Objective To investigate the diagnosis and treatment of tumor of the head of pancreas(THP). Methods The clinical data of 277 cases of THP were restrospectively reviewed. All patients were diagnosed by US,CT, gastroenteric barium meal and /or operation. A comparison was made in 80 cases , who underwent pancreatoduodenectomy were divided into group A(41 patients operated during1982-1995) and group B(39 patients operated after January,1996) . Results 194 patients underwent surgical treatment, including explore laparotomy in 28,bilioenterostomy or T tube drainage in 86,pancreatoduodenectomy(PTD) in 80.Among 80 cases treated with PTD, the operation time, blood loss volume and blood transfusion volume during operation, and serious postoperative complication occurring rate in group A were significantly higher than those in group B(all P 0.05 ).The follow up results were as follows:the average survival time was (4.07 ?1.80) months in patients with bile external drainage, and (8.28 ?2.31) months in hepatojejunostomy,(P

Objective To investigate the application effect of FMEA(failure mode and effect analysis) in prevention of aspiration after stroke. Methods From October 2011 to April 2012 48 stroke patients in our department without implementation of the FMEA management mode were set as the control group. From May 2012 to November 2012,another 48 cases were set as the observation group,in addition to the routine neurological nursing care,we also used FMEA to find forward-looking reason of the failure mode of aspiration and operation process analysis, evaluation, calculation and identify the risk factors of aspiration in patients with cerebral apoplexy and high-risk operation steps, prevention priority action plan and improvement measures. The incidence of aspiration,RPN value of six high risk factors, and satisfaction degree and mastering degree of aspiration related knowledge by patients and their families were compared. Results In the observation group, the crisis values of 6 high risk factors was significantly lower than those of the control group.Significant differences existed in satisfaction degree and mastering degree of aspiration related knowledge by patients and their families as well as hospitalization time. There was no aspiration cases during hospitalization in the observation group while 3 aspiration cases in the control group. Conclusions Application of FMEA to evaluate patients with swallowing disorder after stroke, analyzing and taking relevant measures can effectively prevent risks, reduce the occurrence of aspiration, improve the quality of nursing and promote patients' recovery.

AIM: To investigate the early changes of retinal function in diabetic patients detected by multifocal electroretinogram (mfERG).METHODS: The first-order kernel responses of mfERG were recorded from eyes of 33 normal control subjects,63 diabetic patients without retinopathy and 43 diabetic patients with background retinopathy. The response densities and implicit times of N1 and P1 were compared among the control, diabetic patients without retinopathy and diabetic patients with retinopathy.RESULTS: The response densities of N1 and P1 in central 3 rings were reduced significantly in diabetic eyes with and without retinopathy. And the implicit times of N1 and P1 were delayed significantly only in diabetic eyes with retinopathy.CONCLUSION: mfERG can detect the early changes of retinal function quantitatively in diabetic patients. Analysis of response densities and implicit times of N1 and P1 can reflect the progress of local retinal dysfunction in diabetes.

Objective To study the relationship between the intervals of Mitomycin C treatment and cytotoxicity, apoptosis and drug resistance for bladder cancer cells. Methods The bladder transitional cell cancer line BIU-87 was treated for two hours every time for five times with intervals of 24, 48, 72 and 96 hours respectively.Cytotoxicity was measured by MTT.p53,bcl-2,Bax and p170 expression were analyzed by Western blot. Results The IC_(50)(?g/ml)were 4.41,0.71,2.83,4.51and 6.16 with treatment intervals of 24, 48, 72 and 96 hours respectively, p53 and bcl-2 were significantly down-regulated and bcl-2/Bax was re- duced at 24 hour treatment interval but not changed at 48,72 and 96 hour intervals,p170 was not detected at 24 hour treatment interval but increasingly expressed at 48,72 and 96 hours intervals. Conclusion The in- terval of Mitomycin C treatment is closely related with cytotoxieity and apoptosis and drug resistance of blad- der cancer cells. The intervals of intravesical instillations may play an important role in the effect of chemotherapy.

Objective To probe into the perioperative nursing method during osterior fusion and fmation for idiopathic scoliosis. Methods The study involved 31 patients with idiopathic scoliosis treated with posterior correction nails and selective spinal fusion with hook-rod system fixation. The severe patients were treated with extensive posterior release or anterior correction and fusion posterior to release. Before the operation, the patients were administrated with mental nursing, lung function exercise,self-elongation exercise,wake-up pilot training,self-image appraisement and bed and life skills exercise. After the operation,an observation was done on the change of nervous system, propor turning the body over best clinostatism, drainage tube care, respiratory passage care, functional exercise instruction and discharge instruction. Results The operation was accomplished successfully in all the patients, which showed that the body height was increased 2-15 cm.There found no complications including loosening or breakage of the nails or sticks, nervous system injury, incisional wound infection or press~e sore. Conclusion Preoperative sufficient preparation and fine postoperative nursing are important guarantee to successful operation and fast rehabilitation.

Objective To understand the epidemiological features of syphilis in Qingpu District for developing control strategies. Methods The data of syphilis cases(2009-2018)were collected and analyzed. Results The reported syphilis incidence decreased from 27.17/105 in 2009 to 24.69/105 in 2018. The ratio of secondary syphilis declined from 64.98% in 2009 to 57.14% in 2018, while that of latent syphilis increased from 23.74% in 2009 to 32.89% in 2018. The incidence of syphilis was higher in males than in females, and the average male/female ratio increased from 1.01:1 in 2009 to 1.51:1 in 2018. The incidence of syphilis was the highest in the age group of 20-59 years, but the constituent ratio increased in the aged group(≥ 60 year). The cases were mainly in the local household register, but the composition ratio showed a downward trend from 81.71% in 2009 to 59.14% in 2018. The main source of cases was STD outpatients. It decreased from 89.11% in 2009 to 46.51% in 2018, and the proportion of inpatients and other outpatients increased from 8.56% in 2009 to 50.49% in 2018. Conclusion The incidence of syphilis shows a decreasing trend. The incidence of reported secondary syphilis has decreased in general but increased rapidly in older patients. More measures are urgently in need to control epidemic syphilis effectively.

Chinese Acupuncture & Moxibustion 2006;26(3):195-197
OBJECTIVETo compare therapeutic effects of catgut implantation at acupoint and routine acupuncture on prolapse of lumbar intervertebral disc.
METHODSOne hundred and forty cases were randomly divided into a treatment group and a control group, 70 cases in each group. The treatment group were treated with catgut implantation at acupoint, once each week, 3 sessions constituting one course, and the control group with routine acupuncture, once every other day, 10 sessions constituting one course.
RESULTSThe effective rate was 95.6% after treatment of one course and 88.2% 3 months later in the treatment group, which were better than 84.6% and 72.3% in the control group, respectively (Both P < 0.05).
CONCLUSIONCatgut implantation at acupoint has a better therapeutic effect on prolapse of lumbar intervertebral disc, with lower cost.
