1.Determination of Fluvastatin Sodium Extended-release Tablets by HPLC

OBJECTIVE:To establish a HPLC method for determining the content of fluvastatin sodium in fluvastatin sodium extended-release tablets. METHODS:An Agilent SB-C18 column was used and the mobile phase was CH3CN-0.1% phosphate acid, 50∶50. Flow rate was 1mL?min-1, wavelength was 234nm and the sample size was 20?L. RESULTS:The fluvastatin sodium concentration had good liner relationship from 4.84~96.8?g?mL-1(r=0.999 9), and the average recovery was 100.1%,RSD=0.9%. CONCLUSION:This method was simple, fast, accurate, and precise, could be used to determine content in fluvastatin sodium extended-release tablets.
2.Experimental study on the effects of granulocyte colony stimulating factor on myocardial apoptosis following coronary microembolization

ObjectiveTo investigate the effects of granuloeyte colony stimulating factor (G-CSF) on myocardial apoptosis and left ventricular function following coronary mieroembolization (CME).MethodsA total of 68 male Sprague Dawley rats were randomized into control group(n=24), G-CSF-treated group(n= 24) and sham-operated group(n= 20). The control group and G-CSF -treated group underwent injection of homologous microthrombotic particle suspension into left ventricle when clamping the ascending aorta. The sham-operated group were treated with saline.rhG-CSF(100 yg · kg<'-1> · d<'-1>) were injected subcutaneously for 5 days since 2 h after operation in C-CSF group, meanwhile, the control group and sham-operated group were treated with saline respectively. The rats were sacrificed at day 3,7,14 and 28 after operation. The mRNA expressions of Bcl-2, Bax, Fas, FasL and glyceraldehyde phosphate dehydrogenase (GAPDH) were evaluated by real time PCR. The ratio of Bcl-2/Bax was calculated. The protein expressions of easpase-3 and cleaved poly ADP-ribose polymerase (PARP) were detected by Western blot. Myocardial apoptosis and leftventricular function were checked byterminal-deoxynucleotidyl transferase mediated nick end labeling (TUNEL)staining and hemodynamics respectively. ResultsCompared with sham-operated group,the mRNA expression of Bcl-2, Bax, Fas and FasL were increased whereas the ratio of Bcl-2/Bax markedly decreased in control group(0.28±0.04 vs. 2.98±0.49), P<0.05.Simultaneously, the protein expressions of caspase-3 and cleaved PARP enhanced (0.762±0.129 vs. 0.133±0.027;0.992±0.146 vs. 0.386+0.074) ,all P<0.05. The myocardial apoptosis index was elevated(17.2±1.9 vs. 1.2±0.6)and the left ventricular function decreased significantly (P<0.05). Compared with control group, the mRNA expression of Bct-2 increased and that of Bax, Fas and FasL decreased in G-CSF-treated group. Meanwhile, the ratio of Bcl-2/Bax markedly increased(2.07±0.29 vs. 0.28±0.04)and the protein expression of caspase-3 and cleaved PARP decreased(0.371±0.041 vs. 0.762±0.129;0.548±0.093 vs. 0.992±0.146, all P<0.05). The myocardial apoptosis index was reduced (6.1±1.0 vs. 17.2±1.9)and the left ventricular function increased significantly (P<0.05).ConclusionsG-CSF could lighten the myocardial apoptosis and improve the left ventrieular functionafter CME.
3.A modified Sugiura procedure for variceal bleeding

Objective To evaluate a modified Suguira procedure for the treatment of variceal bleeding.Methods A modified Suguira procedure was performed in 62 patients with acute variceal bleeding (11 cases) that could not be controlled by endoscopic therapy or with a history of massive bleeding (51 cases) after endoscopic therapy.Results Perioperative mortality occurred in 2% (1/62) patients.Esophageal anastomotic leak occurred in 2% (1/62) patients,and anastomotic stenosis developed in 5% (3/62) patients.Twelve months after operation,esophageal varices disappeared in 79% (48/61) patients,diminished in size in 18% (11/61),remained unchanged in 3% (2/61) ; Fundal gastric varices disappeared in 98% (60/61) patients,diminished in size in 2% (1/62).The rebleeding rate was 3% (2/61) and 8% (5/61) in 3 years and 5 years,respectively.Conclusions The modified Suguira procedure is safe and effective for long-term control of variceal bleeding after a failed endoscopic therapy.
4.Systematic evaluation and meta analysis of children bronchial pneumonia treated with tanreqing injection

Objective To evaluate the effectiveness and safety of tanreqing injection to children bronchial pneumonia.Methods PubMed,Ovid,CNKI,and data retrieval system of all places were searched to identify relevant studies from 2003 to November of 2014.The final documents were examined with inclusive and exclusive criteria.Results were pooled with meta-analysis on RevMan software to assess the efficacy of tanreqing injection to children bronchial pneumonia.Results The database was derived from 13 qualified studies that included a total of 1479 patients.Meta analysis showed that,compared to the control group,tanreqing injection treatment group resulted in higher effective rate [OR =2.96,95% CI (2.22,3.96),P <0.01],shorter time of recovering the normal temperature [MD----0.53,95% CI (-0.77,-0.29),P < 0.01],shorter time of disappearance of cough [MD =-1.25,95% CI (-2.14,-0.37),P <0.01],shorter time of lungrale [MD=-1.00,95% CI(-1.4,-0.6),P <0.01],and shorter time of hospitalization time [MD =-1.35,95% CI (-1.83,-0.86),P < 0.01].Significant difference was observed between control and treatment groups.Conclusions This study showed that tanreqing injection appeared to be more effective for children bronchial pneumonia.However,methodology showed low quality.The efficacy needs high-quality clinical trials to verify.
5.Observation of Therapeutic Effects of Shenfu Injection on Sick Sinus Syndrome

OBJECTIVE:To study the therapeutic effect of Shenfu injection on sick sinus syndrome. METHODS: 125 patients with sick sinus syndrome were divided into two groups. 63 patients were given Shenfu by intravenous drip for 14 days as treatment group. The other 62 patients were given glucose insulin potassium(GIK) for 14 days as control group. The changes of clinical manifestations, electrocardiogram(ECG and DCG) and esophagus electrophysiological examination were compared between two groups with a randomized single blind design. Objective To investigate the therapeutic effect of stereotactic surgical treatment and postoperative cognitive functions in patients with treat-resistant obsessive compulsive disorder(TRO). Methods Twelve patients with TRO receiving MRI stereotactic bilateral anterior capsulotomy was retrospectively studied. The evaluation was conducted using the Yale-Brown obsessive compulsive rating scale (Y-BOCS),Wechsler Adult Intelligence Scale-Revised in China (WAIS-RC),Wechsler Memory Scale-Revised in China(WMS-R), Modified Version of Wisconsin Card Sorting Tests (M-WCST) before and 6 months after the operation. Results The scores of Y-BOCS was 5.00(1.00,12.25) and 25.00 (20.25,32.00) after and before surgery, respectively. The scores of Y-BOCS were significantly lower after surgery than before (P<0.05). Most of cognitive function tests were not significantly different in TRO between pre-operation and 6 month post-operation (P>0.05).There were significant improvements in several cognitive tests after surgery including im-mediately logical memory 20.00(12.50,24.00),delayed logical memory16.50(11.50,21.75),immediately visual reproduc- tion 12.00(11.00,14.00),delayed visual reproduction11.00(8.50,14.00) and block design test scores 36.50(29.75,46.75), immediately logical memory14.00(13.00,18.75),delayed logical memory 14.50 (8.75,17.00),immediately visual reproduc-tion 11.00(6.50,11.75),delayed visual reproduction 8.50(6.25,10.00) and block design test scores 30.50(21.00,41.50) (P<0.05). Conclusions MRI-guided bilateral anterior internal capsulotomy is effective and safe treatment for patients with TRO and can improve their cognitive function.

Circulating cell-free DNA (cfDNA) is a kind of free DNA in circulating blood, including circulating tumor DNA (ctDNA) deriving from tumor cells and DNA deriving from normal tissue cells. CtDNA is a specific tumor DNA in cfDNA. CfDNA could reflect the status of tumor to a certain extent, which plays an important role in tumor diagnosis, disease monitoring, treatment evaluation, prognosis and recurrence judgement, and has a potential clinical value as a simple, effective and noninvasive "liquid biopsy"technology. As a new concept, the number of cfDNA analysis in B-cell tumors is increasing, such as the relationship between tumor-specific gene mutations, methylation changes, minimal residual disease (MRD) and prognosis, recurrence, monitoring of treatment effects, targeted therapy by using cfDNA sequencing technology.

Objective Extracorporeal membrane oxygenation is a cardiopulmonary supportive therapy. Since 2004, our institution has adopted venoarterial ECMO for adult patients who otherwise could not be weaned from cardiopulmonary bypass and patients experiencing postcardiotomy cardiogenic shock and/or pulmonary dysfunction unresponsive to conventional treatment algorithms. In this study, we reviewed our experience with ECMO support and tried to identify measurable values which might predict in-hospital mortality. Methods From January 2004 through December 2008, 50 of 21,298 adult patients received VA ECMO. We retrospectively analyzed clinical records of these 50 consecutive patients. Demographics, preoperative measurements, clinical characteristics at the time of ECMO implantation, ECMO related complications and in-hospital mortality were collected. Logistic regression analyses were performed to investigate predictors of mortality. A p value ≤0. 05 was accepted as significant. Results Mean ECMO duration was ( 110 ± 17 ) hours. 38 patients were weaned from ECMO and 33 patients survived upon discharge. The overall survival was 66%. In univariate analyses, duration of ECMO support, receiving cardiopulmonary resuscitation prior to ECMO setup, ECMO setup in ICU, pre-ECMO plasma lactate level, infection, lower limbs ischemia, renal failure, experiencing at least one ECMO related complications were all associated with in-hospital death. In a multiple logistic regression adjusted for other factors mentioned above, blood lactate level before initiation of ECMO was a risk factor associated with in-hospital mortality (OR 1. 27 95% CI 1. 042-1. 542 ). To evaluate the utility of pre-ECMO lactate in predicting mortality, a conventional receiver operating characteristic curve was produced. Sensitivity and specificity were optimal at a cut-off point of 12.6 mmol/L, with an AUC of 0. 752. The positive and negative predictive values were 73.3% and 83.9% respectively. Conclusion ECMO is a justifiable alternative treatment for postoperative refractory cardiac and pulmonary dysfunction which could rescue more than 60 percent of otherwise fatal patients. Patients with pre-ECMO lactate above 12.6mmol/L are at higher risks for in-hospital death. Evidence based therapy for this group of high risk patients is needed.
