1.Cases Analysis of Clinical Pharmacists Participating in the Treatment of Postoperative Drug-resistant Bacterial Infection

Objective: To explore the role of clinical pharmacists participating in the treatment of postoperative drug-resistant bacterial infection through cases analysis of postoperative infection consultation performed by clinical pharmacists.Methods: Clinical pharmacists help surgeons make out the anti-infection medication for 3 cases of postoperative drug-resistant bacterial infection according to the related guideline and mastered antibacterial spectrum and pharmacokinetics of antibiotics, as well as performed the pharmaceutical care. Results: The three cases of postoperative infection were all recovered with the help of participation of clinical pharmacists in the antimicrobial treatment.Conclusion: Clinical pharmacists play an indispensable role by providing pharmaceutical care for surgical therapy team.
2.Effect of Nerve Growth Factor on Expression of Caspase-3 in Hippocampus of Rats with Seizures Induced by Kainic Acid

Objective To observe the effect of nerve growth factor(NGF) on Caspase-3 expression in hippocampus of epileptic rats induced by kainic acid(KA) and explore the protective effect and mechanisms of NGF on the epileptic neurocytes.Methods Sixty healthy rats aged 21 to 30 days were randomly divided into 3 groups,each of which included 20 rats.Rats in group A were treated with 9 g?L-1 sodium,rats in group B were treated with KA,rats in group C were treated with KA and NGF.Concentration of KA was 2 g?L-1 diluted by 9 g?L-1 sodium;KA was injected into rats in group B and group C,intraperitoneally.Then state of seizure was recorded,and EEG and pathological section were performed.Five rats in each group were sacrificed by perfusion with paraform respectively at 6 hour,on the 1st day,on the 3rd day after injection of KA.Specimen was sampled and made into frozen section.Expression of Caspase-3 in hippocampus of epileptic rats was analyzed by immunohistochemistry.And microscope image measuring and analysis instrument was used to analyze image of expression.Results Symptoms of seizure appeared 30 min after injection of KA.Seizure was proved by detection of EEG and pathological section.Expressions of Caspase-3 detected by immunohistochemistry in group C decreased at every time spot were higher than those in group B,and the defferences between 2 groups had statistically significant(Pa

4.Lymph node metastasis pattern in patients with T2 and middle thoracic esophageal squamous cell carcinoma

Objective To investigate the pattern of lymph node metastasis in patients with 17 and middle thoracic esophageal squamous cell carcinoma( ESCC). Methods Retrospective review the clinical data of 246 cases with T2 and middle thoracic esophageal squamous cell carcinoma who were treated by three-field lymphadenectomy. Analyze the relationship between clinical pathological factors and lymph node metastasis. Results Lymph node metastases were found in 129 of the 246 patients (52.4% ).The average number of resected lymph nodes was 25 per patient (rangel5 -59). The rates of lymph node metastasis were 28.9% in the neck, 28.5% in thoracic mediastinum and 22.0% in abdominal cavity for patients with T2 and middle thoracic ESCC. No significant difference in lymph node metastasis' rate was observed among the neck, thoracic mediastinum and abdominal cavity. Logistic-regression showed the length of tumor, tumor cell differentiation and angiolymphatic invasion were factors influencing lymph node metastasis. Conclusion Lymph node metastasis in T2 and middle thoracic ESCC has the characteristics of upward, downward and skip spreading. Patients with T2 and middle thoracic ESCC should be treated with radical surgery with three-field lymphadenectomy.
5.Association between leukemia and genes detected using biomedical text mining tools

Five genes that are closely related with leukemia were detected and identified using COREMINE Medi-cal, and the abstracts of related papers covered in PubMed were analyzed with the biomedical text mining tool, Chilibot, which showed that leukemia interacts with the 5 genes detected using COREMINE Medical.
6.Research on effect of endoscopic submucosal tunnel dissection on patients with large esophageal superficial neoplasms

Objective:To analyze the effect of endoscopic submucosal tunnel dissection on patients with large esophageal superficial neoplasms.Methods: Chosen patients with large esophageal superficial neoplasms in our hospital as research object, randomly divided into control group treated by endoscopic mucosal dissection (ESD) and observation group treated by endoscopic submucosal tunnel dissection(ESTD), compared surgery related indicators, complications and treatment outcomes.Results: 1)Observation group patients’ tumor stripping rate (23.17±4.73)/min was significantly higher than control group patients’ (12.65±2.19)/min; Intraoperative blood loss(9.14±0.67)ml, total length of hospital stay (7.34±1.89) d, were significantly less than control group patients with intraoperative blood loss(21.38±3.14)ml, total length of hospital stay (13.21±3.05)d, t value was 4.965, 5.395, 4.932, respectively(t=4.965,t=5.395,t=4.932;P<0.05); 2)Observation group patients esophageal bleeding ESTD rate(5.26%), esophageal stricture rate(31.58%), mediastinal emphysema rate(5.26%), esophageal perforation rate of 0, were significantly less than control group patients with esophageal bleeding rate(31.58%), esophageal stricture rate(5.26%), mediastinal emphysema rate(21.05%), esophageal perforation rate (26.32%), t value were 4.378, 4.378, 4.471, 5.758, (t=4.378,t=4.378,t=4.471,t=5.758;P<0.05); 3)Observation group patients with no recurrence during the follow-up period, control group patients with local recurrence rate of 21.05%, t value 8.623,P<0.05(t=8.623, P<0.05).Conclusion: Endoscopic tunnel mucosal stripping technique can effectively improve the complete tumor removal rate, during the process of optimization operation at the same time reduce the occurrence of complications, to the improvement of the prognosis of patients with positive clinical significance.
7.Using Metallic Stent to Treat Malignant Stricture of Esophagus and Cardia

Purpose:To evaluate the effect of metallic stent in the treatment of malignant stricture in esophagus and cardia.Materials and Methods:Treated by metallic stent:14patients with malignant stricture in esophagus and cardia,13 males and 1 female aged ranged from 57 to 81,were discussed in this article.Among 12 patients,8 were proved as esophageal squamous carcinoma,4 as esophagus adenocarcinoma,and 1 was complicated with esophagotrachial fistula,l0 of the pa- tients could take fluid food and 4 only water.After catheterizing from mouth,the strictured portions dilated with hallon catheter,15 selfmade Z stents coveted with dacron and 1 strecker stent were in 13 and 1 patients respectively.Results:All stems were successfully placed.Patients could eat more easily after the operations.The esophogotrachial fistula was completely sealed.The average lifetime after the operation was 5.5 months.Conclusions:Metallic stent prosthesis is an effective method to treat malignant stricture in esophagus and cardia,and could elevate the life quality of patients sig- nifieantly.
8.Application of dual-source CT in carotid arterial angiography

Objective: To evaluate the application value and potential faults of Dual-source CT in carotid arterial angiography.Methods: We retrospectively analyzed the images of 477 patients with symptomatic carotid atherosclerosis detected by dual-source CT angiography(CTA) from February 2007 to January 2008,and evaluated the quality of the images,which were classified into 3 quality grades: excellent(with clear display of the lesion and no noticeable motion artifacts,good(with a few motion artifacts but still diagnostically applicable),and poor(with heavy motion artifacts,unclear display and no diagnostic value).Results: Of the 477 images,420(88.1%) were of excellent quality,44(9.2%) of good quality and 13(2.7%) of poor quality.Among the total number of cases,134(28.1%) were shown to be normal and 34(7.1%) abnormal in anatomic structure.Atheromatous plaques were detected in 295 cases(61.8%),carotid artery stenting in 11(2.3%),internal carotid artery occlusion in 1,common carotid artery occlusion in 1,and cervical tumor in 3.Conclusion: Dual-source CT,faster,effective and minimally invasive,is particularly valuable in the diagnosis of carotid arterial lesion and stenosis.
9.Study on Heavy Metal Residues in 100 Chinese Traditional Medicine Materials

Objective:To detect heavy metals in 100 Chinese traditional medicine materials to evaluate the heavy metal residue situation. Methods:The ministry of land and resources Wuhan supervision and inspection center of mineral resources was authorized to examine four heavy metals: lead ( Pb ) , cadmium ( Cd ) , arsenic ( As ) and mercury ( Hg ) . Results: The Chinese herbal medicines showed different degree of heavy metal pollution with the over standard rate of 22%, and the over standard rate of Cd, Pb, As and Hg was 19%, 5%, 2% and 1%, respectively. The situation of two over standard heavy metals in single sample from the same batch exis-ted. As for the medicinal parts, roots and rootstock herbs accounted for 50% in the all over standard. As for the regions, east China and central-southern China had the highest over standard. Conclusion: The examined medicines exhibit heavy metal pollution. It is necessary to strengthen the examination and control for harmful chemical elements in Chinese herbal medicines to enhance the quality and guarantee clinical use safety.
10.The efficacy and safety of long-term thalidomide in the treatment of ankylosing spondylitis

Objective To investigate the long term efficacy and safety of thalidomide in the treatment of refractory ankylosing spondylitis.Methods A total of 232 patients with refractory ankylosing spondylitis were recruited into open study using thaiidomide at a dose of 150 mg/d, bath ankylosing spondylitis disease activity index ( BASDAI) , spinal pain score and thaiidomide related side effects were observed regularly.Results From the third month, BASDAI and spine pain score decreased significantly when compared with those of the base line ( P < 0.05).Such improvement became more obvious as time went on.A total of 148 patients (63.8% ) got >50% improvement in BASDAI and spine pain score, and 76 cases (32.8% ) reported absence of spine pain.The major side effects were drowsiness, constipation, dry mouth, dizziness and dandruff.Thirty two patients (13.8% ) withdrew from the study because of adverse events.Most of the adverse effects disappeared as thaiidomide was stopped.Conclusion Long term thaiidomide is effective and safe for treating resistant ankylosing spondylitis and it has cumulative effect as duration prolongs.
