3.Reflection on the issue of doctors' practice circumstances

Doctors ought to belong to a social stratum with fairly high incomes. The high input and long cycle that is essential to becoming a high-caliber doctor constitute the high costs of the medical profession; high technology, high responsibility and high sacrifices constitute the high risks of the medical profession; the contrast between the input of high intelligence, high academic degree, and high expertise and the output of low economic income tends to warp the minds of doctors. The inversion of the burden of proof in medical lawsuits has practically put doctors on the weak side. Their rights and interests as well as their practice circumstances have been disregarded. For the sake of the development of the health cause, it is imperative to respect the labor of doctors and to provide them with sound survival circumstances.

7.Thoracoscopy combined with laparoscopy in treatment of esophageal cancer radical operation (40 cases)

Objective To explore and analyze the application and problems of esophageal cancer radical opera-tion by thoracoscopy combined with laparoscopy. Methods From July, 2011 to December, 2014, clinical data of 40 patients undergoing esophageal cancer radical operation by thoracoscopy combined with laparoscopy were analyzed retrospectively. Results Except 2 cases being relayed to assisted small thoracic incision because of 1 case with clear tumor invading to trachea and 1 case with obvious tumor invasion to thoracic aortic artery, the remaining 38 cases were successfully completed operation. There were 2 cases with postoperative anastomotic fistula in the neck, 2 cases of anastomotic stenosis and 1 case of hoarseness, there was no perioperative death. Follow-up was within 36 months, 2 patients died 10 months and 24 months after surgery respectively, due to tumor local recurrence. 1 patient with supraclavicular lymph node metastasis underwent local radiation therapy. The remaining was all survival with re-lapse-free. Conclusions Esophageal cancer radical operation by thoracoscopy combined with laparoscopy is a good method with safety and reliability, minimal invasion, fast recovering, and satisfied recent curative effect. It is worthy of applying clinically.
8.Comparison of properties of tumor necrosis factor-alpha converting enzyme (TACE) and some matrix metalloproteases (MMPs) in catalytic domains.

The crystal structural data of TACE, MMP-1, MMP-2, MMP-3 and MMP-9 were obtained from PDB database, and then their catalytic domains' properties including conformation, molecular surface hydrophobicity and electrostatic potential were analyzed and compared by using Insight II molecular modeling software. It was found that the conformation and molecular surface hydrophobicity of catalytic domains of TACE and MMPs were not obviously different, but the molecular surface electrostatic potential of catalytic domain of TACE and MMPs had obvious differences. The findings are helpful in the Rational Drug Design of TACE selective inhibitor.
9.Thyroid-stimulating hormone decreases insulin receptor substrate 1 expression via stimulating TNF-αsecretion in 3T3-L1 adipocytes

3T3-L1 adipocytes transfected with TSH receptor (TSHR) shRNA were incubated with bovine TSH.The concentration of tumor necrosis factor (TNF)-α in culture medium was measured by enzyme linked immunosorbent asssy.Protein level of insulin receptor substrate 1 (IRS-1) was quantified by Western blotting.Tyrosine phosphorylation of IRS-1 was measured by immunoprecipitation.The results showed that 1 mIU/ml TSH significantly sitmulated TNF-α release in 3T3-L1 adipocytes [(341.85 ± 12.00 vs 522.67 ± 36.22) ng/L,P<0.01],along with the decreases in IRS-1 protein expression and its tyrosine phosphorylation (P< 0.01).These effects disappeared when TSHR expression was down-regulated with RNA interference in 3T3-L1 adipocytes.In addition,WP9QY,a TNF-α antagonist,blocked TSH-decreased IRS-1 expresssion.These results suggest that TSH downregulates IRS-1 protein expression and its tyrosine phosphorylation through stimulating production of TNF-α,and thus contributes to the development of insulin resistance.
10.Therapeutic effect of splenic autologous transplantation on rat liver cancer induced by DEN

Objective To investigate the therapeutic effect of autologous transplantation of spleen on rat liver cancer induced by DEN. Methods Forty rats were randomly divided into A and B group on average. Total spleens of 40 rats were resected, which preserved in liquid nitrogen. All splenectomy rats were feeded with DEN to induce hepatocellular carcinoma. After 12 weeks ,all rats which had were successfully induced liver cancer were transplanted by autologous spleen in group A. Group B were sham operation as control. Results Survival period was significantly higher in group A than that in group B. But transplanted splenic tissue had not recovered to normal morphological structure until these rats died. Conclusion Autologous transplantation of spleen has therapeutic effect for liver cancer induced by DEN.
