1. Diagnosis and treatment of neuroendocrine breast cancer: A retrospective analysis of ten cases and review of the literature

Objective: To investigate the characteristics of clinical manifestations, diagnosis and treatment of NEBC (neuroendocrine breast cancer). Methods: This was a retrospective study. Ten consecutive female patients with NEBC were admitted in Department of Breast Surgery, Peking Union Medical College Hospital between January 2004 and December 2010. The average age of the ten patients was 52 years, and the follow-up period was 24-177 months (median 84 months). Results: Clinically, most of the NEBC manifested as breast lump, sometimes as NPBL (non-palpable breast lesion) or nipple discharge. An ultrasound examination usually revealed heterogeneous echoic mass with irregular shape and clear boundary. Mammography showed no significantly different changes from other kinds of breast cancer. Nine patients received curative resection for NEBC, among whom, seven underwent modified radical mastectomy, one underwent nipple and areola-sparing modified radical mastectomy, one with nipple discharge underwent excision of duct and relevant lobule and hook-wire-guided biopsy of breast lump, and later right mastectomy with sentinel lymph node biopsy. The expressions of hormone receptors like ER (estrogen receptor) /PR (progesterone receptor) were usually positive in NEBC, while the expression of HER2 (human epidermal growth factor receptor 2) was usually negative (-) to positive (+). At least one neuroendocrine marker like Syn (synapsin) or CgA (chromgranin A) was positive. During the follow-up period, metastasis was found in two patients and one of them had passed away. Conclusion: NEBC is a clinical entity different from breast cancer with BC-NE (breast cancer with neuroendocrine differentiation). NEBC may affect younger patients in China, ultrasound examination is valuable for diagnosis, and surgical and comprehensive therapy may achieve satisfactory outcome with relatively good prognosis, although there might be cases of metastasis. 2.Study on early-warning of Chinese materia medica price base on ARMA model.

This study sets up an early-warning system framework of Chinese materia medica price, using price index as early warning indicator to establish black early-warning model, with indicator of price index volatility and limit line of "price principal". The research divides warning degree into 5 parts named negative heavy warning, negative light warning, no warning, positive light warning and positive heavy warning, with 5 corresponding lights to describe the change level of the medicine price. Then make an early-warning empirical research based on Chengdu Chinese materia medica price index from December in 2010 to October in 2013. ARMA model is applied to forecast index and the result of early-warning is analyzed, and finally farmer households, companies, customers and the government are recommended respectively.

3.Epidemiology of 10 000 mammary gland disease cases in Yangquan city

Objective To investigate the epidemiology of 10 000 patients with mammary gland disease.Methods 10 000 mammary gland disease cages during 1987~2006 in Yangquan Tumor Institute were collected,all patients had integrity case files and were diagnosed by molybdenum target radiography,near infrared ray,sonography,aspiration-needle biopsy or polyrrhea smear examination.Results In 8919 outpatient treated cases,7493 suffered cyclomastopathy(84%).1081 cases were inpatient cared(11%),the top five mammary gland disease are:breast cancer 342 cases,cyclomastopathy 252 cases,adenofibroma 104 cases,intraductal papilloma 86 cases and ductal ectasia 76 cages.For breast cancer patients,there were 125 cases during 1987~1996,3 were 21~30 years old and 23 were 31~40 years old.The number during 1997~2006 was 217,24 were 21~30 years old and 69 were 31~40 years old.Conclusion In 10000 cases,there are 7745 cyclomastopathy patients(77.45%)which takes the first place.Especially for 342(3.42%)breast cancer cases,the incidence grew up and patients age was much younger.
5.Pathogenic analysis in children with community acquired pneumonia in Jiaxing city

Objective To study the etiology of comumunity acquired pneumonia (CAP) in children of Jiaxing city and provide the basis for the clinical diagnosis and treatment.Methods An etiological study was performed on 800 young children with CAP in Jinxing First Hospital from Jan 2008 to Dec 2010.Sputum samples were collected for isolating bacteria and checking the antigen of respiratory syncytial virus ( RSV),adenovirus ( ADV),influenza virus (IV) and parainfluenza virus ( PIV ).IgM antibodies of mycoplasma pneumoniae (MP),ADV,W and PIV paired serum samples were detected.The 202 cases with positive etiology were divided into three groups:bacteria group ( 109 cases),virus group (54 cases) and MP group (39 cases).The leukocyte,C-reactive protein (CRP) and procalcitonin (PCT) were detected.Results The etiology of CAP was identified in 202 (25.25%) children of all the 800 cases,the distribution of causal agents was as follows:Streptococcus pneumoniae in 46 cases (22.77%,46/202 ),Haemophilus influenzae in 32 cases (15.84%,32/202),Staphylococcus in 5 cases (2.48%,5/202),Klebsiella pneumoniae in 8 cases (3.96%,8/202),Escherichia coli in 12 cases (5.94%,12/202),MP in 39 cases ( 19.31%,39/202),RSV in 48 cases (23.76%,48/202),ADV in 1 case (0.50%,1/202),IV in 3 cases ( 1.49%,3/202),and PIV in 2 cases ( 0.99%,2/202 ),Streptococcus pneumoniae and RSV mixed infection in 3 cases ( 1.49%,.3/202),Streptococcus pneumoniae and MP mixed infection in 3 cases ( 1.49%,3/202).RSV infection occured mainly in infants less than 1-year-old,Streptococcus pneumoniae oceured in all ages but was mostly in 1 ~ 3years,Haemophilus influenzae infection occured mainly in infants less than 3-year-old,MP infection occured mainly in infants more than six years.The PCT levels of the three groups were (7.78 ±6.14) ng/ml,(0.39 ±0.56) ng/ml,and (0.20 ±0.34) ng/ml,respectively.There were significant differences among the three groups ( P < 0.05 ).Conclusion RSV infection occurs mainly in infants of age less than 1 year,Streptococcus pneumoniae and Haemophilus influenzae infection occur mainly in infants of age 1 ~3 years,while MP is the leading cause of pneumonia in children of age more than 6 years in Jiaxing city.We can decide pathogen on ages,clinical manifestation and PCT level to guide the rational use of drugs.
6.Relationship of the occurrence and development of rheumatism arthritis with T lymphocyte subsets and CD4+ TH1/TH2 cell functional subsets

BACKGROUND: Rheumatoid arthritis(RA) is an autoallergic disease,but its immunological pathogenesis has not been completely known. T lymphocytes, especially CD4+ TH1/TH2 cells, may have an important effect in the occurrence and development of RA.OBJECTIVE: To investigate the action of CD4+ TH1 and TH2 cells which separately mediate cellular immunity and humoral immunologic response (respectively) function in the occurrence and development of RA.DESIGN: Case-control, comparative observation.SETTING: Department of Immunology, Medical College of Chinese People's Armed Police Forces.PARTICIPANTS: Totally 15 patients with RA hospitalized in the Department of Internal Medicine of General Hospital of Tianjin Medical University between March 1999 and March 2000 were selected for a RA patient group, consisting of 2 males and 13 females. Of them, 12 patients whose serumal rheumatoid factor (RF)was positive and the three others' was negative. At the same time, 30 healthy individuals from persons receiving health examination in the hospital or from our department were selected for a healthy control group, consisting of 4 males and 26 females. Informed consents were obtained from the participants.peripheral blood mononuclear cells(PBMC) from the two groups of subjects were detected by enzyme-linked immunospot assay (ELISPOT). 200-500 lymphocytes were counted under a normal light microscope, and the perwhich secreted cytokines were detected by ELISPOT, and those which had red spots in their plasma after staining were positive cells. Among them,the cells secreting γ-interferon (IFN-γ) were Tm cells while those secreting interleukin-4 (IL-4) were TH2 cells. 200-500 lymphocytes were counted under a normal light microscope and the percentages of TH1 and TH2 cells square test were used for comparing statistical differences of data between the two groups.MAIN OUTCOME MEASURES: Quantitative analysis of T lymphocyte subsets (including CD3+ T cells, CD4+ T cells and CD8+ T cells) and CD4+ TH1/TH2 cells in peripheral blood from the two groups of subjects.RESULTS: Analysis of the results was from the study on all of subjects percentages of total T cells (CD3+ T cells), CD4+T cells and CD8+F cells in peripheral blood were not significantly different between the two significantly higher in RA patient group than that in healthy control group [(24.44±5.25)%, (14.93±3.82)%, P < 0.05],while in RA patient group the percentage of TH2 cells and ratio of TH1 cells to TH2 cells from peripheral blood were not significantly different as compared with those of control group(P > 0.05).CONCLUSION: Cellular immunologic function mediated by TH1 cells may be associated with the occurrence and development of RA.
7.Lymphostatin (lifA) of rabbit enteropathogenic Escherichia coli possesses both immunomodulation and adhesion properties

The rabbit enteropathogenic E. coli (rEPEC) strain RDEC-1 possesses a lifA homologue adjacent to the LEE pathogenicity island. To study the entire nucleotide sequence and biological function of lifA,the DNA sequence and biological function of RDEC-1 lifA were analysed with gene cloning,gene knock-out and in vivo virulence examination. The result showed that the entire coding sequence of the lifA of RDEC-1 shares nearly absolute homology with the lifA of human isolates. RDEC-1 lifA inhibited IL-2 expression in stimulated rabbit peripheral blood mononuclear cells. We further demonstrated significant reduction in fecal bacterial shedding by RDEC-1 derivative lifA mutant when compared with its parent strain. In a competitive study when rabbits were inoculated with a combination of the WT and the mutant, the WT was the predominant bacteria recovered from fecal samples, while fewer mutant bacteria were recovered. However,the lifA mutant is able to induce A/E type of lesions as efficient as the parent strain. The data provide direct evidence that lifA of rEPEC plays a role in immunomodulation and in in vivo colonization in the intestinal tract.

Fibulin-1 is a widely distributed,versatile secretory calcium-binding glycoprotein which plays an important role on the formation of the structure of the extracellular matrix,the regulation of intercellular signaling and the inhibition of cell motility.Recent studies suggested a tumor-suppressive role for Fibulin-1.Here,this review focuses on the expression,function and mechanism of Fibulin-1 genes in tumor.
10.Alleviative effects of lidocaine postconditioning on pulmonary ischemia-reperfusion injury of rats

Objective To investigate the alleviative effects of lidocaine postconditioning on pulmonary injury following ischemia reperfusion. Methods Seventy-two adult SD rats were randomized to 4 groups; sham group, ischemia-reperfusion (I-R) group, ischemic postconditioning(IPC) group and lidocaine postconditioning group. The pulmonary ischemia-reperfusion model was established by occlusion of the left hilum of lung for 45 min and the reperfusion was taken by removing the clamp for 2 h. At the moment of reperfusion, lidocaine 4 mg/kg was injected as a priming dose following a continuous rate of 4 mg/(kg · h). PaO_2, TNF-α, W/D of left lung, the level of MDA of left lung tissue were measured. At the end of reperfusion left lung was removed for microscopy. Results After reperfusion PaO_2 of lidocaine group was much higher than that of I-R group (P<0.05). Lidocaine postconditioning induced a significant decrease in the level of MDA of lung tissue[(7. 03±1.17) μmol/L] compared with ischemia reperfusion group [(8.77±1.42) μmol/L] (P<0.05). Lidocaine postconditioning resulted in a lower level of TNF-α [(1. 69±0.34) μg/L] than that of I-R group [(2. 52±0. 54) μg/L] (P < 0. 05). Microscopic examination showed that lidocaine postconditioning could decrease the level of edema of left lung and accumulation of neutro-phils. Conclusion Lidocaine postconditioning exerts a protective effect on pulmonary ischemia-reperfusion injury administered in the beginning of reperfusion. The effect may be explained by to the antioxidant effect and the suppression of expression of TNF-α.
