1.The applications of vaginal hysterectomy of non-prolapsed uterus lesion in primary hospital

ObjectiveTo explore the application value and techniques of vaginal hysterectomy of non-prolapsed uterus lesion in primary hospital. Methods140 cases who suffered from uterine fibroid without adhesion,glandular fibroids,functional bleeding without uterine prolapse and malignant disease were selected and divided into observation group(80 cases with vaginal hysterectomy)and control group(60 cases with routine abdominal hysterectomy).Then the clinical treatment effect was analyzed. ResultsCompared with control group,peri-operative bleeding (150 ±23)ml vs(243 ±41)ml,operation time(65 ± 14.0)min vs(90 ± 27.0)min,body temperature recovery time (3 ±0.5)d vs(4 ±0.8)d,anus exhaust time(14 ± 1.8)h vs(26 ±2.7)h,the length of time(5 ±0.8)d vs(11 ±2.9)d in observation group were all reduced,differences were significant(all P<0.01)and no case was transferred to opening appendectomy. ConclusionFor these disease including uterine fibroids with no adhesion,glandular fibroid uterine bleeding,the vaginal hysterectomy was safe and feasible,and had the advantage of smaller trauma,less bleeding,shorter hospitalization time,faster recovery etc.It was worthy to be popularized in primary hospital.
2.The risk factors and the characteristic patterns of development changes of macrosomia

Objective To investigate the risk factors and the characteristic patterns of development changes of macrosomia.Methods 208 cases of macrosoroia were collected.Then the risk factors related to macrosomia by use of Logistic regression analysis were researched.208 infants of normal birth weight were selected as control group.The weight and length in 8 growth development monitoring time according to their health care manual were recorded.Mental development index(MDI)and physical development index(PDI)were tested and calculated by CDCC method when macrosomia and normal birth weight infants were 30 months.The characteristic patterns of development changes of macrosomia wre analyzed statistically.Results The risk factors of macrosomia contain age and height of the pregnant women,weight before pregnancy,weight increasing during pregnancy,nutriture,physical activity,family history of obesity and father's weight.Macrosomia was weighter than normal birth weight infants in the 8 growth development monitoring time and was longer than normal birth weight infants from 3 to 30 months(P <0.05).However,the length of macrosomia was not significantly different with that of control group when they were 36 months(P > 0.05).MDI and PDI in macrosomia group were(107.33 ± 7.29)and(104.71 ± 6.93)respectively.MDI and PDI of normal birth weight infants group were(112.58 ± 7.61)and(109.09 ± 7.14)respectively.The differences were statistically significant(t =4.28,5.33;both P < 0.01).Conclusion There were several risk factors for macrosomia such as environment and heredity.At the period of infants and young children,macrosomia were weighter than normal birth weight infants.The mental development and psycho-moter development of macrosomia were behind normal birth weight infants.

4.Protective effect of edaravone on corneal nerve of rat with experimental diabetic corneal neuropathy

BackgroundWith the number of diabetics increases,people pay more attention to the diabetic keratopathy.The major mechanism leading to diabetic keratopathy is diabetic corneal neuropathy.So it is significant to observe pathologic mechanism of diabetic corneal neuropathy. Objective To investigate the protective effects of edaravone( a free radical scavenger) on corneal nerve of rats with experimental diabetic corneal neuropathy,then explain the effects of oxidative stress in the pathologic mechanism of diabetic corneal neuropathy. Methods Seventy Sprague-Daxley male rats were taken as experimental subjects and 20 of them were used as normal control group.The remaining 50 were induced to be diabetic mellitus by a single intraperitoneal injection of streptozotocin and divided into 2 groups randomly:edaravone treated group and diabetic control group.In the edaravone treated group,edaravone(0.2 g/L) eye drops were used 3 times a day until the animal was killed.Five rats in each group were sacrificed at 6,8,10 and 12 weeks respectively.Then the corneal sensation,number of corneal nerve fibers,morphology,content of malondialdehyde(MDA) and activity of superoxide dismutase(SOD) in the corneal tissue were detected.ResultsIn the diabetic control group,the corneal sensation and the number of corneal nerve fibers were decreased,the density of neural network for cluster was sparse,the nerve activity was decreased,the content of MDA in the corneal tissue was significantly increased,the activity of SOD in the corneal tissue was significantly decreased (P<0.01 ).Accompany with the course of disease,the above change was obvious day after day.Compared with the diabetic control group,the corneal sensation and the morphological abnormalities in corneal nerve of edaravone group were improved significantly which had the partial branches to the 12th week,the content of MDA in the corneal tissue was significantly decreased,the activity of SOD in the corneal tissue was significantly increased (P<0.01).Conclusions Edaravone can lower diabetic corneal nerve of rats with experimental diabetic corneal neuropathyinjury,Oxidative stress may be a critical pathologic mechanism of diabetic corneal neuropathy.
5. Study on anti-tumor activities of ginsenoside structure modification HRG in vivo

OBJECTIVE: To explore the anti-tumor activities of a new compound 3β,12β,20(S)-trihydroxy dammarane-3-O-β-D-glucopyranosyl (1-2)-β-D-glucopyranoside (HRG), which is a substance of ginsenoside structure modification, in vivo. METHODS: Liver cancer H22 tumor model on the chick chorioallantoic membrane (CAM) was established to observe the effects of HRG on tumor growth, the number of induced vessels and the growth inhibition rate. The implanted tumors were dyed by hematoxylin-eosin (HE), and the morphological properties were studied with light microscope. Immunohistochemistry (SP method) was used to detect the expressions of the implanted tumor's MVD and VEGF. RESULTS: HRG inhibited the tumor growth at 25, 50 and 100 μg · mL-1. Compared with the model control group, the inhibitory rates of the tumor growth were 27.73%, 50.02%, and 64.21%, respectively. HRG significantly reduced the number of tumor-induced vessels. At the same time, there existed different degree necrosis among the treatment groups, and the degree of necrosis had an inverse relationship with the number of tumor-induced vessels. In addition, HRG reduced the MVD of the implanted tumors, and decreased the expression of VEGF. CONCLUSION: HRG has good anti-tumor activities in vivo, and can significantly inhibit the growth of liver cancer H22-CAM tumor and the induction of angiogenesis. The mechanisms may be associated with lower tumor MVD and VEGF expression.

8.Reflection on the issue of doctors' practice circumstances

Doctors ought to belong to a social stratum with fairly high incomes. The high input and long cycle that is essential to becoming a high-caliber doctor constitute the high costs of the medical profession; high technology, high responsibility and high sacrifices constitute the high risks of the medical profession; the contrast between the input of high intelligence, high academic degree, and high expertise and the output of low economic income tends to warp the minds of doctors. The inversion of the burden of proof in medical lawsuits has practically put doctors on the weak side. Their rights and interests as well as their practice circumstances have been disregarded. For the sake of the development of the health cause, it is imperative to respect the labor of doctors and to provide them with sound survival circumstances.
9.Application of PASS System in Synchronized Monitoring of Medical Orders for Inpatients with Tuberculosis

OBJECTIVE:To evaluate the medical orders in terms of the medication for the inpatients with tuberculosis in our hospital.METHODS:The medical orders for the inpatients in Tuberculosis institute from Dec.7th,2007 to Jan.6th,2008 were synchronously monitored and analyzed.RESULTS:Of the total 5 592 medical orders monitored,1 461 were warned by PASS,of which,562 were warnings about drug interactions,884 about dosage and 15 were about incompatibility.CONCLUSION:Medical orders synchronously monitored by PASS could help correct and monitor the irrational medication.But the function of PASS remained to be strengthened and the data base remained to be enriched and renewed in time.
10.Biopsy of needle stereotaction under mammography in nonpalpable breast lesion

Objective To discuss the feasibility of diagnosis with needle stereotaction marking by steel wire under mammography and biopsy for nonpalpable early stage breast carcinoma.Methods Eighteen patients with doubtful nidus of breast only in X gram were performed needle stereotaction marking by steel wire under mammography, and removal freemartin biopsies of breast nidus were performed then.Results Localization and biopsy were very successful in all 18 patients. Seven patients were definited to have early stage breast carcinomas and eleven benign diseases.The diagnostic rate for nonpalpable early stage breast carcinoma was 100%.Conclusions Needle stereotaction marking by steel wire undermammography finish off the problem locating accurately breast nidus during operation.It was a credible and perfect technique for diagnoses of nonpalpable early stage breast carcinoma.
