1.Diagnostic Values of Forceps and Brush Biopsy by Percutaneous Transhepatic Cholangiodrainage for the Etiology of Obstructive Jaundice

Objective To study pathological diagnostic value of forceps and brush biopsy cytological examination by percutaneous transhepatic cholangiodrainage(PTCD) for malignant obstructive jaundice.Methods From April 2005 to April 2006,24 cases of obstructive jaundice underwent PTCD and stent insertion in biliary tract.Tissue pathological examination by forceps and brush was done during PTCD.Results 24 cases received forceps biopsy,of whom,14 cases received brush cytological examination at the same time.All of the 24 cases of forceps biopsy obtained pathological tissue and the pathological results as follows: adencarcinoma in 15 cases,4 cases had allotype cells,regarded as positive results;inflammation tissue in 1 case,which was restenosis after stent insertion;fibrous tissue in 1 case;tissue obtained was cholangioepithelia in 3 cases,in which 1 case was diagnosed as lymph node metastasis of peripheral head of pancreas after gastric cancer operation,2 cases were diagnosed as cholangiocarcinoma,the positive rate of forceps biopsy being 79.2%(19/24).4 cases of cytological examinations by brush showed tumor cells in 2 cases,allotype cells in 2 cases,regarded as positive results,the positive rate being 28.6%(4/14),and others were negative results.Conclusions Intracholangio-biopsy by PTCD offers the advantages of simplicity,safety,higher accuracy and less complications.Pathological diagnosis can be obtained at the same time during reducing-jaundice therapy by PTCD.

Objective To investigate the possible mechanism of the biologic ethology effects of NGF and SP(substance P)on primary cultured adrenaline medullary chromaffin cells(AMCC).Methods To establish primary cultured AMCC by means of enzyme digestion and purify the cells by means of isopycnic gradient centrifugation and differential plating.To observe the morphological and ultrastructural changes after the addition of NGF and SP,and detect the concentration of adrenaline and noradrenaline in serum by ELISA.Results Confervaceous processes could be observed after 2 d of addition of NGF to the culture and the processes strenched longer as days went by the observation of electron microscopy.there are some drumstick-like and villiform processes in the cell membrane and some vesiculation be observed near the cell membrane of the primary cultured AMCC cells after the addition of NGF.The bioblast was abundant but the structure was not clear in the intracytoplasm and the concentration of adrenaline were decreased(P
3.Clinical and pathological features of esophageal and gastric junction adenocarcinoma and current status of postoperative adjuvant chemoradiotherapy

The incidence of adenocarcinoma of the esophagogastric junction ( AEG) is rising rapidly in recent years. AEG as an independent disease different from squamous cell carcinoma of the esophagus and gastric adenocarcinoma is getting acknowledged for most of scholars, with the distinctive anatomy location and involute recurrence and metastasis style. A higher rate of relapse and poor prognosis after resection become conscious since most of patients have been diagnosed with advanced disease, and adjuvant chemoradiotherapy needs to be valued further. But so far, there is no clinical data and subgroup analysis aimed at the value of postoperative treatment for AEG specially, leading to controversy is remaining in patients could get benefit from postoperative treatment, and delineation of the radiation target, etc. The design of the irradiation target should in terms of the clinical?pathological characteristics of the AEG, Siewert'''' s subtypes, the lymph node metastasis mapping and recurrence characteristics after surgery.
4.Therapeutic effects of catheter balloon dilatation therapy for neurogenic cricopharyngeal achalasia

Objective To investigate the effects of catheter balloon dilatation on patients with dysphagia caused by neurogenic cricopharyngeal achalasia.Methods Thirty-two patients with dysphagia caused by neurogenic cricopharyngeal achalasia were divided randomly into a treatment group (n =16) and a control group (n =16).The treatment group received catheter balloon dilatation therapy and routine dysphagia rehabilitation training; the control group was treated with routine dysphagia rehabilitation training only.Both groups were tested before treatment,after 3 weeks of treatment (after treatment) and at 6 months after treatment (follow-up).All cases were evaluated using Saitoh's 7-grade swallowing assessment and videofluoroscopy.Results After treatment,5 cases in the treatment group had been cured (31%) and 8 cases showed marked effectiveness (50%) ; in the control group,1 case was cured (7%) and 5 cases showed marked effectiveness (31%).These differences were statistically significant.At follow-up,5 cases in the treatment group had been cured (31%) and 9 cases showed marked effectiveness (56%) ; for the control group,1 case was cured (7%) and 6 cased showed marked effectiveness (38%).Those differences were also statistically significant.After treatment and also at follow-up,the average time for a bolus to pass the pharynx in the treatment group was significantly shorter than that in the control group.Conclusion Catheter balloon dilatation therapy is effective for treating neurogenic cricopharyngeal achalasia in both the short and long term.

6.Ambulatory urodynamics monitoring system in diagnosis of female stress urinary incontinence

Objective To compare the diagnostic accuracy of conventional urodynamic (CUD) and ambulatory urodynamic monitoring (AUM) in diagnosis of female stress urinary incontinence (SUI).Methods Forty women with SUI were prospectively enrolled and performed urodynamic studies after the ICI-Q-SF questionnaire.According to clinical symptoms,patients were divided into three groups:mild,moderate and severe group.Half patients in each groups performed CUD exam,and the other half of patients performed AUM exam.And two micturition cycles were recorded during AUM.Results There were no significant differrences in age,pregnant production times and ICI-Q-SF score between two groups.Three patients with SUI symptoms had negative findings in AUM group and 15 patients in CUD group (P<0.05).Among women with SUI,1 1 patients had positive findings in AUM group and 2 patients in CUD group (P< 0.05).Conclusion AUM can provide objective evidence for the majority of patients with SUI than CUD.
7.Expression of BMI-1 Gene in Children with Acute Leukemia and Its Clinical Significance

Objective To study the expression of BMI-l gene in children with acute leukemia and its clinical significance.Methods The clinical specimens of 46 children with acute leukemia who were diagnosed lately in the Third Affiliated Hospital of Zhengzhou University and other hospitals in Zhengzhou from Jul.1,2008 to Apr.30,2009 were collected,while peripheral blood specimens of 30 healthy children were collected as control group.With the guardians′ informed consent,the experiment was approved through the hospital ethics committee.The level of BMI-1 mRNA′s expression was tested using reverse transcription-polymerase chain reaction(RT-PCR),while data were analyzed through the application of SPSS 12.0 statistical software.Results 1.The level of BMI-l gene′s expression in children with acute leukemia was significantly higher than that in control group(P0.05),after complete remission,BMI-1 mRNA was not detected in the 2 groups;4.Compared with the complete remission group,expression of BMI-1 mRNA in the untreated group and the recurrence group was significantly higher(P0.05).Conclusions BMI-1 gene was highly expressed in children with acute leukemia,and the level of the gene expression in patients of complete remission normalized,which suggests that the gene may be involved in the occurrence and the development process of leukemia;therefore,it is possible to regard the gene as a molecular marker to evaluate the development,relapse and prognosis of the patients with acute leukemia.

Lipases have catalytic active in both aqueous phase and the non-aqueous phase and have a wide range of application in various industrial areas.However,the high cost of lipase production has restricted its extensive use in industry.Solid state fermentation possesses many advantages,such as low requirement for devices,low energy consumption,low production cost,little pollution to environment and easily being popularized,which have made it an important means in microbial production of lipases.Owing to the rapidly increased energy cost and the people's awareness of environmental protection,the solid state fermentation technique,which was regarded as low-tech in the past,has regained attention and developed rapidly since the 1990s.The production of lipase by SSF technique was reviewed.Mainly contents describe its characteristics,including physical and chemical factors and bioreactors.
