Objective To discuss the feasibility of alternant indwelling trocar in bilateral axillary veins to replace the PICC in NICU, and then low down the medical cost for patients when guarantee the safe of treatment. Methods Results Divided 40 premature infants and intensive care infants into control and experimental group randomly, there were 20 infants in each group. Alternant indwelling trocar in bilateral axillary veins was used in the experimental group, the PICC was used in the control group. Compared the clinical effects and the medical costs in the 2 groups. Results The method of alternant indwelling trocar in bilateral axillary veins could reach the satisfactory clinical effects with lower medical costs than PICC. Conclusion Using alternant indwelling trocar in bilateral axillary veins to replace the PICC is safe and feasible. [

OBJECTIVE:To observe the efficacy and safety of Clotrimazole vaginal tablet in the treatment of pregnancy with vulvovaginal candidiasis. METHODS:106 pregnant patients with vulvovaginal candidiasis were randomly divided into observation group (56 cases) and control group (50 cases). Observation group was given one tablet Clotrimazole vaginal tablet,washed the genital area before treatment to keep clean and one Clotrimazole vaginal tablet was sent to intravaginal 5-6 cm,once every 2 day. Control group was given Lactobacillus vaginal capsule,once a day,washed the genital area before treatment and one Lactobacillus vaginal capsule was sent to intravaginal 5-6 cm. The treatment course for both groups was 8 d. No sexual activities during treatment and no other drugs. Clinical efficacy,clearance rate of candidiasis,pregnancy and incidence of adverse reactions in 2 groups were observed. RESULTS:The total effective rate and clearance rate of candidiasis in observation group were significantly higher than control group,carrier rate was significantly lower than control group,the differences were statistically significant (P<0.05). And there were no significant differences in the pregnancy and incidence of adverse reactions between 2 groups (P>0.05). CONCLU-SIONS:Clotrimazole vaginal tablet shows significant efficacy in the treatment of pregnancy with vulvovaginal candidiasis and can clear candidiasis,with good safety.

B-cell specific moloney leukemia virus insertion site 1(Bmi-1)gene is a core member of the polycomb group genes,considered to be a proto-oncogene;Bmi-1 plays an important role in cell self-renewal, proliferation and apoptosis. Several studies have shown that Bmi-1 is highly expressed in non-small cell lung cancer;furthermore,the expression level of Bmi-1 is closely related to the occurrence,development,incursion and prognosis of non-small cell lung cancer. Bmi-1 is expected to become a novel tumor molecular marker,and provides a new direction for the treatment of non-small cell lung cancer.

Objective To discuss the value of ABCD2 score in predicting medium and long-term prognosis in patients with ischemic stroke.Methods 184 patients with ischemic stroke admitted to department of neurology from March 2014 to December 2014 were selected as the research object.According to the ABCD2 score points prior to admission, the patients were divided into 3 groups: 40 cases in the low risk group, 76 in the moderate group and 68 in the high risk group based on whether or to what extent he was able to do self care.The phone call reviews were made for the clinical symptoms of patients discharged from the hospital after six months..According to the modified Rankin scale classification standard, the patients discharged from the hospital after six months were divided into 2 groups: 126 cases in the self-care group and 56 cases in care-needing group.The groups were compared in terms of conditions.Results The self-care rate in the low risk group was 90.00%, the moderate group 77.63% and the high-risk group 45.59%.The care needing rates in the three groups was 5%, 19.74% and 48.53%, respectively.The care needing rate in the low risk group was obviously lower than that in the moderate-risk group and the high risk group (P<0.05).The care needing rate in the moderate risk group was obviously lower than that in the high risk group (P< 0.05).The self-care rates in the low risk group and the moderate group were higher than that in the high risk group (P<0.05).Conclusion The ABCD2 score has a higher predictive value for medium and long-term prognosis in patients with ischemic stroke.

BACKGROUND:The traditional percutaneous pedicle screw fixation for thoracolumbar fractures has a lot of shortcomings,such as restoring the vertebral height.The combination of hyperextension traction-elastic compression and internal percutaneous pedicle screw fixation provides a new treatment strategy.OBJECTIVE:To observe and compare the biomechanical properties of the traditional percutaneous transpedicular screw fixation and hyperextension traction-elastic compression combined with percutaneous pedicle screw fixation in the treatment of thoracolumbar fractures.METHODS:Clinical data of 40 patients with simple vertebral compression fractures undergoing traditional percutaneous pedicle screw fixation (control group) or hyperextension traction-elastic compression combined with percutaneous pedicle screw fixation (treatment group) were analyzed retrospectively,and none had neurologic signs or symptoms.The operation time,intraoperative radiant volume,intraoperative blood loss,hospitalization time or complications were compared between two groups.The sagittal Cobb angle and percentage of anterior height in vertebral body were measured at baseline and different time points after internal fixation.The pain and symptom improvement were evaluated by Visual Analogue Scale and Oswestry Disability Index.RESULTS AND CONCLUSION:All patients were followed up for 12 months.(1) There were no significant differences in the operation time,intraoperative radiation volume,intraoperative blood loss,hospitalization time and complications between two groups (P ＞ 0.05).(2) The sagittal Cobb angle,percentage of anterior height in vertebral body,Visual Analogue Scale and Oswestry Disability Index scores after internal fixation were significantly superior to those before internal fixation in both two groups (P ＜ 0.05).(3) The sagittal Cobb angle and percentage of anterior height in vertebral body in the treatment group were significantly superior to those in the control group at 24 hours,6 and 12 months postoperatively (P ＜ 0.05).(4) The Visual Analogue Scale and Oswestry Disability Index scores in the treatment group were significantly superior to those in the control group at 12 months postoperatively (P ＜ 0.05).(5) These results indicate that hyperextension traction-elastic compression combined with percutaneous pedicle screw fixation exerts remarkable effect on restoring the vertebral height and effectiveness compared with the traditional percutaneous pedicle screw fixation.

Objective To investigate the clinical efficacy of psychological intervention and gefitinib in the treatment of elderly patients with non-small cell lung cancer with limited pulmonary resection. Methods The control group were all treated with limited pulmonary resection. Preoperative chemotherapy was given to gefitinib. The study group was given psychological intervention during the preoperative chemotherapy on the basis of the control group. Results The resection rate in the study group was 93.55％, while in the control group was 74.19％, the difference in the two groups was statistically significant(P<0.05). The transfer rate was 3.23％ and the recurrence rate was 3.23％ in the control group, which was significantly lower than those in the control group the transfer rate of 19.35％, the recurrence rate of 16.13％. 1 years survival rate in the study group 80.65％ was significantly higher than that in the control group 64.52％, the difference was statistically significant(P<0.05). Conclusion On the basis of routine preoperative application of gefitinib combined with local excision treatment, the elderly patients with non-small cell lung cancer with psychological intervention measures can make them achieve more ideal effect and prognosis.
