1.The Analysis on compared between before and post application of coding on classification of disease clinical modification of Beijing

ObjectiveFrom the view of the frequence of codes applying,investigate the situation of the Tenth Revision of the International Classification of Disease Clinical Modification of Beijing and to support the further development and application.MethodsFigure out the difference of the frequence of codes between before and post application of coding on clinical modification,to compare and make analysis for them.Results Got lists of percentage periods frequence of codes on classification of disease calculated from all secondary and above level hospitals in Beijing in the year of 2006,2007 and 2008.Conclusionthe Tenth Revision of the International Classification of Disease Clinical Modification of Beijing is well compatible with WHO ICD-10 on the level of applicable codes set; the refine work is effective and balance for codes applying; the localization should be done first before applying ICD-10.
2.Three biflavonoids from ethanol extract of the root of Daphne genkwa

Aim To investigate the chemical constituents of the secondary metabolites of the roots of Daphne genkwa. Methods The roots of D. genkwa were extracted with 95% ethanol at 60 - 70 ℃ for 7 days to obtain the crude extract. The crude extract was purified by silica gel and Sephadex LH-20 column chromatography as well as the HPLC techniques. The structures of the isolates were elucidated by combined spectroscopic methods including 1D and 2D NMR, MS, UV, IR and CD. Results Three new biflavonoids were isolated from the ethanol extract of the roots of D. genkwa and their structures were identified as daphnodorin H-3-methyl ether (1), daphnodorin H-3"-methyl ether (2) and daphnodorin G-3"-methyl ether (3). Conclusion Compounds 1, 2 and 3 are three new biflavonoids.
3.The clinical effect of treatment with external circular fixator in bone defect after tibial shaft fractures secondary infection

Objective To analyze the clinical effectiveness,safety and significance of treatment with external circular fixator,through the relationship of inducible nitric oxide synthase (iNOS) and bone regeneration to further explore the mechanism of the molecular biology of bone lengthening at the same time.Methods Collected bone defect after tibial shaft fractures secondary infection who were treated with external circular fixator as study group (28 cases),all the cases fixed by external circular fixator,20 healthy volunteers as control group.Study group in different time point measured the levels of serum iNOS content,were compared with the control group.Results All patients were scheduled to reach the extremity lengthening and the bone defect were repaired and healed in study group.The bacterial culture results showed that 18 cases (64.29%,18/28) of resistant Staphylococcus aureus,5 cases (17.86%,5/28) of Hemolytic streptocaccus,2 cases (7.14%,2/28) ofP.aeruginosa,one case (3.57%,1/28) of drug-resistant Enterobacter cloacae,one case(3.57%,1/28) of Staphylococcus epidermidis,one case(3.57%,1/28) of Onion-like aeromonas,the proportion of the former two was significantly higher than other bacteria (P < 0.05).The levels of serum iNOS content 1 d after surgery in study group were increased,compared with control group,difference was no significant (P > 0.05).The levels of serum iNOS content 14 d after surgery,stoped extend,1 d stoped extend,3 d stoped extend in study group were significantly higher than those in control group [(118.8 ± 6.6) U/L vs.(86.4 ± 5.4) U/L,(136.2 ± 8.4) U/L vs.(88.2 ± 3.6) U/L,(132.6 ± 9.0) U/L vs.(87.6 ± 4.8) U/L,(103.8 ± 5.4) U/L vs.(86.4 ± 4.2) U/L] (P < 0.05),there was no significant difference between two groups in other times (P > 0.05).Conclusions External circular fixator for the repair and reconstruction effects in patients with bone defect can achieve the desired therapeutic goal,the higher expression of iNOS may be one of the molecular biological mechanisms of bone lengthening.
4.Analysis for ~1H NMR fingerprint of Radix Daphne genkwa

AIM: To establish ()~1H NMR fingerprint of Radix Daphne genkwa to disclose its chemical composition of the secondary metabolites and provide a quick and credible assay for the detoxicated extraction of the active constituents. METHODS: The ()~1H NMR spectrum of petrol-acetone-methanol extract from Radix Dephne genkwa was detected using CDCl_3/DMSO-d_6(1∶1) as the deuterium reagent. The resonance intensity of the proton in the spectrum was indicated in relative integral area of peaks referenced by the resonance intensity of methyl signal in DMSO. The ()~1H NMR of cascade extraction of Radix Dephne genkwa by the reagent in sequence of petrol→acetone→methanol or chloroform→aether→acetyl acetate→methanol was utilized for observing the variation on their chemical compositions. RESULTS: The ()~1H NMR of Radix Dephne genkwa clearly expressed the information of the protons from long chain aliphatics or genkwadaphnin derivatives, aromatic coumarins or flavonoids as well as glycosides with moiety(ies) of pyran saccharide, symbolizing the existence of aliphatics, genkwadaphnin derivatives, coumarins and flavonoids. Remarkable difference was observed in ()~1H NMR spectrum of the extract by different cascade reagent. As the increase in the polarity of reagent, the intensity of the proton signals in upper field was quickly reduced concomitantly with the rapid enhancement of active proton signals from hydroxyls and saccharides in glycosides in down field. 6.The pathological mechanism of innate and adaptive immunity in the development of uveitis

Human body often contacts with different kinds of pathogens,but these pathogens do not result in disease due to innate immunity.The adaptive immunity is able to discriminate self and non-self pathogen and further maintain immunological health and appropriate response through peripheral regulatory mechanism.Whether innate immune or adaptive immune,lack of control mechanism leads to autoimmunity and autoinflammation.Up to now,some studies demonstrated that autoimmunity and autoinflammation have many common characteristics in intraocular inflammation,but their mechanism relates to different molecular pathways.Hence,ophthalmologists have better understanding in the pathogenesis of uveitis.Herein,it is inferred from the basic mechanism of the activation of immunology pathway that regulation of innate and adaptive immunity might impact how autoinflammatory and autoimmune pathways contribute to disease progress in uveitis.

8.Expression differences and clinical significance of miR-29b,miR-24 and miR-200c in plasma of infants with PCG and normal

Objective To identify the expression differences of mircoRNA-29 b (miR-29b),microRNA? 24 (miR-24) and microRNA-200 c (miR-200c) in plasma of infants with primary congenital glaucoma (PCG) and normal,and analyze its clinical significance.Methods The expression quantity of microRNAs (miR-29b,miR-24,miR-200c) in plasma of PCG group (16 cases) and normal control group (49 cases) were detected by RT-PCR,and the relationship between their expression differences and severity of disease were analyzed.The diagnostic value of miRNAs for PCG was evaluated by receiver-operating characteristic curve (ROC).Results The expression quantity of miR-29b,miR-24,miR-200c in PCG group (0.31 ±0.19,0.17 ±0.16,0.55 ±0.18,respectively) were significantly lower than those in normal control group (1.18-±0.52,2.86 ±2.65,1.62 ± 0.76,respectively) (all P < 0.05);The expression quantity of miR-24 and miR-29b in plasma was related to the severity of PCG,the more severe the disease,the lower the expression;ROC curve indicated that miR-24 and miR-29b had a higher diagnostic value for PCG disease than miR-200c.Conclusion Free miRNAs in plasma may be used as a new plasma markers for auxiliary diagnosis of PCG.
9.Effects of Bushenxingnao Prescription on Cerebral Neurocyte Apoptosis of Experimental Vascular Dementia Rats

Objective To study the pathology mechanism of vascular dementia(VD)rats and study the effect of Bushenxingnao(BSXN)prescription on cerebral neurocyte apoptosis.Method Experimental VD model rats were caused by clipping two side of total artery in neck repeatedly and to examine cerebral neurocyte apoptosis with TUNEL.Results Both the high and low dose BSXN abated excitatory toxic infury and inhibit neurocyte apoptosis after ischemia-refusion.Conclusion BSXN prescription can repress cerebral neurocyte apoptosis,to abate its excitatory nerotoxicity,to protect cerebral neurocyte,which may be one of the mechanisms of to treating the lesion.
