1.Analysis of risk factors of wound complications following the abdominal aorta balloon occlusion

Objective To investigate the risk factors of wound complications following the abdominal aorta balloon occlusion in order to provide the basis for the preoperative management and reduce postoperative complications.Methods Two hundred and sixty-one patients with the sacral tumor aged over 18 were selected as subjects who hospitalized at Peking University People's Hospital from January 2006 to December 2012.All subjects were underwent sacral tumor posterior resection operation after occlusion of the distal abdominal aorta.Patients were divided into two groups according to the existence of postoperative wound disorder.The general information of the patients was collected including intra-operative data and post-operative data.SPSS19.0 software was applied for statistic analysis Results Among 261 cases,89 cases were with incision complications,and the incidence of complications was 34.1% (89/261).There was no significant difference regarding of age,weight,preoperative complications (P < 0.05).Compared with that of no complications group,operation time of wound complications group was longer ((218.27 ± 72.08) min vs.(197.40 ± 53.92) min,P =0.046),and intra-operative bleeding was more ((2368.0 ± 1984.7) ml vs.(1701.6 ±958.8) ml,P =0.000),as well as a longer balloon occlusion time ((78.0 ± 23.8) rain vs.(65.2 ± 17.8) min,P =0.005).Moreover long hospitalized periods was in wound complications group ((47.6 ± 26.3) d,vs.(26.7 ± 9.0) d,P < 0.05).Single factor analysis showed that the factors including operation time,intra-operative bleeding volume,time and times of balloon occlusion balloon occlusion were related to postoperative wound complications.Non conditional logistic regression analysis shows that factors of the balloon occlusion time(OR 1.028,95% CI 1.012-1.045,P =0.001) and block number(OR 6.266,95% CI 1.186-33.119,P =0.031) were related to the wound healing following balloon occlusion the abdominal aorta.Conclusion The risk factors of wound complications following the abdominal aorta balloon occlusion included occurrence balloon occlusion time and blocking times.That single balloon occlusion with 60 min reduced wound complication rate.
2.Quantitative analysis of microcirculation in renal solid lesions with gamma curve fitting technique of contrast-enhanced ultrasound

Objective To evaluate the role of gamma curve fitting technique of contrast-enhanced ultrasound in quantitative analysis of microcirculation in renal solid lesions. Methods A total of fifty patients with renal parenchyma solid lesions were performed contrast-enhanced ultrasound. The images were analysed by computer with gamma fitting analysis of contrast-enhanced ultrasonic system. The quantitative parameters were obtained by the time-intensity curves, such as ascending slope (a3), descending slope (a2), arrival time (AT), time to peak intensity (TTP), basic intensity (BI), peak intensity, amplification (AMP), area under the curve (AUC), mean transit time (MTT) and perfusion index (PI). The parameters were compared between renal malignant and benign solid lesions. Results Fast-in and fast-out was the main perfusion mode in renal malignant tumors while slow-in and slow-out was found in renal angiomyolipoma (AML). The perfusion modes in renal malignant tumors and renal AML were fast-in and fast-out in 28 cases and 0 case, fast-in and slow-out in 4 cases and 1 case, slow-in and fast-out in 5 cases and 1 case, and slow-in and slow-out in 1 case and 10 cases, respectively. There were significant differences in the quantitative parameters such as a2, AUC and PI between renal malignant tumors and renal AML obtained by the time-intensity curves (P<0.05). Conclusion Gamma fitting analysis of contrast-enhanced ultrasound system can provide quantitative information of microcirculation of renal tumors, which helps to differentiate benign renal tumors from malignant ones.
3.The significance of TGF-beta expression in scar in adenosine receptor A(2A) knockout mice.

OBJECTIVETo discuss the mechanism of scar hypertrophy in adenosine receptor A(2A) (A(2A) R) knockout mice.
METHODSAnimal models of hypertrophic scar were established in 12 A(2A) R knockout mice and 12 wild-type mice as control. The thickness and the size of transverse section of the hypertrophic scar were observed by H-E staining. The hydroxyproline (HYP) in the scar was measured colorimetrically. The TGF-beta expression was tested by Western blotting method.
RESULTSThe hypertrophic scar in wild-type mice was more severe than that in knockout mice. Compared with self-control, the increase of the thickness and the size of transverse section of hypertrophic scar was markedly higher in wild-type group than in the knockout group (P < 0.01). There was significant difference in HYP content between the two groups (P < 0.01). Compared with self-control, the increase of TGF-beta expression in wild-type group was much more than that in knockout group (P < 0.01).
CONCLUSIONSThe TGF-beta expression decreases in the A(2A) R knockout mice. The scar hypertrophy is also much less in the A(2A) R knockout mice.

4.Changes in the state of phosphorylation of DARPP-32 of gerbil striatum during ischemia

Using the mongolian gerbil model of transient forebrain ischemia by bilateral carotid arteries occlusion, the levels of phosphorylation and the amount of DARPP-32 in striatum were measured by back-phosphorylation and Western-blotting assay in vitro. Transient forebrain ischemia had no effect on the immunoreaction of DARPP-32 in striatum, but the state of phosphorylation of DARPP-32 showed a significant change. The [32P] phosphate incorporation in DARPP-32 decreased after 2, 7 or 10 min of ischemia, but increased after a 5-min ischemia. In contrast, the result of back-phosphorylation in vivo was opposite to that obtained in vitro.
5.Simultaneous determination of seven chemicals of halogenated alkanes and aromatic hydrocarbons in the air of workplace by gas chromatography.

OBJECTIVETo establish a gas chromatographic method for determination of halogenated alkanes and aromatic hydrocarbons including trichloromethane, 1,2-dichloroethane, carbon tetrachloride, benzene, toluene, ethylbenzene and xylene in the air of workplace.
METHODSAfter the air samples collected with activated carbon tubes and desorbed with CS(2), the target toxicants were separated with FFAP capillary columns and detected with flame ionization detector.
RESULTSThe coefficient of correlation was above 0.999 and the lowest detectable concentrations were 0.2 ∼ 3.6 mg/m(3) with the RSD of 1.2% ∼ 4.6%. The desorption efficiencies was 94.9% ∼ 100.7%.
CONCLUSIONThe method shows lower detection limit, high accuracy and precision. It is feasible for determination of the seven halogenated alkanes and aromatic hydrocarbons in the air of workplace.

6.Clinical outcome after pressure reduction by peritoneal catheterization in 29 patients with malignant ascites-induced abdominal compartment syndrome.

OBJECTIVETo investigate the efficacy of pressure reduction by peritoneal catheterization in patients with malignant ascites-induced abdominal compartment syndrome (ACS).
METHODSClinical data of 29 patients with malignant ascites-induced ACS from October 2002 to October 2008 were analyzed retrospectively. Intra-abdominal pressure (IAP) was reduced by peritoneal catheterization. Changes of intra-abdominal pressure and ascites volume were observed during treatment. Clinical signs and urinary volume were monitored.
RESULTSIAP was less than 25 cm H(2)O in 2 cases, 25 to 35 cm H(2)O in 21 cases, more than 35 cm H(2)O in 6 cases. IAP decreased significantly after drainage of 1000 to 1500 ml of ascites, then IAP curve leveled off. With all the ascites drained, IAP maintained at 11 to 12 cm H(2)O and at 6 to 8 cm H(2)O after 24 hours. Blood pressure was stable without significant changes before and after IAP reduction (P>0.05). The breathing rate and heart rate were improved, and 24 h urinary volume increased significantly after IAP reduction (P<0.01).
CONCLUSIONEarly peritoneal catheterization can improve the cardiac, pulmonary, and renal function in malignant ascites-induced ACS.

7.Expression and relationship of Beclin1 and Bci2 in invasive pituitary adenomas

Objective To detect the expression of Beclin1 and Bcl2 in invasive pituitary adenomas and to explore the relationship of Beclin1 and Bci2 in invasive pituitary adenomas and the relativity between the 2 genes.Methods 61 specimens were classified into invasive group (32 cases) and non-invasive group (29 cases) according to the comprehensive evaluation of invasive pituitary adenomas.lmmunofluorescence analysis and RT-PCR were adopted respectively to detect the protein and mRNA expressions of Beclinl and Bcl2.The difference and relativity of Beclin1 and Bcl2 expression in invasive group and non-invasive group were analyzed.Results 32 specimens of pituitary adenoma were invasive and 29 were non-invasive.Beclin1 protein and mRNA expressions were lower in the invasive group than in the non-invasive group (P <0.01 ).Bcl2 protein and mRNA expressions were higher in the invasive group than in the non-invasive group (P <0.01 ).Pearson related analysis showed that Beclin1 mRNA expression was negtively correlated with Bcl2 mRNA expression in the invasive group ( r =-0.42,P =0.028 ).Conclusions Beclinl expression is decreased in invasive pituitary adenomas.The invasiveness of pituitary adenoma is closely related to the high expression of Bcl2 protein and mRNA,and the low expression of Beclin1 protein and mRNA.The inhibition of the autophagy may lead to the enhancement of the invasiveness of pituitary adenomas and that inhibition may come from the interaction of Beclin1 and Bcl2.
8.Endoscopic surgery using the low-temperature plasma radiofrequency for nasopharyngeal angiofibroma

Objective To evaluate the feasibility of endoscopic surgery using the low-temperature plasma radiofrequency for nasopharyngeal angiofibroma (NA). Methods The clinical data of 4 patients treated between December 2005 and August 2009 were retrospectively analyzed. The tumor in one patient was at stage Ⅰ and three patients were at stage Ⅱ, all these patients were treated by endoscopic surgery using the low-temperature plasma radiofrequency under controlled hypotension anesthesia. Preoperative angiography and embolisation were underwent in all patients. Bipolar coagulation was used to help hemostasis during operation. Results The tumor in all 4 patients was completely removed. The operation time was 60,80, 110, 90 min respectively. The blood loss was 250, 250, 320, 280 ml respectively. Neither blood transfusion nor complications were encountered perioperatively. Follow-up ranged from 6 months to 4 years,none of the patients recurred. Conclusions Endoscopic surgery using low-temperature plasma radiofrequency for nasopharyngeal angiofibroma has many advantages such as less bleeding and total tumor removal. It is a minimally invasive surgical method for nasopharyngeal angiofibroma.
9.Simultaneous determination of 3 phenolic acids in Usnea by HPLC-ESI-MS/MS.

A quick HPLC-ESI-MS/MS method was established for simultaneous determination of three chemical compositions in Usnea, including usnic acid, diffractaic acid, and ramalic acid. The separation was performed on a chromatographic column of Agilent ZORBAX SB-C, (4.6 mm x 250 mm, 5 µm), and the mobile phase was methanol (0.05% formic acid)-0.05% formic acid solution (4 mmol ammonium acetate), with an isocratic elution at a flow rate of 0.8 ml · min⁻¹. Multiple reaction monitoring scanning mode (MRM) was performed combined with the ion switching technology in positive and negative ion switching mode to apply for the quantitative determination. The calibration curves for the above three compounds were linear in corresponding injection amount. Their average recoveries were 95.0%-105.1%, with RSDs of 1.1%-5.2%. The method was simple, rapid, accurate with high repeatability, which could provide a reference for overcalling evaluation the quality of Usnea.

10.Prophylaxis of graft-versus-host disease in mice by chemical modification of graft and OX40-OX40L costimulatory pathway..

OBJECTIVETo explore the prophylaxis effect of pretreatment of allograft with methoxypolyethylene glycol-succinimidyl-propionic acid ester (mPEG-SPA) and anti-OX40L monoclonal antibody (McAb) on acute graft-versus-host disease (aGVHD) after allogeneic bone marrow transplantation (allo-BMT) in mice.
METHODSResponder splenocytes from C57BL/6 donor mice (H-2(b)) were co-cultured with stimulator splenocytes from BALB/c recipient mice (H-2(d)) for 7 days in the presence or absence of anti-OX40L McAb followed by mPEG-SPA chemical modification. Donor bone marrow cells plus the mixed culture of T-cells were then transplanted into lethally irradiated BALB/c mice. The BALB/c recipient mice were divided into four groups: group A (allo-BMT control group), group B(mPEG-SPA modification group), group C (anti-OX40L McAb pretreated group) and group D (mPEG-SPA and anti-OX40L McAb dual-treated group). Survival time and survival rate of the recipients were observed after allo-BMT. GVHD was assessed by clinical signs and histological changes of skin, liver and small intestines. Enzyme-linked immunosorbent assay (ELISA) was used to detect cytokines (IL-4, IL-10 and INF-gamma) production. Flow cytometry (FCM) analysis was used to detect allogeneic chimerism.
RESULTS(1) The mice in group A developed typical clinical signs of aGVHD and all mice died within 17 days after BMT with an average survival time (AST) of (12.1 +/- 5.5) days. The signs of aGVHD were less evident in mice of groups B, C and D, and their AST (36.2 +/- 24.9, 32.0 +/- 24.8 and 44.3 +/- 23.2 days, respectively) were all longer than that in group A (P < 0.05). AST of group D being the longest (P < 0.05). The survival rates at day 60 post-BMT in groups B, C and D were 50%, 41.7% and 66.7%, respectively. (2) Serum IFN-gamma level was increased after BMT in group A, and peaked in day 10 to day 15 post-BMT, while the level was decreased in groups B, C and D, reached the nadir on the day 10 post-BMT, with the lowest in group D (P < 0.01). After BMT, IL-4 and IL-10 levels were slightly decreased in group A, their levels were elevated in groups B and C (P < 0.05) and even more significantly increased in group D (P < 0.01). IL-4 and IL-10 levels peaked between day 10 and 15 post-BMT. (3) The average proportion of H-2(b) positive cells in recipient mice was 95% - 100% on day 60 post-BMT, with complete donor-type implantation.
CONCLUSIONCombination of mPEG-SPA and anti-OX40L McAb can block T-cell activated antigens and co-stimulatory pathway, regulate the T cells differentiation and induce the immune shift of Th0 cells toward Th2 cells. The immune tolerance induced by this method can significantly relieve aGVHD after allo-BMT.
