2.Analysis of perioperative prophylactic use of antibiotics in 140 patients with type Ⅰ incision operations

ObjectiveTo survey the usage of prophylactic antimicrobial agents in type Ⅰ incision surgical operation.MethodsThe data of medication of 140 patients with type Ⅰ incision operation including thyroid surgery,breast surgery and repairing hernia surgery were investigated and statistically analyzed.ResultsThe preventative antibiotics usage in type Ⅰ incision operations was 100%.The frequency ranking of the perioperative antibiotics application was cephamycins ( 116 cases),cephalosporins ( 18 cases) and aminoglycosides ( 8 cases ).127 patients received antibacterials before operation.The total time for medication at least 1 day,up to 4 days.The antibacterials used in single kind and two kinds were 136 cases and 4 cases.ConclusionThe prophylactic use of antimicrobials in patients undergoing type Ⅰ incision operations in this hospital was irrational in the rate of antimicrobials use,choice of drug categories and medication duration,etc.Antibiotic prophylaxis in type Ⅰ incision perioperative should be further strengthened to supervise and management so as to promote rational use.
3.Comparative study on clinical course and prognosis of intermediate uveitis in children and adults

Objective To explore the clinical process and prognosis of middle and intermediate uveitis in children and adults.Methods A total of 121 patients with intermediate uveitis admitted to our hospital from January 2012 to December 2014 were enrolled in the study.Among them,there were 67 cases with monocular involvement and 54 cases with binocular involvement;59 cases were children and 62 cases were adults.All subjects were treated with personalized regimen according to the severity of the disease.The treatment effects and prognosis were compared between patients with different ocular involvement,children and adults.Results The total effective rate of treatment in patients with monocular involvement,children,patients with binocular involvement and adults were 92.5%,93.2%,92.6% and 91.9%,respectively (all P > 0.05).There was no significant difference in acute phase protein and visual acuity between the patients with ocular involvement and children,adults before treatment (all P > 0.05),and there was no significant difference in the indexes between two groups after treatment (all P > 0.05).The acute phase protein in each group after treatment was decreased,and the visual acuity was obviously increased,there were statistical differences before and after treatment (all P < 0.05).Conclusion Individualized treatment regimen for patients with intermediate uveitis can improve the therapeutic effect,control inflammation and improve visual acuity.The curative effect is not significantly related to age and degree of ocular involvement.

The preliminary results of inactivation of model parvovirus M13Mp 18 in plasma by long-waveUVA irradiation combined with psoralen derivatives were described.At 8-MOP concetration of300?g/ml plasma and UVA intensity of 11.5mW/cm~2,the UVA irradiation for 30~120 min couldresult in virus inactivation of 10~(5~9) infectious dose/ml.Quenchers were used to reduce the damage ofUVA to proteins in order to improve the clotting factor recovery after irradiation.2mmol/Lglutathione,or 2mmol/L glutathione with 2 mmol/L mannitol could significantly improve the Frecovery after irradiation.
5.Effect of melatonin on serum MDA, NF-κB, TNF-αand β2-MG levels in secondary brain damage following intracerebral hemorrhage patients

Objective To investigate the effect of melatonin on serum malondialdehyde (MDA), NF-κB, tumor necrosis factor-α(TNF-α) andβ2-microglobulin (β2-MG) levels in secondary brain damage following intracerebral hemorrhage patients.Methods 90 patients secondary brain damage following intracerebral hemorrhage were divided into experimental group and control group, 45 cases in each group.The control group were given conventional treatment after surgical removal of hematoma, and the experimental group, on the basis of control group, were given melatonin capsule (6 mg melatonin) , orally, once a day, for 2 weeks treatment.The levels of MDA, NF-κB, TNF-αand β2-MG in patients’ serum were detected.Results The MDA, NF-κB, TNF-αand β2-MG levels after treatment significantly decreased compared with before treatment in two groups (all P<0.05), and the above indexes in experimental group significantly decreased compared with control group ( P <0.05 ).Conclusion Melatonin can significantly reduce the levels of MDA, NF-κB, TNF-αand β2-MG levels in brain injury patients with cerebral hemorrhage, and repair brain damage, which has a guiding significance for clinical use.
6.Mitral valvuloplasty in patients with mitral insufficiency caused by endocarditis

Objective Valve replacement is a conventional therapy for the mitral insufficiency caused by IE. Mitral valve repair as an optional procedure for the disease has become feasible in recent years. However, concerns from surgeons about the recurrence of endocarditis after mitral valve repair remained. in this study we evaluated the long-term clinical outcomes of patients treated with surgery for the mitral insufficiency caused by infective endocarditis (IE). Methods Between July 1990 and July 2007, 83 consecutive patients (male 62, female 21) with mitral valve IE were enrolled in this study. Forty-one (49.4% )patients received mitral valve repair ( MVP,group A) and 42(50. 6% ) patients received mitral valve replacement ( MVR, group B). Thirty-seven cases had concomitant aortic valve replacement; 1 patient had aortic valve repair; 4 cases had ventricular septal defect repair; 1 case had atrial septal defect repair, 12 cases had bicuspid valve repair; 2 cases had coronary artery bypass graft and 1 case had femoral artery thrombus. Intraoperative transesophageal echocardiography were performed in 18 cases for the evaluation of mitral valve regurgitation. Mean cardiopulmonary bypass time, aortic clamping time and postoperative ventilation time were recorded and analyzed. Mid- and long-term clinical and echocardiographic outcomes were assessed.Results Preoperative left ventricular end systolic diameter, left ventricular ejection fraction and the classification of New York Heart Association in group A were significantly lower than those in group B (P < 0. 05), but no difference was observed between the 2 groups in the cardiopulmonary bypass time and the crossclamping time. However, the intubation time and ICU time were shorter in group A than those in group B ( P < 0.05 ). More vegetations were seen in the MVR group than in the MVP group. Three (3.6% ) patients died after the operation in group B. All patients were assessed as in NYHA Ⅰ-Ⅱ at discharge.A follow-up was done between 1 to 165 months (mean 39 months) with a mean follow-up rate of 95%. In the MVR group, peri-valvular leakage happened in 1 case, cerebral hemorrhage happened in 2 cases and repetitive pleura! effusion in 1 case. One death happened in the MVR group and none in the MVP group. The 10-year survival rate (100% ) in group A was nonsignificantly higher in group A than that (75% ) in group B(P =0.081). Conclusion Mitral valve repair is feasible for treating mitral valve lesions caused by endocarditis, and may provide an optimistic long-term outcome to the patients. The indication for mitral valve repair is mild to moderate mitral valve lesion. Experienced cardiac surgeons, use of antibiotics before and after the operations based on drug-sensitivity test and blood test, as well as follow-up the patients yearly, are important factors for the favorite outcomes.
7.Clinical analysis with 72 cases of respiratory failure rescued by nasotracheal intubation

Objective To investigate the clinical application and significance of nasotracheal intubation (NTI)on patients with respiratory failure.Methods 72 cases of post-operative and non-operative patients with respiratory failure were performed with urgent NTI or blind nasotracheal intubation(BNTI)and then mechanical venfilation.Results In all of the cases NTI were successful.32 cases received direct vision NTI;26 cases were BNTI:9 case8 were NTI through fibrobronchoscope and 5 cases were NTI under awake surface anesthesia.Operation were performed within 20 second to 11 minutes[(5.1±3.1)min].The remaining time of tracheal catheter were 2~21d [(9.3±5.9)d].Conclusion NTI was an important measure in rescuing the patients with respiratory failures.It is an essential rescue technique under the condition without fibrobronchoseope.
8.Association between intima-media thickness and free fatty acids in essential hypertensive patients with metabolic syndrome

Objective To investigate the association between intima-media thickness (IMT) and free fatty acids (FFA) in hypertensive patients with metabolic syndrome (MS). Methods IMT, FFA and body mass index (BMI) were measured in 69 hypertensive patients with metabolic syndrome (MS group) ,57 hypertensive patients without metabolic syndrome ( Non-MS group) ,and 50 healthy controls ( NC group). Results In the MS group,The BMI, serum levels of triglyceride (TG), fasting blood glucose (FPG) and FFA were ( 26.0 ± 2.1 ) kg/m2, (24.0 ±0.81 ) mmol/L and (5.64 ± 0.82) mmol/L respectively, which were significantly higher than those in the Non-MS group ( (24.9 ± 2.3 ) kg/m2, ( 1.93 ± 0. 55 ) mmol/L and ( 5.10 ± 1.08 ) mmol/L respectively ) and the NC group ( (23.6 ± 1.6), ( 1.49 ± 0.36) mmol/L and (4.70 ± 0.90) mmol/L respectively) ( F = 20.06, 30.96 and 15.17,P <0.05 or 0.01 ). FFA in the MS group ((562.11 ± 55.12)μmol/L) were significantly higher than that in the Non-MS group (402.65 ± 49.53 ) μmol/L) and the NC group ( ( 356.23 ± 45.93 ) μmol/L) ( F = 277.28, P <0. 01 ). IMT in the MS group, Non-MS group and NC group were (1. 10 ± 0. 13 )mm, (0. 82 ± 0. 12 )mm and (0.70 ± 0.11 ) mm, respectively, with significantly difference ( P < 0. 01 ). In addition, the incidence of thickening was 28.99% (20/69), 17.54% ( 10/57 ) and 2.00% ( 1/50 ), respectively, with significantly difference ( P <0.01 ). Conclusions Serum level of FFA is associated with IMT in hypertensive patients with metabolic syndrome.

Objective To investigate the opening- up rehabilitation to the inpatients with chronic schizophrenia .Methods We carried out the rehabilitation opening- up in the hospital to 40 inpatients with chronic schizophrenia from 1990 to 1995. We regulated carefully , respected the personality and lawful rights of the patients , set up the equality between the medical staff and patients , evaluated according to their achievements, took the keeping- watch nursery to the patients with severe condition .No accident happened in the study. Results The social function and negative symptoms of most patients improved greatly. Medicine treatment with maintenance dosage is the basic grantee of the safe rehabilitation . The treatment of working can let the patients regain the skill of working and study .The treatment of entertainment can relaxed ,dredged their emotion and stimulated them .The treatment of reading can increase the knowledge of patients .The patients can recognize and solve their own problems by reading. So, it has educational function. The individual or group psychological therapy can eradicate the disturbance of psychology and emotion of the patients .Conclusions It is effective by carrying out rehabilitation opening- up in the hospital to the inpatients with chronic schizophrenia .
10.Effect of ultrasonic cleaning of post space on the apical microleakage following treatment with two kinds of root canal sealers

BACKGROUND:The post space cleaning using ultrasonic file can improve the bonding strength of fiber postscemented with the self-adhesive resin cement, but the effect on apical sealing ability is stil unclear. OBJECTIVE:To evaluate the effect of the post space cleaning using ultrasonic file on apical microleakage fol owing treatment with two kinds of root canal sealers at two different residual lengths of root canal fil ing. METHODS:Eighty extracted human maxil ary anterior teeth with single-rooted canal were selected, and equivalently randomized into groups A and B (n=40 per group), fol owed by fil ed with Cortisomol or AH-plus, respectively. Groups A and B were randomly subdivided into four groups according to the residual length of root canal fil ing and with or without ultrasonic cleaning (n=10 per group):A1, B1 groups:residual 5 mm, no ultrasonic cleaning;A2, B2 groups:residual 5 mm, ultrasonic cleaning;A3, B3 groups:residual 3 mm, no ultrasonic cleaning;A4, B4 groups:residual 3 mm, ultrasonic cleaning. Subsequently, the apical microleakage was measured using dye method and transparent specimen technique under stereomicroscope. RESULTS AND CONCLUSION:The apical microleakage of each group was as fol ows:A1 (0.73±0.21) mm, A2 (1.37±0.55) mm, A3 (1.18±0.36) mm, A4 (2.05±0.62) mm, B1 (0.34±0.18) mm, B2 (0.47±0.14) mm, B3 (0.59±0.23) mm, B4 (1.39±0.50) mm. The three-factor analysis of variance revealed that the ultrasonic cleaning, root canal sealers and residual length of root canal fil ing al had a significant effects on the apical microleakage (P<0.05), and there were significant differences in the apical microleakage between groups A1 and A2, groups A3 and A4, groups B3 and B4, groups A2 and A4, groups B2 and B4 (P<0.05). These results indicate that the ultrasonic cleaning increases the apical microleakage fol owing the use of Cortisomol at 5 and 3 mm residual length, as wel as the use of AH-Plus at 3 mm residual length. Moreover, for the same root canal sealer, the longer residual length of root canal fil ing indicates, the less microleakage after ultrasonic cleaning.
