miRNAs (microRNAs) are small RNA which regulate gene expressions at the post-transcriptional level.Recently studies have suggested that miRNA is associated with the development of diabetes mellitus and its complications.This artcle reviews the regulation of miRNA in diabetes mellitus and its cardiovascular complications,and the biomarkers' value and application of miRNA in the treatment of diabetes mellitus,aiming to develop new insight for the diagnosis and treatment of diabetes mellitus.
2.Promoting the regeneration of rat hepatocytes with rHGF after transplantation

Objective To evaluate the effects of rHGF on liver regeneration after partial transplantation.Methods The partial orthotopic liver transplanted model was established in SD rats with two-cuff technique. In experimental group, immediate after operation, 500 ?g/kg rHGF was administered intravenously twice a day. Same vehicle of saline was administered as control. The rats were (killed) randomly on the day 1, 2, 4 and 7 after operation. The serum levels of albumin and alanine (aminotransferase) were detected. Each liver was removed and weighed. The expression of Ki-67 antigen was detected by using immunohistochemistry assay. The regeneration response of hepatocytes was evaluated by flow cytometry.Results The wet weigh in rHGF group was significantly larger than that in control group 1, 2 days after transplantation. The serum level of alanine aminotransferase was (lower) and that of albumin was higher in rHGF group than in control group 4, 7 days after operation. The proliferating index and Ki-67 labeling index of graft in rHGF group were higher than in control group.Conclusion rHGF can promote the regeneration of hepatocytes after partial transplantation.
3.A Clinical Study on the Anticoagulant Treatment of Pre-thrombosis State in Patients with Cor Pulmonale

Objective To explore pre-thrombosis state in patients with cor pulmonale and the efficacy of anticoagulant treatment with low molecular heparin(LME). Methods 54 patients with cor pulmonale were divided into LMH treatment group and routine treatment group, each group containing 27 patients. The concentrations of Von Willlebrand factor (VWF),prethrombin F 1+2 fragment(F 1+2 ),Fibrinogen(Fg),granule membrane protein(GMP-140)and D-dimer fragment(DD)were measured before and after treatment. Results The concentrations of WF,F 1+2 ,Fg,GMP-140 and DD in the patients with cor pulmonale were significantly higher than those in the controls. In LMH treatment group, the above indices and PaCO 2 markedly decreased and PaO 2 obviously increased, while in routine treatment group, PaCO 2 and PaO 2 slightly improved, and no change was observed in coagulation indices. Conclusion Early detection and diagnosis of pre-thrombosis state in patients with cor pulmonale and immediate treatment with LMH can be beneficial to the prognosis of these patients in acute stage.
4.Relationship between epithelial cadherin and syndecan-1 expressions and invasive characters in hepatocellular carcinoma

Objective To study the relationship between expressions of E-cadherin and syndecan-1 in hepatocellular carcinoma(HCC) and the invasive characters of HCC. Methods The expression of E-cadherin and syndecan-1 were observed in 47 HCC cases using immunohischemistry. Results Most HCC cases showed decreased expression of E-cadherin and syndecan-1 (55%, 60%). Objective To explore the influence of percutaneous gastrostomy-jejunostomy ( PEG-J) on the nutritional status and prognosis of critically ill patients in ICU .Methods Totally 58 patients who received me-chanical ventilation in the ICU of Changshu No .1 People's Hospital from January 2013 to December 2015 were di-vided into two groups according to random number table:treatment group ( n=28, enteral nutrition by establis-hing PEG-J tube in 120 hours after excluding contraindications );and control group ( n=30, nasogastric tube as the way of giving enteral nutrition and if delayed gastric emptying happened , nasogastric tube would be replaced by nasointestinal tube).The incidence of ventilation-associated pneumonia (VAP) within 28 days, duration of ICU stay, and duration of mechanical ventilation were compared between these two groups .Results The inci-dence of 28-day VAP (7.14%vs.26.67%, P=0.049), duration of ICU stay [ (13.6 ±5.9) d vs.(16.8 ± 4.4) d, P=0.024], duration of mechanical ventilation [ (8.1 ±5.0) d vs.(10.7 ±4.5) d, P=0.041] were significantly lower or shorter in treatment group than in control group .Conclusions The use of PEG-J tube for enteral nutrition in critically ill patients is reliable and safe .It also helps to reduce VAP .
7.Development and evaluation of the heroin abstainers' cue-sensitization questionnaire.

In order to develop the heroin abstainers' cue-sensitization questionnaire (HACSQ), and evaluate its reliability and validity, during March and April in 2006, the cross-sectional sample survey and cluster sampling were adopted among 939 volunteer participants who were heroin abstainers from 5 compulsive detoxification centers in China. The data were collected by the self-report questionnaires. The reliability analysis included Cronbach's alpha coefficient, split reliability and test-retest reliability, while construction validity was analyzed in the light of exploratory and confirmatory factor analysis. Based upon the exploratory factor analysis, 4 factors were extracted, which were social emotion cue (SEC), direct implement cue (DIC), personnel cue (PC) and indirect implement cue (IIC), and they together explained 64.52% variance. According to the data, test-retest reliability coefficient ranged from 0.57 to 0.63, and Cronbach's alpha coefficient ranged from 0.86 to 0.95 across the 4 factors and HACSQ. The adjusted goodness of fit index was 0.75. It was suggested that the HACSQ demonstrated good validities and reliabilities, and may be used as a reliable assessment tool for the cue-sensitization among the heroin abstainers.

8.Clinical Analysis on 33 Neonates with Septic Shock

Objective To study the clinical diagnosis and therapy of neonatal septic shock. Methods The data of clinical features,lab examination and medical treatment of 33 neonatal septic shock patients were restrospectively analyzed. Results 10 patients had been in the situation of septic shock when they were in hospital among 33 patients and 23 patients catched septic shock while in hospital. 30 patients entered hospital through neonatal transport systems, and 10 patients got locate first aid,5 patients were intubated and supplied oxygen.Finally,23 patients were healed,5 patients given up therpy, and 5 patients died of MODS. Conclusion It is important to strengthen early recognition and effective therpy in order to improve prognosis of neonatal septic shock.
9.Clinical features and individualized treatment of 39 cases of adefovir dipivoxil-induced renal hypophosphatemia and osteomalacia

Objective To investigate the clinical features,early diagnosis and individualized treatment of renal hypophosphatemia and osteomalacia induced by adefovir dipivoxil (ADV) in patients with chronic hepatitis B (CHB).Methods Thirty-nine CHB or hepatitis B virus (HBV)-related cirrhosis patients of renal hypophosphatemia and osteomalacia induced by ADV were consecutively collected.The clinical features were analyzed and treatment outcome was followed up.Results The mean age of the 39 patients was 54 (27-71) years old.There were 26 male and 13 female patients,and 19 patients with cirrhosis.The mean ADV treatment duration was 69 (range 18-116) months,and 31 patients were treated for 36-96 months.The mean serum phosphate was 0.68 (0.42-0.79) mmol/L.Twenty-six cases developed renal hypophosphatemic osteomaolacia,of which 14 had bone pain and 19 had abnormally elevated alkaline phosphatase (ALP).Three patients had increased serum creatinine and 24 patients had decreased estimated glomerular filtration rate (eGFR).After individualized treatment,patients gained normal serum phosphate in mean of 2.0 (range 0.5-6.0) months,and had bone pain remission in the mean of 0.8 (range 0.2-1.0) month and bone pain disappeared in the mean of 1.5 (range 0.5-5.0) months.Function indices of liver and kidney were improved gradually,and the bone mineral density examination improved slowly.Conclusions CHB and HBV-related cirrhosis patients treated with longterm ADV could develop renal hypophosphatemia and hypophosphatemic osteomalacia,which is partially reversible.Monitoring serum phosphate,creatinine and cystatin C is necessary during long-term ADV therapy.After confirmed diagnosis,withdrawal or dosage reduction of ADV,and switch to telbivudine or entecavir should be considered.Meanwhile,serum phosphate and HBV DNA level should be monitored.
10.Effects of Qingjin Huatan Decoction on Expressions of NE, EGFR, and MUC5AC mRNA in ;Lung Tissue of COPD Rats

Objective To investigate the action mechanism of Qingjin Huatan Decoction on regulating COPD rats with mucus hypersecretion in airway. Methods Fifty 6-8 week old Wistar male rats were randomly divided into healthy group, model group, Qingjin Huatan Decoction group, clarithromycin group, 10 rats in each group. Except for healthy group, all the other groups were used the combination method of injecting LPS and smudging to establish COPD models. The last three groups were fed with normal saline, Qingjin Huatan Decoction, clarithromycin respectively for 30 days. On the 31st day of the experiment, the rats were put to death to take lung tissue. 6 rats from each group were chosen randomly, and the protein expressions of NE, EGFR and MUC5AC mRNA in lung tissue were detected by real-time fluorescent quantitative PCR. Results The expressions of NE, MUC5AC mRNA in lung tissue of COPD rats were higher than the healthy group. Compared with model group, the expressions of NE mRNA and MUC5AC mRNA in Qingjin Huatan Decoction group and clarithromycin group were markedly lower, while the expressions of NE, MUC5AC mRNA in Qingjin Huatan Decoction group were significantly lower than those in clarithromycin group. The expression of EGFR mRNA in Qingjin Huatan Decoction group was lower than that in model group. Conclusion Qingjin Huatan Decoction can inhibit mucus hypersecretion in airway through NE/EGFR/MUC5AC signal transduction pathway.
