively selected.The dual-isocenter plans based respectively on Halcyon 3.0 linear accelerator and Truebeam linear accelerator were designed,and the dosimetric parameters included conformity index(CI)values of target region,homogeneity index(HI)values and doses to organs at risk(OAR)of the two plans were calculated as statistic method.And then,the dosimetric performance of Halcyon 3.0 dual-isocenter plan was compared and analyzed.Utilizing two dose verification tools,ArcCHECK and Portal Dosimetry,to assess the precision of beam delivery of Halcyon 3.0 dual-isocenter plan.Results:The Halcyon dual-isocenter plan and the Truebeam single-isocenter plan had similar plan quality.There were not statistically significant differences(P>0.05)in dosimetric parameters such as CI,HI,exposure dose to 2%of the target volume(D2%)and exposure dose to 98%of the target volume(D98%).Compared to the average monitor unit(MU)of Truebeam single-isocenter plan,the MU values of Halcyon 3.0 dual-isocenter plan increased by 366 MU,while the difference was not statistically significant(P>0.05).The Halcyon 3.0 dual-isocenter plan provided comprehensive protection for OAR,which was better than that of the Truebeam single-isocenter plan.The values of the left side of lung volume(V20)that were covered by 20 Gy dose of the Halcyon 3.0 dual-isocenter plan and the Truebeam single-isocenter plan were respectively 20.41%±1.56%and 24.88%±2.95%,and the difference was statistically significant(t=6.413,P<0.05).There were not significant differences in other OAR dosimetric parameters between the two kinds of plans(P>0.05).The verification pass rates of the Halcyon 3.0 dual-isocenter plans on the ArcCHECK and Portal Dosimetry platforms were respectively 97.6%-98.9%and 98.1%-100%when the ratio of dose tolerance(DT)to distance to agreement(DTA)was set at 2%/2 mm.The verification pass rates of the Halcyon 3.0 dual-isocenter plans on the ArcCHECK and Portal Dosimetry platforms were respectively 99.1%-100%and 99.7%-100%when DT/DTA was set at 3%/3 mm.The beam delivery precision of the Halcyon 3.0 dual-isocenter plan could meet verification standards of clinical treatments.Conclusion:The Halcyon 3.0 dual-isocenter IMRT plan demonstrates a higher feasibility in the postoperative radiotherapy of radical mastectomy for left side of breast cancer.The precision of dose delivery of plan is high,and the dose of target area is sufficient,uniform and favorable conformability.It can effectively limit the OAR exposure dose at the same time.
8.Dosimetric evaluation of different aperture shape controller parameters based on the Halcyon 3.0 accelerator in the hippocampal avoidance-whole brain radiotherapy

Objective:To evaluate the dosimetric effects of different aperture shape controller (ASC) parameters based on the Halcyon 3.0 accelerator in the hippocampal avoidance-whole brain radiotherapy (HA-WBRT) plans.Methods:This study enrolled 13 patients treated with WBRT using a Halcyon 3.0 accelerator at the Department of Radiotherapy of the Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine. According to different aperture shape controller (ASC) parameters, the HA-WBRT plans were divided into six groups, namely non, very low, low, moderate, high, and very high ASC settings, marked by AO, AVL, AL, AM, AH, and AVH, respectively. This study presented a statistical evaluation of the effects of different ASC parameters on conformity index (CI), homogeneity index (HI), radiation doses to the hippocampus and other organs at risk, and the plans’ execution efficiency. Furthermore, using two dose verification tools, namely ArcCHECK and Portal Dosimetry, this study delivered a comprehensive analysis of the differences in beam delivery precision of the plans with different ASC parameters.Results:The six groups all met the requirements for clinical treatment. They had similar CI and HI values, with no statistically significant differences ( P > 0.05). The AVH group showed better protection effects on organs at risk. Compared to the control group, the AVL group showed lower Dmax of brainstems but higher Dmax of chiasma opticum ( F = 6.26, 8.04, P < 0.05). Compared to the control group, the AH group showed lower Dmax of eyeballs but higher Dmax of optic nerves ( F = 2.04, 1.37, P < 0.05). In contrast, the AVH group exhibited lower Dmax of brainstems, eyeballs, and lens than the control group ( F = 6.26, 2.04, 2.02, P < 0.05). No statistically significant differences were observed in dosimetric indices of other organs at risk ( P > 0.05). As verified using ArcCHECK and Portal Dosimetry, the γ passing rates of the six groups were over 98% at 2%/2 mm and 100% at 3%/3 mm. The overall γ passing rates verified using ArcCHECK were lower than those verified using Portal Dosimetry. The maximum difference in the monitor unit among the six groups was less than 15, and these groups did not show significant differences in terms of execution efficiency. Conclusions:The HA-WBRT plans based on the Halcyon 3.0 accelerator can meet the requirements for clinical treatment. Different ASC parameters can significantly optimize the dosimetric parameters. Among them, the AVH parameters can highly reduce the radiation dose to organs at risk. Furthermore, different ASC parameters show insignificant effects on beam delivery precision and plan execution efficiency, meeting the verification standards for clinical therapeutic doses.
9.Long-term prognosis of cobalt-chromium sirolimus-eluting stent implantation in patients with coronary heart disease complicated with diabetes mellitus

Objective:To evaluate the long-term efficacy and safety of a novel type of cobalt-chromium sirolimus-eluting stent (CoCr-SES) in patients with coronary artery disease (CAD) complicated with diabetes mellitus (DM ) .Methods:A total of 1 045 Chinese patients with DM were enrolled into a prospective ,multicenter registered research and followed up for the incidence of death ,myocardial infarction (MI) ,blood circulation reconstruction and thrombosis (ST) among other events . The primary endpoint was a major adverse cardiovascular event (MACE) ,which was composed of cardiovascular death ,non-fatal myocardial infarction (MI) and target vessel revascularization (TVR) ,with a clinical follow-up of 3 years .Results:During the 3-year follow-up ,among 1 045 cases of coronary heart disease complicated with diabetes mellitus ,the rate of MACE was 10 .0% ,including 41 cases (3 .9% ) of cardiovascular death ,38 cases (3 .6% ) of non-fatal MI and 25 cases (2 .4% ) of TVR . The incidence of stent thrombosis (ST ) was 1 .1% in 3 years ,including 7 cases (0 .7% ) of early ST and 4 cases (0 .4% ) of advanced ST .Conclusions:The incidence of MACE and ST is low in patients with coronary heart disease complicated with diabetes at 3 years after implantation of CoCr-SES ,providing compelling evidence for the wide use of domestic CoCr-SES in patients with coronary heart disease complicated with diabetes .
10.A biomechanical case study on the optimal orthodontic force on the maxillary canine tooth based on finite element analysis.

Excessive forces may cause root resorption and insufficient forces would introduce no effect in orthodontics. The objective of this study was to investigate the optimal orthodontic forces on a maxillary canine, using hydrostatic stress and logarithmic strain of the periodontal ligament (PDL) as indicators. Finite element models of a maxillary canine and surrounding tissues were developed. Distal translation/tipping forces, labial translation/tipping forces, and extrusion forces ranging from 0 to 300 g (100 g=0.98 N) were applied to the canine, as well as the force moment around the canine long axis ranging from 0 to 300 g·mm. The stress/strain of the PDL was quantified by nonlinear finite element analysis, and an absolute stress range between 0.47 kPa (capillary pressure) and 12.8 kPa (80% of human systolic blood pressure) was considered to be optimal, whereas an absolute strain exceeding 0.24% (80% of peak strain during canine maximal moving velocity) was considered optimal strain. The stress/strain distributions within the PDL were acquired for various canine movements, and the optimal orthodontic forces were calculated. As a result the optimal tipping forces (40-44 g for distal-direction and 28-32 g for labial-direction) were smaller than the translation forces (130-137 g for distal-direction and 110-124 g for labial-direction). In addition, the optimal forces for labial-direction motion (110-124 g for translation and 28-32 g for tipping) were smaller than those for distal-direction motion (130-137 g for translation and 40-44 g for tipping). Compared with previous results, the force interval was smaller than before and was therefore more conducive to the guidance of clinical treatment. The finite element analysis results provide new insights into orthodontic biomechanics and could help to optimize orthodontic treatment plans.
