1.Advances in laparoscopic sphincter-preserving surgery for low rectal cancer

The incidence of low rectal cancer, which severely threatens human health, has been increasing annually in China. Sur-gical operation is the most effective treatment for low rectal cancer. In-depth analyses on the physiology and anatomy of the rectum and the biological characteristics of low rectal cancer have led to the wide application of anus preservation operation as a treatment for this disease. Anus preservation operation is widely used, especially because the application of the laparoscopic technique has recently be-come a popular topic in colorectal surgery. Alternative laparoscopic sphincter-preserving procedures for this disease include the follow-ing: laparoscopic anterior resection; and laparoscopic anterior resection with inter-sphincter resection or with prolapse technique. Colorectal anastomosis with double-stapler technique, hand-sewn coloanal anastomosis, and colonic J-pouch-anal anastomosis are com-mon reconstruction procedures. Each procedure has its own adaptive range, advantages, and disadvantages. Radical surgery must be pri-oritized, but we should also consider the patients' life quality when selecting a mode of surgery.
2.Eyesight and internal carotid artery stenosis: a case report

AIM:To report a case of a chronic glaucoma patient whose reduced eyesight is related to ocular ischemia.METHODS:A case report.RESULTS:A 71 year-old man presented with a decreased left eye vision for over 3 years.During the last two months,he had deteriorated blurred vision when he woke up in the morning.The loss of his lower-half peripheral vision was especially serious.The patient suffered from an episode of transient amaurosis fugax three months ago.According to transcranial Doppler ultrasound scan and cerebral computerized tomographic angiography,the patient was found that he had severe stenosis of the bilateral intracranial internal carotid artery and decreased blood flow velocity within the left ophthalmic artery.His symptoms alleviated after an internal carotid artery endarterectomy.CONCLUSION:When vision loss glaucoma cases are presented,more attention should be paid to the presence of ischemia within the ophthalmic artery except glaucoma.
3.Influence of alimentary tract reconstruction on gastric cancer patients complicated with type 2 diabetes mellitus (a report of 67 cases )

Objective To explore the influence of alimentary tract reconstruction on blood glucose level in gastric cancer patients complicated with type 2 diabetes mellitus. Methods Clinical data of 67 gastric cancer patients complicated with type 2 diabetes mellitus and the level of blood glucose before operation, one month, three month, six month after operation were retrospectively analyzed. BMI of 53 patients was lower than 25kg/m2,9 patients between 25~29.9 kg/m2 and 5 patients was higher than 30kg/m2 .Total gastrectomy with P-type jejunal pouch Roux-en-Y esophagojejunostomy was performed in 26 cases, and total gastrectomy with Roux-en-Y esophagojejunostomy was performed in 11 cases, distal subtotal gastrectomy with Roux-en-Y gastrojejunostomy in 30 cases. Results Operations on sixty-seven patients were all uneventfully. The mean fasting blood glucose level in the morning was 9.6±3.3 mmol/L before operation, 7.4±2.6 mmol/L one month after operation, 7.5±2.3 mmol/L three month after operation, and 7.7±2.9 mmol/L six month after operation. There were significant differences between the blood glucose level of before operation and one month after operation (P<0.05), and there were no significant differences between three month,six month and one month after operation (P>0.05). Conclusions Alimentary tract reconstruction has obviousinfluence on blood glucose level in type 2 diabetes mellitus patients. It takes about one month for reveal the effect of operation. This phenomenon is of value for clinical application. Its mechanism needs further research.
4. Genetic optimization, recombinant expression and functional study of human augmenter of liver regeneration

Objective: To construct recombinant expression vector of human augmenter of liver regeneration (ALR) and study its protective effect on liver function. Methods: ALR cDNA was synthesized and inserted into expression vector pET28a+. The recombinant plasmid was tranformed into BL21 and the expression of ALR was induced by isopropyl-β-D-thiogalactoside(IPTG). MTT method was used for cell proliferation assay; the protective effect of recombinant product on liver function was observed in CCl4-induced acute toxic mouse model. Results: Recombinant expression plasmid of ALR was confirmed correct by restriction enzyme digestion and sequencing. The purified expression product had strong stimulatory effect on hepatocyte proliferation. Low and medium dosages of expression product decreased aminotransferase level in acute chemical injury mouse model. Conclusion: The recombinant expression vector of ALR has been correctly constructed and the expressed rALR can simulate hepatocyte regeneration.
5.Development of control system for B→O blood conversion device

B→O blood conversion has very important meanings in the blood transfusion during the period of peace and war.In the leader of Professor Zhang Yangpei,from Academy of Military Medical Sciences,B→O blood conversion has been achieved by using gene engineering.They obtained the great breakthrough in the domain of blood type conversion.In order to realize standardization and automatization,it is necessary to develop a device applying to B→O blood conversion.This device automatically accomplishes the process of B→O blood conversion,shortens the time of B→O blood conversion to quarter than the time by manual operation,and ensures the standardization and obturation.In the condition of one-off pipeline,this device meets the clinical demands for blood,and lays a foundation for clinical expansion of B→O blood conversion.This paper presented the control system of B→O blood conversion device,including design of software and hardware.This device provides a safe and efficient tool for B→O blood conversion.

8.Biologic Characteristic Study of β-mercaptoethanol and bFGF to Induce Rat Mesenchymal Stem Cells to Neuron-like Cells

[Objective] To compare the biological characteristics of neuron-like cells from rat mesenchymal stem cells (MSC) induced by β-mercaptoethanol (BME) and basic fibroblast growth factor (bFGF) respectively. [Method] BME and bFGF were added to rat MSC respectively for 9 h and 5 days. The neuron-like cells from MSC with the neuron specific protein NF200 were identified using immunofluorescence. The gene expression of NF were analyzed using RT-PCR and the protein expression of NF200 were analyzed using Western blotting. [Results] MSC induced by BME appeared morphology change in an hour and the neurite can be seen at six hour. Some cell bodies became lighter and died. MSC induced by bFGF had neurite in the third day, and appeared network structure in the fifth day, then cells died in the seventh day. At the same time, The result of immunofluorescence showed the NF200 expression of neuon-like cells induced by two ways were beth positive, but MSC induced by bFGF were stronger than those induced by BME. The results of RT-PCR and western blotting were same as that of immunofluorescence. [Conclusion] Both bFGF and BME can induce rat MSC to neuron-like cells, but the neuon-like cells induced by bFGF were more like real neurons in morphology and expression of neuron specific protein and mRNA.
9.Safety and Adverse Drug Reactions of Antongding

OBJECTIVE:To evaluate the adverse drug reaction(ADR) induced by Antongding and its safety in use.METHODS:According to the organ/system type recommended by WHO,34 cases with ADRs induced by Antongding were classified.The ADRs in this series were analysed concerning clinical manifestations,severity of symptoms and inducing mechanism and the safety of use of Antongding was discussed.RESULTS:8 organs/systems were involved in 34 cases with ADRs,of them,18 cases had moderate and severe ADRs(52.94%).In vitro experiments showed that Antongding could inhibit CFU-GM in man.CONCLUSION:Antongding is apt to induce ADRs,so it has potential risk in use.
10.Clinical analysis of hyperthyroidism complicated with periodic paralysis

Objective To explore relevant factors of hyperthyroidism with periodic paralysis of hypokalemic (TPP).Methods The clinical data of 112 cased with hyperthyroidism complicated periodic paralysis of hypokalemic were retrospectively analyzed and to analyze the relevant factors. Results Compared to control group, the hyperthyroidism with periodic paralysis group had no obvious differences in general factors ( P > 0. 05 ), but had obvious difference in Gender( P <0. 05 ) ;Compared to control group, the hyperthyroidism with periodic paralysis group had obvious differences in induction factors(P < 0. 05);Compared to recurrent group, the incipient group had no obvious differences in several factors( P >0. 05) ,but had obvious difference in hyperthyroidism medical history over one year( P <0. 05) ; Hyperthyroidism with periodic paralysis was strongly associated with hypokalemia, age, gender, tiredness, getting a cold, satiation and hyperthyroidism genetic history ( P < 0. 01 ). Conclusion The relevant factors of hyperthyroidism with periodic paralysis were hypokalemia, gender, tiredness, getting a cold, intemperance, satiation and hyperthyroidism medical history.
