Chinese Journal of Postgraduates of Medicine 2014;37(15):43-46
Objective To investigate the relationship between the changes of blood coagulation index before and after neoadjuvant chemotherapy and the chemotherapeutic efficacy in patients with breast cancer. Methods Ninety patients with breast cancer were selected, and detected the changes of blood coagulation index before and after neoadjuvant chemotherapy (TAC regimen),and the relationship between blood coagulation index and the chemotherapeutic efficacy were analyzed. Results The levels of fibrinogen (Fib),platelet count (PLT) after chemotherapy were significantly higher than those before chemotherapy [(3.42 ±0.50) g/L vs.(2.44 ±0.46) g/L,(267.83 ±61.34) × 109/L vs.(174.18 ±48.65) × 109/L] (P < 0.01),D-dimer (D-D) was significantly lower than that before chemotherapy [(176.32 ± 101.41) mg/L vs.(214.55 ± 106.34) mg/L] (P < 0.01).The remaining indicators before and after chemotherapy had no significant differences (P > 0.05).The level of D-D in 68 cases was reduced after chemotherapy, the effective chemotherapy in 53 cases (77.9%,53/68),ineffective in 15 cases (22.1%,15/68);the level of D-D in 22 cases was increased after chemotherapy, the effective chemotherapy in 9 cases (40.9%,9/22),ineffective in 13 cases (59.1%,13/22),the difference was statistically significant (P=0.000).The level of PLT in 10 cases was reduced after chemotherapy, the effective chemotherapy in 3 cases(3/10),ineffective in 7 cases (7/10);the level of PLT in 80 cases was increased after chemotherapy, the effective chemotherapy in 59 cases (73.8%,59/80),ineffective in 21 cases (26.3%,21/80),the difference was statistically significant (P=0.044).Conclusions The Fib and PLT increase while the D-D decreases after chemotherapy,so it is necessary to monitor Fib,PLT,D-D level during chemotherapy. The D-D and PLT are effective indexes in evaluating curative effect of neoadjuvant chemotherapy in patients with breast cancer.
4.Optimization of the Technology of Extracting the Paeoniflorin and Puerarin from Tongqing Granules
Chinese Journal of Information on Traditional Chinese Medicine 2006;0(06):-
Objective To optimize the technology of extracting Paeoniflorin and Puerarin from Tongqing Granules. Methods The experiment used the orthogonal experimental design (4 factors and 3 levels) with the shift-rate of Paeoniflorin and Puerarin as experimental criterion in screening the factors affecting the technology of extracting Tongqing Granules. Results All the 3 factors designed in the orthogonal experiment non-signifantly affected the extracting technology. In consideration of the industrial practice, the best condition of extracting the constituents from Tongqing Granules was decided:water extraction for 3 times, extracting with 12 fold water for 1 hours for the first time, with 5 fold water for 0.5 hours for the second time, and with 5 fold water for 0.5 hour for the third time. Conclusion The condition is the optimized technology of extracting Paeoniflorin and Puerarin from Tongqing Granules.

8.Clinical Observation of Early Application of Tirofiban Combined with Different Lipid-regulating Drugs Inter-vention in Elderly Patients with Coronary Heart Disease
China Pharmacy 2015;(29):4118-4120,4121
OBJECTIVE:To observe short-term efficacy and safety of early use of tirofiban combined with different lipid-regu-lating drugs percutaneous coronary intervention(PCI)in elderly patients with coronary heart disease. METHODS:562 elderly pa-tients with coronary heart disease underwent selective PCI were selected and randomly divided into atorvastatin group and probucol group, with 281 cases in each group. Both group received routine treatment;atorvastatin group was additionally given tirofiban combined with Atorvastatin tablet, and probucol group was additionally given irofiban combined with Probucol tablet. Clinical indi-cators and main adverse cardiac event(MACE)were compared between 2 groups before and after treatment. RESULTS:After treat-ment,3 level of thrombolysis in myocardial infarction(TIMI)in atorvastatin group accounted for 69.4%,which was significantly lower than 95.7% of probucol group, with statistical significance(P<0.05). After 12h treatment,CK,2 groups had lower CK-MB and LVEF and higher cTnI;LVEF decrease of atorvastatin group was more significant, with statistical significance(P<0.05);af-ter 8 weeks of treatment,NO,NO/ET-1,GSH,GSH/GSSG of probucol group were increased significantly, and ET-1,GSSG and Eh decreased significantly;above indexes of atorvastatin group had no significant change, there was statistical significance between 2 groups(P<0.05). The incidence of MACE in atorvastatin group and probucol group were 21.7% and 21.4%,there was no sig-nificant difference (P>0.05). CONCLUSIONS:Tirofiban combined with different lipid-regulating drugs can effectively improve the prognosis of elderly patients with coronary heart disease after PCI operation with good safety, but tirofiban combined with pro-bucol has more obvious therapeutic effect.
9.First 100 cases of off-pump coronary artery bypass grafting: Experience from a training cardiac surgeon
Chinese Journal of Minimally Invasive Surgery 2005;0(11):-
Objective To report the experience of first 100 cases of off-pump coronary artery bypass(OPCAB) grafting by a training cardiac surgeon, and to discuss the feasibility of the training method. Methods The trainee, who had no previous experience of coronary surgery, received an OPCAB training program under the guidance by a director who had completed more than 2000 cases OPCAB.After 28 months of training, the trainee began to be an independent operator. The data of the first consecutive 100 cases of OPCAB by the trainee(Training Group) were collected for comparison with the director's 100 cases of OPCAB surgery(Control Group). Results There were 5 cases of conversion to on-pump surgery in the Training Group. The number of distal anastmosis in the Training Group(2.9?0.6) was smaller than that in the Control Group(3.5?0.7)(t=-6.508,P=0.001).Compared with the Control Group, there were less circumflex grafts(67/100 vs 88/100;?~2=12.645,P=0.000) and sequential grafts(3/100 vs 27/100;?~2=22.588,P=0.000) in the Training Group. There was no significant difference in mortality between the Training Group(3%,3 cases) and the Control Group(1%,1 case)(?~2=0.255,P=0.614).No significant difference was observed between the two groups in other postoperative severe complications. Conclusions The first 100 cases of OPCAB surgery performed by the trainee give favorable results, indicating a stable transition from training period to independent performance. It also testifies the feasibility of this training program.
