1.Surgical treatment of large hepatocellular carcinoma

Hepatocellular carcinoma (HCC) with a diameter > 5 cm is defined as large HCC. Hepatic resection is the first choice for solitary large HCC with intact capsule and without satellite nodules. The key to successful large HCC resection is to judge the resectability and estimate the remnant liver function preoperatively. Moreover, the liver must be exposed and dissociated adequately, and familiarity with the anatomy of the liver is crucial. Choosing the right technique of hepatic blood flow occlusion and avoiding excessive resection of the liver are important elements. Special attention should be paid to the efficacy of liver transplantation for patients with large HCC exceeding Milan criteria, h is advisable to resect large HCC actively.The hepatic resection is safe and feasible in selected patients with large HCC.
2.Real-world study: a potential new approach to effectiveness evaluation of traditional Chinese medicine interventions.

Some conclusions drawn from explanatory randomized controlled trial (ERCT) lack practical value in application under real-world clinical settings, and there are also some limitations on the generalization of pragmatic randomized controlled trial (PRCT) results. In some countries, real-world studies (RWSs) have been conducted to explore the effectiveness and safety of clinical interventions or postmarketing drugs, which is a new tendency in clinical researches. By reviewing some RWSs carried out in Western countries, we sum up the basic characteristics of the research design, and discuss the differences and connections between PRCT and RWS. The design ideas of RWS are different from PRCT. Researchers in RWS tend to perform long-term evaluation based on large quantities of subjects and quite large sample and focus on outcome measures which are clinically meaningful. Strict control on data collection, management and analysis are very important to RWS and PRCT. Owing to the complexity of clinical interventions and evaluation, PRCT is not completely suitable for clinical researches in traditional Chinese medicine (TCM). It is an inevitable tendency to apply RWS in clinical trials of TCM. We can explore the efficacy of TCM interventions through PRCT, and better understand the effectiveness through RWS. RWS will become a powerful approach to TCM clinical trials and postmarketing evaluation of Chinese medicines.
3.Research Progress in the F Gene and Protein of the Respiratory Syncytial Virus.

The respiratory syncytial virus (RSV) belongs to the family Paramyxoviridae and subfamily Pneumovirinae. The RSV can cause acute infections of the lower respiratory tract in infants. The F gene of the RSV is a conservative gene and varies only slightly in its expression. Few studies focusing on the variability of the F gene have been carried out. F protein (fusion glycoprotein) is a transmembrane glycoprotein that mediates fusion and penetration between the virus and host cells. Neutralizing antibody against the F protein can protect against infection by RSV subtypes A and B. Hence, F protein has become the main target for the development of a monoclonal antibody and vaccine against the RSV. An effective vaccine is not available, so a monoclonal antibody against F protein is now the most important method to reduce the morbidity and severity associated with RSV infection in high-risk children. However, a monoclonal antibody can lead to the production of drug-resistant strains of the RSV. This review focuses on genetic variation of the F gene of the RSV as well as progress in the development of a monoclonal antibody against F protein and a vaccine in the last decade.

4.Clinical Observation of Yin-nourishing and Qi-tonifying Recipe Decoction in the Treatment of Type 2 Diabetic Stress Urinary Incontinence

OBJECTIVE:To observe clinical efficacy and ADR of Yin-nourishing and qi-tonifying recipe in the treatment of type 2 diabetic stress urinary incontinence. METHODS:Sixty female patients with stress urinary incontinence based on T2DM were random-ly divided into the treatment group(n=30)and the control group(n=30). Control group received diet control,blood glucose control, blood pressure control,elvic muscles exercise. Based on control group,treatment group additionally received Yin-nourishing and qi-toni-fying recipe decoction,one dose a day(200 ml/dose),morning and night,for 4 weeks. Clinical efficacy and ADR of 2 groups were observed. RESULTS:Compared with control group, total effective rate of TCM syndrome efficacy and clinical efficacy both were in-creased significantly in treatment group,while urinary incontinence score and average leakage frequency of daily urine were decreased significantly;there was statistical significance(P<0.05). 2 patients in treatment group suffered from aggravated thirsty and dry phar-ynx,and those symptoms disappeared after adjusting the dose. CONCLUSIONS:Yin-nourishing and qi-tonifying recipe decoction com-bined with pelvic muscles exercise has definite clinical effect on female patients with type 2 diabetic stress urinary incontinence.

8.Effect of Serum Containing Bushen Prescription on Chondrocyte Proliferation

[Objective] To observe the effect of serum containing Bushen prescription (BP) on inhibition of chondrocyte proliferation and nitric oxide (NO) production in chondrocyte induced by interleukin-1 (IL-1). [Methods] Four New Zealand rabbits were administered BP 2.5mL (1.15kg/L) for 3 days for the preparation of BP-containing serum. Rabbit chondrocytes were cultured in-vitro and were added with DMEM culture containing IL-1 1, 10 and 20 ?g/L to examined the inhibition concentration of IL-1 for chondrocyte proliferation. After then, BP group was added with 20 ?g/L IL-1 and various concentrations of BP-containing serum and the model group with 20 ?g/L IL-1 and various concentrations of blank serum. Alter 3-day culture, chondrocyte proliferation were dectected by thiazolyl blue colorimetry (MTT) and NO content by Griess method. [Results] The inhibition concentration of IL-1 for chondrocyte proliferation was 20 ?g/L; various concentrations of BP-containing serum could counteract chondrocyte proliferation and inhibit NO pnxluction in chondrocyte induced by IL-1. [Conclusion] BP can counteract chondrocyte proliferation directly or indirectly by the mediation of NO, which may be one of its therapeutic mechanisms in treating osteoarthritis.
9.Determination of Catechin Compound in Salacia amplifolia by HPLC

Objective To determine the content of ECEGC in Salacia amplifolia by HPLC.Method HYPERSIL C18 column was used with a mixture of methanol and water(30∶70) as mobile phase,and detected at 283 nm.Result The linear range was 0.444 8~7.116 8 ?g(r =0.999 9) for ECEGC.The average recovery was 100.30%.Conclusion The method is simple,rapid,accurate and reliable.
