1.Effects of transient intensive insulin treatment on free fatty acid in newly diagnosed type 2 diabetic patients

Objective To study the effect of transient intensive insulin treatment on the serum free fatty acid (FFA) in newly diagnosed type 2 diabetic patients.Methods Sixty-four newly diagnosed type 2 diabetic patients were treated with transient intensive insulin.The fasting plasma glucose (FPG),2 hours post-prandial glucose (2hPG),lipid,fastin insulin (FINS),and serum FFA was examined hefore and after treatment.Results The levels of FPG,2hPG,total cholesterol (TC),triglycerides (TG),low density lipoproteins cholesterol (LDL-C),FFA and HOMA-IR after treatment were (9.68 ± 2.02) mmol/L,(12.77 ± 1.35) mmol/L,(4.26 ± 1.07) mmol/L,(1.52 ± 0.58) mmol/L,(2.50 ±0.75) mmol/L,(435.84 ± 190.94) μmol/L,0.51 ± 0.62,and they decreased obviously compared with those before treatment [(14.66 ± 3.50) mmol/L,(17.43 ±4.89) mmol/L,(5.03 ±0.94) mmol/L,(2.05 ± 1.42) mmol/L,(2.91 ±0.78) mmol/L,(586.68 ±229.45)μmol/L,0.65 ± 0.89](P<0.05).The level of HOMA-β increased obviously (2.70 ± 0.83 vs.1.74 ± 1.04)(P<0.05).The increase of HOMA-β and the decrease of HOMA-IR was positively correlated with the decrease of FFA.Conclusion The transient intensive insulin treatment can evidently decrease the level of FFA that can improve beta-cell function and relieve insulin resistance in newly diagnosed type 2 diabetic patients.
2.Effect of alpha-interferon treatment in children with hepatitis B e antigen-positive chronic hepatitis B-a Meta-analysis

Objective To conduct a Meta-analysis to evaluate the efficacy and safety of interfer on-α for treating HBeAg-positive chronic hepatitis B in children.Methods PubMed and Chinese Biomedical Database were searched from the beginning to April 2006,and the references of eligible studies were manually screened.Randomized controlled trials published in the English and Chinese literature comparing interferon-α with non-antiviral interventions(placebo or no treatment)in children with chronic hepatitis B were eligible for inclusion.Studies were included if patients were treated for at least 3 months and followed up for at least 6 months after cessation of therapy.Two investigators independently assessed the quality and extracted the data.The methodological quality of trails was assessed by the Jadad-scale plus allocation concealment.Heterogeneity was examined by Chi-square test.Fixed effects model or random effects model was used to pool the data.Sensitivity analyses were used in the treatment course.Results Seven randomized controlled studies with a total of 360 child chronic hepatitis B virus carriers who were positive for hepatitis B surface antigen and hepatitis B e antigen werc identified.It was found by Meta-analysis that,compared with the control,at the end of therapy,interferon-α could significantly clear HBeAg[22.1%vs 6.7%,OR 3.56,95% CI(1.74, 7.28),P=0.000 5],HBV DNA[33.7% vs 12.6%,OR 3.50,95% CI(2.03,6.06),P<0.01], HBsAg [6.5% vs 0.5%,OR 7.10,95% CI(1.52,33.12),P=0.01],and achieve HBeAg seroconversion [17.3% vs 2.9%,OR 5.62,95% CI(1.65,19.18),P=0.006],but was not more effective in HBsAg seroversion[2.0% vs 0,OR 3.55,95%CI(0.35,35.93),P=0.28]and alanine aminotransferase(ALT)normalization[24.2% vs 16.2%,OR 1.72,95% CI(0.84,3.52), P=0.14].Conclusions Interferon a may be efficacious in clearance of HBeAg,HBV DNA and HhsAg, and achievement of HBeAg seroversion.Little evidence is available on HBsAg seroversion and ALT normali zation.Rigorously designed large sample size randomized double blind clinical trials with large sample size are required to further confirm and support the conclusion.
3.Localization of parathyroid glands with intravenous low-dose methylene blue in thyroidectomy

[ABSTRACT]OBJECTIVETo explore the efficacy and safety of localization for parathyroid glands with intravenous low dosage of methylene blue in thyroidectomy. METHODSWe retrospectively reviewed the clinical data of 41 patients who suffered from thyroid papillary carcinoma between Aug, 2014 and Jan, 2015 (9 males and 32 females, with a median age of 46 years). Thirty eight patients underwent primary thyroidectomy and 3 patients underwent second operation. A variety of thyroidectomy was performed in all patients, and who also underwent intravenous (3-4) mg/kg methylene blue in operation. RESULTSEighty four parathyroid glands were stained. Among 39 patients who's parathyroid glands were stained, the mean dyeing time was (31.27±9.41) min. Dyeing rates and dyeing time were not significantly different between 3 mg/kg group and 4 mg/kg group (t=0.24 and 0.20, all P>0.05). None of patients had the hypoparathyroidism problem such as peri-oral numbness, tingling, muscle aches and spasms. According to postoperative monitoring of parathyroid hormone, all of patients had no permanent hypofunction of the parathyroid gland. Neurotoxic effects and other serious side effects were not observed in all patients. CONCLUSIONIntravenous low-dose methylene blue in thyroidectomy is a safe, easy, and effective technique that facilitates rapid identification of parathyroid gland.
4.PROTECTIVE EFFECTS OF OFFSPRING OF MOTHER-MICE INFECTED WITH TRICHINELLA SPIRALIS AGAINST SCHISTOSOMA JAPONICUM INFECTIONS

Objective To explore the acquired immunity against Schistosoma japonicum infections in the offspring of mother-mice infected with Trichinella spiralis.Methods The offspring of mother-mice infected with Trichinella spiralis and the mice after passively transferring the offspring's sera were sacrificed at day 42 post infection with S. japonicum cercariae. The percentage of reduction in worm burdens was calculated to determine the protective effects. Results Comparing with the control group, the reduction rates of the worm were 24.31% in the offspring, 18.65% and 20.77% in mice with passively transferring the offspring's sera by intravenous injection and muscular injection respectively. Conclusion The offspring of mother-mice infected with Trichinella spiralis and the mice with passively transferring the offspring's sera could acquire protective immunity against Schistosoma japonicum infections.

Objective To investigate the correlation between midkine(MK) and VEGF expression with angiogenesis and biological features in recurrent gastric carcinoma.Methods The expression of MK was examined using immunohistochemistry in 9 cases of gastric carcinoma and 9 cases of adjacent normal tissues.Microvessel density(MVD)was determined by VEGF immunohistochemical staining.Results Majority of recurrent gastric carcinoma tissues showed positive reaction to immunostaining,but no specific positivity was detected in adjacent normal tissue.Values of MVD and MK positive group(88.8%,79.75) was higher than those in the negative group(15.0%)(P

Objective To know the vaccination status of BCG in Gaoxin district of Chongqing and explore the associated impact factors on seroconversion rate .Methods PPD was used to test the seroconversion status after PDG vaccination in infants under 3 months .Results There was no significant differences of seroconversion rate between different sex ,birth weight and vaccination age (P>0 .05) .But the rate was significantly higher in infants with normal birth weight than that in fetal macrosomia which was signif-icantly higher than that in low-birth weight .The rate was significantly decreased in infants of 1-2 months ,2-3 months and 0-1 months in order .The rate of infant with vaccination scar was significantly higher than that without scar (χ2 =80 .057 ,P<0 .01) . Conclusion There was no significant differences of seroconversion rate between different sex ,birth weight and vaccination age ,and the scar can indirectly show the seroconversion condition of vaccination .
7.Screening anti-human IgG for colloidal gold conjugate with dot immunogold filtration assay

Objective:To screen the appropriate anti-human IgG (secondary antibody) for conjugating with colloidal gold by dot immunogold filtration assay,and put forward the method of using multiple evaluation indicators for secondary antibody comparing and analyzing. Methods:Three different secondary antibodies A,B and C from three different companies were screened. Firstly the titration was used to determine the optimal reaction conditions. Then three secondary antibodies were conjugated to the colloidal gold,tested with a dot immunogold filtration assay for hydatidosis specific antibody detection. The optimal conjugated secondary antibody were compared with the standard indirect enzyme-linked immunosorbent assay. Results:The optimal pH of three secondary antibodies were 8. 5,the binding capacity were 38. 4,24 and 19. 2μg /ml colloidal gold. According to the comprehensive evaluation,the diagnostic effects of sec-ondary antibody B was better than others. Its diagnostic effects in dot immunogold filtration assay was compared with enzyme-linked im-munosorbent assay. The Kappa was 0. 895(P<0. 05) and showed the two methods were in good agreement. Conclusion:The appropriate secondary antibody for conjugating with colloidal gold could be screened by dot immunogold filtration assay and the evaluation indicators which put forward by this study,the screening method would have an important reference value for all kinds of colloidal gold test kit to screen the suitable secondary antibody.
8.Clinical and genetic research in a Chinese family with Stickler syndrome type 1

Background Stickler syndrome is a genetic connective tissue disorder that affects the ocular,skeletal,orofacial and auditory systems.To determine the gene mutation loci can offer a basis for genetic diagnosis and management of Stickler syndrome.Objective The aim of this study was to research the clinical characteristics of a pedigree with Stickler syndrome and identify the disease-causing gene mutation.Methods This study was approved by Ethic Committee of Peking Union Medical College Hospital.The clinical study and pedigree analysis were performed in one family with Stickler syndrome type Ⅰ (STL Ⅰ).Nine family members were examined with informed consent.The entire coding regions of COL2A1 gene with flanking intronic regions were amplified by PCR and directly sequenced.The detected sequence change was confirmed to be mutationloci by examining whether they existed in normal control individuals.Mutant proteins were predicted with online software.Results There were 4 generations and 11 members in this family,and 2 members died,including 1 patient.Three patients were found in 9living families.Inheritance of this family complicd with an autosomal dominant inheritance mode.All affected individuals showed the consistent phenotypes with STL Ⅰ,including high myopia,membranous vitreous anomaly and surface central flat,short nose,palatoschisis,etc.Mutation screening of COL2A1 gene revealed that the first base of intron 12 was deleted(IVS12+1G del).Nucleotide sequence analysis showed that this mutation led to the functional abnormal of this gene by forming termination cordon in advance.This mutation occurred in all affected individuals,however,no mutation was observed in any unaffected member or 100 normal unrelated individuals.Conclusions This study identifies a novel splice-site mutation(IVS12+ 1G del)in COL2A1 gene in a Chinese STL Ⅰ pedigree.This is the first report on a mutation in a Chinese STL Ⅰ family.
9.Effect of advanced glycosylation end products on oxidative stress and MCP-1 in human renal mesangial cells.

OBJECTIVETo investigate the effects of advanced glycosylation end products (AGEs) modified bovine serum albumin (AGE-BSA) on the expression of reactive oxygen species (ROS) and monocyte chemoattractant protein-1 (MCP-1) in human renal mesangial cells (HRMCs).
METHODSHRMCs were cultured in vitro with medium containing different doses of AGE-BSA or BSA (50,100, 200, 400 mg/L) for 48 hours, or with AGE-BSA (200 mg/L) for different times (12, 24, 48, 72 h). Immunocytochemistry assay was used to estimate the protein level of RAGE. The ROS in cells were measured by flow cytometry and the mRNA expression of MCP-1 were analyzed by semi-quantiative reverse transcription-polymerase chain reaction (RT-PCR) after treatment with AGE-BSA or BSA.
RESULTSThe protein level of RAGE was upregulated in the HRMCs with AGE-BSA. The expression of ROS and MCP-1 significantly enhanced by incubation of AGE-BSA in a time- and dose-dependent manner. The effects of AGE-BSA-induced up-regulation of ROS and MCP-1 level was significantly blocked by neutralizing antibodies to RAGE, while the expression of ROS and MCP-1 stood nearly unchanged after cultured with huamn IgG.
CONCLUSIONThe expression of ROS and MCP-1 in HRMCs is induced by AGE-BSA through RAGE, which may have potential effects in the pathgenic mechanism of diabetic nephropathy.
