The primary success rate of intraluminal angioplasty for long superficial femoral artery (SFA) occlusions is low due to the long occlusion length and the hard component of the occlusion. To overcome this problem, subintimal angioplasty has been previously proposed and this technique is now considered as an effective method for the treatment of SFA occlusions. Subsequently, various devices and strategies have been developed to increase the success rate of subintimal angioplasty for SFA occlusions. Here, we present a case in which a long chronic total occlusion of SFA was successfully recanalized by the retrograde subintimal angioplasty through the popliteal artery after the failed antegrade subintimal approach.

The acute technical failure of endovascular treatment of chronic total occlusions is most often due to the inability to re-enter the true lumen after occlusion is crossed in a subintimal plane. True lumen re-entry catheters are very effective at gaining wire passage back to the true lumen and facilitating successful endovascular treatment of chronic total occlusions that would otherwise require open bypass. These case reports describe our initial experiences with a new catheter system (the Outback(R) LTDtrade mark catheter) that is designed to allow fluoroscopically controlled re-entry of the true arterial lumen after subintimal guidewire passage during recanalization procedures of arterial occlusions.

Injury of the femoral artery with a femoral intertrochanteric fracture is rare, and usually occurs on the deep femoral artery during surgery. We experienced a case of preoperative injury of the superficial femoral artery by a lesser trochanteric fragment. We repaired the femoral artery through an anterior approach before internal fixation.

The objective of the present study is to obtain information on the hemodynamic characteristics in the anastomotic sites of femoral artery through the vascular implantation. Three dimensional steady and physiological blood flows in the femoral artery are simulated using the finite volume method. The geometrical shape of the anastomotic sites is made based on the vascular anatomy of a white rabbit. Wall shear stress distributions in the anastomotic sites for the physiological flow are compared with those for steady flow. Blood flow phenomena in the anastomotic sites of the femoral artery are discussed extensively.

A study aims at comparison of the ultrasound Doppler of 30 patients and digital angiography of 24 patients among 30 these patients (6 patients without angiography) in the vascular pathology of Saint Philibert - came hospital during 11/1997-3/1998. The results have shown that the ultrasound found the superficial femoral arterial stenosid in cases with the stenosid grade more than 60% with the confidence of 100%. 5/6 cases with the stenosid grade less than 60% found with the sensitivity of 84%.

The iliac and femoral arterial segments are the most common sites of atherosclerotic steno-occlusive lesions. As a less invasive treatment, endovascular therapy for these arterial segments has been widely accepted as a treatment of choice in selected patients. The long-term patency rate after endovascular treatment using angioplasty and/or stent for the iliac artery lesions was similar to those of surgical bypass. However, the results of femoral artery are still challenging. In this article, the indications, techniques and results of endovascular therapy for iliofemoral steno-occlusive lesions will be reviewed.
