Objective To discuss the efficacy of the modified subinguinal approach for the treatment of acetabular anterior column and wall fractures.Methods A clinical retrospective study was performed of the 5 patients who had been treated at our department from January 2011 to April 2015 for acetabular anterior column and wall fractures and conformed to follow-ups.They were 3 males and 2 females,aged from 19 to 45 years (mean,35.6 years).The duration from injury to surgery averaged 7.6 days (range,from 4 to 10 days).The subinguinal ligament approach was adopted in the 5 cases,through which the inguinal ligament was preserved intact after sharp dissection of its insertion at the anterior superior iliac spine and medial-upper advancement,the advantage of expanded exposure of the first window by the Farid sub-ilioinguinal approach was retained,and additional lesions due to iliac osteotomy were avoided.The operation time,intraoperative bleeding,postoperative reduction and time for fracture union were documented.Merle d' Aubigne and Postel scoring system was used to assess the hip joint function of the affected limb at the final follow-ups.Results The operation time ranged from 110 to 150 minutes (mean,125 minutes);the intraoperative bleeding ranged from 415 to 550 mL (mean,450.2 mL).By the Matta's criteria,the postoperative reduction was rated as excellent in 4 cases and good in one.The 5 patients were followed up for an average of 32.4 months (range,from 6 to 43 months).Their fractures united clinically after an average of 4.6 months (range,from 3 to 6 months).The hip joint function was rated as excellent in 2 and good in 3 by the Merle d' Aubigne and Postel scoring system at the final follow-ups.Follow-ups observed no serious complications like inguinal hernia,internal fixation failure,myositis ossificans,or avascular necrosis of the femoral head.Conclusions As the modified subinguinal approach can provide a broad surgical exposure,lead to minimal injury to the inguinal ligament,avoid lesions caused by iliac osteotomy,and decrease operation time and bleeding,it may be a better approach for acetabular anterior column and wall fractures.

Objective To evaluate the therapeutic effect of catheter-directed thrombolysis (CDT) for the treatment of acute deep venous thrombosis (DVT) of the lower extremity.Methods Clinical data of 195 patients of acute DVT treated by CDT and adjunctive angioplasty and stenting from August 2010 to February 2014 were analyzed retrospectively.CDT by antegrade puncture of popliteal vein,CDT by great saphenous vein and CDT by retrograde puncture of contralateral femoral vein were used in these cases.Venous recanalization was graded by a thrombus score based on pre-and post-treatment venography.Results Technique success rate,clinical success rate,in popliteal vein group,great saphenous vein group and contralateral femoral vein group were 94.6％,72.2％ and 90.3％,97.3％,83.3％,and 90.0％.Patent rate of deep venous,patent rate of stenting and mild post-thrombotic syndrome (PTS) rate were 88.6％,66.7％ and 75.0％,83.3％,57.1％ and 88.9％,8.3％,26.7％ and 20.8％.Conclusion CDT by antegrade puncture of popliteal vein group combined with adjunctive angioplasty and stenting is safe and effective with higher clinical success rate and better long term results than other approaches for the treatment of DVT patients.

To improve students' experimental research skills,innovative consciousness,promote the construction of functional experimental center,we established series of “comprehensive,contrivable,innovative” experiments in medical students.

After 28 foreign species of AM fungi were inoculated in sterilized soil, the effects of the AM mycorrhizal colonization and the medicine quality of Paris polyphylla var. yunnanensis were observed by combination of inoculation test in pot at room temperature and instrumental analysis. The results showed that, compared with control group (CK), the inoculation of foreign AM fungi in the soil influenced the spore density, mycorrhizal infection rate, and colonization intensity of AM fungi in root system of P. polyphylla var. yunnanensis. The inoculation of foreign AM fungi enhanced the mycorrhiza viability of P. polyphylla var. yunnanensis by increasing the activity of succinic dehydrogenase (SDH) and alkaline phosphatase (ALP) in intraradical hyphae. The content of single steroid saponin in rhizome of P. polyphylla var. yunnanensis showed variation after P. polyphylla var. yunnanensis was inoculated by different foreign species of AM fungi, which was beneficial for increasing the medicine quality; however, the kinds of steroid saponin showed no difference. In a degree, there was a selectivity of symbiosis between P. polyphylla var. yunnanensis and foreign AM fungi. And we found that the Claroideoglomus claroideum and Racocetra coralloidea were best foreign AM fungi species for cultivating P. polyphylla var. yunnanensis under field condition.

Objective To investigate the distribution ofβ2-adrenergic receptor (β2-AR)genetic polymorphisms among the included parturient women and observe the relationship between the genetic polymorphisms and labor progress after labor analgesia.Methods A prospective observational study was conducted from March to June in 2014 at Nanjing Maternity and Child Health Care Hospi-tal.A total of 1 65 nulliparous women were enrolled in the study,and finally 1 52 patients were includ-ed after exclusion for several reasons.Theβ2-AR gene polymorphic variations at nucleotides encoding amino acids 1 6 and 27 were detected by polymerase chain reaction-restriction fragment length poly-morphism technique.And the demographic data,rate of oxytocin usage,length of labor progress and the incidence of cesarean section after labor analgesia were observed and recorded.Results The allele frequencies ofβ2-AR genetic polymorphism in the parturient women were dominantly presented in the form of Arg1 6Arg (AA)and Arg1 6Gly (AG)at 1 6 position and Gln27Gln(CC)at 27 position respec-tively.The demographic and background characteristics of the subjects didn’t present any significant differences among the groups with differentβ2-AR genetic polymorphism.And the differences of labor progress among β2-AR 27 genetic polymorphisms parturient women were not significant.However, the length of the first stage and the total labor progress in the β2-AR Arg1 6Arg homozygous were both significantly longer than that in the other two groups (P <0.05),and the incidence of cesarean section was also significantly higher (P <0.05 ).Furthermore,we also found that β2-AR Arg1 6Arg homozygous was related to cesarean delivery after labor analgesia through multivariate logistic regres-sion analysis (OR 2.87,95%CI 2.69-3.09).Conclusion The labor progress of the parturient women with β2-AR Arg1 6Arg homozygous is relatively slower,which maybe an important risk for the nullip-arous women transforming to cesarean delivery after labor analgesia.

"Mistreatment by warming therapy" refers to various symptoms/syndromes caused by improper use of fire methods mentioned in Treatise on Febrile Diseases, which are similar to damaged yin induced fire-heat syndrome (injury of yin, consumption of qi, loss of yin, consumption of body fluid) manifested after radiotherapy and/or chemotherapy in modern clinics. Better efficacy was obtained in treating various symptoms/syndromes by using nourishing yin purging fire method. [Objective] To explore risk factors for ectopic pregnancy to provide basis for its prevention.[Methods] A case-control study was done on 180 cases of ectopic pregnancy treated in Deqing Chinese medicine hospital from January 2012 to December 2016 and another 180 cases of intrauterine pregnancy were as control who received artificial abortion in outpatient clinic during the same period. The data on the factors that might bring about ectopic pregnancy were analyzed by single-factorial and multi-factorial logistic regression. [Results] The multi-factorial analysis confirmed that the risk factors associated with ectopic pregnancy were prior abortion history, pelvic inflammation, ectopic pregnancy history, fallopian tube surgery history and others. [Conclusion] Ectopic pregnancy is associated with multiple factors, whose occurrence should be prevented and reduced by taking corresponding measures against it.
