3.Policy, experience and implications of outpatient security in typical developed countries

This paper reviews outpatient security policy of typical developed countries .It provides reference for outpatient security system in China .Analysts believe that the experience for Chinese reference should be acquired in the following ways:The pooling of outpatient service is an inevitable trend in the development of health insurance , which is in principle an integral part of payment and financing system; the implementation of outpatient community first diagnosis system should be adapted to the local conditions; the community first diagnosis system is a necessary condition for capitation , so it is closely related to compensation methods and the elaborate degree of outpatient treat -ment catalog;Outpatient services should be supervised and payment policies should be developed respectively .There are two supervision models about outpatient service:external supervision and self supervision , but the key is to focus on the management of outpatient doctors .External regulation should be consistent with the current regulatory approach of the situation and patients have to bear some responsibilities for outpatient treatment expenses in some proportions , but the payment cap line should not to be low .The implementation of certain preferential policies for vulnerable groups can be considered under the harmonized system to partially reduce their medical expenses .

7.Analysis of the Utilization of Oxycodone and Acetaminophen in Outpatient and Emergency Department of Our Hospital during 2013-2015

OBJECTIVE:To provide reference for rational use of oxycodone and acetaminophen in outpatient and emergency department. METHODS:By retrospective study,the application of oxycodone and acetaminophen in outpatient and emergency de-partment of our hospital during 2013-2015 was analyzed statistically. RESULTS:During 2013-2015,there were 22654 outpatient and emergency prescriptions of oxycodone and acetaminophen in our hospital. The prescription number,amount and consumption sum of oxycodone and acetaminophen were increasing year by year. Top 3 clinical departments of oxycodone and acetaminophen in the list of prescription number were orthopedics department(78.34%),emergency department(15.52%)and oncology department (2.09%). The age of patients using oxycodone and acetaminophen mainly aged 19-64 years old(78.62%);the ratio of male to fe-male was 1:1.08. The amount of oxycodone and acetaminophen was 5 mg,mainly 4 times a day(86.91%). In all kinds of diagno-sis,knee osseous arthritis (47.26%),pain in waist and lower extremities (6.18%) and abdominal pain in dispute (5.92%) were top three. CONCLUSIONS:A large amount of oxycodone and acetaminophen were used in outpatient and emergency department of our hospital during 2013-2015;there is unreasonable drug use so that we need to strengthen management and standardize drug use.
8.Effect of c-Jun NH2-Terminal Kinase Signal Transduction Pathway on Hyperoxia-Induced Lung Injury in Rats

Objective To explore the effect of c-Jun NH2-terminal kinase(JNK)signal transduction pathway on hyperoxia-induced lung injury in rats.Methods Twenty-four Wistar rats aged 3 weeks were randomly divided into 3 groups(n=8):room-air control group,7 d hyperoxia exposure group,and 7 d hyperoxia exposure with inhibitor of JNK intervention group.The rats in hyperoxia exposure group were exposed to high concentration of oxygen [fractional concentration of inspired oxygen(FiO2)≥950 mL?L-1] at normal pressure.The rats in room-air control group were placed in room air(FiO2=210 mL?L-1)at normal pressure.The rats in JNK inhibitor intervention group were intraperitoneally injected 30 mg?kg-1 SP600125 and exposed to hyperoxia 2 h later.The histopathological changes of lung tissues were observed by means of light microscope,therefore the changes of lung W/D weight ratio,total protein in bronchoalveolar lavage fluid(BALF)and lung permeation index were detected.The extent of lung cells apoptosis was analyzed by terminal deoxynucleotidyltrans-ferase-mediated dUTP nick end labeling assay.The protein level of p-JNK was measured by Western blotting analysis.Results Compared with room-air control group,conspicuous hyperaemia,edema,hemorrhage and extensive inflammation cells infiltration in the lung tissues were significantly observed in 7 d hyperoxia exposure group.The lung W/D weight ratio,total protein in BALF,lung permeation index,cell apoptotic index and the p-JNK protein levels of lung tissues all significantly increased in 7 d hyperoxia exposure group compared with those in room-air control group(Pa
9.Rehabilitation for Thallium Poisoning: One Case Report

ObjectiveTo investigate the clinical manifestations, rehabilitation evaluation and treatment for Thallium poisoning. MethodsA patient with Thallium poisoning was reviewed. ResultsGastroenteritis, peripheral polyneuropathy and alopecia were the typical symptoms of Thallium poisoning. After the shortwave therapy, medium frequency electrotherapy and physical therapy, the index of sensory, extremities perimeter and muscle strength, sitting-stand-walking, ADL, and emotions were apparently improved. ConclusionComprehensive rehabilitation with shortwave therapy, medium frequency electrotherapy and physical therapy might be effective treatments for the patients with Thallium poisoning.
10.Phylogenetic Analyses of HPV53 and Prediction of B and T Cell Epitopes

To construct phylogenetic trees based on HPV53 full length sequences, and predict the physical and chemical parameters, secondary structure, B and T cell epitopes of HPV53 proteins(E1, E2, E4, E6, E7, L1, and L2). The full-length sequences of HPV53 variants were retrieved from the National Center for Biotechnology Information(NCBI), and a phylogenetic tree was constructed to delineate variant lineages. The physical and chemical parameters of HPV53 proteins were analyzed by ProtParam. The secondary structure of proteins was analyzed using PSIPRED and SOPMA. The B and T cell epitopes for HPV53 proteins were predicted by the IEDB analysis server and the ABCpred server, respectively. Then, to select the potential dominant B and T cell epitopes, more parameters including flexibility, hydrophilicity, surface accessibility, antigenicity of predicted B and T cell epitopes were further predicted by bioinformatic methods such as VaxiJen. Finally, for homology analysis, the potential dominant B and T cell epitopes were compared with the 13 high-risk HPV subtypes using NCBI BLAST tool. A total of 54 full-length HPV53 sequences were retrieved from the NCBI database, with 48 entries remaining after deduplication. These 48 HPV53 isolates from different countries/regions were clustered into three main evolutionary branches labeled as lineages A, B, and C. The physicochemical properties of three different HPV53 variants(representing A, B, and C lineages, respectively) were similar. The secondary structure of the E1, E6, and E7 proteins was predominantly α-helices, while E2, E4, L1, and L2 predominantly exhibited random coils. After prediction and screening, a total of 6 potential B-cell epitopes and 9 potential T-cell epitopes were identified on HPV53 proteins. Among these epitopes, B cell epitopes TTPIRPPPPPRPWAPT in E4 region, CYRCQHPLTPEEKQLH in E6 region, and T cell epitopes SGVHSYEEIPMQ in L2 region showed high homologous to HPV56(all > 90%). Bioinformatics analysis and prediction revealed that HPV53 isolates could be clustered into three main evolutionary branches labeled as A, B, and C. These branches exhibited similar physicochemical properties, with minor differences in their secondary structure. Moreover, HPV53 viral proteins contained both B-cell and T-cell antigenic epitopes. These results lay the foundation for further research on vaccines and drugs based on HPV53-related peptides.
