1.A review of medical violence management policy change based on the perspective of punctuated equilibrium theory

Under the impact of medical and health system reform and focal events in China, the medical vio-lence policy change is characterized by partial discontinuous equilibrium. It gradually evolves from policymaker-based discontinuity to participatory one, from single decision-making field to the collaborative one, and from the ex-cessive protection of unilateral interest to the maintenance of public interest as a core. In the future, if we want to re-alize the effective minimization of medical violence by public policy, the participation of all parties in the policy deci-sion should be strengthened, the focal events should be analyzed as policy issues, and the policy should be clear in order to overcome the negative influence of the inactivity of old policies.
2.Multiple fundus imaging diagnosis: knowing the principles well for a rational application

Multimodal fundus imaging techniques, based on lights and shadows, appear to have a rapidly great progress in recent years.Fundus imaging techniques are divided into anatomical approaches and functional approaches according to the working characteristics and principle of different instruments.Nowadays, fundus imaging techniques are developing toward a wide field, microview, multimodal, quantitative, and intelligent way.However, to our knowledge, the fundus is very broad with many unexplored territories and unsolved mysteries until now.The road of clinical industrialization of emerging imaging techniques is still long, and there is also a potential challenge in the application of artificial intelligence deep learning in ophthalmology.Multimodal fundus imaging techniques are beneficial for us to accurately diagnose and dynamically monitor eye diseases, but with so many examination methods, how to choose a highly sensitive and specific way to avoid the waste of medical resources and reduce unnecessary financial burden of patients is the responsibility of ophthalmologists.
3.Challenges to medical libraries in big data era and their countermeasures

After a description of the concept and meaning of big data, the application of big data in biomedical field was investigated, the changes in medical library functions were analyzed, the challenges to medical libraries were pointed in terms of their data, hardware infrastructures, software technologies, talents and service modes, and how to respond to the big data in medical libraries was proposed from the aspects of strengthening the external collabo-ration, co-developing cloud computing center, intensifying the construction of related organizations and talents, and changing the service modes.

6. Effect of Haobie Yangyin Ruanjian Decoction on rats with hepatic fibrosis induced by CCl4 composited factor

Objective: To observe the effect of Haobie Yangyin Ruanjian Decoction (HYRD) on rats with hepatic fibrosis induced by CCl4 composited factor. Methods: Wistar rats were randomly divided into seven groups: control group, model group, HYRD groups of low, medium, and high doses, positive groups of Fufang Biejia Rungan Tablets, and colchicin. Model of hepatic fibrosis was induced by CCl4 composited factor. Malondialdehyde (MDA), total protein (TP), albumin (ALB) in serum and hydroxyproline (Hyp) in liver were detected by chromatometry; MMP-2 and MMP-9 activities in liver were detected by gelatin zymography; α-SMA expression in liver was evaluated by immunohistochemical method. Results: MDA in serum and Hyp in liver were decreased gradually in HYRD groups compared with those of model group; TP and ALB in serum were increased gradually in HYRD groups compared with those of model group; MMP-2 activity in liver was decreased in HYRD groups compared with those of model group; MMP9 activity had no significant change in each group and α-SMA expression was decreased gradually in HYRD groups compared with those of model group. Conclusion: HYRD can antagonize the hepatic fibrosis induced by CCl4 composited factor.
7. Prevention of cistanche salsa extract on hepatic fibrosis induced by carbon tetrachloride in rats

Objective: To explore the antifibrotic effect of echinacoside on carbon tetrachloride (CCl4)-induced hepatic fibrosis in rats. Methods: Male Wistar rats were randomly divided into normal control (n = 8), model (n = 14), and echinacoside treatment (n = 14) groups. The hepatic fibrosis model was induced by CCl4 compositor. The rats were ig administered with echinacoside at a daily dose of 50 mg/kg. The anti-oxidant status, liver function parameters, and hepatic hydroxyproline content were detected by chromatometry. The serum levels of hyaluronic acid (HA), type IV collagen (CIV), type III precollagen (PIIIP), and laminin (LN) were assayed with radioimmunoassay. The hpatic injury was detected by haematoxylin-eosine staining. The deposition of collagen was observed with Masson staining. Results: Echinacoside increased the superoxide dismutase activity and reduced the levels of malondialdehyde, aspartate aminotransferase, alanine aminotransferase, HA, CIV, PIIIP, and LN in serum. Echinacoside could also reduce the hydroxyproline content in liver, alleviate hepatic injury, and inhibit collagen deposition. Conclusion: Echinacoside possesses antihepatic fibrosis effect. © 2013 Tianjin Press of Chinese Herbal Medicines.
8. Evodiamine combined with radiation inhibits proliferation, adhesion and migration of human tongue squamous cell carcinoma cell line Tca-8113

Objective: To investigate the effects of evodiamine (EVO) combined with radiation on radiosensitivity and adhesion as well as migration abilities of human tongue squamous-cell cancer Tca-8113 cells. Methods: Cell proliferation inhibitory rates of Tca-8113 cells were detected by MTT method at different time points after treatment with EVO at different concentrations. The colony-formation ability of Tca-8113 cells treated with EVO plus radiation was detected by colony-formation assay. The changes of adhesion and migration abilities of Tca-8113 cells were examined by using cell adhesion assay and wound healing assay, respectively. Results: Survival rate of Tca-8113 cells decreased in association with the elevation of EVO concentration and longer treatment time. However, the survival rates of Tca-8113 cells at different time points after treatment with EVO at different concentrations in the EVO plus radiation group were significantly lower than those in the radiation group ( P<0.01). The calculated values of sensitizing enhancement ratio (SER) Do, SERDq and SERSF2 through clony-forming assay were 1.35, 1.75 and 1.67, respectively. The homotypic adhesion test revealed that the numbers of adhering cells in the EVO plus radiation group and the radiation group were significantly higher than that in the control group at 20, 40 and 60 min after vibration ( P<0.01). The number of adhering cells in EVO plus radiation group was significantly higher than that in the radiation group at 40 and 60 min after vibration ( P<0.01). The endothelial cell adhesion assay indicated that the adhesion rates of Tca-8113 cells in the EVO plus radiation group obviously decreased compared with those in the radiation group and the control group at 20, 40 and 60 min after vibration ( P<0.05). The wound healing assay revealed that the migration rate of Tca-8113 cells in EVO plus radiation group obviously decreased compared with those in the radiation group and the control group (<0.01). Conclusion: Evodiamine can significantly enhance the radiosensitivity of human tongue cancer Tca-8113 cells, and can change the adherence and migration capacities of Tca-8113 cells.
9. High-dose rate and low-dose rate intracavitary brachytherapy for cervical carcinoma: A systematic review

Objective: To assess the clinical effectiveness and safety of high-dose rate (HDR) and low-dose rate (LDR) intracavitary brachytherapy for cervical carcinoma. Methods: This study retrieved relevant randomized and quasi-randomized control trial in PubMed (1966 to December 2007), EMBASE (1974 to December 2007), Cochrane Library (Issue 4, 2007), Chinese Biomedical Literature Database (CBM, 1978 to December 2007), China Journal Full Text Database (CJFD, 1994 to December 2007), and Chinese Scientific Journals Full Text Database (CSJD, 1989 to December 2007). We also traced back the references to collect the relevant trails. Inclusion criteria were as follows: randomized or quasi-randomized controlled trails reported in English or Chinese; high- or low-dose rate intracavitary brachytherapy in the treatment of local advanced cervical carcinoma. Two reviewers assessed the quality of included trials and extracted data independently. The RevMan 4. 2. 10 software was used for Meta-analysis. Results: Four studies were involved and 1 253 patients were included. Meta-analysis showed that there was no significant difference in 3-, 5-, 10-year overall survival rate, 3- and 5-year pelvic control rate, 3-, 5-, and 10-year relapse-free survival rate between HDR group and LDR group. The pooled relative risk value and 95% CI were 0.96 (0.80-1.16), 0.92 (0.83-1.01), 0.86 (0.70-1.05); 0.96 (0.86-1.07),0.95 (0.87-1.05); 1.02 (0.84-1.23), 0.98 (0.89-1.07), and 1.02 (0.88-1.19), respectively. There was no significant difference in local-regional recurrence, local recurrence with distant metastasis, para-aortic lymph node metastasis, and distant metastasis between the two groups. The relative risk value and 95% CI were 1.03 (0.83-1.30), 2.23 (0.78-6.34), 1.06 (0.50-2.24), and 1.00 (0.76-1.32) respectively. The 3-5 grade complications in the bladder, rectosigmoid colon, and small intestine had no statistical difference between the two groups. The relative risk value and 95% CI were 1.08 (0.18-6.63), 0.90 (0.25-3.21), and 3.12 (0.96-10.15), respectively. Conclusion: This study suggests that HDR has similar effects with LDR in improving the survival rate and tumor control rate of patients with cervical carcinoma. Because of the small number and uneven qualities of samples, the conclusion needs further verification by large sample, multi-center, randomized controlled trials.
