1.Early therapeutic effect of coronary artery bypass grafting with and without cardiopulmonary bypass

Objective:To evaluate whether off pump coronary artery bypass grafting (OPCABG) has advantages over conventional coronary artery bypass grafting(CCABG). Methods:A total of 362 patients undergoing coronary artery bypass grafting were divided into 2 groups: OPCABG group:152 cases undergoing OPCABG and beating heart via sternotomy; CCABG group,210 cases undergoing CCABG with extracorporeal circulation. Preoperative and postoperative variables of 2 groups were compared. Results:There were no significant differences in sex, age, weight, acute or remote myocardium infarction, hypertension, diabetes and type of bypass grafts between 2 groups.The numbers of bridge vessels were not significantly different between the 2 groups; the incidences of low cardiac output syndrome, atrial fibrillation and atelectasis in OPCABG group were significantly less than those of CCABG group( P

Objective:To evaluate the therapeutic effectiveness of peritoneal dialysis(PD) for acute renal failure(ARF) following cardiac surgery with cardiopulmonary bypass and its effect on hemodynamics. In the current study, nine flavonoids were isolated and purified from 75% ethanol extract of Selaginella uncinata (Desv.) Spring by column chromatographic techniques over macroporous resin, polyamide, silica gel, Sephadex LH-20 and pre-HPLC. On the basis of their physico-chemical properties and spectroscopic data analyses, these compounds were elucidated as cirsimarin (1), nepitrin (2), apigenin-6-C-α-L-arabinopyranosyl-8-C-β-D-glucopyranoside (3), apigenin-6-C-β-D-glucopyranosyl-8-C-α-L-arabinopyranoside (4), apigenin-7-O-β-D-glucopyranoside (5), 2,3-dihydroamentoflavone (6), 4'-O-methylamentoflavone (7), 2,3-dihydro-4'-O-methyl-amentoflavone (8), and 2,3,2",3"-tetrahydron-4'-O-methyl-robustaflavone (9). Compounds 1-5 belong to flavonoid glycosides and were isolated from the genus Selaginella for the first time.

4.Social Cognition and Its Correlation with Social Function in Attenuated Psychosis Syndrome

Objective To investigate the social cognition and its correlation with social function for attenuated psychosis syndrome (APS). Methods From August, 2014 to December, 2015, 39 patients with APS were recruited as research group. Another 40 normal healthy persons with similar gender, ages, and education levels were selected as control group. The Faux Pas Recognition Test (FPR) and Yoni Task Test were used to evaluate the social cognition, and Social Disability Screening Schedule (SDSS) was used to evaluate the social function. The correlation between FPR, Yoni Task Test and SDSS in the research group was analyzed. Results In FPR test, the faux pas questions score, control questions score and total score of FPR were significantly lower in the research group than in the normal control group (t>2.378, P<0.01). In Yoni Task Test, the cognitive theory of mind total score (Cog), cognitive theory of mind first-level score (Cog1) and cog-nitive theory of mind second-level score (Cog2), and affective theory of mind total score (Aff), affective theory of mind first-level score (Aff1) and affective theory of mind second-level score (Aff2) were significantly lower in the research group than in the normal control group (t>2.341, P<0.01). There was no significant difference in control theory of mind total score (Phy), control theory of mind first-level score (Phy1) and control theory of mind second-level score (Phy2) between two groups (t<1.430, P>0.05). The SDSS total score was signifi-cantly higher in the research group than in the normal control group (t=13.032, P<0.001). In the research group, FPR's faux pas questions score and FPR's total score were negatively correlated with SDSS score (r>0.473, P<0.01); in Yoni task test, Cog's total score and factor scores, Aff's total score and factor scores were negatively correlated with SDSS score (r>0.448, P<0.01). Conclusion Social cognition func-tion in APS is impaired. It is associated with social dysfunction in APS.
5.Efficacy of combining ultrasonic with three-dimensional imaging guided microwave ablation in treating large hepatocellular carcinoma: a comparative study

Objective To investigate the efficacy of combining ultrasonic with three-dimensional imaging guided microwave ablation in treating large hepatocellular carcinoma.Method The clinical data of 262 patients with large unresectable liver cancer who were admitted to the Air Force General Hospital from Jan 2011 to Jun 2014 were retrospectively analyzed.Of these patients,136 underwent transcatheter arterial chemoembolization (TACE),and the remaining 126 patients underwent combined ultrasonic with three-dimensional imaging guided microwave ablation (MWA).The AFP levels,tumor ablation rate,postoperative complication rates and survival rates between the two groups were compared.Result There were significant differences in the AFP levels in the two groups before and after surgery (P <0.05),but no significant differences were found between the 2 groups (P > 0.05).A significant difference was also observed on tumor ablation rate.In the MWA group,11 patients (8.73%) developed complications,while 21 patients (15.44%) in the TACE group developed complications,(P < 0.05).The 3-month,9-month,1-year,2-year and 3-year survival rates were 99%,95%,81%,70% and 57% in the MWA group,and 98%,94%,63%,36% and 28% in the TACE group.There were significant differences in the 1-year,2-year and 3-year survival rates,but no significant difference were observed in the 3-month and 9-month survival rates.Conclnsion Combined ultrasonic with three-dimensional imaging guided microwave ablation increased the rate of tumor ablation and prolonged the survival time of patients with large hepatocellular carcinoma.
6.Effects of lidocaine on adhesion molecules expression in lung injury after ischemic shock in rats

Objective To study the effect of lidocaine on the expression of CD11b/CD18 and MPO in lung injury after hemorrhagic shock in rats. Methods Eighty male Wistar rats weighing 230-270 g were randomly divided into 4 groups: sham operation group ( group Ⅰ, n = 8 ) received operation without shock, shock group ( group Ⅱ, n = 8 ) received hemorrhagic shock without resuscitation , normal saline treatment group (group Ⅲ, n = 32 ) received normal saline after shock, lidocaine treatment group ( group Ⅳ, n = 32 ) received lidocaine after shock. The animals were anesthetized with intraperitoneal pentobarbital sodium 40 mg? kg-1 . Hemorrhagic shock was induced by withdrawing blood from right femoral artery at the rate of 2.0 ml?kg-1? min-1 , MAP was maintained at 40 mm Hg for 60 min before resuscitation. Direct arterial MAP and HR were continuously monitored. Group Ⅳwas received lidoacaine at the beginning of resuscitation with a bonus dose 2.0 mg? kg-1 and then followed continuous infusion1.0mg?kg-1?h-1 for 2 h. Group III was received the same dose of normal saline. Flow cytometric analysis was used to assess the expression of CD11b/CD18 on leukocyte and the change of myeloperoxidase (MPO) in the lung was studied at the end of shock (groupⅡ ) and at 2,4, 8, 12 h after resuscitation (group Ⅲ, group Ⅳ) . The rats were sacrificed and the piece of lung was immediately removed for electron microscopic examination. Result In group Ⅲand group Ⅳ ,the expression of CD11b/CD18 on PMNs and MPO in lung were increased significantly compared with Group Ⅰ. Microscopic examination indicated the longer time of reperfusion ,the more serious injury of the lung. And in groupⅣ ,the changes of CD11b/CD18 and MPO were reduced as compared with group Ⅲ (P

Reduced radiosensitivity of lung cancer cells represents a pivotal obstacle in clinical oncology. The hypoxia-inducible factor (HIF)-1α plays a crucial role in radiosensitivity, but the detailed mechanisms remain elusive. A relationship has been suggested to exist between hypoxia and autophagy recently. In the current study, we studied the effect of hypoxia-induced autophagy on radioresistance in lung cancer cell lines. A549 and H1299 cells were cultured under normoxia or hypoxia, followed by irradiation at dosage ranging from 0 to 8 Gy. Clonogenic assay was performed to calculate surviving fraction. EGFP-LC3 plasmid was stably transfected into cells to monitor autophagic processes. Western blotting was used to evaluate the protein expression levels of HIF-1α, c-Jun, phosphorylated c-Jun, Beclin 1, LC3 and p62. The mRNA levels of Beclin 1 were detected by qRT-PCR. We found that under hypoxia, both A549 and H1299 cells were radio-resistant compared with normoxia. Hypoxia-induced elevated HIF-1α protein expression preferentially triggered autophagy, accompanied by LC3 induction, EGFP-LC3 puncta and p62 degradation. In the meantime, HIF-1α increased downstream c-Jun phosphorylation, which in turn upregulated Beclin 1 mRNA and protein expression. The upregulation of Beclin 1 expression, instead of HIF-1α, could be blocked by SP600125 (a specific inhibitor of c-Jun NH2-terminal kinase), followed by suppression of autophagy. Under hypoxia, combined treatment of irradiation and chloroquine (a potent autophagy inhibitor) significantly decreased the survival potential of lung cancer cells in vitro and in vivo. In conclusion, hypoxia-induced autophagy through evaluating Beclin1 expression may be considered as a target to reverse the radioresistance in cancer cells.
8.Effect of fast track surgery combined with quality nursing in operating room

Objective To study the effect of fast track surgery (FTS) combined with quality care in operating room on postoperative infection and quality of life of prognosis.Methods A total of 172 patients with surgical treatment were divided into two groups according to random number table method.The patients in the control group were given quality care in the operating room,and the patients in the observation group underwent FTS based on high quality care in the operating room.The general situation,surgical site infection,complications and prognostic quality of life were compared.Results The patients in the observation group had shorter infusion quitting time,awake time after anesthesia,the first time to exhaust and hospitalization time than the control group (P < 0.05).The incidence of surgical site infection was 2.33% (2/86) in the observation group,which was lower than 10.47% (9/86) in the control group (P < 0.05).There was no significant difference in the quality of life (physiological function,mental state,social function and affective activity) between the two groups before treatment (P > 0.05).The scores of physical function,mental state,social function and affective activity of the two groups were higher than nursing before,and the study group was higher than the control group (P < 0.05).The incidence of postoperative complications was 5.81% (5/86) in the observation group,which was lower compared to 15.12% (13/86) in the control group (P < 0.05).Conclusion FTS surgical concept combined with operating room quality care can reduce the occurrence of postoperative infection,shorten treatment time,reduce complications,promote incision rapid healing and prognosis,so it is worth promoting.

10.The isolation of acinetobacter strain from burn wound and the analysis of its antibiotic resistance.

OBJECTIVETo investigate the distribution and drug resistance of acinetobacter isolated from burn wounds.
METHODSThe acinetobacter strains were isolated and identified by routine methods. Based on the recommendation of NCCLS, AmpC enzyme was determined by cefoxiti three-dimensional test, ESBLs by disk diffusion method and bacterial susceptibility by Kirby-Bauer agar diffusion method.
RESULTSAmong the 69 strains of acinetobacter clinically isolated from burn wounds, 52 were baumannii (75.6%). The acinetobacter strains were identified to be highly resistant to 17 kinds of antibiotics. The drug resistance rate of beta-lactamase-producing strains (68.25%) was higher than that of non-beta-lactamase-producing strains (20.33%). The strains isolated in our burn ward exhibited multiple drug resistance which was mainly due to the production of many kinds of beta-lactamases. Among the 38 strains of beta-lactamase-producing acinetobacter, those producing AmpC beta-lactamase (AmpC BLA) accounted for 42.1%.
CONCLUSIONAcinetobacter strain was one of the pathogens in burn wound infection, and its isolation and identification of its drug resistance could be beneficial to the doctors to make right choice of antibiotics.
