1.Medical order-obeying behavior in 164 patients with essential hypertension

BACKGROUND: Medical order-obeying behavior(MOOB) in patients with essential hypertension(EH) cau directly affect the progression and prognosis of the disease. What factors, in turn, affect MOOB?OBJECTIVE:To analyze factors affecting MOOB in EH patients.DESIGN: Cross-sectional study.SETTING: Foreign Guests Ward of Qindu Stomatological Hospital, Fourth Military Medical University of Chinese PLA.PARTICIPANTS: A total of 205 patients with EH were admitted into the Foreign Guests Ward of Qindu Stomatological Hospital, Fourth Military Medical University of Chinese PLA from January 1998 to August 2000. Inclusion criteria: In accordance with WHO diagnostic criteria for hypertension. Exclusion criteria: Diabetes and other cardiovascular diseases. Altogether 164 patients with the mean age of (51 ± 5) years, 122 men and 42 women , accorded to the inclusion criteria.METHODS: The 164 patients were investigated with the self-designed questionnaire concerning MOOB in EH patients; patients' sex, educational level, family economic conditions, disease duration and other factors affecting MOOB were observed.MAIN OUTCOME MEASURES:Factors related to the patients' MOOB.RESULTS:53.0% (87/164) patients followed the medical orders and persisted in exercise therapy, 58.5 % ( 96 / 164 ) patients followed reasonable diet, 51.6% (32/164)patients quitted smoking and alcohol drinking, 82. 3% (135/164) received regular recheck-up, 64.6% (106/164) followed the medicinal treatment prescribed. Sex did not influence MOOB significantly. The rate of medical-order disobedience was higher in illiterate people and those with education below junior middle school level, but lower in those with education above senior middle school level(x2 =7.25, P < 0.01) . The disobedience rate was lower in patients with the mean income of > 500 yuan/month than in those with < 500 yuan/month(x2 = 39.4, P < 0.01),and was also lower in patients with disease duration < 1 year than those with disease duration > 1 year (x2 =5.66, P < 0.05).CONCLUSION; Medical order-obeying behavior in EH patients is poor, and it is significantly influenced by patients' educational level, family economic conditions and disease duration.
2.Multiple enamel pearls on left maxillary third molar: a case report.

Enamel pearl is an ectopic enamel, which usually occurs in the root bifurcate or approaching enamel-cementum site of the first maxillary molar. A case of multiple enamel pearls on the left maxillary third molar is reported in this paper, and relevant literature was reviewed.

3.Effect of Shenqi Wan on Calcium Channel Protein CaV1.3 in Cochlear Hair Cells of Guinea Pigs with Ototoxic Deafness Induced by Gentamicin

Objective To observe the effect of Jingui Shenqi Wan (JSW) on the content of calcium channel protein CaV1.3 of guinea pigs with ototoxic deafness induced by gentamicin (GM). Methods Forty healthy guinea pigs were randomly divided into normal group, model group and high-, middle-, and low-dose JSW treatment groups. The model group was given intramuscular injection of GM ( 120 mg/kg) per day for 10 continuous days. The high-, middle-, and low-dose JSW treatment groups were given intramuscular injection of GM (120 mg/kg) and intragastric administration of JSW in the dosage of 20.3, 13.5, 6.08 g·kg-1·d-1 respectively per day for 10 days. Immuno histochemistry was used to detect the mean optical density value of CaV1.3 expression in cochlear hair cells. Results There were significant differences of the optical density value of CaV1.3 in cochlear hair cells between the model group and normal group (P<0.01), and between the model group and high-dose JSW treatment group (P<0.05). Conclusion JSW has certain effect on preventing and treating guinea pig with ototoxic deafness induced by GM, thus to protect the hearing function.
4.Effects of the aqueous extract of dendrobium nobile lindl on NF-κB and IL-6 expression of renal tissue in diabetic rat model

Objective To observe the effects of dendrobium nobile Lindl(DNL) on Nuclear factor(NF)-κB and interleukin(IL)-6 expression of renal tissue in diabetic rat model .Methods The diabetes mellitus rat model was induced by streptozotocin .All rats were divided into four groups at random :the normal control group(group N) ,diabetic control group(group diabetes mellitus) ,drug group one(group DNL1) ,drug group two(group DNL2) ,and administrated for 12 weeks .The levels of the fasting plasma glucose , urea nitrogen ,serum creatinine ,urine creatinine ,24 hours urinary protein were determined .NF-κB and IL-6 protein expression levels were measured by Western-Blot .IL-6 mRNA expression levels were measured by Real-time fluorescence quantitative PCR . Results Compared with group N ,the blood sugar ,urea nitrogen ,24 hours urinary protein of group diabetes mellitus were signifi-cantly higher(P<0 .01) ,creatinine clearance was significantly reduced (P<0 .01) ,the NF-κB、IL-6 protein expression levels and IL-6 mRNA level increased significantly (all P<0 .05) .Compared with group diabetes mellitus ,the blood sugar ,urea nitrogen ,24 hours urinary protein of group DNL1 and group DNL2 were significantly decreased(P<0 .05) ,creatinine clearance increased significantly (P<0 .05) ,the NF-κB、IL-6 protein expression levels and IL-6 mRNA level decreased significantly(all P<0 .05) .Conclusion The DNL could protect the kidney of diabetic rats ,reduce the levels of NF-κB 、IL-6 protein and IL-6 mRNA expression in renal tissue of diabetic rats .
5.Application of the NuMED covered CP stent in treatment of adult coarctation of the aorta

Objective To evaluate the NuMED covered CP stent for the treatment of adult aortic coarctation.Methods 6 patients with coarctation of the aorta received covered CP stent implantation.There were 4 males and 2 females cases with mean age of (20.13±5.16) years old.The size of the covered CP stents and balloons were determined by preoperative aortic CTA assessment.Stent implantation was per formed through femoral approach and the puncture site was sealed by vascular closure device after the operation.Results All the NuMED covered CP stents were implanted successfully.The pressure gradient dropped from (59.32±15.78) mmHg to (10.83±3.66) mmHg (P<0.001) after stenting.The diameter of the coarcation at the narrowest point increased from preoperative (5.1±1.2) mm to (19.7±2.4) mm after stenting (P<0.001).In 2 cases with PDA, the shunting disappeared spontaneously after operation with normalized of lower limb blood pressure.All the patients were followed up clinically for 12-36 months and all of them reported improvement in clinical symptoms post operation.Follow up Doppler echo or aorta CTA examinations at 3 months showed no restenosis.No mortality recorded during follow up.Conclusions NuMED covered CP stent for the treatment of aortic coarctation in adults showed satispactony short term outcome.Its long term outcome still requires further in vestigation.
6.Acute Effect of Air Pollution on Cardiovascular Disease Mortality in Old People in Taiyuan City

Objective To understand the acute effect of the air pollution on daily mortality of cardiovascular diseases in the old people aged over 65 years in Taiyuan, Shanxi, China. Methods Case-crossover design was used to assess the association between air pollution and daily cardiovascular disease mortality. Pollutants considered were nitrogen dioxide (NO2), carbon monoxide (CO), inhalable particles (PM10) and sulfur dioxide (SO2). Results CO exposure was significantly associated with the mortality of five kinds of cardiovascular diseases. No association was found for arrhythmia. As CO concentration increased by 100 ?g/m3, the corresponding OR of cardiovascular disease mortality were as follows, total cardiovascular disease mortality were1.000-1.013, cardiac mortality were1.000-1.024, ischemic heart disease were 1.005-1.032, cardiac failure were 1.006-1.112, myocardial infarction were 1.009-1.050 respectively. The higher OR value was found in unidirectional control design than that in the bidirectional design. Considering the lag effect of air pollution, the 24 hr and 48 hr average of air pollution levels were analyzed in the present study respectively. Conclusion The current air pollution level (PM10, CO) in Taiyuan City has a acute effect on the cardiovascular disease mortality in the local old people aged over 65 years.
7.DISCRETE RADIOFREQUENCY CATHETER ABLATION FOR PAROXYSMAL ATRIAL FIBRILLATION IN A DOG MODEL

To investigate an approach of treatment for atrial fibrillation with discrete ablation lesion, 12 dogs with experimental atrial fibrillation underwent radiofrequency catheter ablation. Atrial lesion was made at the most vulnerable site. The results showed that in 10 of 12 dogs persistent fibrillation could be ablated. There was no significant change in sinus heart rate, P wave duration, and corrected sinoatrial node recovery time (cSNRT). The results suggested that paroxysmal atrial fibrillation could be eliminated by point ablation of the lesion, and that trigger activity might be involved in the mechanism of atrial fibrillation.

9.Detection of ESBLs in Enterobacter cloacae by potentiation test with aminophenylboronic acid and clavulanic acid

Objective To study the function of aminophenylboronic acid(APB)and clavulanate(CA)for detecting ESBLs in Enterobacter cloacae.Methods The phenotype of ESBLs of 61 Enterobacter cloacae isolates was detected with adding single beta-lactamase inhibitor CA to ceftazidime(CAZ)and cefotaxime(CTX),and double beta-lactamase inhibitors CA/APB to ceftazidime(CAZ)and cefotaxime(CTX)respectively.PCR was used to detect ESBLs genes of 61 Enterobacter cloacae isolates.The results of the enzymatic inhibitor potentiation test and PCR were compared and analyzed.Results With adding single enzymatic inhibitor CA to CAZ,28 isolates of Enterobacter cloacae producing ESBLs were detected,while 14 isolates were detected with adding CA to CTX.With adding double enzymatic inhibitors CA/APB to CAZ,28 isolates of Enterobacter cloacae producing ESBLs were detected,while 44 isolates were detected with adding CA/APB to CTX.By PCR positive ESBLs genes were detected in 47 isolates of Enterobacter cloacaes.Conclusions The potentiation test with double beta-lactamase inhibtion can be used to detect ESBLs in Enterobacter cloacae.
10.Research the Expression of MDR1 Genes in Vaginal Candida Albicans Resistance to Azole Agents

Objective To investigate the expression levels of MDR1 gene from azole resistant strains in vaginal Candidaalbi-cans.Methods 60 strains of Candida albicans from recurrent vagintis patients were collected and identified by CHROM agar from Aug.2011 to Aug.2012.The drug resistance was detected by disk diffusion method.According to the result of drug sensitivity test,the strains were diveded into four groups:sensitive group moderately susceptible or resistant to flucon-azole group moderately susceptible or resistant to detoconazole group and moderately susceptible or resistant to miconazole group.Each group was selected 12 strains randomly.The expression of the MDR1 gene in these 48 strains were detected by real-time RT-PCR and analysed the datas by t test statistics.Results There were 22 sensitive strains,26 strains of moder-ately susceptible or resistant to detoconazole,12 strains of moderately susceptible or resistant to fluconazole and 38 strains of moderately susceptible or resistant to miconazole in 60 strains of Candidaalbicans.The relative expression of MDR1 gene in sensitive group was 0.41±0.47,in moderately susceptible or resistant to detoconazole group fluconazole group and micon-azole group were 3.32±4.46,2.27±3.05 and 0.9±0.81 respectively.Compared with the sensitive group,t values of mod-erately susceptible or resistant to detoconazole group,fluconazole group and miconazole group were-2.177,-2.130 and-2.094.The expression level of MDR1 gene had no statistical significance between the sensitive strains and moderately sus-ceptible or resistant strains(P>0.05).Conclution The relationship between MDR1 expression and the resistance to azole agents in vaginal Candida albicans requires further study.
