1.Cephalometric analysis of Class Ⅱ division 1 malocclusion after bite-opening treatment with fixed inclined bite plate

Objective: To evaluate the effects of fixed inclined bite plate on jaw bone and dentition in juveniles with Class Ⅱ division 1 malocclusion.Methods: Eighteen patients(10 male and 8 female aged 11.5~13.8 years old) were treated with fixed inclined bite plate for bite-opening, twelve patients(6 male and 6 female) were involved as the control. Cephalograms were taken before and after the treatment.Results: After treatment ANB was significantly reduced and mandibular development was improved; AFH and PFH were significantly increased; the first molar in mandible was siginificantly elongated.Conclusion: Fixed inclined bite plate is effective in the improvement of sagittal skeletal relationship and in the orthodontic treatment in the juveniles with Class Ⅱ division 1 malocclusion.
2.Standard Trauma Big Bone Flap Pressure-reduction Operation Treat Severe Brain Injury

[Objective]To explore the clinical effect of standard trauma big bone flap pressure-reduction operation on severe brain injury.[Method]90 cases take the said therapy,then observe the cure effect.[Result]Make follow-up after one year,75 cases survived,among which,50(55.5%) had good prognosis,15(16.6%) middle handicapped,5(5.5%) severely handicapped,2(2.2%) lived as plant,18(20.0%) dead.Among the dead ones,they basically had platycoria or abnormal breath,or their CT showed missing sicterna ambiens and upper sella ambiens,mostly caused by multi-organ failure and lung infection,etc.[Conclusion]The said therapy can fully reduce the pressure,effectively protect brain function,markedly improve the prognosis of the said disease,and decrease death rate.
3.Effect of circulation factors on ventilation therapy in severe asphyxial neonates

Objective To analyze the effect of circulatory failure on ventilation therapy of respiratory failure after resuscitation in neonates with severe asphyxia. Methods Three hundred and eighty-two neonates with mechanical ventilation following severe asphyxia were retrospectively analyzed. Two groups were divided according to the efficacy of mechanical ventilation: effective and noneffective. Results The non-effective rate was 24.1% at 1 hour after ventilation (19.4% in full term babies,34.5% in preterm babies). The degree of asphyxia in noneffective group was significantly more severe than the effective group. The incidence of persistent pulmonary hypertension of the newborns(PPHN),exsanguine shock,cardiogenic shock,DIC and cardiac failure was 84.3%、51.0%、80 4%、49.0% and 54.9% respectively in term babies,while the incidence of exsanguine shock,cardiogenic shock,DIC and cardiac failure was 65.9%、68.3%、75.0% and 58.5% respectively in preterms babies. All of them were significantly higher than the effective group. After treatment of the circulation problems,the blood gas turned to normal in 94.1% term babies and 92.7% of preterms. No difference was shown in the recovery rate in both groups. Conclusion The circulatory failure is a main cause of non-effective mechanical ventilation in asphyxia neonates. To correct the pathophysiologic changes of the circulation in time can effectively improve the prognosis.
4.Evaluation of improved covered metallic stents for proximal malignant esophageal strictures

Objective To explore the efficacy and safety of improved covered metallic stents for proximal malignant esophageal strictures.Methods A total of 56 patients with proximal malignant esophageal strictures were treated with improved covered metallic stents whose diameter was 14 mm or 16 mm at Shanxi Province Tumor Hospital from Feburary 2013 to August 2014.Swallowing condition, physical and life quality were compared before and after surgery and analyzed.At the same time, side effects and complications were recorded during and after surgery.Results All 56 patients were treated successfully with improved covered metallic stents, no complications related with stents occurred after surgery.Swallowing conditions were improved after surgery, there were 45 patients with obvious improvement, 10 patients with better improvement and 1 patient's stent taken out due to stent intolerance.Remission rate of swallowing conditions was 98.2% (55/56).Two weeks after surgery, Stooler of swallowing conditions obviously improved (t =7.05, P < 0.05), physical conditions (ZPS) also improved (t =22.49, P < 0.05), but there was no significant difference in life quality (Kamofsky scores) (t =1.07, P > 0.05).During 90 days of follow-up there was no hemorrhage or perforation.Conclusion The application of improved covered metallic stents for proximal malignant esophageal strictures is safe and effective.
5.CDH Legacy instrumentation for scoliosis:techniques and results

[Objective] To evaluate the surgical techniques and its clinical results of CDH Legacy in the treatment of scoliosis.[Method]Nine patients(7 female and 2 male)with scoliosis instrumented with posterior CDH Legacy system were recruited for this retrospective study.The etiological classification of scoliosis were idiopathic for 7,congenital for 1,neurofibromatosis(NFl)for 1.The average age was 13.5 years(ranged 11~18 years).The average preoperative Cobb's angle was 54?(ranged 48?~68?).Seven AIS and one CS patients received one stage posterior CDH Legacy instrumentation;the NFl patients received one stage anterior epiphysiodesis and second stage posterior CDH Legacy instrumentation.[Result]There was no death,no infection and no neurological complication.No intra-operative fracture,no dura matter or pleuml laceration occurred.Superior Mesenteric Artery Syndrome occurred in one patients and recovered with conservative treatment.Posterior Cobb's angle averaged 20? with correction rate of 63%.The duration of follow-up averaged 23 months(ranged 20~30 months).Surgical time averaged 260 min(ranged 210~300 min)and mean EBL was 700 ml(300~1 000 ml).Bony fusions were achieved in all patients and no coronal or sagittal plane decompensation occurred.The loss of correction at last follow-up was 4? with 7.4% loss of correction rate.[Conclusion]The clinical results of CDH Legacy are as good as the other three generation implants,its advantage lies in lower profile,solid inter-locking,versatile implants and handy manipulation.
6.Allograft versus autograft grafting in posterior selective thoracic fusion in adolescent idiopathic scoliosis

Objective To compare the clinical outcomes of selective thoracic spinal fusion in adolescent idiopathic scoliosis using either iliac bone or autologous rib combined with allografts. Methods A retrospective study was made in 84 cases of idiopathic thoracic scoliosis, who were operated on with posterior instrumentation of CD, CDH or TSRH from 1999 to 2000. These patients were divided into two groups. In Group A, 34 patients, who were 12 to 20 years with a mean age of 14.5 years with a mean Cobb angle 56?(range, 40? to 82?), received autologous iliac bone grafting, and no thoracoplasty were performed. In group B 50 patients, who were 13 to 19 years with a mean age of 15.4 years with a mean Cobb angle of 54? (range, 38? to 80?), had allograft bone grafting combined with the ribs from thoracoplasty. The patients were followed up for at least 2 years with an average of 3.4 years regarding the clinical and radiographic outcomes. Results For group A, an average of 73.2% correction rate was achieved, and average 10% loss of correction and 3% of pseudoarthrosis was recorded at 2 year follow-up. The incidence of donor site morbidity was 26%, which included deep infection, hematoma, delayed healing of incision and continuous pain. For group B, an average of correction rate was 70.4% with 9% average loss of correction and 2% of pseudoarthrosis at 2 year follow-up. The complications related to thoracoplasty consisted of intraoperative tear of parietal pleural, pneumothorax, effusive pleuritis, and chest pain. Conclusion The clinical outcomes are comparable between the group of iliac bone grafing and the group of rib grafing associated with allografts in terms of fusion rate and loss of correction. If chose appropriately, allografts plus autologous rib harvested from thoracoplasty will be a good alternative to iliac crest regarding its safety, efficacy and reliability.

9.Clinical study of anti-neurohormone drugs on heart function and the plasma concentrations of neurohormone of patients with different grade heart function after myocardial infarction

ObjectiveTo observe the effects of anti-neurohormone drugs on ventricular remodelling and the plasma concentrations of neurohormone of patients with different grade heart function after myocardial infarction.MethodsAccording to ejection fraction of patients,103 consective patients with myocardial infarction were divided into two groups:normal heart function group:ejection fraction ranged from 50% to 70%.Abnormal heart function group:ejection fraction was less than 50%.All patients were given the same beta-receptor blocking agent and angiotensin-converting enzyme inhibitor and then were followed up 6 months.Heart function and the plasma concentrations of neurohormone were compared before and after anti-neurohormone drugs were given;at the same time,changes of heart function and the plasma concentrations of neurohormone were compared in statistical aspect between two groups.ResultsThe improvement of heart function and change of the plasma concentrations of neurohormone were significantly different in statistics before and after drugs were used.Besides,the patients with different grade heart function had significantly different changes of heart function and plasma concentrations of neurohormone.ConclusionAnti-neurohormone drugs can significantly reduce the level of the plasma neurohormone,improve heart function and inhibit heart remodeling.What’s more,there are better treatment effects in patients with heart failure.
10.Effect of purslane decoction on oxidative stress products and soluble protein of senile cataract patients tears

Objective To study the effect of purslane decoction on oxidative stress products and soluble protein of senile cataract patients tears.Methods 120 cases of cataract patients (160 eyes) were selected, according to different drugs were randomly assigned to two groups.The control group were treated with Imidacloprid sinnock sodium drops eye drops, 1 drop each time, 4 times per day,the patients in the experimental group on the basis of the control group with Portulaca oleracea decoction 100 mL, for three consecutive months.SOD, MDA, soluble protein level and visual improvement were compared after the end of treatment.Results Compared with control group, the visual acuity of the experimental group was higher(P<0.05), MDA was lower, SOD was higher(P<0.05), soluble protein content was higher(P<0.05).Conclusion Portulaca oleracea decoction can apparently improve the cataract patients with tear of SOD, MDA, soluble protein level and improve visual acuity.
