1.Effect of different compatibility of zhizi dahuang decoction on pharmacokinetics of naringenin and hesperetin.

An HPLC-UV method was developed for the determination of total naringenin and total hesperetin in rat plasma after oral administration of Citrus aurantium Immaturus extracts and Zhizi Dahuang decoction. Plasma samples were pretreated with liquid-liquid extraction procedure and acid hydrolysis method was used for converting conjugated naringenin and hesperetin to their respective free forms. Plasma samples were separated on a C18 column (4.6 mm x 150 mm, 5 microm), using 0.1% phosphoric acid and methanol as mobile phase at a flow rate of 1.0 mL x min(-1) with gradient elution. DAS 2.0 software was applied to calculate the pharmacokinetic parameters while the SPSS 16.0 software was used for statistical analysis. Significant differences were observed, the C(max) AUC(0-t) of total naringenin in ZS group was 73.5% and 65.9% higher than those in ZZDHD group, respectively; the C(max), AUC(0-t) of total hesperetin in ZS group was 63.5% and 119.1% higher than those in ZZDHD group, respectively. There is a obvious decrease in C(max) and AUC(0-t) of total naringenin and total hesperetin after compatibility and their pharmacokinetic characteristics changed greatly due to the combination of other herbs. The established method was rapid, sensitive, selective and accurate, and it could be applied in the determination of total naringenin and total hesperetin in rat plasma.

3.Protective Effect of Heparin on Kidneys of Children with Henoch-Schonlein Purpura

Objective To explore protective effect of heparin on renal of children with Henoch-Schonlein purpura(HSP).Methods Two hundred and fifteen cases with HSP were divided into heparin-treated group(110 cases,male 64,female 46) and control group (105 cases,male 61,female 44).Serum IL-6 and IL-8 levels were examined by using radioimmunoassay respectively.These patients were followed up for more than 6 months.Results 1.The level of IL-6,IL-8 did not show significant difference between heparin-treated group and control group before treatment.The serum concentrations of IL-6,IL-8 in heparin-treated group were obviously lower than those in control group,after treatment,there was significantly different[IL-6(116.50?19.45)vs(123.88?22.55)ng /L,t=2.573 P=0.011;IL-8(0.161?0.043)vs(0.173?0.048)?g/L,t=2.024 P=0.044].2.During more than 6 months follow-up there were 26 cases suffering from purpura nephritis in heparin-treated group, the morbidity was 25%; the morbidity in control group was 39.8%,the morbidity in treatment group was significant lower than that in control group(?2=5.061 P=0.024).The time of renal injury was significantly delay in treatment group(35.0?13.2)d compared with that of control group (26.0?12.1)d(t=2.659 P=0.010).Conclusions The serum IL-6 and IL-8 concentrations in HSP obviously decrease after heparin treatment.Heparin therapy can decrease the morbidity of renal injury in children with HSP and delay the time of renal injury effectively.
4.How to carry out the translational medicine research effectively in gastrointestinal tumors.

The development of gastrointestinal surgery reflects the idea of translational medicine. To carry out the translational study, the gastrointestinal surgeon must learn how to find subjects from clinical problems, how to collect complete information and tissues, how to collect complete information and tissues, how to collaborate with others from different fields and how to utilize all kinds of resources. By translational studies, gastrointestinal surgeons may further improve the survival of patients with gastrointestinal tumor.

5.Pharmaceutical Care in the Management of Nausea and Vomiting Induced by Adjuvant Chemotherapy for a Postoperative Patient with Gastric Cancer

Objective To provide reference for clinical pharmacist participating in management of nausea and vomiting induced by tumor chemotherapy. Methods The process of pharmaceutical care for a patient with severe vomiting caused by adjuvant chemotherapy after gastric cancer operation was described. Antiemetic application and drug adverse reactions were analyzed. A new treatment plan was given by clinical pharmacist. Results The suggestions were adopted by clinician. The vomiting was controlled and drug adverse reactions were dealt with. Conclusion To reduce the risk and improve the income of antiemetic,clinical pharmacists should pay more attention to clinical practice guideline,drug interaction and adverse reactions, provide the most suitable suggestions for clinicians according to pharmacology and evidence-based medicine.
6.Mechanism and strategy of the secondary resistance to EGFR-TKI in patients with lung cancer

Epidermal growth factor receptor-tyrosine kinase inhibitor (EGFR-TKI) is one of the most important targeted drugs for lung cancer patients carrying EGFR sensitive mutations.However,almost all patients that are effective to this treatment will eventually develop secondary resistance to EGFR-TKI.The most accepted mechanisms of resistance mainly include T790M mutation,MET amplification,PIK3CA mutation,down-regulation of PTEN expression and activation of Fas-transcription factor-κB.Recent years,many new drugs are developed to overcome this resistance.Although most of drugs are in the stages of cell or animal experiment,some new drugs get positive clinical results.
7.Effects of transcranial magnetic stimulation on the recovery of neurological function and the expression of neurofilament protein in rats with cerebral infarction

Objective To investigate the effects of transcranial magnetic stimulation(TMS)on the recovery of neurological function and the expression of neurofilament protein(NFP)-200 in rats with cerebral infarction.Methods The string-fastening method was used to establish focal ischemia-reperfusion models in 60 Sprague-Dawley rats and the rats were divided into the TMS group and the sham TMS group(composed of 30 rats each).Then each group were divided into the treated 1 d,3 d,7 d,14 d and 21 d subgroups(composed of 6 rats each).TMS group rats were administered with 2 sessions of TMS(200 pulses total)daily,keeping on corresponding days in different groups.The recovery of neurological function and the optical density of NFP-200 by immunohistochemistry in the infarct peripheral zone in two groups were detected after therapy.Results At the 14 d and 21 d after treatment,neurological deficit scores of TMS group(2.67?0.82,1.50?0.55) were significantly lower than those of the sham TMS group(3.67?0.52,3.17?0.75)(P
8.ACTH-independent Cushing syndrome(report of 89 cases)

Objective To study the diagnosis and treatment of adrenocorticotrophic hormone (ACTH) independent Cushing syndrome. Methods A retrospective review was done on 89 patients with ACTH independent Cushing syndrome, including 85 cases of adrenal tumors with hypercorticism, 2 cases of adrenocortical adenocarcinoma and 2 cases with ACTH independent macronodular adrenal hyperplasia. Results Favorable results were observed from the surgery for 85 cases of adrenal tumors combined with hypercorticism.Two cases with ACTH independent macronodular adrenal hyperplasia were cured by bilateral adrenolectomy. Adrenal cortical adenocarcinoma was unresectable for both the patients who survived 7 and 9 months respectively. Plasma ACTH,dexamethasone suppression test,B ultrasonography and CT scan were useful for the diagnosis and differentiated diagnosis of ACTH independent Cushing syndrome. Conclusions Prognosis of adrenal tumors with hypercorticism and ACTH independent macronodular adrenal hyperplasia is favorable after surgery,whereas the prognosis of adrenal cortical adenocarcinoma is poor.
