2. Evaluation of hemodynamics with perfusion-weighted magnetic resonance imaging in patients with cerebral vasospasm after aneurysmal subarachnoid hemorrhage

Objectives: To observe the changes of cerebral blood flow parameters showed by perfusion-weighted magnetic resonance imaging (PWI) in patients after aneurysmal subarachnoid hemorrhage (aSAH) and to investigate the correlation between cerebral hemodynamics and cerebral vasospasm ( CVS ). Methods: A total of 38 patients with aSAH ( ≤7 d) were selected for cerebral angiography. The severity of CVS was observed and PWI was performed. The parameters of regional cerebral blood flow (rCBF) , regional cerebral blood volume (rCBV) , mean transit time (MTT) , and time to peak (TTP) were collected in the territories of anterior cerebral artery, middle cerebral artery, and basal ganglia. The correlation between these parameters and CVS was analyzed. Results: DSA showed that 28 of the 38 patients had vasospasm at least in one branch, 9 of them had mild vasospasm; 10 had moderate vasospasm, and 9 had severe vasospasm; 10 did not find any CVS. Circled digit twoAs compared with the control group,the rCBF and rCBV in ACA, MCA, and BSGL territories in the severe vasospasm group were decreased, and there were significant differences ( P < 0.05 to P < 0.01 ) , while the rCBF and rCBV in all the blood supply territories in the non-, mild, moderate, and severe vasospasm groups had a tendency of reduction, however, the degrees of reduction were different. As compared with the control group, the TTP and MTT had an increasing tendency in the ACA, MCA and BSGL territories in the mild, moderate, and severe vasospasm groups; among them, there was significant difference between the moderate and severe vasospasm groups ( P < 0. 05 to P < 0.01). Circled digit threeThe severity of CVS was negatively correlated with rCBF and rCBV, and it was positively correlated with TTP and MTT. Conclusions: PWI may quantitatively provide the information of cerebral blood flow perfusion. Among the parameters of rCBF, rCBV, TTP, and MTT in all the blood supply territories collected by PWI, the parameters of TTP and MTT had better consistency with the severity of CVS.
3. Advances in the study of arrhythmogenic mechanisms

Arrhythmia is a common complication of cardiovascular diseases and a risk factor for human health. Especially, ventricular tachycardia and ventricular fibrillation may not only exacerbate original heart diseases, but also cause cardiac sudden death which has been an mportant death reason in China. However, anti-arrhythmic drugs nowadays cannot effectively treat these arrhythmias, with an efficiency of only 30%-60%, which indicates that our knowledge about arrhythmias is limited. Hence, to explore the potential mechanism, look for novel targets, and develop drugs with multiple-channel action are the focus of the research direction. Recent studies displayed that the atrial-specific potassium channels such as IKur and IKAch were involved in atrial fibrillation, which provided a prospective target for atrial fibrillation treatment. Calcium leak, gap junction protein and autoantibody against ICaL channel were shown to participate in arrhythmogenesis. These findings provided a theoretical basis for the development of more effective anti-arrhythmic drugs. Remarkably, as a kind of mportant RNA regulating gene expression, microRNA (miRNA) was shown to possess anti-arrhythmic activities which may prevent cardiac sudden death. miR-1, miR-133 and miR-590 regulated the arrhythmia in various types of animal models. Because of the multiple-gene regulation actions of miRNA, it has the potential to be developed as novel anti-arrhythmic target.
4.Application of Mg-PLGA-rhbFGF stent on angiogenesis in hindlimb ischemia

ObjectiveTo prepare recombinant human basic fibroblast growth factor (rhbFGF) loaded magnesium-poly(D,L-lactide-co-glycolide) (Mg-PLGA) stent and to evaluate its angiogenesis effect in rat model of hindlimb ischemia.MethodsThe stent was prepared with spiral magnesium (Mg) and loaded with therapeutic agent rhbFGF and PLGA matrix.In vitro drug release study was carried out and the effect was evaluated using a standard animal model of rat hindlimb ischemia.A mechanical drill was conducted and the stent was implanted.The concentrations of Mg2+ in the muscle adjacent to the stent,rat plasma,urine and stools of the experimental animals were tested to analyze the degradation and metabolism of metal Mg.Immunohistochemical staining was performed to evaluate the angiogenesis effect of the stent.ResultsThe drug loaded in the stent could release continuously for about 4 weeks.The concentrations of Mg2+ in the rat plasma,urine and stools were within normal range.Immunohistochemical and quantitative analysis showed that the effect of Mg-PLGA-rhbFGF stent on angiogenesis of rat limb ischemia was better than that of the control group.ConclusionRhbFGF loaded MgPLGA stent could promote angiogenesis of rat limb ischemia,and it may provide theoretical basis for the critical patients suffered from lower limb ischemic disease.
5.Analysis of Drug Use in National 1 248 Sample Hospitals during 2012-2013

OBJECTIVE:To understand the drug use of sample hospitals,and to provide reference for drug production,man-agement and service department. METHODS:Statistical analysis was conducted in the data of drug use in national 1 248 sample hos-pitals during 4th quarter of 2012-3rd quarter of 2013. RESULTS&CONCLUSIONS:Anti-infection medicine,Chinese patent medi-cine,nervous system drugs has being ranked the top 3 in the list of consumption sum,and that of various drugs is in an upward trend. Top 20 drugs in the list of consumption sum mainly are biological products,drugs for the nervous system,digestive system drugs;that of human albumin and deproteinized calf serum have increased greatly. Top 10 imported pharmaceutical enterprises or joint ventures in the list of consumption sum mainly are multinational enterprises,and they are stable in ranking. The development of pharmaceutical enterprises should focus on brand,quality,price and service,etc.,so that their competitiveness can be improved.
6.Experimental Study of Thyrotropin-Releasing Hormone for the Treatment of Acute Spinal Cord Injury

Purpose: To confirm the therapeutic effects of thyrotropin-releasing hormone(TRH) on the experimental spinal cord injury. Method: Wistar rats were subjected to incomplete spinal cord injury using the modified Allen method. The effects were observed by means of neurologic scoring, quantitative enzyme histocytoehemistry、quantitative immunohistocytochemistry and electron microscopy. Results: 1) The treated group exhibited significantly higher scores than the control group. 2) Acetylcholinesterase (AchE) activity of anterior horn cells in the treated group was higher than that of the control group invarious time, and its activity restored to normal level two weeks after injury; Acid phosphatase(AePase) activity was lower than that of the control group in various time. and its activity restored to normal level four weeks after injury. The content of Nissl's bodies in anterior horn cells in the treated group was higher than that of the control goup one week after injury. 3)The number of axon in a certain area of spinal white matter in the treated group was more than that of the control group. 4)Ultrastructural observation showed that both the nerve tissue injury and the extent of hemorrhage were milder in the treated group than that of the control group. Conclusion: The therapeutic effects of TRH on the spinal cord injury" are shown not only in the improving recovery of motor function, but also in the morphology.
7.Therapeutic effect and safety of finasteride for aged patients with isolated systolic hypertension compli-cated benign prostatic hyperplasia

Objective:To analyze the therapeutic effect and safety of finasteride on aged patients with isolated systolic hypertension (ISH)complicated benign prostatic hyperplasia (BPH).Methods:A total of 84 aged ISH + BPH pa-tients admitted from Jun 2011 to May 2013 were selected.According to medication plan,they were divided into fin-asteride group (n=44)and routine treatment group (n=40,only received routine treatment because they cannot use finasteride).Course of treatment was all six months.International prostate symptom score (IPSS),quality of life (QOL) questionnaire score,prostate-specific antigen (PSA),incidence rate of acute urinary retention,maximum u-rinary flow rate (Qmax),postvoid residual urine (PRV),prostate volume (PV)and incidence rate of adverse reac-tions were compared between two groups.Results:Compared with routine treatment group,there were significant reductions in IPSS [(13.2±2.4)scores vs.(9.4±1.1)scores],PRV [(21.4±4.6)ml vs.(9.0±2.3)ml],PV [(36.8±3.8)ml vs.(29.5±3.0)ml],PSA [(2.45±0.74)μg/L vs.(1.26±0.48)μg/L]and incidence rate of acute urinary retention (17.50% vs.9.09%),P<0.05 or <0.01,and significant increase in Qmax [(10.2±2.4) ml vs.(13.7±3.8)ml]and QOL score [(3.1±1.0)scores vs.(4.7±1.5)scores]in finasteride group (P<0.05). There were no significant difference in incidence rates of adverse reactions between two groups,P>0.05 all.Con-clusion:Therapeutic effect of finasteride is significant and adverse reactions don’ t increase significantly in aged pa-tients with isolated systolic hypertension complicated benign prostatic hyperplasia.
8.Status and advance in chemotherapy for biliary tract carcinoma

Biliary tract carcinoma is a rare cancer with poor prognosis.Although having limited clinical benefit,chemotherapy represents the cornerstone of management for patients with inoperable and postoperative recurrence of cholangiocarcinoma.Substantial breakthroughs in the study of chemotherapy for treatment in advanced cholangiocarcinoma are rare due to the heterogeneity of cholangiocarcinoma,the use of various chemotherapeutic agents and the small size of existing patient cohorts.Currently,no standard chemotherapeutic strategies exist for cholangiocarcinoma,though some combinatorial stratagies,such as gemcitabine plus cisplatin,have proven to prolong survival significantly.Still,further studies are needed to improve the treatment of patients with biliary tract cancer.
9.Post-stroke Depression and Rehabilitation of Neurological Function

Objective:To detect depression in stroke patients and observe its influence on rehabilitation of neurological function and the effect of fluxetine therapy Methods:132 acute stroke patients (78 with infarction, 54 with hemorrhage) were included in the study After inclusion, we did five times of assessments at the end of 2 weeks, 1 month, 3 months, 6 months and 1 year respectively using HAMD assessment to determine whether there had depression or not, as well as SSS (modified Edingburgh-Scandinavian Scoring Scale) for neurological function, ADL (activity of daily living), and self rate depression and anxiety (Zung's self rate depression scale and anxiety scale) Of those with depression, 25 received fluxetine 20mg/d, other 32 with only regular treatment of stroke as control Results:According to results of HAMD, 59 of the 132 had depression (44 7%) The rate of depression had no significant difference between male and female, between those with infarction and with hemorrhage Depression had negative influence on neurological function and activity of daily living Depression had no relation to location, size and unilateral or bilateral of the loci Fluxetine improved depression, neurological function and activity of daily living from 3 months after stroke; its influence on neurological impairment was most significant at 1 year after stroke Conclusion: Post stroke depression is common after cerebral infarction or hemorrhage, it has negative influence on rehabilitation of neurological function and activity of daily living Fluxetine improves both neurological impairments and activity of daily function, as the same time of its improvement of depression
10.Affects upon renal function after conventional surgery or endovascular graft exclusion for infrarenal abdominal aortic aneurysms: A comparative investigation

Objective To assess and compare the affects upon renal function after endovascular exclusion (EVE) or conventional surgery (CS) for infrarenal abdominal aortic aneurysms(IAAA). Methods The records of 157 consecutive patients with IAAA from 1997 to 2002 were retrospectively reviewed. There were a group of 115 patients undergoing EVE and a group of 42 patients undergoing CS. The postoperative changes of plasma Cr and BUN with EVE and CS were analyzed respectively and compared. Results The plasma Cr and BUN were significantly increased in the group of CS postoperatively, but no significant difference were shown before and after endovascular repair was discovered in the group of EVE. Moreover, there was a case with acute renal failure in CS group. Conclusion The affects upon renal function with EVE are much less than CS for IAAA patients.
