1.Effect of total thyroidectomy on quality of life in patients with nodular goiter
Chinese Journal of Biochemical Pharmaceutics 2017;37(7):431-433
Objective To analyze the influence of total thyroidectomy (TT) on quality of life of patients with nodular goiter with iodine deficiency. Methods 70 patients who received TT in the hospital from June 2014 to July 2015 were randomly divided into two groups, 35 cases in each.Control group received subtotal thyroidectomy and observation group received total thyroidectomy. The situation of operation, thyroid function, therapeutic effects and quality of life in two groups were compared. Results Operation time and hospital stays of observation group were significantly longer than that of control group (P<0.05);There was no significant difference in intraoperative bleeding volume and curative effective rate between the two groups; Before surgery, there was no significant difference in thyroid function between the two groups; After surgery, thyroid function of both two groups decreased compared with pre-operation, but there was no significant difference; There was significant difference in VAS and health description system scores between the two groups. Conclusion TT in treatment of patients with nodular goiter with iodine deficiency has significant clinical efficacy with high cure rate and less complication, and it can improve quality of life of patients, which is beneficial to postoperative recovery and worthy of generalization in clinical application.

5. Effect of pemetrexed combined with oxaliplatin on the growth of human gastric cancer cell BGC-823
Tumor 2011;31(12):1067-1071
Objective: To investigate the growth inhibition effects of pemetrexed and oxaliplatin on human gastric cancer cell line BGC-823 in vitro, and to explore the possible mechanism. Methods: The BGC-823 cells were treated with different concentrations of pemetrexed or oxaliplatin alone or in combination. The growth inhibition rate of BGC-823 cells was detected by MTT method, and the change of cell cycle distribution was analyzed by flow cytometry (FCM). The expressions levels of proliferating cell nuclear antigen (PCNA) and cell cycle D1 (cyclin D1) proteins in BGC-823 cells were examined by Western blotting. Results: The growth of BGC-823 cells was inhibited by pemetrexed or oxaliplatin in a dose- and time-dependent manner (P £?0.05). The percentage of cells in G1 phase was increased while which in S phase was decreased by the treatment of pemetrexed or oxaliplatin. The expression levels of PCNA and cyclin D1 proteins were significantly decreased, and the combination regimen of pemetrexed and oxaliplatin exerted a synergistic effect. The growth inhibition rate of BGC-823 cells treated with pemetrexed (80 μg/mL) for 24 h followed by oxaliplatin (40 μg/mL) for 24 h was significantly decreased as compared with those of the cells treated with pemetrexed (80 μg/mL) combined with oxaliplatin (40 μg/mL) at the same time or treated with oxaliplatin (40 μg/mL) for 24 h followed by pemetrexed (80 μg/mL) for 24 h (P<0.05). Conclusion: Pemetrexed and oxaliplatin can both inhibit the proliferation of gastric cancer BGC-823 cells and arrest the cells at G1 phase. The sequential regimen of pemetrexed followed by oxaliplatin may have the strongest growth-inhibitory effect on gastric cancer cells among different doublet regimens. Copyright© 2011 by TUMOR.

An analysis of the present development of general practitioners in China, probed into the bottlenecks found. The paper proposed to set forth from the training, use and incentives of GPs to promote the systematic design of China's GP system. These measures center on the regulation of GPs development, while building a multi-channel and multi-level GP development system.Other recommendations include raising the professional standing and attractiveness of GPs as a discipline, and enhancing GPs capacity building to promote general practitioners development.

Objective To investigate prognostic value of the CHA 2 DS2-VASc score [ congestive heart failure , hypertension , age≥75y (doubled), diabetes mellitus, stroke (doubled)-vascular disease, age 65~74 and sex category (female)] in an initial car-dioembolic stroke with atrial fibrillation .Methods A total of 346 initial cardioembolic stroke patients with atrial fibrillation was en-rolled.The clinical feature, the National Institutes of Health Stroke Scale (NIHSS) score on admission, and clinical outcome [modi-fied Rankin Scale (mRS) score after 90 days] were obtained and evaluated retrospectively according to the CHA 2DS2-VASc score. Results The rate of female, smoking, hypertension, congestive heart failure, anticoagulation and the level of age, NIHSS score, and CHA2 DS2-VASc score were significantly different between two prognostic groups ( P <0.05 ) .A positive correlation was observed be-tween CHA2DS2-VASc score and NIHSS score on admission (ρ=0.324, P <0.01), CHA2DS2-VASc score and mRS score (ρ=0.228, P <0.01).Logistic regression analysis showed that age , smoking, anticoagulation, the CHA2DS2-VASc score, and the NIH-SS score on admission were related to clinical outcome independently .Conclusions The CHA2 DS2-VASc score may be useful for not only evaluation of stroke risk but also prediction of clinical outcomes after stroke .

Objective To analyze the outcome of reoperation in drug resistant epilepsy after failed surgery or recurrence in children, and to identify the predictors of the outcome of reoperation and inspiration of epilepsy resection surgery. Methods Analysis was done on data of 4 patients with drug resistant epilepsy diagnosed in Department of Neurosurgery Epileptic Center,Fuzhou General Hospital of Nanjing Military Command from Sep.2011 to Sep.2012.Clinical manifestation, preoperative evaluation, postoperative follow-up of the children were analyzed. Results After first epilepsy resection surgery,3 patients had little decrease in seizure rate(Engel class Ⅳ),the other one had seizure freedom (Engel class Ⅰ),but got recurrence after 2 years.Four cases were all underwent extended resection reoperation du-ring the study period.Types resection included temporoparietooccipital (3/4 cases) and frontal (1/4 cases),mean follow-up 11.25 months. One of the 4 patients had seizure freedom (Engel class Ⅰ),and 3 cases of the 4 patients had seizure reduction (Engel class Ⅱ) at last follow-up,in addition, none of them had significant or unacceptable complication. Conclusions Patients with recurrent drug resistant epilepsy after an initial resection surgery can regain seizure freedom outcome when they underwent repeat resection surgery after comprehensive reevaluation.Predictors with likelihood of seizure freedom after reoperation are:(1) retained medial temporal structures or relevant magnetic resonance imaging structural lesion;(2) original surgery suboptimal; (3) preceding evaluation suggests solitary identified focus;(4) semiology relate to preoperative seizures.
