1.Therapeutic effect of ipratropium bromide on bronchial spasm in asthma and COPD

Objective To investigate the therapeutic effect of ipratropium bromide on bronchial spasm of patients with asthma and chronic obstructive pulmonary disease (COPD).Methods 174 patients with asthma and COPD were selected and divided into two groups, 87 cases in the control group treated with budesonide and formoterol fumarate powder for inhalation , 87 cases in the experimental group received ipratropium bromide on the basis of the control group, pulmonary ventilation function, airway resistance and cytokine levels in induced sputum, the clinical effect and incidence of adverse reactions were compared after the treatment.Results The effective rate in the control group(81.61%)was lower than the experimental group (93.11%), with significant difference (P<0.05); compared with the control group, levels of peak expiratory flow (PEF), maximum midexpiratory flow (MMEF), forced expiratory volume in one second to forced vital capacity ratio (FEV1%), forced vital capacity (FVC) were higher in the experimental group after treatment, levels of airway resistance (Raw), Raw% were lower, level of specific airway conductance (Gsp) was higher after treatment, induced sputum levels of vascular endothelial growth factor (VEGF), intercellular adhesion molecular-1 (ICAM-1), interleukin-13 (IL-13), IL-17 were lower after treatment, with significant difference (P<0.05);there was no significant difference in the incidence of adverse reactions between the two groups.Conclusion The clinical effect of ipratropium bromide in the treatment of asthma and COPD was exactly , ipratropium bromide can significantly improve the pulmonary ventilation function in patients, relieve bronchial spasm, reduce airway resistance, inhibit airway inflammation, and the safety is higher.

3.Analysis of high-frequency stimulation-evoked synaptic plasticity in mouse hippocampal CA1 region.

Extracellular recordings of field excitatory postsynaptic potential (fEPSP) is one of the most common ways for studies of synaptic plasticity, such as long-term potentiation (LTP) and paired-pulse plasticity (PPP). The measurement of the changes in the different components of fEPSP waveform, such as the initial slope, initial area, peak amplitude and whole area, were commonly used as criteria for the judgement of potentiation or depression of synaptic plasticity. However, the differences in the conclusions drawn from measuring different components of fEPSP waveform at the same recording have still been largely ignored. Here we compared high-frequency stimulation (HFS)-evoked synaptic plasticity, both LTP and PPP, by measuring different components of fEPSP waveform, including the initial slope, initial area, peak amplitude, whole area and time course. The results not only indicated the acceleration of α-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid (AMPA) receptor kinetics underlies LTP in hippocampal CA1 region of mice, but also showed that different measurements of fEPSP waveform at the same recording result in different magnitudes of LTP and different forms of PPP in hippocampal CA1 region of mice. After HFS, the paired-pulse ratio was slightly decreased by measurement of the initial area, but obviously increased by measurement of the initial slope of the pair fEPSPs. These results might draw apparently contradictory conclusions. Therefore, careful and complete analysis of the data from different parts of fEPSP waveforms is important for reflection of the faithful changes in synaptic plasticity.

4.Epidemiological characteristics and risk factors of rotavirus infection in children aged 3-5 years who seek treatment for diarrhea in Jining area

Objective:To analyze the epidemiological characteristics and risk factors of rotavirus infected in children aged 3-5 years seeking treatment for diarrhea in Jining area.Methods:The epidemiological data of 698 children aged 3-5 years seeking treatment for diarrhea in Jining area hospitals from September 2022 to September 2023 were collected, and fecal samples were collected for laboratory examination. The epidemiological characteristics of the rotavirus infection were analyzed, and logistic regression analysis was applied to analyze the risk factors of rotavirus infection in children aged 3-5 years seeking treatment for diarrhea in Jining area. A predictive model of rotavirus infection in children aged 3-5 years seeking treatment for diarrhea in Jining area was established based on decision tree, and the predictive performance of the model was verified by receiver operating characteristic (ROC) curve.Results:The detection of rotavirus antigen showed 302 positive cases, with a positive rate of 43.27% (302/698). Gender distribution: There was no statistically significant difference in the positive rate of rotavirus infection in these diseased children etween different genders ( χ2=1.862, P=0.172). Age distribution: The positive rate of rotavirus infection in these diseased children showed a decreasing trend with age ( χ2=28.893, P<0.001). Time distribution: There was a statistically significant difference in the positive rate of rotavirus infection in these diseased children among different months ( χ2=241.607, P<0.001), showing obvious seasonal characteristics, the high incidence months were from October to the following March, the highest positive rate was in December, and the lowest was in July. Rotavirus genotype: The G genotype result showed that G9 was the most common, P genotype result showed that P[8] was the most common, and the G/P combination genotype result showed that G9P[8] was the most common. Multivariate Logistic regression analysis showed that having finger sucking habits ( OR=4.193, P=0.018), lack of vaccination against rotavirus ( OR=1.947, P=0.002), whether to clean hands before feeding ( OR=4.719, P=0.007), and a history of contact with rotavirus infected children ( OR=4.976, P=0.001) were independent risk factors for rotavirus infection in children aged 3-5 years old who seek treatment for diarrhea in Jining area. The sensitivity of the decision tree model was 58.94%, the specificity was 87.12%, and the AUC was 0.814, indicating that the four selected risk factors could predict rotavirus infection well. Conclusions:Children aged 3-5 years old who seek treatment for diarrhea in Jining area have a relatively high positive rate of rotavirus infection, showing obvious age and time characteristics. The genotype distribution was mainly G9, P[8], and G9P[8]. Rotavirus infection is related to finger sucking habits, lack of vaccination against rotavirus, whether to clean hands before feeding, and a history of contact with rotavirus infected children.
5.Anti-tumor mechanism of active components from extract of Actinidia rufa root.

OBJECTIVETo observe effect and mechanism of n-Butanol lysate of alcohol extracts from Actinidia rufa root (monomer of R6,R8).
METHODTunel, Wright's stain with Giemsa's stain dyeing, and Hoechst 33258-PI double dyeing assay were used to detect the apoptosis of SGC7901 tumor cells treated with R6, R8. The SGC7901 tumor cells were randomly divided into control group and two treatment groups administered 0.05 g x L(-1) R6, R8, respectively, for 72 h). FCM assay was used to detect the apoptosis. Agarose electrophoresis assay was used to detect DNA strand break of tumor cells and reveal anti-tumor action mechanism.
RESULTThe apoptosis percentage of the tumor cell in 24 h, 48 h, 72 h was (17.08 +/- 2.78)% , (29.68 +/- 2.96)%, (52.46 +/- 3.81)%; (14.75 +/- 2.14)%, (27.35 +/- 3.79)%, (45.64 +/- 5.24)%, respectively, for the treatment group, significantly higher than that in the control group (1.94 +/- 1.55)%, (2.78 +/- 1.84)%, (11.8 +/- 2.79)% (P < 0.01) by tunnel assay. Wright's stain with Giemsa's stain dyeing assay, Hoechst 33258-PI and FCM double dyeing assay showed same action. R6 and R8 had the effect of inducing the DNA histogram of tumor cells (P < 0.01).
CONCLUSIONThe anti-tumor mechanisms may be associated with inducing the injury of DNA and stimulating apoptosis.

6.Sleep and quality of life among rural elderly in Anhui province

Objective The primary purposes of this article were to examine the distributions and correlation between conditions of sleep and the quality of life,so as to explore the risk factors of abnormal sleep among elderly in the rural areas of Anhui province.Methods Elderly who aged 60 years or more (n= 1680) were randomly selected from rural areas in Anhui province.Conditions of sleep and quality of life of the subjects were assessed independently,using the pittsburgh sleep quality index (PSQ1) scale and SF-36 scale.A generalized logistic model analysis was performed,using sleep-related factors as explanatory variables.Results The average PSQI score was 7.43 + 4.13.Persons whose sleep conditions were graded as well,common or poor were accounted for 29.4%,24.3%,46.3%,respectively.Sex and sleep quality were different in terms of quality of life among all the factors (P<0.05).Men had better quality than women,and poor sleepers had poorer quality of life as compared to the one with better conditions.Factors as being married/living alone/economically independent,having better social function,often eating meat or wheat were significant predictors for having good sleep conditions.However,factors as better education received,living alone,with poor vitality/general health,having chronic diseases (back pain,coronary heart or stomach disease),less meat intake,rice as major foods etc.were predictors for poorer sleeping condition.Conclusion Our data showed that it was unsatisfactory on the quality of sleep among the rural elderly in Anhui province.Conditions on general,health and nutrition should be improved in order to prevent the poor sleep condition.
7.Relationship of childhood physical abuse and internet addiction disorder in adolescence: the mediating role of self-esteem

Objective To find out whether the effects of childhood physical abuse on internet addiction disorder in adolescence could be mediated by self-esteem.Methods 3798 high school students selected from 76 classes in Grade One and Grade Two,were asked to fill in the anonymous questionnaire,which including the demographic characteristics of students,Young' s Internet Addiction Scale,Parent-Child Conflict Tactics Scales and Rosenberg' s Self-Esteem Scale.Results Childhood physical abuse could directly predict less self-esteem and internet addiction disorder (r=-0.108,P<0.01,r=0.057,P<0.01 ) and had significant indirect effects on intemet addiction disorder which could be mediated through self-esteem (a=-0.703,standardized b=-0.104,z=5.052,P<0.001 ).Self-esteem had mediated 22.5% of the childhood physical abuse cases on their internet addiction disorders during the period of adolescence.Conclusion Self-esteem could partially mediate the relationship between childhood physical abuse and internet addiction disorder.The mediating roles of self-esteem suggested that salient leverage points could make a change through empowerment training,self-esteem group training on self-esteem enhancement in the stage of adolescence.
8.Effects of early intervention with low-dose dobutamine on pneumonia complicated by sepsis

Objective:To investigate the effects of early intervention with low-dose dobutamine on pneumonia complicated with sepsis.Methods:We retrospectively analyzed the clinical data of 200 patients with pneumonia complicated by sepsis who received treatment in the First People's Hospital of Taizhou from January 2015 to January 2018. We divided these patients into control and observation groups with 100 patients/group according to different treatment methods. The control group was treated with immunoglobulin and methylprednisolone and given ventilator-assisted ventilation. The observation group was given low-dose dobutamine based on the treatments given in the control group. Clinical efficacy, pulmonary function, the incidence of adverse reactions, length of hospital stay, time to dyspnea disappearance, organ failure rate, and mortality were compared between the two groups.Results:Total response rate was significantly higher in the observation group than in the control group [96.0% (96/100) vs. 77.0% (77/100), χ2 = 15.45, P < 0.05]. After treatment, improvements in the pulmonary function indexes [forced vital capacity, forced expiratory volume in one second, and forced expiratory volume in one second/forced vital capacity] in the observation group were superior compared with those in the control group ( t = -15.25, -34.56, -3.77, all P < 0.001). Length of hospital stay and time to dyspnea disappearance in the observation group were (4.23 ± 0.89) days and (3.21 ± 0.58) days, respectively, which were significantly shorter than those in the control group [(8.96 ± 1.58) days, (7.26 ± 0.24) days, t = -26.08, -64.52, both P < 0.001]. The incidence of adverse reactions, incidence of organ failure, and mortality in the observation group were 2.0% (2/100), 1.0% (1/100) and 2.0% (2/100) respectively, which were significantly lower than those in the control group [18.0% (18/100), 20.0% (20/100), 10.0% (10/100), χ2 = 16.80, 19.20, 5.67, all P < 0.05). Conclusion:Early intervention with low-dose dobutamine for the treatment of pneumonia complicated by sepsis can greatly improve clinical efficacy, reduce adverse reactions, decrease the incidence of organ failure and mortality, improve pulmonary function, and shorten the length of hospital stay and time to dyspnea disappearance.
9.Clinical analysis of diffuse axonal injury:a report of 280 cases

Objective To discuss the clinical features and pathological changes of diffuse axonal injury (DAI), and explore the treatment and prognosis of patients with DAI. Methods Two hundred and eighty patients with DAI, admitted to our hospital from January 1999 to December 2009,were enrolled in our study; bilateral decompressive craniotomy was performed in 76 patients and conservative treatment in 204 patients; the clinical data and treatment efficacy of these patients were retrospectively analyzed. Results Good recovery was achieved in 122 patients (43.6%), mild disability in 86 (30.7%), severe disability in 37 (13.2%), persistent vegetative state in 2 (0.7%) and death in 33 (11.8%). Conclusion Decompressive craniotomy is needed as early as possible for patients with brain swelling, unilateral or bilateral mydriasis; early tracheotomy should be performed for patients in coma for a long time to maintain airway smooth; 20% mannitol plus furosemide plus albumin, together with methylprednisolone and mild hypothermia therapy, can be employed to treat patients with early dehydration.
10.Application of high perfusion therapy combined with fasudil and prostaglanddin E1 in patents with cerebral infarction after receiving surgery for severe traumatic brain injury

Objective To investigate the traumatic cerebral infarction treatment in patients with severe craniocerebral trauma combined with cerebral hernia after performing large decompressive craniectomy.Methods Thirty-two patients with cerebral infarction after receiving decompressive craniectomy for severe traumatic brain injury combined with brain hernia,admitted to our hospital from January 2009 to April 2012,were chosen in our study; these patients were given saline (35 mL) +dopamine (100 mg)+Aramine (50 mg) micro-pump infusion for 2-3 mL/h,24 hours of high perfusion combined with fasudil and prostaglanddin E1 for antispasmodic therapy.The clinical data and treatment efficacy of these patients were retrospectively analyzed.Results Postoperative head CT confirmed the disappearance of infarction focuses and good recovery of brain tissues,including cure in 17 patients,good recovery in 12,mild disability and severe disability in 1; no persistent vegetative state or death were noted.Conclusion Under the premise of full blood volume,high perfusion combined with fasudil and prostaglanddin E1 for antispasmodic treatment enjoys good effect in patients with severe craniocerebral trauma combined with cerebral hernia after performing large decompressive craniectomy,which is economical,practical,and worthy to be popularized.
