3.Design of Voltage-clamp-controlled Current-clamp

Previous research rcvealed that distortion is detected in transient voltage signal recorded with traditional patch clamp amplifier under current clamp mode, which is essentially resulted by electronic design of the headstage of the patch clamp. A new kind of headstage is designed to modify the defect, the circuit of which not only measures the transient potentials as the classical voltage follower does but also is quite suitable for the standard voltage-clamp mode. Furthermore, the technique of voltage-clamp-controlled current-clamp is applied for modifying the conventional patch-clamp amplifier, the variable low-pass filter is added into the circuit to reduce the response speed of voltage-clamp module, thus the transient potentials changes can be measured while membrane potential is kept at a constant value. Bridge balance circuitry is designed to eliminate the voltage drop while the variable current injected into the electrode. And fast capacitance compensation stage of conventional PCA is modified to nentralize the capacitance and accelerate system response speed for current-clamp mode. The experirnental results on cell model demonstrate that modified PCA meets the rcquirement of monitoring transient potential changes in electrophysiology research.
4.Analysis of the Drugs for Antiacid and Peptic Ulcer Diseases in 22 Hospitals of Nanjing Area during 2003~2005

OBJECTIVE: To analyze the application and tendency of the drugs for antiacid and peptic ulcer diseases in Nanjing area.METHODS: In ranking of amount of money of drugs and DDDs,the drugs for antiacid and peptic ulcer diseases in 22 hospitals of Nanjing area during 2003~ 2005 statistically analyzed.RESULTS: During this period,the amount of money of these drugs had progressively increased by 30.8% per year;The DDDs of omeprazole increased remarkedly by 36.8% per year.CONCLUSION: With the morbility of peptic ulcer increasing,the drugs of PPI for peptic ulcer become the first choice in clinical application,the constituent ratio of amount of money of the drugs increased every year.
5.Influence of Screening Dementia with MMSE Combining with Delay Memory Test

Objective: To study the influence of screening dementia with mini-mental state examination (MMSE) combining with delay memory test . Methods: MMSE and delay memory test are used in screening 301 residents. The diagnosis of dementia is according to the DSM-Ⅲ-R criterion. The validity and reliability were studied when the MMSE and delay memory test were used to screen dementia alone or combined. Results: When MMSE was used to screen dementia alone, the specificity, sensitivity, false positive proportion and false negative proportion were 85.40%, 52.43%, 47.56% and 14.60% respectively. When delay memory test was used to screen dementia alone, the specificity, sensitivity, false positive proportion and false negative proportion were 74.45%, 92.07%, 7.93% and 25.55% respectively. If parallel connection of MMSE and delay memory test were used to screen dementia, the specificity, sensitivity, false positive proportion and false negative proportion were 95.62%, 49.39%, 50.61% and 4.38% respectively. If MMSE was in series with delay memory test, the specificity, sensitivity, false positive proportion and false negative proportion of screening dementia were 64.23%, 95.73%, 4.27% and 35.77% respectively. Conclusion: The parallel connection of MMSE and delay memory test can elevate sensitivity of screening dementia and decrease false negative proportion, so it is important in epidemiologic investigation. MMSE in series with delay memory test will raise specificity of screening dementia and decrease false positive proportion, so it is useful for diagnosing early dementia.
6.Effect of Occupational Therapy on Activities of Daily Living of the Children with Cerebral Palsy: 62 Case Report

ObjectiveTo observe the effect of occupational therapy (OT) on activities of daily living (ADL) of the children with cerebral palsy (CP).MethodsSixty-two CP children were treated with OT. The ADL scores of all children were assessed within one week after admission and training goal and plan were made according to evaluated result. The therapeutic effect was assessed before discharge.ResultsAfter OT treatment, the ADL scores of the children increased significantly ( P<0.01) and disable grades decreased (χ2=10.48, P< 0.01). The total effect rate was 87.1%.ConclusionOT training can improve ADL of the children with CP significantly.
7.Application of 3D printing technique in bilateral sagittal split osteotomy.

OBJECTIVETo investigate the application of osteotomy template manufactured via 3D printing technique in bilateral sagittal split osteotomy (BSSO).
METHODSThe patient group consisted of 32 patients (17 males and 15 females) with mandibular hypoplasia who underwent a BSSO setback (Hunsuck modification; 64 splits). The mean age at the time of surgery was 23.5 years (range 1.9-35 years). All patients were scanned using cone beam computed tomography (CBCT) to create a 3D model of the mandibular, and the osteotomy template of the inner horizontal ramus of mandible bone incision was manufactured via 3D printing technique. All splits underwent operation with or without the osteotomy template by either a young doctor (attending doctor) or a doctor with extensive orthognathic surgery experience (chief doctor). The time it took to perform the mandible bone incision of the inner horizontal ramus of each group was recorded. Postoperative CBCT scan was performed, and the lingual split scale was used to assess the effect of the operation.
RESULTSPrimary healing of incisions was observed in all patients, and no serious complications occurred. The time it took to perform the mandible bone incision of the inner horizontal ramus was significantly different in the four groups (F=30.059, P<0.05), and it was longer in the group of the attending doctor without osteotomy template than in the other three groups (P<0.05). No significant difference was observed in the other groups (P>0.05). Although all splits (n=64) were performed according to the standardized protocol, only 59.38% (38/64) of the fracture lines run according to the Hunsuck's description. By contrast, only 21.88% (14/64) run through the mandibular canal and 18.75% (12/64) was split. The database was analyzed using crosstabs and via Fisher exact test. The split pattern was influenced by the application of an osteotomy template (P<0.05).
CONCLUSIONThe application of osteotomy template manufactured via 3D printing technique in BSSO was effective, which enabled the young doctor to complete the operation faster and with good results.

8.Clinical follow-up study of jaundice type of human cytomegalovirus hepatitis in infants

Objective To explore the recovery time of hepatic function in infants with jaundice type of human cytomega-lovirus (HCMV) hepatitis and its influencing factors. Methods The clinical data of 73 infants with jaundice type of HCMV hepa-titis admitted to hospital from February 2005 to October 2012 were retrospectively analyzed. The effects on hepatic function of nine factors including age, sex, liver size, total bilirubin (TBil), direct bilirubin (DBil), alanine aminotransferase (ALT), aspar-tate aminotransferase (AST), total bile acid (TBA) and the loads of HCMV DNA were assessed by Kaplan Meier method, and further analyzed by Cox proportional hazards regression model. Results The results of Cox proportional hazards regression showed that age≤3 month (RR=0.27, 95%CI:0.10-0.70), DBil≤90μmol/L (RR=0.16, 95%CI:0.08-0.32), male (RR=0.49, 95%CI:0.26-0.94) and enlarged liver size<3 cm (RR=0.50, 95%CI:0.27-0.93) were independent factors that shorten the time for TBil back to be normal. Furthermore, AST≤120 U/L (RR=0.16, 95%CI:0.08-0.33) and enlarged liver size<3 cm (RR=0.28, 95%CI:0.15-0.49) were independent factors that shorten the time for AST back to be normal. The time for TBil back to be normal was (2.23 ± 1.54) months, which was significantly shorter than that [(3.63 ± 1.93) months] of AST (t=10.37, P<0.001). Conclusions Jaundice type HCMV hepatitis had good outcome and varied in disease course. The recovery of hepatic function was significantly adversely affected by the degree of cholestasis and hepatomegaly as well as AST level, and early treatment was conducive to the recovery.
9.Vena cava thrombosis after vena cava filter placement:Incidence and risk factors

Background The objective of this study was to assess the clinical safety and efficacy of vena cava filter (VCF) placement,with particular emphasis on the incidence and risk factors of inferior vena cava thrombosis (VCT) after VCF placement.Methods Clinical data of patients with venous thromboembolism (VTE),with or without placement of VCF,were analyzed in a retrospective single-center audit of medical records from January 2005 to June 2009.The collected data included demographics,procedural details,filter type,indications,and complications.Results A total of 168 cases of VTE (82 with VCF; 86 without VCF) were examined.Over a median follow-up of 24.2months,VCT occurred in 18 of 82 patients with VCFs (11 males,7 females,mean age 55.4 years).In 86 patients without VCFs,VCT occurred in only 6 individuals (4 males,2 females) during the study period.VCT was observed more frequently in patients fitted with VCFs than in those without VCFs (22% vs.7.0%).Conclusions The incidence of VCT in patients with VTE after VCF implantation was 22% approximately.Anticoagulation therapy should be continued for all patients with VCF placement,unless there is a specific contraindication.Almost all instances of VCT in patients with VCF implants in our study occurred after stopping anticoagulation treetment.The use of VCFs is increasing,and more trials are needed to confirm their benefit and accurately assess their safety.
